A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. It was report that the port MDE had a ruptured expansion joint causing the J/W to drain into the bilge and therefor had to secure the MDE. Conduct repairs and test to the satisfaction of attending CG Marine Inspector prior to carrying passengers.

Each hand portable fire extinguisher shall be marked with a number and the location where stowed shall be marked with a corresponding number at least 1/2 inch high. Where only one type and size of hand portable fire extinguisher is carried, the numbering may be omitted. Stations #6 & #10 were missing numbers. Replace missing numbers to the satisfaction of attending CG Marine Inspector.

CG was notified with vessel loss of steering while trying to back out of terminal. Conduct repairs, test and conduct satisfactory sea trials to the satisfaction of USCG prior to carrying passengers.

The lights prescribed in these rules shall have an intensity as specified in Annex I to these rules, so as to be visible at the minimum ranges. Vessels of 50 meters or more in length must have a white all around light for 3 miles. All around white light was found with one light not working and the 2nd all around light working, the off with an alarm at helm station and then flickering. Make repairs and test to the satisfaction of attending CG Marine Inspector.

CG was notified stating the port clutch was banging/thumping every time clutching in. MI was notified stating vessel was struck by lightening. Multiple electronics have been affected. Repair or replace with in-kind equipment and make available for attending MI.

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. Vessel lost power on #1SSDG for approximately 20 seconds during transit. Prove proper operation of #1SSDG to satisfaction of CG.

The Type II retro-reflective material approved under approval series 164.018 must be placed on the boat to meet the arrangement requirements in IMO Resolution A.658(16). Renew retro-reflective tape on rescue boat. Tape was found missing peeling off.

The inspection shall be such as to insure that the vessel, as regards to life-saving appliances and other equipment is in satisfactory condition and fit for the service for which it is intended. A minor leak found on hydraulic accumulator for the rescue boat davit. Make effective repairs IAW manufacture instructions.

At each annual inspection, the inspector shall examine the tank tons and bilges in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. Excessive accumulation of oil and water mixture was discovered in the bow thruster space bilges during the inspection.

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter F (Marine Engineering) of this chapter. MI discovered signs of multiple coolant leaks (crystalized coolant) on main engines and ship service generators. Provide local OCMI proposed repair plan and/or findings.

Trouble shoot and determine the cause of the inoperative #2 SSDG breaker, found during morning boat checks while out of service. Provide findings in regards, make proper repairs, and conduct satisfactory operational test to the satisfaction of the Coast Guard.

Bilge system must be capable of pumping from any watertiht compartement. Vsl #1 port void bilge suction is inop. Vsl Must provide bilge pumping capabilities iaw this part.

BILGE PIPING MUST BE CAPAPLE OF PUMPING FROM AND DRAINING ANY WATERTIGHT COMPARTMENT. VSL SELF REPORTED A 1/4" HOLE IN THE SUCTION SIDE OF THE MAIN BILGE SYSTEM. VSL MUST PROVIDE PIPING CAPABLE OF PUMPING AND DRAINING WATER FROM ANY WATERTIGHT COMPARTMENT IAW THIS PART. 46 CFR 56.50-50

Crew reported a loss of electrical power on the vessels #1 SSDG, while in service. Trouble shoot, make proper repairs, and report findings to the satisfaction of the local marine inspector.

Operating generators must provide continuous and uninterrupted source of power for the ship's service load under normal operational conditions. Master reported no voltage on #2 service generator rendering inoperable. Make permanent repairs to the satisfaction of the attending Marine Inspector. Vessel authorized one time transit to Cedar Island Terminal with no passengers for repairs.

PASSENGER VESSELS OVER 500GT MUST CARRY ON EACH SIDE OF THE VESSEL AT LEAST ONE RESCUE BOAT. VSL HAS ONLY ONE RESCUE BOAT INSTALLED. VSL MUST PROVIDE SECOND RESUCE BOAT OR AN APPROVED EXEMPTION LETTER FROM THE OCMI. 46 CFR 199.202

The hull, structual bulkheads, decks, and deck houses shall be constructed of steel or other equivalent metal construction of appropriate scantlings. Vessel has temp repair on bulkhead in EDG space. Permanent repairs must be conducted to the satisfaction of the attending marine inspector.

The crew reported the vessel's emergency diesel generator subsequently did not take the vessel's electrical load as design, upon loss of the normal electrical load.

Each self propelled vessel must have both main and auxiliary steering gear, except when the main steering gear includes two or more identical power units. Master reported the #2 steering system kicked out to 0 degrees when they passed 35 degrees to port. Make permanent repairs to steering system. Provide technician's report of findings and repair. Prove proper operation of steering system to USCG Marine Inspector.

During morning checks the crew discovered the vessel's emergency diesel generator was not taking the ships electrical load as designed. Trouble shoot and determine the cause, make proper repairs to the satisfaction of the Coast Guard.

All installations shall be of marine type engines suitable for the intended service, design, and constructed in conformance with the requirements of this subchapter. A fuel leak was detected on starboard main engine fuel filter housing appox. 1/2" cracked weld was cause of casualty. Make effective repairs and prove proper operation to the satisfaction of the attending marine inspector.

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he/she deems necessary to be assured of the safety of the vessel. Perform operational tests of the fixed fire extinguishing systems IAW table 46 CFR 71.25-22(a)(2) in the presence of the attending Marine Inspector.

The installation of all systems of a marine engineering nature with details with details of installation shall be in accordance with the requirements of sub-chapter F. The starboard generator lube oil hose IVO gauge is wasted.

It shall be the duty of the owner , master or person in charge to see that the vessel's firefighting equipment is maintained. #19 portable CO2 in the engine room has worn out hose.

Each credentialed crewmember must become familiar with the relevant characteristics of the vessel's fire fighting systems. Chief engineer was not familiar with the engine room fixed fire extinguishing system.

Bilge pumping plant must be capable of pumping from any watertight compartment. During operational test of the engine room, bilge suction crossover valve was found open.

Vessels contracted for prior to January 18, 2017, must meet the following requirements: (1) Previously installed extinguishers with extinguishing capacities smaller than are required in Table 76.50-10(a) of this subpart need not be replaced and may be continued in service so long as they are maintained in good condition to the satisfaction of the Officer in Charge, Marine Inspection; and (2) All new equipment and installations must meet the applicable requirements in this subpart for new vessels. During an expanded examination, the attending inspector discovered eleven (11) fire-extinguishers, that were manufactured after the applicable date and did not have previous identification IAW the vessel Certificate of Inspection. These fire extinguishers do not meet the new classification system required by regulation. Provide change out plan and or renew the fire-extinguishers on board to comply with the regulations of table 76.50-10(a) and to the satisfaction of the Coast Guard.

All emergency lights shall be marked with a letter "E" at least 1/2 inch high. All emergency light on upper passenger deck are fade or painted over.

In all ventilation systems dampeners provided be served by the systems for shutting off the passage of air in the event of fire. Ventilation shutdown switch for bow thruster space was found rigged in closed position.

Before vessel leaves port and at all times during the voyage each life saving appliance must be in working order and ready for immediate use. During spot check M/I noted a wasted life ring buoy and two float lights with defective batteries not being maintain properly by crew.

Rescue boat davit hydraulic reservoir small drop of fluid at the spot weld. ID cause. MI observed for 90 mins and could not duplicate.

Passenger compartment to galley ladder handrail loose.

Bowthruster lagging improperly installed. Causing a burn/fire haz.

Properly install tank room overhead hatch gasket.

Reinstall spot light cover gasket.

Fixed firefighting system (CO2 bottles and local actuation) for bow thruster is located within the bow thruster space. Provide evidence of prior USCG approval or modify arrangement to ensure bottles are not located inside space iaw 46 CFR 76.15-20

Actuation for engine room fixed firefighting is located within engineering space. Provide evidence of prior USCG approval or modify arrangement to ensure remote actuation is not located inside space iaw 46 CFR 76.15-10

Ensure all portable fire extinguishers are in approved locations in accordance with 46 CFR 76.50-10 TABLE and provide evidence of servicing of semi-portable extinguisher iaw 46 CFR 76.50-10

Update fire control plan as needed iaw 46 CFR 71.65-5(d)(6)

Install low lube oil alarm for bow thruster and provide associated audible/visual alarm is installed in the engineering booth iaw 46 CFR 62.35-50 and 46 CFR 62.25-30(b)(5)

Submit updated electrical plans to MSC for approval for newly installed wiring/alarms on bridge from engine room, aft steering to control booth, and bow thruster to engineering booth iaw 46 CFR 70.65-10 and 46 CFR 71.55-1.

Replace/repair broken watertight door indicator iaw 46 CFR 170.270(e).

Master reported high compression on the starboard engine. A requirement was issued to determine cause and report findings to the coast guard inspector.

Captain reported oil leaking from the starboard main engine block to Coast Guard. All repairs are to be made to the satisfaction of the attending Marine Inspector prior to carrying passengers.

The vessels chief engineer license did not meet the conditions of the manning per the COI. License presented - stated in limitations motor propelled for less than 500 GT, vessel is greater than 500. Provide prove of proper manning within 90 days, per CPREV's direction.

Upon arrival to vessel all doors and windows were observed open due to A/C inoprative. After checking engineering logs A/C has been having problems since Dec 2019 and currently isnt working due to compressor issue.

Rescue boat port bow gel coat cracks in several areas above rub rail. Sand and repair.

Rescue boat davit hydro tank spacers wasted and must be replaced.

MIs noted hole in the E-Gen aft bulkhead due to corrsion. Temp repair will be made until dry-dock dated 31 Jul 2020.

#11 main deck fire ext. missing safety pin.

Bow thruster thur hull auto valve leaking sea water into vessel when operating. Vessel must correct prior to passengers this may require dry-dock.

DUplex sea strainer for A/C untit from seachest not functioning properly causing the valve to leak by. Replace valve with new.

Steering rams leaking port and stbd. MIs noted drip pans and rags under steering rams. This will be correccted in the 31Jul dry-dock

E-gen vent screen not properly installed/ pushed in.

Hot water heater wasted. Replaced water heater and mounts.

Potable water head tank wasted at bottom and mounts.

Water tight doors to a/c room and main deck to bow thruster room losse dogs were noted. Tighten and rechaulk test.

Rescue boat would not start. Problem found, adjust netural so vessel will start.

Crew passage way hall lighting lights were 100% out. Install new light bulbs.

Life jacket stowage boxes upper deck found to have broken hinges and rivots.

The rescue boat davit pin retaining clips on craddle were wasted and damaged.

The rescue boat port and starboard side life lines are worn and wasted.

The galley and crew berthing has loose tiles which are damaged and creating a slip and tripping hazard.

The ladder well stairs has loose ladder treads.

The bow entrance to passenger compartment overhead emergency light has water and filled with insect.

The main deck life ring buoy line is not properly installed.

The fire pump has metal shaving in the bilge area.

Both MDE's and SSDG's resilant mounts for gauges are damaged.

The port side main engine shaft seal cooling pump is inoperative.

The pilot house air handler has an unusal noise vibration.

Both MDE's have damaged gauges (on local box)

The #1 SSDG gauge panel has two open hole in the panel.

The #1 SSDG overspeed and emergency stop is inoperative.

Conduct satisfactory sea trials and drills.

The MDE port shaft seal cooling gauge is inoperative.

The aft stern light is not displaying the proper arc (lens is shifted) Prior to night operations.

Unclog condensate drain lines IWO crew quarters

Repair erratic shaft tach operation P&S

STBD shaft leaking approx. 2 GPM due to shaft seal and fwd to aft movement of shaft.

The discharge side of Fire Pump #2 has a pin hole leak.

Found an exhaust leak on the #2 MDE IWO the exhaust silencer flange. Renew the exhaust gasket.

Unable to test/ verify the low lube oil alarms on #1 MDE, #2 MDE, and the Bow Thruster. Install fittings to facilitate test.

Verify proper operation of fuel filter/separator differential pressure guages (MSD space fuel unit). Verify proper operation of fuel check valve (before the primary racors) IWO #1 generator. The above requirements are due to the #1 generator becoming airbound due to clogged remote primary fuel filters.

Attending Inspectors noted fractured weldment IWO port quarter main deck stanchion along with buckled bulwark(s).
