Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces. Engine room is protected by a CO2 fire suppression system. The access hatch was found not dealing properly due to wasted rubber gasket. 46 CFR 116.330.

Each hatch exposed to the weather must be watertight. The following hatches were found with deteriorated rubber gaskets, allowing large mounts t flood the compartment: a. port side collision bulkhead hatch cover b. port side void 2 hatch cover. c. stbd side collision bulkhead hatch cover d. stbd side void 2 hatch cover. 46 CFR 174.220

All escape hatches and emergency exits used as a means of escape must be clearly marked on both sides. The text must read: "EMERGENCY EXIT, KEEP CLEAR". The lettering must be clearly legible and at least 2 inches. Both Port and stbd engine room forward excape hatches are not marked "EMERGENCY EXIT, KEEP CLEAR" in both sides. 46 CFR 185.30-15.

A-Class bulkheads or decks must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads, and decks. They must be so constructed that, if subjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. In addition, they must be so insulated with approved structural insulation, bulkhead panels, or deck covering so that, if subjected to the standard fire test for the applicable time period listed below, the average temperature on the unexposed side does not rise more than 139 °C (250 °F) above the original temperature, nor does the temperature at any one point, including any joint, rise more than 181 °C (325 °F) above the original temperature. The structural fire protection (fire insulation) was found either deteriorated or hanging in the following locations: a. under passenger stairways port side by engine room access, and next to the blower. b. under passenger stairways stbd side by engine room access and next to the blower. c. adjacent to the ventilation louvers, inside of the engine room port side, outer side. 46 CFR 116.415 (i) 46 CFR 72.05 46 CFR 72.05-20 46 CFR 72.05-10

All piping, valves, and fittings of ferrous materials shall be protected inside and outside against corrosion. The fire main piping located in the port side engine room was found to have installed at least 02 clamps with evidence of corrosion of a suspected leak. Remove the clamps and conduct permanent repairs to the fire main piping. 46 CFR 181.20-20(c).

Except as further modified, where fire extinguishing systems or equipment are not required but installed, the system or equipment and its installation shall meet the requirements of this part. The fire pump pressure gauge is not required, but found with a broken glass display. 46 CFR 181.05-5.

At each inspection for certification, the marine inspector shall view the vessel afloat and shall inspect the hull structure. Examine all accessible parts of the hull. Areas with deep pitting and corrosion were found, at both sides of the keel in the port side void number 2. These areas are deep enough to request the vessel to undergo repairs, to cut and renew the affected areas identified during the exam. 46 CFR 176.25-5 (a)(1). Prior Carrying Passengers.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Vessel master and crew lack of knowledge, lack of communication, on how to operate the main diesel engines override system after simulation of the CO2 fire suppression systems. Provide evidence of training for all crew and master in how to operate the emergency system, and carry out the testing again, with the same crew and master, in presence of an attending USCG Marine Inspector. 46 CFR 115.830

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. A hole penetration was found in both port and starboard forward ventilation rooms at the deck, adjacent to the ventilation fans. Rectify to the satisfaction of competent Marine Inspector. 46 CFR 116.415

Internal combustion engine exhausts, boiler and galley uptakes, and similar sources of ignition must be kept clear of and suitably insulated from combustible material. Engine Room is a Class 10 space, requiring A-60 fire protection in all its boundaries. Multiple areas as identified during the exam, and provided photographic evidence shows that there are areas with missing approved thermal insulation, in both port and starboard sides. Install missing fire retardant insulation as approved in the original vessel plans. 46 CFR 116.405 46 CFR 116.415

Connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable. All connecting devices must be suitable for copper stranded conductors. The connections must be made within an enclosure and the insulated cap of the connector must be secured. Dead end wires found under operating station. Rectify to the satisfaction of the attending Marine Inspector. 46 CFR 111.60-17

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of— Either: (i) The proper method of donning and adjusting life jackets of the type(s) carried on the vessel including a demonstration of the proper donning of a lifejacket, or (ii) That passengers may contact a crew member for a demonstration. During safety orientation, there was no demonstration of how to don a lifejacket. Rectify to the satisfaction of competent Marine Inspector. 46 CFR 122.506(a)(3)

For vessels other than those covered in paragraphs (b) and (c) of this section, the name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel name missing from bow, Port & Starboard. Add name to both Port & Starboard of bow. 46 CFR 67.123(a)

Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by noncombustible hangers or blocks. Exhaust leak present in starboard side engine room exhaust piping. Rectify to the satisfaction of the attending Marine Inspector. 46 CFR 119.430(b).

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural examination at least once every two years. The vessel has lapsed on its drydock inspection deadline. A drydock inspection to the satisfaction of the cognizant OCMI must be conducted prior to clearance of this deficiency. 46 CFR 115.600 (c)(1)

46 CFR 155.800 (a) A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Photo and video evidence of the welds to repair the damaged rub rail show impurities. Submit a repair proposal to [email withheld] for approval prior to conducting repairs. Repairs to be completed at next DD

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level in a machinery space bilge. Both high water level visual alarms for the port engine room did not properly go off. Repair to ensure audio and visual are operational. 46 CFR 119.530(a)(2) 16c

The training program must address firefighting proficiency and must include fire and smoke detection and automatic systems on board. Crew was unfamiliar with proper testing of fire and smoke detection systems, resulting in melting of the detectors. Familiarize crew with fire control plan and proper testing of detectors. 46 CFR 122.420(b)(1)(vi) 16c

The owner or managing operator shall provide satisfactory evidence of the required servicing to the marine inspector. No firefighting servicing reports were provided for the smoke and heat detectors. Provide the servicing reports to the Coast Guard. 46 CFR 115.810(b)(2) 16c

Each vessel must be equipped with at least two operable portable battery lights. One light was broken. Vessel requires two operable portable lights. 46 CFR 120.430 16c

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel reported a total loss of both port and starboard main diesel engines. 1. Vessel is allowed to return back to Ceiba for repairs without any passengers on board. 2. Prove proper operation of both port and starboard main diesel engines under load during sea trials with an attending USCG Marine Inspector. 3. Provide a written technical report of root cause analysis of what caused the loss of propulsion to the satisfaction of an attending USCG Marine Inspector. 46 CFR 119.200 17C

Each floating water light must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant. The water lights installed on board do not have the battery type required by the manufacturer. 46 CFR 117.70

At each inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. 01 deck, port side door leading to the aft outside seating area does not latch closed. 46 CFR 115.910

Repair port fuel tank main return line at point of leak. Pressure test to ensure repairs effective.

Remove all sea valves for inspection.

Replace zincs on sea chests.

Make repairs to the Starboard Hull engineroom hull forward of the main engine. A 12-14" crack was reported allowing water to compromise the watertight envelope of the starboard hull. Make repairs to the temporary reparis in the port forward void. Also weld the missing welds in the internal structure in the port fwd void.

Vessel has a 12' hairline fracture next to the sea chest on the stbd side engine room. 46cfr116.100(a)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Vessel STBD side aft corner has a fracture around 1" in length.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port side Jet lost communication with engine. Provide a tech report attesting to the satisfactory working condition of the engine for the approval of the attending marine inspector.

A door in a bulkhead required to be A-60, A-30, or A-15 Class must be of hollow steel or equivalent material construction, solidly filled with approved structural insulation, and capable of meeting the requirements of an A-15 Class bulkhead; Both engine room doors had piercing penetrations around 1/2" diameter compromising fire boundary.Deficiency has to be corrected at DDX.

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. Loose cables on port and stbd engine room must be secure.

The vessel Port MDE failed which resulted in trouble shooting and a top-half over hall

Submit a repair proposal addressing the following deficiencies found in the following locations: a. Damage found (2'5" long x 7" high x 2" deep) in both aluminum side shell and bottom plating at the shine, port side-outside of the engine room. b. Unapproved repair of two plug welds identified between frames 3 and 4, and 1.5" from the waterline, port side. c. Vessel owner/operator must follow the mitigation proposal submitted to USCG Sector San Juan. 46 CFR 116.100

Vessel has a 2" by 1/8" hairline crack in the hull on the port side inboard engine room approx 18" above the water line.

Capt of vessel noticed RPM reduction and white smoke on port side exhaust from manin engine. Provide the following to the attending marine inspector: -Repair Plan -Diesel tank report -Test report of fuel

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. The vessel rep stated that there would be a design change to the internal frames in the Port and Starboard voids. Also in the starboard #2 void a design change was discussed to repair the fractures in the crotch of the wet deck under frames 11-15.

A-Class bulkheads or decks must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads, and decks. In addition, they must be so insulated with approved structural insulation. In the starboard engine room, forward bulkhead found the structural insulation missing around the raw water heated exchanger mounting brackets.

At each inspection for certification and at every other vessel inspection all observed unsafe practices and other hazardous situations must be corrected. Starboard transom, in board of the water jet, found an unused plastic open stuffing tube penetrating the hull.

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. a. Starboard engine room, inboard, found cable penetration that was not completely sealed b. Port engine room, forward bulkhead near the overhead, found an unused plastic electrical cable stuffing tube penetrating the bulkhead.

Vessels to which this subchapter applies must meet the applicable provisions in subchapter S (Subdivision and Stability) of this chapter, except that the requirements in subpart K of this part may be met in lieu of the requirements of §§ 171.124 through 171.155 in subchapter S of this chapter. Repair plans 0183-114-v01 and 183-118-v00 modified internal framing in P/S #2 and #3 voids. Submit documentation to the Marine Safety Center addressing the change in the vessel lightship characteristics; TECHNICAL NOTE (MTN) NO. 04-95, CH2.

The shapes specified in these Rules shall comply with the provisions of Annex I of these Rules (33 CFR part 84.06. Vessel missing two balls and a diamond day shapes.

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship and that each person handling garbage follows the plan. The West Marine Placard on the bridge did not meet all the elements for the garbage management plan.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Mark the top deck with maximum capacity (75 passengers) at the entrance of that deck. Mark entrance of second deck Maximum Passenger capacity of 195 on upper two decks.

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. The Certificate of Inspection was not posted.

When, in accordance with § 170.120 in subchapter S of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. The Stability Letter was not posted.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] repair of welds as marked in #3 Port Void, #2 Starboard void and #3 Void.

An Emergency Instruction placard containing the following information will stratify the requirements of 46 CFR 122.510 or this part. No Emergency Instructions on board.

Gauges to indicate engine revolutions per minute (RPM), jacket water temperature, and lubrication oil pressure must be provided for all engines installed in the vessel. The gauges must be readable at the operating station The RPM, jacket water temp. and oil pressure gauges or the STBD electronic display were not operational. Crew to take local readings every 15 minutes until the electronic display is operational.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port side turbocharge for MDE is not operational.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port side jacket water cooling system is not operational.

AIS Carriage - a self propelled vessel that is certified to carry more than 150 passengers. The AIS is not working correctly.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Capt of vessel notice RPM reduction and exercise smoke on port side main engine. Provide the following to the attending marine inspection: - Provide repair plan. - Sound Diesel tank report. - Provide a test report of the fuel.

Vessel under went modification, a dead weight was conducted and stability calcs have been submitted to MSC. Prior to operating with passengers, the vessel must poses a valid stability letter.

Vessel will be operating in Sector Puerto Rico's AOR conduct a NTZ and demonstrate proper drills.

Repairs or alterations to the hull that affect the safety of the vessel must not be made without the approval of the OCMI. The owner or operator shall submit drawings describing the details of any proposed alterations to the [name withheld] alterations must be approved before the work is started. Vessel must submit drawings for approval and prove proper installation of alteration to the satisfaction of a Coast Guard Marine Inspector prior to the carriage of passengers.

A Vessel must be provided with a satisfactory arrangement for draining any watertight compartment under all practicable conditions. Prove function of bilge pumping system. 46 CFR 119.500

Non-metallic hoses must be in accordance with SAE-J1942. Prove compliance of flexible connections in bilge piping system. 46 CFR 119.720

At every inspection, spaces may be examined to determine if they are in a sanitary condition. Sewage is leaking out of the tank in the engine room. 46 CFR 115.818

Machinery, equipment, material, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practices. Fuel return lines must be cleaned of leaking fuel and joints resealed. 46 CFR 115.800

A life jacket must be provided on board for every adult passenger. Vessel is short 27 Life jackets and will be limited to 350 total persons on board. 46 CFR 117.71

A device must be provided to automatically shut down engines that draw intake air from a protected space prior to the release of extinguishing agent into the space. Prove function of engine shutdowns for fire suppression system. 46 CFR 118.410 (b)(10)

A vessel with more than one deck must have a public address system from the control station. Vessel must prove operation of PA system. 46 CFR 121.610(c)

Machinery, equipment, material, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practices. Bilge manifold piping must be labeled for space it services.46 CFR 115.800

Electrical equipment on a vessel must be installed and maintained to protect persons and vessel from electrical hazards. Prove proper installation and circuit protection of engine room LED light system. 46 CFR 120.200(b)

A station bill must be posted by the master on a vessel over 65ft in length and requiring more than four crew members. An updated station bill must be drafted and posted. 46 CFR 122.514(a)(2)

Initial inspections may include inspection of certificates and operating manuals including certificates issued by the FCC. No FCC documentation was available for inspection. 46 CFR 115.402(c)(4)

Multiple fractures found in girders/structures beneath propulsion reduction gear IWO P/S enginerooms. Reduction gears have extensive history of severe vibration. COI has been inactive for nearly 4 years. To be corrected prior to inspection for certification.

Provide report from engine manufacturer which identifies root cause of turbocharger failures and recommended solution and repairs. Complete servicing and permanent repairs to both main engines and demonstrate proper operation of same to the satisfaction of the attending marine inspector.

Repair and demonstrate proper operation of starboard main engine. Inboard turbocharger had failed during operation.

Pressurized oil supply tubing for inboard turbocharger on starboard main engine was worn from contact with mountings, due to worn anit-chafing materials, replacement of tubing and mountings required.

Jacket water cooling line on starboard engine had a o ring failure resultant from excessive vibration. Section of line was replaced and a new bracket was installed to prevent further casualties.

Permanently repair starboard reduction gear to eliminate excessive vibration and demonstrate proper operation of same.

FOUR CHILD SIZE PFDS MISSING, CORRECTED.

SEA PAINTERS IMPROPERLY SECURED TO VESSEL, CORRECTED.

FUEL SUPPLY PIPING LEAKED AT 1ST UNION FROM PORT FUEL TANK, CORRECTED.

Provide/install bilge suction strainers for engineroom pick-ups with an area 3 times that of the pipe diameter.

Repair fractured welds as noted on work-list.

Carry no passengers until fractured welds and fractured shell plate and fractured bulkhead are repaired. Crop & renew fractured shell plate & bulkhead in way of #1 & #2 STBD tanks. Renew all fractured welds. Drill stop and weld all other fractures in Tank #2 STBD.

Carry no passengers prior to replacing expired hydrostatic release unit on inflatable survival craft.

Resolve comment #4 of MSC letter (ser H2-9501237) dated April 6, 1995 regarding the FORMICA brand laminate used in the type 5A spaces. The laminate must meet the requirements of 46 CFR 164.012.

Place retro-reflective material on ring buoys.

Repair fractured welds IAW worklist dated 25 JULY 1996.

Provide durable placard at VHF radiotelephone installation with emergency broadcast instructions and information, specific to the vessel, IAW 46 CFR 121.506.

Resolve comment #6 of MSC letter (H2-9501237) dated April 6, 1995 regardingthe durability of the Arrest-A-Flame XLL-B carpet flame treatment. Providethe cognizant OCMI test results of the durability of the Arrest-A-Flame when applied to carpet subject to foot traffic of typical passenger ferry.

Resolve comment #3 of MSC letter (ser H2-9501237) dated April 6, 1995 regarding the fire resistance of the fixed seating in the passenger spaces.The fabric must meet NFPA standard 701 and the foam padding must meet ASTM D-1692. As per 46 CFR 72.05-55
