Vessel rep reported to SSD that the vessel impacted a rouge wave and caused damage to the stbd forward Bulwark. No water ingress was reported, the vessel ended the voyage and safely returned to San Diego. Verbal no sail was issued to vessel.

On vessels described by 46 CFR 175.110(d) of this chapter, a passenger safety bill must be posted by the master in each cabin or stateroom, and in passenger accommodation spaces. Vessel did not have the passenger safety bill posted in passenger accommodation spaces. Post passenger safety bill to the satisfaction of the attending marine inspector.

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.”. (2)Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. Vessel was missing required CO2 warning signs. Install CO2 warning signs for aft compartment and machinery space entrance to satisfaction of attending marine inspector.

Emergency egress training for each member of the crew, to occur for all members of the crew - (i) At least monthly while such members are employed on board the vessels; and (ii) Each time a crew member joins the crew of such vessel. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics. Egress training records were not available. Conduct and log monthly egress training.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. An electrical receptacle was found behind the forward berthing stairs unmounted and hanging by the cables. Properly mount electrical receptacle and install cable supports.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel did not install an interconnected fire detection system by 27DEC2022. Vessel is restricted to a LIMITED COASTWISE route and is prohibited from allowing passengers to utilize overnight accomodations until it's compliance with the interconnected fire detection requirements of 46 CFR 181.405 have been verified by a USCG marine inspector.

All passenger accommodations must be arranged and equipped to provide for the safety of the passengers in consideration of the route, modes of operation, and speed of the vessel. The deck in the forward passenger bunk room was found to be constructed of general plywood and with areas of severe delamination, warping and soft areas due to chronic moisture damage. Owner plans to replace after dry dock but prior to using the space for passenger accomodation. Replace floor to the satisfaction of the attending marine inspector prior to using for passengers but no later than 01APR2023.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended in this section. Portable 2-A fire extignuisher located in the forward passenger berthing area was found with a depleted charge. Service or replace fire extinguisher.

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. One deckhand had taken his preemployment test but had not received results at the time of inspection. Vessel was manned above the minimum required by the COI. Provide documentation of negative test prior to employment in a safety sensitive position.

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. The carbon dioxide fixed fire suppression system activation pressure switch did not shut down either generator located in the engine room. Install and demonstrate operation of engine shut downs.

Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in § 185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessel's emergency instructions were not vessel specific. Provide vessel specific emergency instructions.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Records of tests and inspections required by 46 CFR 176.810(b) were not available. Provide records.

The Rules concerning shapes shall be complied with by day. The lights and shapes specified in these Rules shall comply with the provisions of Annex I of these Rules (33 CFR part 84). Vessel was not carrying day shapes. Provide day shapes.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly portable extinguisher inspections records not available. Conduct and record inspections.

Fire, abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Drill records only contained the date and general type of drill (fire, MOB, abandon ship). Conduct and log drills to include the above information.

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. All vessels with overnight accommodations must be equipped with a general alarm system. The public address system required by §184.610 of this chapter may be used to sound the general alarm signal. Vessel's Public Address and General Alarm system was not sufficiently audible during normal operating conditions in the passenger bunk spaces. Demonstrate operation of Public Address and General Alarm of adequate volume in all accommodation spaces.

Each ring buoy on a vessel mus be approved in accordance with §160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Port forward ring buoy was found deteriorated and worn. Provide ring buoy meeting specifications of 46 CFR 160.050.

On a vessel on a voyage of more than 24 hours duration, passengers shall be requested to don life jackets and go to the appropriate embarkation station during the safety orientation. If only a small number of passengers embark at a port after the original muster has been held, these passengers must be given the passenger safety orientation required by paragraphs (a) or (b) of this section if another muster is not held. Vessel did not require passengers to don life jackets during the safety orientation for a three day voyage. After discussion between inspector and vessel master, passengers donned life jackets and deficiency was corrected.

Emergency instructions missing instructions for man overboard and rough weather. Provide additional instructions.

Aft crew bunk room missing speaker for public address and general alarm. Provide speaker.

Provide current and complete first aid kit.

Provide fire hose nozzle capable of straight and wide spray.

Provide spanner wrenches at all fire stations.

Remove permanent extension cord powering bilge pump.

Provide plan to replace mattresses or demonstrate compliance with flammability standards.

Unground outlets found throughout Demonstrate proper grounding of outlets. Due by the end of next drydock.

Provide dead end covers on distribution panel in pilot house.

Provide log showing fire, MOB, abandon ship drills as well as epirb & fire ext. inspections.

While underway or drifting at sea, every vessel shall at all times maintain a proper look-out by sight and hearing. The designated watchmen required by 46 CFR 185.410 cannot preform both roving patrolling and look-out watch simultaneously.

Vessel required to have the appropriate number of independent modular smoke detection and alarm units in accommodation spaces that are in compliance with UL 217. Must install units in closed berthing spaces amidships of vessel to satisfaction of Coast Guard Inspector.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from passenger and crew accommodation spaces below the main deck. Must install emergency lighting in forward sections of port and starboard fwd berthing spaces to satisfaction of CG Marine Inspections.

All major appliances on board vessels must be hard wired to the outlet. Freezer in aft accommodation space was plugged into an extension cord. Must rectify to satisfaction of CG Marine Inspector.

Provide pre-employment test results for crew member prior to working on board with PAX.

Hard wire freezer in aft space or provide an outlet so that extention cords are not used.

Repair frames in the bow area as indicated.

Due to water ingress at port chine just aft of the collision bulkhead; open area up for further inspection, make proper repairs, and complete a post repair sea-trial prior to carrying passengers.
