Each ship transporting more than six passengers for hire equipped with a radiotelephone station must carry a radio operator who holds a marine radio operator permit or higher class license. 47 CFR 80.159(e)(1)

Irrespective of build date, a vessel to which this subchapter applies must meet 46 CFR 181.405 and 46 CFR 181.450. Vessel is not fitted with interconnected fire detectors in the galley, engine room, and generator room. Install an approved fire detection system to the satisfaction of the attending marine inspector.

Stations in the maritime service must be licensed by the FCC and issued for a term of ten years. Vessel was missing updated FCC Station license, needs original form to be inspected.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Bilge Alarm in pilothouse has 02 lights come on at the same time. Must reflect correct space with the activated alarm.

Exposed hazards, such as gears and rotating machinery, must be properly protected by a cover, guard or rail. Guard for port side motor located in the laundry room is loose/unlatched.

Hinged watertight door must be fitted with quick action closing device. Watertight doors must be tightened so quick action closing device have little to no slack to allow water entry.

Distribution panel must be totally enclosed. Main distribution panel is missing a blank to cover an opening in the panel, exposing back wires.

Exhaust gas must not leak from the piping or any connections. Port side exhaust pipe has a pin hole leak, causing exhaust to leak out and solidify.

Flexible shall be fabricated with an inner tube and a cover of synthetic rubber or other suitable material reinforced with wire braid. Replace F/O hose to #3 main diesel engine fuel pump.

Portable fire extinguishers shall be installed to the satisfaction of the Officer in Charge, Marine Inspection. 01 undercharged portable fire extinguisher at No. 8 fire station inside engine room. Must be properly charged.

Inspection of all watertight closures in the hull, decks, and bulkheads to be ready for inspection. Insert needed to replace bulkhead penetration and surrounding damage in the generator room, port side.

Exhaust gas must not leak from the piping or any connections. Exhaust leak on the #2 Generator, first joint, exposed after removing lagging.

All wiring and cables shall be suitable for marine service and for particular installation used. Protective sleeves for wires on both generators are deteriorating, exposed to oil and fuel.

Voltmeter and an ammeter shall be provided that can be used for measuring voltage and current of each generator. Ammeter for #1 Generator is not reading/registering.

Each vessel must have adequate emergency lighting. Generator room emergency light to be cleaned to show adequate light and able to highlight escape hatch sign in emergency.

A hatch exposed to the weather must be watertight. Escape hatch in engine room is not watertight, evident in gap.

All accessible parts of the exterior and interior of the hull to be ready for inspection. Pin hole on the starboard side shell under the escape hatch in the engine room.

Prepared to test the steering systems of the vessel and to the extent necessary to determine that they are in suitable condition. All steering gear posts have cracked welds at the joint.

Vessel must carry either a first-aid kit that meets the the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents. First Aid kit box latch fell and some medication were expired.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Lines for the life float and life ring on the 01 deck behind the pilothouse, attached to lights, are fraying and deteriorating.

All accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks are prepared to conduct tests and ready for inspection. Cracks in the mid body and first vent box on both sides, top side, are present. To be monitored and then repaired at next dry dock.

A hatch cover exposed to the weather must be watertight and must have securing devices and operable from both sides. All main deck "Freeman" hatches are not closing within a reasonable time. Recommend to method to adjust all hatches to find the locking mechanism with ease.

All cable/wire must be protected from the weather. Main deck light port side contain loose wire stuffing, exposing the wire and penetration to weather.

All accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks are prepared to conduct tests and ready for inspection. 03 cracked skip welds on the side shell near starboard generator.

A hatch exposed to the weather must be watertight. Passenger space emergency escape needs adjusting.

Fuel lines must be protected from mechanical injury to prevent cracking and bending. Port generator F/O return line found to be bent. 46CFR182.455

At each inspection for certification vessel must be made ready for inspection of the hull structure to include all accessible parts of the exterior and interior of the hull. On the bulkhead of the generator room side of exhaust overboard discharge found excessive salt build up as well as dampness on the salt buildup. Appeared to have cracked welds. Could not locate cracked welds due to salt build up. Requires cleaning and cracked welds to be repaired. Multiple cracks and cracked welds on stern rub rail require repair. Cite: 46CFR176.802(a)(1)

Piping, valves, and fittings of CO2 cylinder for paint locker must be securely supported and protected against damage. Paint locker cylinder hose dry-rotted, to be replaced.

Doors exposed to the weather must be weathertight. Gasket on main deck weathertight door is not serviceable(missing pieces) and is not weathertight.

Vessel is required to have Safety Radiotelephony and Bridge to Bridge certificates. Radio call sign incorrect on both Certificates.

Port and Stbd Main Engine water hoses located on the top of the mains outer coating are deteriorating, vsl will replace in kind.

When a vessel is not in compliance with its Certificate of Inspection or fails to comply with a regulation of this subchapter, the cognizant OCMI may permit the vessel to proceed to another port for repair, if in the judgment of the OCMI, the trip can be completed safely." The vessel experienced a head-on collision underway with another vessel, incurring hull penetrations in the port side forepeak compartment, approximately 6x12 inches, 6 feet above the waterline. Per the vessel transit plan provided 21FEB2019, the following conditions must be maintained throughout the entire voyage: 1) a 1 inch diaphragm pump 2250 gph must be onboard and ready for immediate use, 2) a polymer baloon filled with approximately 2psi must be placed in the affected area to prevent water ingress, 3) conduct hourly rounds of the affected space, 4) conduct hourly radio updates with the Sector New Orleans Command Center, 5) the vessel will transit from Venice, LA, to Houma through Cat Island Pass and come up the Houma Nav canal then turn east in the ICW east bound to mile marker 51 at Diversified R and R boats. Upon arrival, vessel must conduct permanent repairs to the satisfaction of the attending USCG Marine Inspector. 46 CFR 176.202(a) 705c

Vessel's #2 main engine PTO fire monitor was unable to gain suction. Marine Inspectors tested primary fire pump, test satisfactory. Fire monitor to be repaired and tested in the prescence of attending Marine Inspector.

Two lifefloats were found in unsatisfactory condition vessel is restricted to 22 persons onboard. One lifefloat is servicable. Repair or replace lifefloats to satisfaction of the attending Marine Inspector.

Paint locker has a hole in it. Repair to the satisfaction of the attending Marine Inspector.

Wire rungs in engine room are passing through at watertight bulkhead but are not sealed properly. Make all penetrations watertight.

Gauges on air receivers are maxed at 200 psi. MAWP is 200 psi. Relief valve set at 150. Replace gauges (1.2-2 relief valve settings) above MAWP.
