OUTWARD BOUND

A vessel that carries more than 49 passengers that is not more than 65 feet is required to have 1 fixed power bilge pump and 1 portable hand pump. A secondary powered bilge pump may serve as an alternative to the hand pump. The engine driven bilge pump was not operational at the conclusion of the drydock examination. Repair engine driven bilge pump and prove proper operation. Vessel is authorized to transit to homeport in Boston Harbor with the powered fire pump serving as the bilge pump.

Cable must be installed with metal supports spaced not more than 24 inches apart. Cables in the engine room have metal supports spaced more than 24 inches. Ensure all cables in the engine room have supports spaced no more than 24 inches.

C&R the sbd side forpeak as marked to good metal.

C&R the lower aft steering to Engine room bulkhead to good metal.

All observed hazardous conditions must be corrected. The light for the exterior stairwell was not operational, Prove proper operation of the exterior stairwell light.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Trash can on board was constructed of plastic with no lid. 46 CFR 177.405(f)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Battery operated portable flashlight at the operating station was inoperable. 46 CFR 183.430

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. During inspection, crew was not able to demonstrate full competency in recovering a person falling overboard due to not conducting sufficient drills to achieve satisfactory results. Master and crew are to conduct sufficient training and demonstrate satisfactory drills to the satisfaction of the attending marine inspector prior to carrying passengers.

The master of the vessel shall ensure that each EPIRB must be tested monthly using the integrated test circuit and output indicator to determine that it is operative and shall be logged or otherwise documented. After reviewing the vessel's log, there was no documentation or log entry of monthly EPIRB testing. Informed vessel master of monthly test requirement and log entry. Provide evidence of EPIRB monthly test log entry to the satisfaction of the attending marine inspector.

The master of a vessel shall keep a correct, written count of all passengers that embark on and disembark from the vessel. Prior to departing on a voyage, the passenger count must be communicated verbally or in writing, and available ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. During inspection, master stated vessel documents passenger count via vessel log entry, but has not established a method of relaying passenger count ashore. Vessel must establish means of communicating passenger count ashore prior to each voyage with passengers. Provide proof of means to the satisfaction of the attending marine inspector.

At each initial and subsequent inspection for certification of a vessel the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including operational test of the means provided for pumping bilges. During inspection, the bilge suction in the forward compartment and forward cabin were not operable. Fix and provide proof of proper operation of all bilge suction points to the satisfaction of the marine inspector.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. During inspection, there was no documentation of abandon ship and man overboard drills being done. Master is responsible for ensuring such drills are properly documented and available for review by Coast Guard upon request.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. During inspection, the vessel did not have a valid Certificate of Documentation (COD). Vessel is not allowed to operate or move until evidence is provided to marine inspector that a new COD has been paid for and is in processing.

A vessel must carry either a first-aid kit that meets the requirements in 46CFR199.175(b)(10) or a kit with equivalent contents and instructions. Several items in vessel's first aid kit were found expired. Replace expired medication items in first aid kid.

To maintain a valid Certificate of Inspection, you must complete an annual inspection within the periods specified in § 176.500(b)(1) and your Certificate of Inspection must be endorsed. The vessel missed the annual inspections for CY 2022 and 2023. Vessel is prohibited from carrying passengers for hire until a satisfactory COI renewal inspection is completed.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and internal structural at least once every 2 years. Vessel did not conduct the require hull exam before the due date of 31MAR23. Vessel must complete satisfactory hull exam prior to the carriage of passengers for hire.

All vessels of 26 feet in length and over shall be provided with individual bilge lines and bilge suctions for each watertight compartment except as follows: The space forward of the collision bulkhead need not be fitted with a bilge suction line when the arrangement of the vessel is such that ordinary leakage may be removed from this compartmant by the use of a hand portable bilge pump or other equipment and such equipment is provided. MI observed the bilge system was unable to draw suction from the forward compartment and there was no other equipment available to remove water. Correct prior to carrying passengers

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; The EPIRB test required shall be logged or otherwise documented. Vessel representative stated they do not keep a log of EPIRB testing.

One life raft sent out for service. Vessel restricted to one mile from shore untill IBA is returned.

Operator requested replacing existing life floats with IBAs. Provide installation proposal fro approval, install if approved, schedule inspection of installation with CG. Must be corrected prior to carrying passengers for hire.

Each life float must be fitted with a light. Water light on life float intermittently operates.

A vessel's hull must not carry current. Main switchboard ground detection system indicated current on the hull in excess of 10 amps. Must be corrected prior to carrying passengers for hire.

Overboard discharge on stbd side has no check/positive action valve. Ensure no water ingress by installing valve or terminating discharge if not in use. Must be corrected prior to carrying passengers for hire.

Wasted transverse frame on starboard side forepeak. Crop and renew in kind to good metal. Must be corrected prior to carrying passengers for hire.

Wasted frame on port side support to Laz hatch. Crop and renew in kind to good metal. Must be corrected prior to carrying passengers for hire.

Main deck aft wasted IWO emergency steering tiller access cap. Crop and renew in kind to good metal. Must be corrected prior to carrying passengers for hire.

Wasted areas IWO port engine room bilge. Conduct thickness gauging and provide repair proposal for approval. Must be corrected prior to carrying passengers for hire.

Wasted steel IWO aft pilot house and deck. Provide repair proposal. Must be corrected prior to carrying passengers for hire.

Repair leak in transmission on main diesel.

Minimize or eliminate amps to ground.

Install appropriate hydrostatic release unit on EPIRB.

Remove or replace in electrical box, dead end wires in void #2 and engine room.

Replace line on life ring on bow.
