Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. The electrical enclosure for the toilet pumps in both heads are not properly secured and are just sitting on the deck. Additonally, the deck penetration stuffing tube in the aft head where the wire enters the engine room is not properly secured which would allow the passage of flames or smoke in the event of a fire in the engine room. 46 CFR 183.220

The following spaces must be equipped with a fire detection and alarm system of an approved type installed in accordance with 46 CFR part 76; (1) A space containing propulsion machinery. Port engine room heat detector did not provide an audible alarm at the operating station when tested. 46 CFR 181.405(a)(1)

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. Aft MDE's on port and starboard sides did not have insulation around the turbos which exceeded temperatures of 150 °F. 46 CFR 177.970

Each survival craft must be Stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. Inflatable liferaft sea painters were not properly connected to the hydrostatice release units. 46 CFR 180.130

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Expired items were found inside the first aid kit. 46 CFR 184.710

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Starboard inboard jet drive shaft was missing 1 guard and both were found missing on the starboard jet drive shafts. 46 CFR 177.960

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Starboard engine room had excessive accumulation of oil near the aft MDE and the starboard engine room had excessive amounts of coolant in the bilge. Oil absorbent pads were found throughout the bilges. Make repairs to all oil and coolant leaks, remove debris and conduct satisfactory power trials to the satisfaction of attending Marine Inspector. 46 CFR 176.830

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Starboard forward MDE coolant expansion tank was zip tied above the engine. 46 CFR 182.100

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions. Starboard forward engine room light was constantly flickering. 46 CFR 183.200

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions. A junction box with missing cover plate was located in the bilge in starboard aft engine room. 46 CFR 182.200

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Bridge life jacket storage area had 4 listed but only two were present. The port forward storage container was not marked. 46 CFR 185.604(f)

The name of the vessel must be marked or painted in clearly legible letters on each EPIRB. Vessels EPIRB was missing the registration sticker. O/O Rep provided the registration form but the vessels name was not marked on the EPIRB. 46 CFR 185.60(C)

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Emergency fuel shut off cables are ran through a hole in the overhead which is causing corrosion from dissimilar metals and chaffing the cable. 46 CFR 176.800

Found 4 cracked railings. 2 on top deck, 1 on stairwell, and 1 on swim platform. Make repairs to cracks.

Provide repair proposal for damaged area on starboard side outboard bottome plate. Area of damage is below fuel tank, approximately 6' by 6" from likely grounding.

Complete fire drill, man overboard and sea trials to the satisfactory of an attending CG Marine Inspector prior to carriage of passengers.

Provide current FCC Safety Radio Telephony certificate.

Install missing bilge check valve and strainer in #2 port void space.

Install missing bilge check valve and strainer in #2 starboard void space.

Repair fuel leak on fuel fill piping in port side engine room overhead in way of coupling just forward of elblow in the aft section of engine room.

Starboard side bilge has excessive oil accumulation. Remove all standing oil from bilge. 701 (c)

Starboard aft (#3) MDE has an excessive oil leak from the forward section of the engine. Make proper repairs to correct leakage 701(c).

Identified the magnetic compass was nearly void of fluid. Refill or renew compass.

Secure sewage system valve to prevent ovbd discharge when in U.S. Territorial waters.

#4 Main diesel engine suffered a thrown rod through the crank case. Renew/repair engine.

Conduct drills in Spring 2018.

Make AC raw water cooling line bulkhead penetration valve operable from the bulkhead deck or provide 1 valve on either side of the bulkhead.

Repair and prove proper operation of the port and stbd engine room high level alarms.

Conduct all tests examinations required to be conducted when the vessel is in the water.

Identified an AC saltwater supply hose wich has excessive breaks and cuts in the exterior covering in the port engine room. Renew with approved hose and fittings.

Renew or remove fallen insulation in the #3 void of both hulls.

Simplify the generator saltwater supply piping in the starboard engine room. Remove unused splices and utilize approved hoses and connections.

Starboard main and generator remote fuel oil valve failed to operate.

Replace ring buoy located by wheelhouse. 46CFR180.70

Conduct drill (MOB, fire, flooding, abandon ship). 46 CFR 185.520

The fuel pump coupling for the #1 starboard main engine broke.

Replace #1 MDE. At this time it appears that a rod has broken and put a hole in the block. Vessel may operate on 3 MDE's while operating in the Sitka area only.

Grind out and reweld broken weld for external engine exhaust box located inboard on the stbd hull.

Crop and renew port corner of transom where side shell is dented in and stern is "S" curved. area approx. 12-18 inches.

Make repairs to aft deck stair access to upper deck on port side where portable railing stanchion socket is torn away from the deck.

Make repairs to deck at the entrance to the stbd #2 void where previous damage has caused a small crack in the deck plate.

Pick up missing welds in water tight bulkhead where missing in port fuel void.

Drill stop, V, & reweld the 1 inch crack located in the aft bulkhead of the stbd engine room outboard of the shaft coupling.

Pick up weld at top of longitudinal plate stiffener located on top of the tunnel in the portside void forward of the collision bulkhead where "suck back" created small hole in plate

Renew gasket on raw water filter in port jet room

Repair weather-tight doors for both port and starboard engine rooms.

Replace missing dead cover in pilothouse 120/240 AC panel.

Remove 14/2 dead ended wires in engine rooms that used to provide power to old fire alarms. Or, terminate properly and place in "J" box.

Provide retention for wire bundle at forward end of port engine room where two retention devices have failed.

Provide watertight hatches in main deck in lieu of increased coamings in weathertight doors as discussed in MSO Juneau waiver letter of 2004

Drill stop, V & weld the crack located at the top stbd corner of the front main deck windows.

Replace missing bolts that hold in the widows on the main deck. More are missing on the stbd side than port.

Remove vent for Marine Sanitation Device where it is connected to overboard discharge. Run vent properly up through deck and provide flame screen. As installed vent is designed to allow overflow to freely discharge overboard.

Replace missing cover to florescent light in pilot house.

Pick up welds on bilge pipe penetrations of water tight bulkheads where improperly welded from only one side. Especially noted is the penetration between the stbd engine room and fuel void for the lower bilge pipe.

Repair remote shut-off for port fuel tank.

Pick up welds where not double continuous for collision bulkheads in port and starboard hulls

Crop and renew pipe penetration from port jet room to engine room where one 2" hole was cut in wrong place and one 2" hole was cut, replaced and 1 & 1/2" hole was cut out of center for pipe penetration, then both were rewelded improperly.

Clad weld as appropriate, two areas of hull where hit or cut by unknown object displacing metal approx. 1/4" wide x 1/16" deep x 2' long in 3/16" hull plate.

Replace lower latches which are missing for both forward doors on main deck.

Pick up weld where missing in stbd engine room hull plate forward of frame on outboard side where marked.

In port engine room repair support for control cables to Mathers Controls. Examine and repair as necessary the cable which is partially pulled out of the control box.

Reweld side shell stiffener where displaced and welds are broken by inset hull plate outboard of the stbd fuel tank.

Replace three expired orange smoke floats.

Remove fuel from bilge in port fuel tank space. Determine source of fuel, examine tank for leaks/cracks and provide report of findings back to the Coast Guard.

Replace inoperable fire alarm sensor in port engine room. Existing vessel built in 1995 - system is not required. But installed system must be maintained if depended upon to accurately work by crew.

Renew inoperable stern light. Vessel not allowed to operate after dark until renewed.

Replace missing battery for new life ring lantern.

Repair Fuel leak at injector piping on top of forward cylinder of forward engine in port engine room.

When the ten person IBA gets installed notify MSO Juneau so that the installation can be inspected.
