A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Two 2.5 gallon fire buckets were not present on board the vessel. Provide required (3) 2.5 gallon fire buckets to the satisfaction of the attending marine inspector.

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificate will be issued for various periods of time based upon the endorsements the mariner holds. Provide valid medical certificate.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. Missing required analgesic medication and gauze.

Vessels described by 46 CFR 175.110(d) must submit plans to the cognizant OCMI, in accordance with 46 CFR 176.700, for the installation and use of monitoring device to ensure the wakefulness of the watchmen required in paragraph (a) of this section. Vessels with a keel laid date after March 28, 2022, must include plans for the monitoring device(s) within the plan submissions required in 46 CFR 177.202. The Coast Guard will work with the vessel operators to determine a reasonable implementation schedule once the plans are accepted. The monitoring device(s) must:(1) Ensure the wakefulness of the crew in the event that the watchman required in paragraph (a) of this section is unresponsive;(2) Remain operable during the nighttime watch; and(3) Be arranged to ensure proper coverage of the passenger accommodation spaces, common areas, and spaces with potential fire hazards. Operationg station watch alarm not working as designed. Repair watch alarm and provide valid test to the attending marine inspector.

Each ring life buoy on a vessel must: (1) Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150; If on a vessel on an oceans or coastwise route, be orange in color. Lifebuoy was faded white on the front, no vessel name label visible, and deteriorated buoy line. Vessel replaced during inspection. Changed action to 10c. Deficiency Rectified.

An adult life jacket must be provided for each person carried on board a vessel. (1) foam lifejacket was found with cracks and missing foam. Vessel replaced onsite. Changed action to 10c. Deficiency Rectified.

Each survival craft must be: (1) Secured to the vessel by a painter with a float-free link permanently attached to the vessel except that a float-free link is not required if the vessel operates only on waters not as deep as the length of the painter; Survival craft sea painter was not connected to hydrostatic release. Vessel corrected on site. Action code changed to 10c Deficiency Rectified.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and systems, including the following: Inspection of all generators, motors, lighting fixtures and circuit interrupting devices located in spaces or areas that may contain flammable vapors. Vessel generator would shutdown immediately upon startup. MI could not inspect generator functionality. Repair generator and provide functionality test in the presence of marine inspector.

You are responsible for all actions of your officials, r epresentatives, and agents in carrying out the requireme nts of the DOT agency regulations. Vessel did not have enrollment with service agent for drug testing. Vessel provided proof. Action changed to 10c. Deficiency Rectified.

Each distribution panel or switchboard must be totally enclosed and of the dead front type. Distribution panel near rear of pilothouse was missing a conduit plug. Vessel corrected and plugged side panel on site. Action code changed to 10c. Deficiency Rectified.

Electrical equipment on a vessel must be installed and maintained to:Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Bow compartment light was filled halfway with water. Disconnect circuit and repair or replace bow light to satisfaction of marine inspector.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Sluice valves are not permitted in watertight bulkheads. A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: (1) The pump is listed by an independent laboratory as meeting the requirements in UL 1113 (incorporated by reference; see 46 CFR 175.600). Bunk room submersible electric bilge pump was inoperable. Pump and on board spares were identifieable as meeting UL 1113. Provide UL 1113 listed pump and demonstrate operation prior to carrying passengers or cargo.

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Ring life buoy floating waterlight was found inoperable and battery was replaced in the last year in accordance with manufacturer instruction. Replace battery and demonstrate proper operation.

Except as otherwise provided in this section, rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Vessel rail in way of port boarding gate was found loose and missing screws from three stanchions. Repair or provide equivalent protection for passengers.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. At time of inspection neither the mast head light nor the port running light were functioning. Corrected prior to completion of inspection. Code changed to 10C.

A vessel must carry one floating water light, unless it is limited to daytime operation, in which case no floating water light is required. At time of inspection water light was not functional when tested. Corrected prior to completion of inspection. Code changed to 10C.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard, and to make sure that each crew member is familiar with his or her duties in case of fire. Deckhand on board had not participated in drills on vessel, but on questioning was familiar with vessel equipment and procedures. Conduct drills with assigned crew members.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: (1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection; (2) Perform the duties and functions of patrolmen or watchmen required by this chapter; or, (3) Are specifically assigned the duties of warning, mustering, assembling, assisting, or controlling the movement of passengers during emergencies. Assigned crew memebr at time of inspection was not part of the regular crew, but was filling in. Crew member was not part of marine employer's chemical testing program, but was subject to random testing under the program of another vessel. Add crew member to marine employer's chemcical testing program or provide documentation showing current status in another program along with arrangements for marine employer to be notified immediately upon a positive chemical test result.

The applicant for a merchant mariner credential must possess a valid medical certificate. The master's medical certificate was not available for inspection. Provide valid medical certificate

Except when individual pumps are provided for separate spaces, individual bilge suction lines must be led to a central control point or manifold and provided with a stop valve at the control point or manifold and a check valve at some accessible point in the bilge line. The check valve in the lazarette bilge line was not functioning properly. Repair or replace check valve and demonstrate proper operation.

Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in § 185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessel's emergency instructions were not vessel specific. Provide vessel specific emergency instructions.

The Rules concerning shapes shall be complied with by day. The lights and shapes specified in these Rules shall comply with the provisions of Annex I of these Rules (33 CFR part 84). Vessel was not carrying day shapes. Provide day shapes.

Primary lifesaving not on board. Provide IBA.

Drug and alcohol program documentation not on board. Provide documentation.

Stern light and anchor light not operable. Demonstrate proper operation of all navigation lights.

Ring life buoy grab line frayed. Replace grab line and/or ring buoy.

Demonstrate operation of bunk room emergency lighting. Lighting not operable during inspection. Correct prior to passengers occupying bunkroom.

Fire and lifesaving equipment maintenance and inspections not logged. Conduct and log.

Rot observed in structural members above and forward of fuel tank port and starboard in the engine space. Provide proposal to remove rot and treat where able and replace/repair if necessary.
