Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel failed to schedule nor complete an annual inspection within three months of anniversary date (03 Feb 2025). Schedule COI inspection with our office (at [email withheld]) and complete COI inspection. 46 CFR 176.500(b)(1)

Each life jacket stowed overhead must be supported in a manner that allows quick release for distribution. Life jacket net zippers were seized, not allowing life jackets to be accessible. 46 CFR 180.78(3)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Vessel has 02 inoperable bilge high level alarms. 46 CFR 182.530(a)

Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. Vessel could not provide valid DOC. 46 CFR 67.7

To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. Vessel could not provide valid EPIRB registration. Without this, vessel fails to meet the primary lifesaving requirements set forth in the 46 CFR 180.200 table. 47 CFR 80.1061(f)

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. All of vessel's flares were expired. Vessel is restricted to short runs of 30 minutes or less until rectified. 46 CFR 180.68(a)

All fuel tanks must be electrically bonded to a common ground. Fuel tank fill pipe bonding wire broke free. 46 CFR 182.440(b)(4)

A propulsion engine control system, including pilothouse control, must be designed so that a loss of power to the control system does not result in an increase in shaft speed or propeller pitch. Two new outboards controlled by electronic control modules installed on vessel. 46 CFR 184.620(c)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. Vessel conducting repower, replacing two 225 HP outboards with two 250 HP outboards, without the involvement or notification of the Coast Guard. 46 CFR 176.700(a)

Exhaust blower motors shall be outside of the ducts, and if mounted in any compartment required to be ventilated by this section, must be located as high above the bilge as practicable. Blower blades must be nonsparking with reference to their housings. Exhaust blower motor is installed in the duct. Old T 46 CFR 182.15-45(c)

Provisions must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Operator was unable to provide closure devices for two supply duct openings and two exhaust duct discharge openings. 46 cfr 182.465(h)

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturer's instructions. Fuel tank compartment has multiple openings and wire penetrations that are not vapor tight. Make compartment vapor tight. 46 CFR 181.420(a)(4)

Exhaust blower switches shall be located outside of any space required to be ventilated by this section, and shall be of the type interlocked with the starting switch and the ignition switch so that the blowers are started before the engine starter motor circuit or the engine ignition is energized. Vessel's blowers are inoperable and are not interlocked to the starting switch. Configure the blowers to ensure they are interlocked to the starter motor circuit and are energized prior to starting of the engines. Old T 46 CFR 182.15-45(d)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel's navigation lights are inoperable. Repair navigation lights and demonstrate operation to attending Marine Inspector. 46 CFR 183.420

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Fireboy is past due for servicing and inspection. Provide documentation that Fireboy was serviced by a qualified servicing facility. 46 CFR 176.810(b)(2)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with §80.59 once every five years. Vessel does not have a valid Safety Radiotelephony Certificate. Renew Safety Radiotelephony Certificate. 47 CFR 80.903

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing MMSI and LAT/LONG for DSC operation. 47 CFR 80.905

Except as provided in 80.13(c), stations must have an authorization granted by the Federal Communications Commission. Vessel could not produce current FCC station license. Provide correct FCC license within two weeks. 47 CFR 80.405(a)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances. Unable to conduct annual leak test on collar. Conduct collar leak test in presence of attending marine inspector during deficiency check. 46 CFR 176.802(a)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Conduct man overboard drill in the presence of attending marine inspector during deficiency check. 46 CFR 176.808(g)

At each initial and subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions to be specified by the inspector. Conduct firefighting drill in the presence of marine inspector. 46 CFR 176.810(d)

Shutoff valves, installed so as to close against the fuel, shall be fitted in the fuel supply line. Demonstrate the operation of the port and starboard fuel tank shutoff valve to the attending USCG Marine Inspector.

Spaces containing gasoline machinery or fuel tanks shall have natural supply and mechanical exhaust ventilation as required by this section.... The vessel doest not have two natural ventilation inlet ducts or two mechanical ventilation ducts, the vessel's exhaust blower is installed in line with the exhaust duct, and the vessel's exhaust blower is not interlocked with the main propulsion engines starting switch.

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant OCMI on or after March 11, 1996, must comply with this part. Replacement or existing equipment, not including wire or cable, installed on the vessel prior to March 11, 1996 need not comply with the regulations in this part. Electrical equipment and wiring installed in the vessel's fuel tank space are not explosion proof or part of an intrinsically safe system as required by 46 CFR 183.530 (a).

The name of a vessel must be marked on painted in clearly legible letter and numbers: on each life jacket. 13 life jackets are not marked with the vessel's name.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. The vessel does not have a donning placard posted for the current outfit of life jackets.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made with out the approval of the cognizant OCMI. The vessel's starboard propulsion outboard engine is not operational.

A vessel not required to have a power driven fire pump must have three buckets, with an attached lanyard satisfactorily to the cognizant OCMI. The vessel is currently outfitted with only two fire buckets.

Batteries shall be effectively screened by cage or similar structure to minimize the danger of accidental accidental spark through dropping a metal object across terminals. The vessel's batteries to not have a cover installed.

The vessel must carry a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. The following items are expired or missing in the vessel's first-aid kit, aspirin, eyewash, slint, and amonia.

Vessel needs to show satisfactory operation of the onboard remote fuel shutoff valves as required in 46 CFR 182.455.

Vessel needs to show that all onboard extinguishing units (fixed and portable) are within serviceable standards as required in 46 CFR 176.810.

Vessel needs to ensure the forward bilge pump is in satisfactory working order as required in 46 CFR 182.520.

Vessel needs to show proof of having a valid FCC Radio Safety Certificate as required in 47 CFR 13.

Vessel needs to post all required placards for emergency procedures as required in 46 CFR 185.602.

Vessel needs to provide proof that its pre-engineered fire extinguishing unit and portable fire extinguishers are within serviceable standards and are operationally ready.

Vessel high bilge alarms need to produce a visual and audible indication when in contact with water.

Vessel needs to be equipped with a functional fume vapor detector unit capable of providing a visual and audible alarm.

Vessel’s starboard side at handrail needs to be re-welded to support it.

Vessel needs to conduct satisfactory sea trials and perform required drills to an attending marine inspector.

Vessel needs to be equipped with full complement of working navigational lights as required by the vessel’s route IAW Navigation Rules of the Road.

Vessel needs to have one ring buoy equipped with a floating water light attached with a lanyard and corrosion resistant clip and easily able to disconnect from buoy.

Vessel needs to provide a visual indicator showing operation of bilge pump.

Vessel fuel strainers need to have proper flame protection for gasoline applications.

Vessel needs to be equipped with a Class D Radio capable of GPS. 47 CFR 80.905.

Vessel operators are to hold a valid FCC Marine Radio Operator’s Permit. 47 CFR 80.159.

Vessel needs to have a valid FCC Safety Certificate.

Vessel needs to properly mark quantity storing locations of both adult and child lifejackets.

Vessel needs to include the company’s policy for in-water emergency procedures to the emergency instruction list that is required to be posted on the vessel for its passenger to view.
