Midbodies of all tank vessels certificated to carry a pollution category I oil cargo listed in 46 CFR table 30.25-1 must undergo an initial gauging survey and periodic regauging surveys as follows:(1) An initial midbody gauging survey must be accomplished no later than the next drydocking inspection after the midbody becomes 30 years old. Regaugings:..(ii) Midbodies of all other tank vessels must be regauged at intervals not exceeding 5 years. Provide proof of completed midbody guaging surveys or carry out the required midbody gauging survey as required above. 46 CFR 31.10-21a

Attending MI observed plate deterioration and wastage on bottom and side shell plating during hull walk. Ultrasonic testing has been reviewed by our office and has determined the following: 1.) Crop & Renew bottom plate identified on Windward report dated 09May2025 displaying Ultrasonic Readings (UT) per the ABS Rules PART 7 Section 4 TABLE 1 standard below the 25% uniform wastage threshold for 3/8" plate. 2.) Ensure USCG attending Marine Inspector (MI) is afforded adequate notice for call-out work for the following at a minimum: Fit-up, back-gouge, Final NDT as deemed appropriate by the attending MI. Code: 705c - prior to DDX credit

Forward and Aft rake goose necks on the topside of barge shall be opened and available for USCG MI to inspect. Aft rake goose neck is missing closure device. Cite: 46 CFR 31.10-21(a) Code: 705c - prior to DDX credit

Except as provided in paragraphs (b) through (g) of this section, each tank vessel must undergo dry dock, internal structural, and cargo tank internal examinations. Vessel did not complete a dry dock or internal structure exam by 28FEB2025 as required by the vessel's COI. Vessel is authorized to discharge cargo. Vessel is not authorized to load any additional cargo. Vessel is restricted from movement until dry dock. Vessel requires successful DDE, ISE, and COI Renewal prior to resuming operations. 46CFR 31.10-21

All cargo tank hatches must be clearly marked in not less than three inch lettering “Danger—Keep Out". Cargo tank hatches were mssing the required signage.

For vessels having a square bow, the name of the vessel must be marked on some clearly visible exterior part of the bow in a manner to avoid obliteration. The name and hailing port must be marked on some clearly visible exterior part of the stern. Vessel's name was found to be incorrectly marked.

Nozzles, flanges, or other fittings for pipe connections must be welded or brazed to the tank. The tank opening in way of pipe connections must be properly reinforced where necessary. Where liquid level indicating devices are attached to the tank, they must be of heat resistant materials adequately protected from mechanical damage. Sight glass on deck tank was made with PVC fittings.

Your vessel must undergo an annual inspection 3 months before or after each anniversary date. Vessel did not undergo an annual inspection within the required window. Vessel must complete a COI Renewal inspection and have a valid COI prior to operations.

A vessel being pushed ahead, not being part of a composite unit, shall exhibit at the forward end, sidelights and a special flashing light. Sidelights did not work. 33 CFR 83.24(f)(i)

Marine employers shall establish programs for the chemical testing for dangerous drugs on arandom basis of crew members on inspected vessels who (1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection. Crew member(s) are not enrolled in a random chemical testing program. 46 CFR 16.230(a)

Cargo hoses must have no unrepaired kinks. The 3" diameter 100' fuel hose is kinked. Conduct 150%MAWP hydro test or replace hose prior to using using.33CFR156.170(c)(2).

Cargo pump relief valves shall be tested at least once a year. The cargo pump relief valve has not been tested in the past year. 46CFR33.35-70(b)

Cargo pump pressure gage shall be tested at least once a year for accuracy. The cargo pump gage has not been tested for accuracy. 46CFR35.35-70(c).

Vacuum Pump Engine located in the hazardous location does not meet Class I Division I requirements. Submit plans to address pump location/requirement issued to USCG.

Vessel is not authorized to transfer cargos with a flash point below 140 degrees Fahrenheit.

Vessel had a vacuum tank install without proper plan review or submitting request/plans to MSC. Vessel shall not operate until proper plan review and approval is met.

Prime mover was leaking oil as well as black smoke from exhaust. In addition, hydraulic oil was leaking from the HO return tank. Make effective repairs to the satisfaction of attending CG Marine Inspector prior to receiving Certificate of Inspection.

Vessel's overfill protection devices (liquid overflow sticks) are inoperable. Make effective repairs and display proper operation to the satisfaction of attending CG Marine Inspector.

Vessel rep has requested to be removed from Vapor Control System requirements under 46 CFR 39. In order to do so, rep must contact Marine Safety Center, cap the Vapor Control line, and re-submit a new application for inspection for grade "D" cargoes and lower.

Submit 30 year mid-body survey report IAW 31.10-21a.

Make permanent repairs to bottom plate IWO #2 stbd void. Repairs to be completed to the satisfaction of the attending CG Marine Inspector.

Gauge bottom plate and provide gauging report to cognizant OCMI.

PRIOR TO USE OF THE CLOSED GAUGING SYSTEM, MAKE THE SYSTEM AVAILABLE FOR USCG TESTING/INSPECTION IN ACCORDANCE WITH THE USCG/MSC APPROVED PLAN.

PRIOR TO USE OF THE VAPOR RECOVERY SYSTEM, MAKE THE SYSTEM AVAILABLE FOR USCG TESTING/INSPECTION IN ACCORDANCE WITH THE USCG/MSC APPROVED PLAN.

RENEWED ENTIRE SIDE SHELL, TRANSOM AND RAKE SLOPE SHEET TOGETHER WITH ADJACENT STRUCTURAL SUPPORT MEMBERS. BOTTOM INSERTED IN #4P & #3S WING VOIDS. DECK INSERTED #1P,#2S,#3S,34P WING VOIDS.

REPLACE/REPAIR INSULATION TO STEAM HEADER AND PROP TUBE PIPING, TO INCLUDE FLANGES AND VALVES. ALSO HYDROSTATICLY TEST MAIN STEAM PIPING (ALL PIPING WITH A GREATER THAN 3" DIAMETER) TO 1 1/2 TIMES THE MAWP OF THE SYSTEM. COMPLETE PRIOR TO STEAM SYSTEM BEING USED BUT NOT LATER THAN 23OCT86.

MAKE PERMANENT REPAIRS TO THE EMERGENCY SHUTDOWN CABLE AT THE CARGO PUMP ENGINE. THIS ITEM IS TO BE COMPLETED PRIOR TO OPERATING THE PUP ENGINE OR THE DATE OF COMPLIANCE, WHICHEVER OCCURS FIRST.
