The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. An Internal Structural Exam was conducted for purpose of drydock extension. During the inspection, sections of bow ramp were noted with wastage. Vsl rep provided bill of materials for replacing ramp in kind. Vsl rep proposed continuation of operations without loaded trucks. MI made recommendation to OCMI to accept proposal until drydock extension due date of 31Dec2025. In consideration with the overall satisfactory material condition of the vessel, [name withheld] may continue operations but without loaded trucks of any kind; including dump trucks, fuel trucks, garbage trucks, cargo trucks, etc. until 31Dec2025. Owner is responsible for ensuring the safe operation of the vessel.

The following spaces must be equipped with a fixed gas fire extinguishing system, in compliance with § 181.410, or other fixed fire extinguishing system specifically approved by the Commandant, except as otherwise allowed by paragraph (b) of this section:(1) A space containing propulsion machinery; CO2 bottle in E/R was free standing and was not secured with bracket. 181.400(a)(1)

A hinged watertight bulkhead door must be fitted with a quick action closing devise operable from both sides of the door and indicator lights at the operating station showing whether the door is open or closed. Quick closing device did not seal WTD due to gasket alignment.46 CFR 179.330(b)

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. Two employees did not have current enrollment dates listed in APCA. 46 CFR 16.201

A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. 01 Child PFD needs replace.

Boundaries that separate machinery spaces from accommodation spaces, service spaces, and control spaces must be lined with noncombustible panels or insulation approved in accordance with § 164.009 in subchapter Q of this chapter. Missing portions of overhead insulation in E/R overhead. Replace missing portions of insulation during upcoming drydock.

A hatch exposed to the weather must be watertight. Self reported hatch on main deck was cracked. Replace during upcoming drydock.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. Safety cable on port bow was frayed. Replace at upcoming drydock.

Fire pump outlet must be fitted with a pressure gauge. New gauge was installed but did display as designed. Replace at upcoming drydock.

Electrical equipment on a vessel must be installed and maintained to:(a) Provide services necessary for safety under normal and emergency conditions;(b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock;(c) Minimize accidental personnel contact with energized parts; and(d) Prevent electrical ignition of flammable vapors. Wiring below operating station was not properly installed IWO batter box. Make all necessary corrections to prevent potential safety hazards.

A visual and audible alarm must be provided at the operating station to indicate a high water level. Bilge alarm panel was found disconnected. Prove proper operation of all bilge alarms.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Crew was not proficient with installed CO2 system. Conducted training and system component labeling was installed.

Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. (i) You must contact the cognizant OCMI to schedule an inspection at a time and place which he or she approves. No written application is required. MI reached out to company as courtesy to request annual inspetion prior to expiration. COI lapsed prior to inspection date. Conduct satisfactory COI prior to issuance of COI. 46 CFR 176.500 (b)(i)

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection, the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Objective evidence indicators, including deficiencies #02 through #05, demonstrate substandard conditions that are not being proactively managed by the vessel owner/ operator. 46 CFR 176.25-50

At each initial and subsequent inspection for certification, the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition and fit for the service for which they are intended: Fuel systems. Tanks, tank vents and other appurtenances, piping, and pipe fittings shall be checked. The fuel systems for the main propulsion engines, all auxiliaries and all other fuel systems installed shall be checked. All valves in the fuel lines shall be tested by operation locally and at remote operating positions. After starting #1 SSDG, significant diesel leak observed from the fuel water separator. 46 CFR 176.25-10

At each initial and subsequent inspection for certification, the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition and fit for the service for which they are intended: Fuel systems. Tanks, tank vents and other appurtenances, piping, and pipe fittings shall be checked. The fuel systems for the main propulsion engines, all auxiliaries and all other fuel systems installed shall be checked. All valves in the fuel lines shall be tested by operation locally and at remote operating positions. During operational test of starboard fuel tank remote emergency fuel shutoff valve, the extension rod sheared due to corrosion, preventing the valve from being remotely operated. 46 CFR 176.25-10

At each initial and subsequent inspection for certification, the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition, and fit for the service for which they are intended: Electrical cable. The cable shall be examined as far as practicable without undue disturbance of the cable or electrical apparatus. #1 main engine starting battery charger inoperable. Observed a portable battery charger attached to the battery starting circuit with alligator clips to open ended wires. 46 CFR 176.25-15

At each initial and subsequent inspection or certification and reinspection the firefighting equipment of all vessels shall be inspected and all deficiencies found shall be corrected. The marine inspector shall conduct the following examinations and tests: The fire pump shall be tested and the fire hose shall be subjected to the available fire pump pressure. During the fire pump test, observed significant leaks from the electrical fire pump and the pressure gauge displayed 50 psi. In accordance with 46 CFR 181.10-1(c), the minimum pressure requirement shall not be less than 60 psi. 46 CFR 176.25-25 46 CFR 181.10-1(c)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. High water alarm indicator at the operating station for the engine room was inoperable during testing. 46 CFR 182.530(a)(2)

A voltmeter and an ammeter shall be provided that can be used for measuring voltage and current of each generator that is in operation.For alternating-current generators a means for measuring frequency shall also be provided. Additional control equipment and measuring instruments shall be provided as necessary to insure satisfactory operation of the generator or generators. #1 SSDG gauges inoperable. 46 CFR 183.10-5(g)

At each initial and subsequent inspection for certification, the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition and fit for the service for which they are intended:Auxiliary machinery. All machinery essential to the routine operation of the vessel shall be checked. #2 SSDG air filter is kept in place with a ratchet strap. #1 SSDG air filter is missing. 46 CFR 176.25-10

At each initial and subsequent inspection for certification and reinspection the inspector shall examine and test all items in the ship's outfit, such as ground tackle, navigation lights, compass etc., which are required to be carried by the regulations in this subchapter. Such examination and tests shall be to the extent necessary to determine that the condition of the item is satisfactory and that it is fit for the service intended. Vehicle loading ramp shackles are corroded. Remove shackles and descale for reinspection or replace with shackles rated for service of the loading ramp. 46 CFR 176.25-40

The Rules concerning lights (§§ 83.20 through 83.31) shall be complied with from sunset to sunrise, and during such times no other lights shall be exhibited, except such lights as cannot be mistaken for the lights specified in these Rules or do not impair their visibility or distinctive character, or interfere with the keeping of a proper lookout. Masthead light inoperable. Vessel restricted to daytime operations only until masthead light is operational. 33 CFR 83.20(b)

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. On systems in which the CO2 cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methodsof discharging the system should the manual release or stop valve controls fail to operate. Each control valve to branch lines must be marked to indicate the related space served. Main deck CO2 control labels are illegible. 46 CFR 181.20-15(d)

All life preservers and ring life buoys shall be marked with the vessel's name. All 3 lifebuoy markings are faded or missing. Mark all 3 lifebuoys with vessel name for reinspection. 46 CFR 185.30-5

Marine inspectors noted the port and starboard rudder post bearings were last replaced in December 2016. MIs noted excessive play in the rudder posts during exam. Remove rudder posts for inspection. Provide repair proposal if the rudder assemblies will be modified in order to accommodate rudder post removal.

Crop and renew wasted 3 ft section of bilge piping in fuel tank void

Forepeak & #2 void (adjacent) main deck connections of longitudinal angle supports and transverse flat bar supports improperly welded and/or missing welds. Correct in accordance with approved welding standard.

Remove standing water from E/R in way of shaft seals to prepare for inspection.

Descale port and starboard exhaust flanges to prepare for inspection.

Remove port and starboard exhaust lagging for inspection due to exhaust leak repair. Verify leak correction at sea trials.

Prepare seachest gate valve for inspection.

Widespread wastage observed on the vehicle ramp frame vertical and horizontal square tubing. Provide repair proposal for the ramp.

In general, compliance with the standards established by a recognized classification society will be considered satisfactory evidence of the structural adequacy of a vessel. 02 overboard pipe penetrations were observed to be wasted with holes in the starboard aft void behind the house. Make repairs to wasted piping areas to the satisfaction of USCG.

Make necessary repairs to port and starboard props.

Recommend replacing wasted zincs

Heavy scaling was observed IWO stbd quarter deck overhang. Remove rust and make available for inspection. Repair as necessary.

Heavy scaling was observed IWO stbd midship angle iron overhang x2. Remove scaling and make available for inspection. Make necessary repairs.

Cleats on main deck were found wasted and corroded. Replace as necessary.

Bulwarks on stbd side were found with holes from anchor brackets that were previously mounted. Make necessary repairs. This includes the area IWO drainage opening on stbd side.

Hinges on stbd side main deck passenger access were found corroded. Replace hinges on stbd side main deck access.

Clean and make necessary repairs to main deck bilge connection, port side.

Steering compartment: Remove hydraulic fluid from bilge. Find source of hydraulic leak and make necessary repairs. Remove all scaling and rust within this compartment and make available for inspection. Provide repair proposal plan for wasted metal.

During the USCG initiated fire drill, crew was not familiar with their duties and responsibilities in the event of a fire and the procedures for activation of the fixed C02 system and the crew was not familiar with their duties in regards to abandoning ship procedures.

Ships port side life ring was faded. All life rings require the correct amount of SOLAS tape and need to be re stenciled with appropriate markings

Ships port side fire hose had numerous holes. Ship must provide adequate replacement.

Ship must provide instructions for operation of the fixed gas fire extinguishing system at the release station. Instruction placard was not legible.

Engine room watertight door markings are not legible. Engine room doors make up the boundary for a CO2 protected space and must be kept closed while the vessel is transiting.

Storage space door for fixed C02 system was blocked by buckets operational waste and the door could not be opened outwards. Space was also being used to store shipboard equipment.

Life jacket stowage containers need to be appropriately marked with the number and type of PFD.

Child life jackets shall be separated from adult life jackets in the event not to confuse proper utilization during an emergency.

Ship must ensure all navigation lights are in compliance with appropriate UL1104 standards for vessels over 65ft. All navigation lights were not operational. *****Vessel is restircted to daytime operations only until proper navigation lights are installed.*****

Lighting fixture in the E/R did not have a cover. Electrical box in aft void space did not have a cover.

Ships port engine had an exposed loom of electrical wire subject to mechanical abuse that needs an appropriate cover.

Vessels EPIRB battery expiration date is due within 14 days.

Provide a speaker on the rear of the vessel or other means to properly deliver the passenger safety orientation before every trip and so the PA system can be heard at the rear of the vessel.

Ships emergency fuel shut-off valves were painted over and not operable and not stenciled with the purpose and direction of operation.

Replace all wasted void cover plates and gaskets and ensure watertight hatches on the weather deck are attached to the boat by a chain or other means.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Bilge suction did not work in the fuel room or lazarette.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces The vessels bilge high level alarms were not mounted at the helm and wires were loose and not working properly.

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. Bridge navigational equipment was shutting down intermittently and disrupting the operation of the radios, AIS and rudder angle indicator.

A vessel on a limited coastwise route must carry Six hand red flare distress signals approved in accordance with §160.021 in subchapter Q and Six hand orange smoke distress signals approved in accordance with §160.037 in subchapter Q. Remove unapproved flares from the vessel and provide six (6) additional approved distress flares.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Remove excess oil from the vessel bilges.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” Prove proper operation of stern light and remove all unapproved bulbs and replace with UL1104 bulbs.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible, U.S. Coast Pilot or similar publication, Coast Guard Light List, and Tide tables. Provide all navigation publications.

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the Passenger safety orientation. Provide a means for passengers on the rear portion of the vessel to clearly hear the safety orientation.

A vessel of 12 meters or more in length shall be provided with a whistle.

Spanner wrenches must be provided where a firehose is required. Provide spanner wrenches for all fire hydrants.

Provide four (4) adult PFDs. COI restricted to 145 passengers until new PFDs provided.

CO2 system was found to be out of annual service and CO2 bottles were out of hydrostatic testing date.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including inspection and operation of all weathertight closures above the weather deck. Forward watertight door to engine room was hit by a vehicle and bent and was found latched open by rope and could not be closed. Aft watertight door was open and was blocked by a 55gallon oil drum preventing its closure.

Each life jacket kept in a stowage container must be readily available. Life jacket stowage box behind the house was latched down with line and not ready for immediate deployment. Life jacket box forward of the house was not secured to the deck and could cause injury to passengers.

Port main engine reduction gear overheated rendering the port shaft inoperable. Complete repairs/replacement of reduction gear and demonstrate successful sea trials prior to the carriage of passengers, vehicles or cargo.

Running lights inoperable and bulbs not UL 1104 listed. Replace all bulbs with UL 1104 kind. Prove proper operation of all running lights prior to operating at night.

Compass light inoperable. Repair or replace prior to operating at night.

AIS not transmitting static data. Prove proper operation.

Cooling water temperature and lube oil pressure guages not operable from the operating station.

Radar inoperable. Prove proper operation.

FCC Station License expired in 2018.

Make DAPI program documents available for inspection.

Provide current logbook entries for Fire/MOB drill, and monthly EPIRB testing.

Provide emergency broadcast placard at the operating station.

Provide current CO2 fixed firefighting servicing report.

Emergency generator room had 6 inches of standing water, batteries located on the deck, and unsafe wiring conditions. Make emergency generator room safe for inspection.

Port and starboard engine room ventilation closures found warped and wasted. Vent closures will not close properly and are not water tight. Make proper repairs.

Show proper operation of all remote fuel oil valves in fuel oil space. Port cross connect valve inoperable and port fuel tank valve reach rod wasted.

Crop and renew damaged gunwale in way of starboard side aft.

Demonstrate proper operation of high bilge alarm in water tank room.

Provide guards for aft generator.

Remove portable welder from engine room.

Repair excessive leaks from expansion tanks for port main diesel engine and generators.

Remove dead end wires in way of aft circuit breaker panel.

Repair excessive leaking on aft bilge pump.

Provide complete required first aid kit.

Remove sharp edges on anchor on inboard of starboard bulwarks.

Provide FCC radiotelephony and bridge to bridge readio telephony certificates.

Provide visual indication at operation station to indicate when any automatic bilge pump is operating.

Prove proper operation of engine instrumentation at operating station.

Prove proper operation of fire pump pressure gauge.

Remove excess rust, inspect and paint noted area of port side fuel tank on aft bulkhead at bottom seam.

Hydrostatically test (04) cO2 storage tanks.

Provide fire service report.

Engine room hydrant not installed. Not valve to secure fire main.

Provide door knob on 2nd deck door.

Provide flame bowl on starboard side main engine fuel filter.

Remove excessive buckets of oil from engine room.

Remove batteries not in service from vessel.

Remove or properly wire bilge pump in aft machinery space.

Prove proper operation of power interlock for shore power/generators.

Replace holed fire hose in engine room.

Repair or replace leaking fire hydrant on main deck port side, aft.

Properly secure EPIRB hydrostatic release cover to vessel.

Remove all exhaust piping insulation. Make all insulation and exhaust piping available for inspection. Renew or replace saturated insulation.

Make engine room ready for inspection. Remove all oily residues as well as all exhaust soot from all surfaces. Remove oily water from bilges.

Replace cable thimble on port side ramp cable.

Renew all deck plate and hatch opening seals, make watertight.

Ensure all deck plates and hatches are properly secured with appropriate hardware.

Crop and renew with a minimum insert of 18" x 18" marked hole in deck, starboard side.

Stop drill and weld all marked cracks in aft void spaces forward of lazarette. Crop and renew marked area of wastage.

Provide check valve for bilge piping to starboard side void space two spaces forward of lazarette.

Provide working door knobs for both main deck doors as well as 2nd deck door.

Provide window for starboard side main deck door.

Properly support and arrange fire main piping to ensure no chaffing between fire main piping and other piping. Crop and renew wasted/chaffed piping.

Identify all exhaust leaks, provide a repair proposal for leaking exhasut piping.

Hydraulice steering lines found to be leaking in way of engine room overhead above port side main engine.

Prove proper operation of bilge high water alarms.

Investigate and correct flooding through bilge piping into lazarette.

Vessel limited to carrying vehicles weighing less than 5 tons due to wasted ramp.

Submit plans for ramp replacement.

Complete ramp repairs.

Make air receivers available for CG inspection

Make crew available for drills.

Replace missing hinge pins on cargo ramp.
