In the event of an accident to a boiler, unfired pressure vessel, or machinery tending to the render the further use of the item unsafe until repairs are made, a report shall be made by the chief engineer immediately to the OCMI. A notification was made but the wrong Ferry Quick Reference guide was utilized. When the TILLIKUM lost one of the two main engines, the vessel should have been in "take home mode only". The vessel continued with its service for at least one more stop before taking itself out of service. All WSF vessels and operators shall use the most current and approved Ferry Quick Reference Guide. 46CFR78.33-5a

Vessel reported a fuel leak on the crossover manifold connecting fuel banks on propulsion generator #1. Vessel is authorized to continue service with passengers IAW approved WSF matrix, USCG will attend vessel to attest to satisfactory repairs. 46CFR71.55-5(a)

Relief valve at the whistle tank reducing station will not reseat. Repair or renew relief valve.

Steam heating system condensate return pipe adjacent to the boiler flat has an external temperature greater than 150 degrees F.

Each passenger vessel of 500 tons gross tonnage and over must carry on each side of the vessel at least one rescue boat approved under approval series 160.156 that is equipped as specified in table 199.175 of this part. The #2 rescue boat is inoperable. Vessel shall reduce pax capacity by 50% until rescue boat is fully operational and tested.

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. Properly troubleshoot root cause of loss of propulsion and provide proof of repairs to CG.

Aux steam boiler is only equipped with one low water cut off device. ASME/ANSI CSD-1 (incorporated by reference) requires two such devices. Install a second low water cut off device in accordance with all code requirements at the vessel's next yard period.

Approximately 155 gallons of combustible liquids in 5 gallon containers is being stored in the machinery space. 46 CFR Subchapter N only allows 55 gallons of combustible liquid stores to be kept in the machinery space. Relocate excess combustible liquid outside the machinery space.

Wiring to heater fan (over boiler) includes an inappropriate inline splice. Provide continuous cable connection or splice in accordance with IEEE Standard 45.

Numerous fire doors in the machinery spaces found with manual hold backs installed. Hold backs are only allowed if they can be remotely released. Remove manual hold backs on fire doors in the engineering spaces.

Illustrations and instructions must be conspicuously displayed at each muster station; the method of donning life jackets. Life Jacket drop points on all four corners of vessel car deck missing required signage. 46CFR199.80(c)(5)

All general alarm bells shall be identified by red lettering at least 1/2 inch high: "GENERAL ALARM - WHEN BELL RINGS GO TO YOUR STATION." All general alarm bells on vessel car deck missing signage, signage faded and/or missing throughout vessel. 46CFR78.47-7(a)

Each auxiliary boiler must be available for examination by the marine inspector at intervals specified by Table 61.05-10. Boiler valve inspection required as per Table 46 CFR 61.05-10 has not been conducted within 5 years from the date of the last inspection. Vessel to make boiler valves available for inspection at next scheduled drydock. 46 CFR 61.05-10(a)

(a) Each self-propelled vessel, except as provided in paragraph (d) of this section, must have an electric or mechanical engine order telegraph system from the navigating bridge to the engine room. (c) On a double-ended vessel that has two navigating bridges, this system must be between the engine room and each navigation bridge. Engine order telegraph cable on number two end is broken.

The main steering gear must be of adequate strength for and capable of steering the vessel at maximum ahead service speed, which must be demonstrated to the satisfaction of the cognizant Officer in Charge Marine Inspections. At normal operating speeds the pedestal bearing oil unit is showing a high temperature alarm. Cite: 46 CFR 58.25-10(b)(1)

Prove the proper operation of the AMS and propulsion control systems. When attempting to prove PSTP PCS-13 the failure of the AUX 1 PLC did not leave the propulsion system unaffected. Propulsion control was lost from each pilothouse. 46 CFR 62.25-25, Code 60 c

Provide a means to acknowledge and silence the annunciator for the EOT failure in both the pilot house and the engineer's operating station. 46 CFR 62.25-20

Separate the over-speed alarm from the other safety shutdowns,the main propulsion prime movers safeties e.g. e-stop, CO2, etc. also result in an over-speed indication. Alarms must be independent of each other. 46 CFR 62.25-20

Update the the vessel's PSTPs to reflect the condition found on the vessel. The majority of the procedures and the expected results did not match the condition found on the vessel as noted on the shipboard copy dated 2021-2022. Submit the revision to the USCG MSC for approval. 46 CFR 61.40-10

Repair the fractured butt weld in way of the overhead deck transverse stiffener located in the No.2 steering gear room and above the e-steering station. 46 CFR 72.01-15\ Repair at next DDX.

Repair the inoperable RPM indicators located in the central control station. 46 CFR 62.25-20

Prove all non-completed PSTPs. The engineering systems were not ready for inspection. 46 CFR 61.40-6, Code 701 c

Qualify the emergency power system. The frequency continually surges 60-69 hz, and when a load is applied the amps and voltage fluctuates for approx. 10 seconds. 46 CFR 112.05-5

Reconnect and qualify the remote operating valves serving the fire main/sprinkler isolation valve and the dirty oil tank which were found disconnected.

Prove the proper operation of the low lube oil alarm on the in-port generator as it was inoperable during the test. Prove the proper operation of the e-gen over-crank alarm, it worked 1 out of 4 attempts. 46 CFR 62.35-50

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, sop valve controls and in the CO2 cylinder storage room. The instructions did not match the labeling of the CO2 pulls. 46 CFR 76.15-10(h)

Class 3 sliding watertight door must operate in accordance with incorporated reference ASTM F1197. Door does not close completely, with an approximate 1" gap from full closure. Limit switch is inoperable. 46 CFR 170.270(c)(1)

As designed and noted in the periodic safety test procedures, #2 main diesel engine did not shut upon activation of the fixed fire extinguishing system. Corrected on the spot. 46 CFR 61.40-6(a)

Boiler safety controls must control and cycle the unit in the proper manner and sequence. According to the PSTP, the boiler's pressure limit control's setting was 9 PSI but the boiler did not secure until 12 PSI. The switch setting indicated it was set at 9 PSI. 46 CFR 61.35-3 (a)(1) & (a)(8)

The hot water heating boiler must have at least one safety relief valve. The safety relief valve was marked for 15 PSI. The relief valve did not relieve at 15 PSI and the crew member utilized the remote hand pull to relieve the pressure at 17 PSI. 46 CFR 53.05-2(b)

Wasteage on vehicle deck beyond tolerance.

In all ventilation systems, annually operated dampers or other suitable means shall be provided in accessible locations, for the shutting off the passage of air in the event of fire. The Engine room NO2 Supply fire damper is not operating properly. 46CFR72.05-50(i) 701 C Vessel requested an extension for 30 days. Granted with the requirement to wire shut the damper and not use the fan, until fixed.

The lifesaving plan onboard the vessel does not accurately reflect the amount of lifejackets onboard. Update plan to reflect the change in amount and location of lifejackets. Submit plans to the Officer in Charge, Marine Inspections.
