No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel shall complete an Internal Structural Examination and repairs satisfactory to the marine inspector prior to departure.

All tank vessels, the construction or conversion of which is started on or after July 1, 1951, shall conform to the requirements in this subpart in construction of hulls. Barge had 03 pinholes in port stern knuckle (14' forward of stern log at 2' draft) and 03 pinholes in starboard stern side plate weld seam (5' forward of stern log above the loaded waterline). Barge was not known to have been involved in any incidents that may have resulted in damage and damage appeared to be "normal wear and tear". Divers attended on 09OCT22 and applied marine epoxy patches to damage areas as a temporary repair, no further water ingress reported. Barge is granted permission to move for on October 19 or 20, Barge will transit from Turn Services Michoud Fleet, LA to Copper Fleet (Mobile River Mile 3),Mobile AL on October 21 or 22. to off load cargo, then immediately proceed to [name withheld] Yard in Chickasaw, AL for permanent repairs. Repairs must be made to the satisfaction of the attending CG Marine Inspector prior to loading of cargo. 46 CFR 35.60-1(a)

No extensive alterations involving safety of a tank vessel either in regard to hull or machinery shall be made without the approval of the commandant. A single crack was identified at the seem of the bilge knuckle to sidewall approximately 3” in length. Once the barge has offloaded cargo, the owner or operator shall survey the barge to assess for further damage or leaks and must notify the CG of their findings. The barge may then proceed directly to Three Mile Drydock and Repair. A repair proposal shall be submitted to CG and approved by CG prior to making any repairs.

Water ingress into #1 and #2 P/S Wing tanks was reported. Vessel to discharge cargo at Olin Facility in McIntosh, AL. A temp repair proposal shall be submitted to OCMI, Sector Mobile. Once temp repairs are completed to the satisfaction of Marine Inspector, the barge shall proceed directly to 3 Mile shipyard in Mobile, Al for permanent repairs. A portable pump shall be provided for transit to 3 Mile Drydock and crew must conduct routine inspections of the #1 & #2 wing tanks to ensure there is no ingress of water during transit. Notify Sector Mobile upon arrival to 3 Mile Shipyard. Provide permanent repair proposal to approved by attending Marine Inspector.

Vessel must have approved Vessel Security Plan. Vessel must provide approved Vessel Security Plan prior to cargo operations.

Replace coupling flange on the #1 forward cargo hatch prior to next ISE scheduled for 15JUL2015.

VESSEL IS TO PROCEED TO PETRO UNITED, MILE 200.0 AHP AND OFF LOAD CARGO. _x001C_ TOW BOAT DOUGLAS IS TO PUSH THE BARGE BOW END FIRST. DISCHARGE PUMPS AND STERN VOID ARE TO BE CHECKED EVERY TWO HOURS.

AFTER OFFLOADING CARGO, VESSEL IS TO PROCEED TO PLAQUEMINE SHIPYARD, FOR _x001C_ REPAIRS. REPAIRS AND AIR TEST TO BE DONE TO THE SATISFATION OF COGNIZANT_x001C_ OCMI.

RENEWED ALL BILGE KNUCKLE EXCEPT FOR APPROX. 30' ON STBD SIDE #1 & #2 WING VOIDS.

SLOPE SHEETS RENEWED AS ORIGINAL.

REPAIR AND REPLACE CARGO PUMP. MAINTAIN THE BLANK FIT UP TO THE MOUNTING FLANGE UNTIL THE PUMP IS REPLACED.
