GENERAL II

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge, Marine Inspection (OCMI) on or after March 11, 1996, must comply with this part. Replacement of existing equipment, not including wire or cable, installed on the vessel prior to March 11, 1996 need not comply with the regulations in this part. Current generator configuration is not an approved installation and was installed without USCG oversight or notification. Provide formal written proposal for current generator configuration to the USCG for review, specifically on how you as the owner intend to meet the applicable regulations. Provide all specifications, data, and all relative information for currently installed generator on board. This requirement is in addition to previously issued 835. 46 CFR 183.115(c)

Auxiliary machinery shall be suitable in type and design for the purpose intended. Hydraulic equipment for vehicle ramp was not in working condition. Make necessary repairs. 46 CFR 182.10-1

Repairs or alterations to the hull, machinery or equipment that effect the safety of the vessel must not be made without the approval of the OCMI. This includes and not limited to replacement, repair, or refastening of deck or hull planking, plating, and structural members. It was explained to the owner that CG oversight is required throughout all processes of steel repairs including proposed scope of repairs. 46 CFR 176.700(a)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, and means of escape in case of a sudden unexpected casualty. A dry-dock examination of the vessel found serious safety and structural deficiencies including holes, cracks and corrosion beyond 25% of the main deck plating, cracked and corroded internal framing beyond 25%, loading ramp framing cracks and broken welds, wasted/broken steps on the accommodation stairways, wasted fire-main piping, wasted bilge piping, and other areas of concern noted in the following drydock worklist. 46 CFR 177.100

The Coast Guard, on its own initiative, may examine or inspect or re-inspect at any time any vessel subject to inspection under subtitle II, title 46 of the U.S. Code, title 46 and title 33 U.S. Code. If during such examination, inspection, or reinspection, any failure to comply with any applicable requirement of law and/or applicable regulations in this chapter, or any defects or imperfections become apparent tending to render the navigation of the vessel unsafe, or that repairs have become necessary, the Coast Guard will so notify the master and state what is required. Patterns of structural deflection were observed in centerline void numbers 06, 07, and 08 located transversely across the mid-body. This could be indicative of overloading the vessel which resulted in hogging of the hull. These issues if related, along with visible signs the vessel has been overloaded, bring into question the overall structural integrity of the vessel. These conditions warrant further examination which is to be carried out by an attending naval architect/professional engineer with sufficient background in steel structural repairs. The report of survey shall contain a full strength assessment of existing structure and direction for repairs. Upon completion of the survey, the owner is required to provide a report of survey from the naval architect to the Coast Guard. 46 CFR 2.01-10(b)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Marine inspectors identified excessive amounts of oily water in the engine room bilges, oil covered machinery, oil covered electrical equipment and batteries, and oil covered structural fire protection insulation. 46 CFR 176.830

Repairs must not be made without approval of [name withheld] observed incomplete and unsatisfactory repairs IWO engine room overhead plating including slug welds, cracks, improper fit-up, incomplete back gouging and weld penetration. Remove all remaining overhead insulation for MI’s to inspect and determine full scope of repairs in this area. After the scope is determined by CG MI’s, all engine room overhead plating shall be repaired to satisfactory condition IAW recognized standard. 46 CFR 176.700(a) 46 CFR 610.(b)

Batteries shall be secured against shifting with the motion of the boat. Spring clips or other temporary type clamps are not permitted. Batteries in the engine room were found stacked, not enclosed, and unauthorized terminal connections installed. Additionally, wires at the battery box were found covered in oil and other petroleum products from lack of cleanliness in E/R. 46 CFR 183.05-20(b) 46 CFR 183.05-20(c) 46 CFR 183.05-20(d)

All electrical installations shall be to the satisfaction of the Officer in Charge of Marine inspections. Multiple electrical connections and wires were exposed in the E/R and not secured. Additionally numerous dead end wires were found throughout the engineering space. Secure all exposed wire connections within the E/R. All dead end wires must be properly capped w/ approved devices or removed. 46 CFR 183.01-15(a)

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the supply lines. The shutoff valve at the tank shall be operable from outside the tank compartment, preferably from an accesible position on the weather deck. MI's observed that the portside emergency fuel shutoff needed to be re-routed and properly connected. Demonstrate proper operation to MI's at follow up inspection. 46 CFR 182.20-40(b)(3)

The bilge suctions shall be fitted with suitable strainers having an open area not less than three time the area of the bilge pipe. Multiple bilge suction pipes were fitted with strainer types that were not authorized in accordance with approved plans approval. Replace bilge suction strainers with approved type. 46 CFR 182.25-5(b)

Repairs must not be made without approval of [name withheld] observed heavy corrosion in addition to multiple repairs on fire main piping located in engine room. Replace section of vital system piping IWO corrosion and previous repairs; re-secure fire main piping with bracket. 46 CFR 176.700(a)

Vessel must comply with ABS Steel hull operating in protected waters. Vessel's port fire station piping is wasted beyond allowable limits. 46CFR 177.300(e)

Each ring life buoy must be stowed in a way that it can be rapidly cast loose. Ring life buoy station does not meet the requirements. 46CFR 180.70(b)(3)

Machinery and equipment must be installed in such a manner to afford adequate protection. Starboard shaft seal displayed excessive leaks. 46CFR 182.100

At each inspection operator must demonstrate satisfactory inspection of all railings. Railing on bridge has broken concourses. 46CFR 176.802(a)(4)

On a vessel more than 65ft, the public address system must be audible during normal operations. PA system not audible on car deck. 46CFR 184.610(b)

Piping exceeding 150 degrees must be suitably insulated. Starboard engine exhaust had exposed piping. 46CFR 177.970

A vessel must carry an approved first aid kit. Vessels first aid kit was expired. 46CFR 184.710

Vessel must comply with 33CFR 83 in regards to appropriate navigation lights. Vessel's navigation lights were not visible at time of inspection. 33CFR 83.20

Gauges to indicate lube oil pressure must be present at operating station. Engine oil pressure gauge on bridge was not operational. 46CFR 182.410(b)

The inspection of maintenance of equipment of this section shall be logged for review by the Coast Guard. Vessel could not provide logged equipment inspections. 46CFR 185.702(d)

Materials and arrangements not covered standards in this subchapter may be inspected IAW with standards acceptable to the OCMI as good marine practice. Signage throughout the vessel is faded and not legible to passengers. Signage is imperative to passenger safety. 46CFR 176.800(a)

As appropriate a vessel must carry adequate and up to date publications. Vessel publications were not up to date at the time of inspection. 46CFR 184.420(a)

Materials and arrangements not covered standards in this subchapter may be inspected IAW with standards acceptable to the OCMI as good marine practices. Vessel had a battery on the deck of an escape route. 46CFR 176.800(a)

On a vessel more than 65ft each hose must conform to UL 19. Port Fire hose has damage that rendered it not suitable for use. 46CFR 181.320(b)(1)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Marine inspectors observed vessel had excessive oily water within the engine room bilge.

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. The ground tackle and mooring lines provided must be satisfactory for the size of the vessel, the waters on which the vessel operates, subject to the approval of the cognizant [name withheld] lowering the cargo ramp to offload vehicles, the starboard cable parted and the shock-load sheered the shackle that secures the port side cable to the vessel causing the ramp to fall downward 180°.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Adult and child lifejackets were stowed together on the stern under the stairs with no separation.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its certificate of inspection. Drydock examination MISLE activity #7043082 dated 13AUG2020 required ultrasonic gauging. The gauging report showed two areas of the vessel car deck under the house that were at 25% wastage. Both areas are located at the center and port sides of the car deck under the house, forward of the aft bulkhead approximately 20'. The listed car deck areas must be cropped and renewed prior to receiving credit for the next drydock not later than 30JUN2021 in accordance with the COI.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its certificate of inspection. A drydock examination of your vessel found serious safety and structural deficiencies, including fire main piping, bilge piping systems, internal framing, hull plating, deck house bulkheads, car deck plating, bow ramp structural components, hydraulic systems piping, railings, stairways and other vessel parts with excessive wastage as noted in the vessel work list with 100+ items documented in MISLE activity 7043082 and provided to the vessel owner. These deficiencies demonstrate the vessel owner/operator has not proactively managed the vessel's safety, construction, arrangements, and overall operations. This vessel is detained until the owner/operator rectifies all deficiencies and demonstrates proactive safety management to the OCMI.

Emergency fuel shutoff valve cable frayed and broken. Replace cable.

Fire pump is inoperable. Prove proper operation of pump and 60 psi pressure at the gauge.

Fuel lines on main engines found worn, wasted and leaking. Replace all fuel lines.

Replace engine room exhaust lagging at both engines.

Re-stencil all life rings.

Replace gasket on E/R door.

Replace bathroom light and bathroom exit handle.

Public address system for passenger safety orientations inoperable. Demonstrate proper operation.

Replace damaged life ring.

Replace wasted oil pressure gause sending line on engine.

Vessel did not undergo an annual inspection within the three months before or after each anniversary date.

Make satisfactory repairs/replacement of the vessel's ship service generator. Vessel's ship service generator overheated after a cooling line failed. All repairs/installations/tests must be witnessed by attending Coast Guard Marine Inspector prior to conducting cargo or passenger operations.

Make permanent repairs to coolant leaks on prime mover of ship's service generator.

Make permanent repairs to leaking hydraulic rams for vehicle ramp or provide invoice for replacment rams.

Hard wire power supply for engine room blower fan.

Replace damaged expanded steel on ramp.

Replace upper deck rail courses.

Repair holes above bridge windows.

Mark vehicle lanes on main deck.

Show proper operation of bilge remote operated valve forward of collision bulkhead.

Re-install port main diesel engine PTO machinery guard.

Provide record of monthly portable fire extinguisher inspections.

Provide spanner wrench at each fire hose station.

Show proper operation of main deck lifebuoy water lights.

Re-mark vessel name on life buoys.

Provide covers for batteries in center of engine room.

Provide installed means of charging generator start batteries.

Provide junction box cover where missing inboard of generator.

Provide 2 hose clamps on each side of starboard main diesel engine cooling hose.

Prior to carrying passengers or cargo demonstrate proper operation of fire pump.

Prior to carrying flammable or combustible cargos provide approved type V semi-portable fire extinguisher.

Remove excessive oil and water from engine room bilge.

Pins on the ramp door were not secured while underway. Ramp could not be completely closed.

Fire station piping on starboard side was cracked and hoses on both fire stations had holes in them.

Vehicles were butted up against the fire hydrant and hoses causing damage to the fire main and making the fire station not ready for immediate use.

Prove proper operation of secondary power driven bilge pump.

Replace port side ring buoy.

Replace fan belt guard on generator.

Provide approved mounting bracket for fire extinguisher in port side engine room companionway.

Paint lane lines on vehicle deck.

Remove pollution threat caused by leaking and pooled hydraulic fluid on main deck and ingine room bilge.

Complete all deck insert welding and make available for inspection.

The vessel was issued a CG-835 requiring installation of a new vehicle ramp by 16 Dec 2016. The company failed to make this replacement.

Make permanent repairs to cracks on underside of vehicle ramp.

Make permanent repairs to outboard frame on the port side of vehicle ramp.

Submit plans for approval for replacement ramp.

Replace vessel's vehicle ramp.

Make permanent repairs to cracks and wastage on vehicle ramp at next dry docking.

Make permanent repairs to prevent steering linkage bar from chaffing on deck plating.

Provide (01) type V semi portable fire extinguisher. Vessel restricted from carriage of flammable or combustable liquids in portable tanks or tank trucks until cleared.

Vessel was not made available for inspection within 3 months of the COI anniversary date. Previous COI initial date was 21Jan2015.
