Each lifebuoy on a towing vessel must be marked in block capital letters with the name of the vessel. Inspector noted buoys had missing/faded vessel names and retroreflective materials peeling off. Vessel rep is required to rectify the proper markings and retroreflective materials on the ring buoys.

Functional Requirements. A towing vessel's survival craft must meet the functional requirements of paragraph (b)(1) through (5) of this sections. Inspector noted expired life raft on board. Vessel rep is required to service the life raft or remove it from the vessel as it is not required to be on board for vessel's routes and conditions.

Each cable and wire on a towing vessel must be installed to meet the following paragraph (1) through (11) of this section. Inspector noted exposed wiring found in the following location: behind the radio on the stbd side of the bridge, outlet in the galley next to the refrigerator, and the battery bank outside the bridge port side. Vessel's rep is required to rectify exposed wiring by next drydock.

Electrical equipment and electrical cables must be suitably protected from wet and corrosive environment. Exposed lights with no covers were noted in the following location: stbd exterior walkway on 1st deck, all lights in berthing spaces, and a light in the bridge. Vessel's owner/operator is required to provide covering for exposed light boards by next drydock.

The vessel has been both in satisfactory service insofar as structural adequacy is concerned and does not cause the structure of the vessel to be question by either the OCMI, or TPO engaged to perform an audit or survey. Inspector noted structural issues in the following: bottom 2" of the entire superstructure attachment just above the main deck is heavily corroded, structure just below the attachment to the first deck above main deck appears to have temp repairs IVO the galley entry door on the stbd side, first deck above the superstructure deck is corroded in multiple locations, and the port and stbd triangle brackets attached to the center steering cylinder post are fractured at the toe of the weld attachment to the compensation plate on the main deck. Repair the the satisfaction of the MI.

There must be an installed or portable bilge pump for emergency dewatering. Unable to draw suction from installed bilge pump. Demonstrate proper operation prior to vessel operations. 46CFR 143.275

Fuel system requirements for towing vessels require filters and/or purifiers. Where filters are used, the fuel filters must be replaced in accordance with manufacturer's requirements. Racor filters are installed with clear plastic sight glass without protective metal flame bowls. Plastic covers only meet the manufacturer's specifications when installed with the metal flame bowls in a marine application. Install flame bowls to meet the requirement. 46 CFR 143.255

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. Paper charts were out of date. Replace/update charts to current standard.46 CFR 140.705

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Fuel leak from starboard aft fuel tank inspection cover bolts. Make temporary repairs and permanent repairs during next dry dock. 46 CFR 143.200(a)

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. First aid kit and eyewash station were both expired. Replace expired items.46CFR 140.435

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. Compressor missing a guard over rotating belt. Install adequate guard. 46CFR 144.820

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Water pump leaking on Port generator. Repair to stop leaking.46CFR 143.205(a)

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and must be either: (1) Automatic, battery-operated with a duration of no less than 2 hours; or (2) Non-electric, phosphorescent adhesive lighting strips that are installed along escape routes and sufficiently visible to enable egress with no power. Inadequate emergency lighting noted in engine room and galley. Install additional lighting.46CFR 143.410(b)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Multiple lighting fixtures were found missing covers. Install covers.46CFR 143.400

Any piping system that penetrates the hull below the waterline must be fitted with an accessible valve, located as close to the hull penetration as is practicable, for preventing the accidental admission of water into the vessel either through such pipes or in the event of a fracture of such pipe. Soft patch found on fire main. Make permanent repairs at next dry dock.46CFR 143.2050(d)

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems must be maintained to ensure proper operation. Vessel reported failure of oil filter gasket and subsequent shutdown of the port main engine. Make repairs and demonstrate proper operation of propulsion machinery to the satisfaction of an attending CG marine inspector prior to towing operations.

Each towing vessel must comply with the requirements of part 4 of this chapter for reporting marine casualties and retaining voyage records. Vessel failed to report above marine casualty immediately after addressing any resultant safety concerns. Vessel is directed to immediately notify the Sector Columbia River Command Center for any involvement in future, potential marine casualties.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The vessels auxiliary generators do not have alarms visible in the wheelhouse. Provide operable alarm system for axillary generators. Complete prior to 20 July 2019.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Cables supplying lighting fixtures located in forward compartment and aft compartment were disconnected and hanging from the overhead. Wires were not energized at the time of the inspection, but were still connected to the breaker panel. Light fixtures were broken and missing covers. Renew and properly wire lighting fixtures. Complete prior to 20 July 2019.
