A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. 01 fire bucket was fractured. Replace the fire bucket. 46 CFR 181.610

Each collision bulkhead must not be fitted with any type of penetration or opening except penetrations may be made if they are located as high and as far inboard as practicable and they have a means to make them watertight. There was water intrusion near the starboard side collision bulkhead. Seal the penetrations. 46 CFR 179.310

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Wood rot near 02 backing plates next to the starboard water slide. Crop and renew the wood. 46 CFR 46 CFR 177.300 (a)

All vessels shall be provided with a suitable steering apparatus. Steering gear for the two outboard engines stopped working upon arrival to the dock. 46 CFR 182.30-1 Code 17C.

The bilge pipe in "s" vessels shall be not less than one inch nominal pipe size. The bilge suction shall be fitted with with a suitable strainers having an open area not less than 3 times the area of the bilge pipe. None of the fixed bilge hand pump hose outlets has a strainer installed to prevent debris to clog the pipe. 46 CFR 182.25-5 (b), Code 16C

vessel limited coastwise route must carry six hand red flare and six hand orange smoke distress signals. any of the hand orange smoke distress signals may be substituted for A rocket parachute flare, or A hand red flare distress signal. Vessel was missing one orange smoke and one red distress signals. 46 CFR 180.68 Code 16C.

Every vessel shall be fitted with a suitable compass. Compass transparent plastic display is damaged, and all glycerin fluid leaked to empty 46 CFR 184.20-1, code 16C.

Fuel flexible hose may be used provided it is fitted with suitable connection fittings and has high resistance to saltwater, petroleum oils and vibrations. Fuel hoses from the engine to the filters, and from the filters to the portable gas tanks shows signs of UV damage. Replace flexible fuel hoses in both port and starboard engines, and filters. 46 CFR 182.15-40 code 16C.

All vessels shall be provided with a suitable steering apparatus. Steering gear for the two outboard engines stopped working upon arrival to the dock. The port side handheld wood steering tiller is showing signs of wood rot at the base. Replace tiller. 46 CFR 182.30-1, Code 16C.

A vessel at anchor shall exhibit where it can best be seen In the fore part, an all-round white light or one ball. Vessel is missing the ball day shape while at anchor. 33 CFR 83.30 (a). Code 16C.

Sternlight means a white light placed as nearly as practicable at the stern showing an unbroken light over an arc of the horizon of 135 degrees and so fixed as to show the light 67.5 degrees from right aft on each side of the vessel. Sternlight was not found in compliance with with angle and was loose. 33 CFR 83.21 (c), Code 16C.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space. Wood rot found in the compartment between mid ship and portside at the transom, on which a structural backing plate was found rotten, crop and renew. 46 CFR 176.600 (d), Code 16C.

The master of the vessel shall ensure that the EPIRB is tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. No evidence was found that EPIRB was being tested and logged. Deficiency corrected on the sport by conducting EPIRB testing and logging it. 46 CFR 185.25-20, Code 10C corrected on the spot.

Aluminium support stanchion bases for the stern bench both were found corroded, allowing water to enter the under deck into the lazzarette.

The construction and arrangements of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection considering provisions for a seaworthy hull. Provide a repair proposal with details of how the following areas found with wood will be repaired and addressed: a. Crop a renew two ring frames located in the starboard side hull, close to midship found rotten due to water saturation. b. Area of wood rot was found both port and starboard side hulls by mid-ship below waterline. c. Fracture found in centerline transverse frame, directly below where the mast is located. Once the hull plywood has been sanded, vessel owner shall notify USCG and request an attending USCG Marine Inspector to verify the condition of the wood, prior determine if the vessel shall conduct a credit for dry dock prior scheduled date in YR2025. 46 CFR 177.100

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the required safety items as established by Regulation 46 CFR 185.506(a). Vessel has been operating without providing an accurate safety orientation as required. 46 CFR 185.506(a)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high-water level in each of the following normally unmanned spaces and a visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The port visual alarm indicator was found inoperative. 46 CFR 182.530(a)(c)

At each inspection for certification and at every other vessel inspection, quarters, toilet and washing spaces, galleys, serving pantries, lockers, and similar spaces may be examined to determine that they are serviceable and in a sanitary condition. Vessel was found with pests, conduct a professional contracted fumigation service and provide a fumigating certificate. 46 CFR 176.818

14404 - When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include— (1) Closing each valve leading to an overboard discharge and removing the handle; (2) Padlocking each valve leading to an overboard discharge in the closed position; or (3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. Marine inspector discovered that vessel is equipped with a type III holding tank with a macerator pump installed on the discharge side. The sewage piping seacock was missing the handle, the master stated that the valve was in the open position and they controlled the discharge of sewage by activating the breaker and manually starting the macerator pump. 33 CFR 159.7 (c) 705 C - prior to using marine head

14404 - After January 30, 1980, subject to paragraphs (e) and (f) of this section, marine sanitation devices on all vessels on waters that are not subject to a prohibition of the overboard discharge of sewage, treated or untreated, as specified in paragraph (a)(1) of this section, shall be designed and operated to either retain, dispose of, or discharge sewage, and shall be certified by the U.S. Coast Guard. If the device has a discharge, the effluent shall not have a fecal coliform bacterial count of greater than 200 per 100 milliliters, nor suspended solids greater than 150 mg/1. Marine inspector questioned master how they discharge sewage from their type III holding tank, the master stated that he activates the macerator pump and discharges sewage every trip. The vessel's route is confined to 3 NM from Fajardo, an area where untreated sewage discharge is prohibited. 40 CFR 140.3 (d) 705 C

Each ship transporting more than six passengers for hire equipped with a radio telephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements; where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. At the time of the inspection the vessel Master did not have a FCC Marine Radio Operator Permit.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency instructions were not posted at the operating station.

Each vessel that operates on the high seas must have on board a FCC Type Accepted Category I, 406MHz EPIRB installed to automatically float free and actuate. The hydrostatic release was both expired and not functional.

At least one ring life buoy must be fitted with a lifeline. The lifeline attached to the ring life buoy was deteriorated and exhibited broken fibers throughout its entire length.

A vessel not required to have a power driven fire pump must have at least three 2.5gal buckets with an attached lanyard and the words "FIRE BUCKET" must be stenciled in a contrasting color on each bucket. Two fire buckets did not have a lanyard, and one fire bucket was not properly stenciled.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. The two batteries stored within the operating station had wing nuts on all four battery terminal posts.

A hatch exposed to the weather must be watertight. Hatches on port and starboard side were not watertight.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and informatio, specific to the individual vessel. An emergency broadcast placard was not posted at the operating station near the radio.

No major repairs or alterations affecting the safety of the vessel with regards to the hull, machinery or equipment, shall be made without the knowledge and approval of the OCMI, Marine Inspection. The portside rudder tube assembly in the lower and outer part of the hull was found to have a hull fracture of about 3" long; and the base of the of the fiberglass rudder tube where bond with the fiberglass bottom plate was found with a fracture that caused water ingress to the portside hull. 1. Submit a repair proposal to conduct permanent repairs for hull fracture and rudder tube base fracture. 2. Conduct all repairs must be conducted as in kind (original) approved plans and repair proposal. 46CFR176.20-1

No major repairs or alterations affecting the safety of the vessel with regards to the hull, machinery or equipment, shall be made without the knowledge and approval of the OCMI, Marine Inspection. USCG Marine inspector found multiple unapproved welding repairs on existing fractures in several locations of the outboard engines mounting sled, including all structural stringers. 1. Uninstall engine sled to further examination for more stress fractures and damages. 2. Submit a repair proposal to address how these fractures will be corrected. 46CFR176.20-1

At each initial and subsequent inspection for certification, and reinspection the marine inspector shall examine the main steering apparatus to the extent necessary to determine that its condition is in satisfactory and that its fit for the service intended. 1. Remove and uninstall starboard side rudder post for inspection. 2. Replace all port & stbd cutless bearings; found wasted with excessive play. 46CFR176.25-35

The Marine Inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that they are in satisfactory condition. The wooden deck next to the net attachment, portside, was found to have multiple areas of wood rot. Crop and renew all areas found with wood rot. 46CFR176.10-5

No major repairs or alterations affecting the safety of the vessel with regards to the hull, machinery or equipment, shall be made without the knowledge and approval of the OCMI, Marine Inspection. Proposed alterations shall be approved by the OCMI, Marine Inspection, before work is started. Drawings will not be required for repairs or replacements in kind (original approved design). The attending USCG Marine Inspector noticed that the forward net attachment was altered from its original design noted in the plan SR 50-9 Rev. B dated 04/20/95. Vessel forward net attachment must be returned to its original construction as designed, or submit the existing net arrangement for approval to the USCG. 46CFR176.10-5

The Marine Inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that they are in satisfactory condition. The forward netting ropes between port and starboard hull are showing signs of wastage and some are frayed. All forward netting ropes must be replaced by a strong, UV resistant rope 3/8" black poly as DWG# SR 49-5. 46CFR176.10-5.

The Marine Inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that they are in satisfactory condition. The wood and fiberglass structure that holds the forward ladder to embark passengers was found to have wood rot on several pad eyes, where the rope secures it in place. Also, it was noticeable several fractures at different locations. Uninstall and conduct proper repairs (if reparable), otherwise a new structure must be made in kind (original). 46CFR176.10-5.

The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Marine inspectors observed multiple sections of the vessel's deck being removed due to wood rot. No repair proposals, plans, or requests were sent to OCMI for approval. Vessel is required to submit plans/scope of work being conducted as well as pictures of wood rot extent and current repair progress. Final repairs must be observed in person and approved by attending marine inspector prior to vessel operating.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. The starboard side bilge high level alarm was inoperable.

Provide the current main and jib sail square footage dimensions from a 3rd party sail and rigging surveyor in order to compare it current approved Marine Safety Center approved Sail Plan. Last sail and rigging 3rd party survey was missing the current sails dimensions.

The Rigging Preventive Maintenance Plan (PMP) general description, examination & maintenance schedule, inventory of rigging components must be reviewed and updated. Last version of the PMP found does not have any date and most of the information has changed. Conduct a full review and update of the PMP and submit to USCG Prevention, Inspections Office via email at [email withheld].

The OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary. There was no record of the vessel conducting standing rigging maintenance as prescribed in their rigging survey.

Each EPIRB must be tested monthly and each test must be logged monthly. The 2020 vessel log did not show evidence of monthly EPIRB tests.

The master or managing operator shall instruct each crew member at least once every three months, the duties that the crew member is expected to perform in an emergency. The 2020 vessel log did not show evidence of conducting emergency drills.

Each ring life buoy must be marked with Type II retroreflective material. The vessels ring life buoy did not have Type II retroreflective tape.

A vessel must carry one floating waterlight. The battery within the waterlight had an expiration date of 01/20.

The items contained in an approved first-aid kit shall be as listed in Table 160.041-4(b). The vessels first-aid kit did not have all necessary contents.

Vessel is required to have valid Radiotelephony certificate on board. Radiotelephony certificate on board was expired. 47CFR80.903 16c

Twist-on type may not be used for making joints in cables. The pump in the head on the starboard side of the vessel had two twist-on type joints in its wiring. 46CFR183.340.i.2 16c

Port and STBD void fwd of collision bulkhead are full of water. pump out and make ready for inspection
