Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port. During the hull walk, the Port and Starboard ring buoy(s) were discovered missing the name of the vessel and the home port of the vessel.

Before the vessel leaves port and at all times during the voyage each lifesaving appliance must be in working order and ready for immediate use. The ring life buoy on the bridge needs grab lines replaced, markings renewed & retro reflective material renewed.

Each life raft or group of life rafts must be arranged for float free launching. During the inspection of the HRU unit it was discovered that the expiry date (2 years past the date of install) was not properly marked on the unit.

Vessel navigational charts are currently not up do date. Ensure updated paper charts are provided at minimum within 6 months of USCG inspection.

Vessel shall provide proof of safety radio certificate.

Vessel Master shall provide medical certificate in conjunction with his license.

Immediately after the addressing of resultant safety concerns, the owner, agent, master, operator, or person in charge, shall notify the nearest Sector Office, Marine Inspection Office or Coast Guard Group Office whenever a vessel is involved in a marine casualty consisting in a loss of main propulsion, primary steering, or any associated component or control system that reduces the maneuverability of the vessel. The owner/operator failed to notify Sector New York that the vessels shaft and broke. Owner/operator to submit a CG2692 prior to 28 January 2024, as a result of the shaft break.

It was discovered that the vessel was hauled out at North River Shipyard, to have a shaft replaced. The local Officer in Charge, Marine Inspection (OCMI) was never notified of the haul-out or of the damaged shaft. The master, owner, operator, or agent of the vessel shall notify the OCMI whenever the vessel is to be dry-docked regardless of the reason for dry-docking, the OCMI, upon notification, will determine whether to assign a marine inspector to examine the underwater hull of the vessel.No repairs affecting the safety of the vessel shall be made without the knowledge of the Officer in Charge, Marine Inspection (OCMI), when necessary, the repairs or alterations must be inspected by a marine inspector. **Prior to going back in the water** (a) A USCG Marine Inspector is to attend the vessel to examine the installation of the repairs or replacement made to the vessel shaft. **MUST CALL TO SCHEDULED** (b) Provide to the OCMI the following: 1) What was the cause of the need to replace the tail shaft 2) The date, time and location when damage to the tail shaft was discovered. 3) The name of the Master of the vessel when the damage occurred.

Existing structure previously approved will remain satisfactory so long as it is maintained in good condition. Minor repairs and alterations may be made to the same standard as the original construction. Four small portholes above engine room have missing wing nuts required for tightening and closure against the weather elements. Additionally, the porthole frame starboard side aft has separated from the steel substructure.

No means shall be provided for locking doors giving access to either of the two required means of escape. Observed a slide bolt type lock on the emergency escape door from the engine room to the wheel house. Remove lock to ensure accessibility for escape from engine room.

The inspector shall examine all accessible parts of the vessel's hull to be assured that it is in a satisfactory condition. In the inspection of hulls the standards established by the American Bureau of Shipping apply. During next dry dock, descale and treat forward peak, void space #1 and lazerette/steering space.

Logbook must include information on emergency training drills. Logbook entries must include date & time of drill, survival craft & fire extinguishing equipment used in the drills, inoperative or malfunctioning equipment & the corrective action taken, identify crew members that participated, & the subject of the drill. Log book entries do not provide sufficient details aside from date & name.

On vessels less than 20m in length, sidelights shall be fitted with matt black inboard screens. Vessel navigation lights are set against a white background.

1. Submit plan addressing how vessel will recover "helpless" man overboard.

F/O return piping on main engine weaping fuel. Repair/replace.

Mark lifejacket storage IAW 46CFR199.70

Provide lifjacket donning instructions.

Provide lifering buoys IAW COI.

AA-04 Reduction gear oil hoses to cooler is of improper material. Replace IAW 46CFR56.60-25

Properly insulate dry exhaust pipe IAW 46CFR58.10-10

Provide FCC radio station licsens.

Remove rubber hose to L/O gauge in wheelhouse & install tubing.

Repair the leak on the salt water ovbd pipe check valve.

Install support bracket on teh salt water discharge pipe on the MDE.

Remove duct tape from the MDE exhaust lagging.

Remove the non-mettalic flex hose from the bilge/firemain system and mount the piping.

Prove proper operation of the A/C generator to the satisfaction of attending Marine Inspector.

Renew steering cable

Renew line on the block and tackle for the emergency steering tiller.

Install cable raceway & metalic cable hangers in the E/R to get all the electrical cable off the piping.

Make all cable splices IAW 46CFR111.

Provide blank covers in the 32 VDC distribution panel.

Remove the wooden deck plates from compartment #2

Proper mount the portable fire extinguishers below decks.

Remove the rubber hose on the F/O return line & install hard pipe as discussed.

Properly install inflatable raft.

Install hydrostatic release on inflatable raft.

Make top of house weathertight.

Make all junction boxes weathertight

Make repairs to port flood light

Make repairs to stbd flood light

Post COI with all pages visible.

Post stability letter.

Make repairs to magnetic compass light.

Provide gaskets for all three (3) deck hatches.

Prove proper operation of aft deck watertight hatch.

Port stern qtr hull plating at shear strake holed and wasted. C/R for credit hull exam.

ID'd various transverse framing in lazarette wasted. C/R for credit hull exam.

Replace wasted deck plating on port quarter.

Replace Port, aft bridge Window.

Remove repair protruding steel where the bulwark is wasted.

Renew/replace gaskets on deck hatches and make water tight.

Label heavy load area on main deck.

Label main deck to restrict crew from going fwd of the pilot house while underway.

Replace and make weather tight, the port, aft, port hole.

Prove proper inspection of the cold water liferaft prior to sailing on or after 15OCT01.

Remove or properly terminate, dead end wires under bridge console.

Do not operate generator until repaired to the satisfaction of the attending MI.

Prior to installing bridge heater, submit plans to USCG for review.

Repair and make weather tight engine room vent covers, stbd side aft and ontop of cabin.

Renew/repair shaft packing on main propulsion shaft.

Complete work to hand rails on stbd side of vessel.

Mount shore tie cable, make secure from tension and provide bracket, make weathertight.

Label fuel oil cut off valve on deck.

Label fire/bilge manifold.

Weld aft 2 deck griders in compartment #3

Permenantly mount overhead lights in compartment #3 with approved wiring, submit plans if new installation.

Provide full penatration welds in compartment #3 where hatch was removed.

Repair / replace gasket & closure dogs on engine room overhead hatch cover.

Repair leak on main engine raw water pump.

Permanently label bilge/fire main manifold.

Make emergency steering arrangement on deck watertight.

Install proper stuffing tubes on engine room cable penetrations as needed.

Provide 1 assitional ring bouy IAw 46 CFR 199.640(i).

Provide 1 additional BII fire extinguisher for machinery space IAW 46 CFR 95.50-10(a).

Install clamps / secure cables on mast, minimum every 24 inches.

Provide sufficient liferaft on board vessel IAW 46 CFR 199.640 prior to 31OCT00.

Provide up to date log book for vessel. Show logging of required fire and man overboard drills.

Provide garbage pollution placard.

Repair aft bulkhead in wheelhouse; ensure compliance with structural fire protection IAW 46 CFR 92.07-10.

complete all worklist items from plan review worklist.

Replace missing cover on generator connection box.

Install voltmeter and ammeter for generator.

Replace A/C electric sub-pump with hand operated portable pump.

Mount ammeter between alternator and 32V battery bank.

Hard mount 12V charger for generator starting battery.

Replace #12 AWG cable from 32V battery bank to 12V house panel with #5 AWG conductors.

Install interlock between generator and shore tie breakers.

Submit plans for diesel cabin heater prior to installation or operation.

Mount shore tie connection from power panel.

Install remote f/o cut-out-valve in supply line to generator, capable of being closed from outside compartment.

Provide means of closing lower compartment vents IAW 95.15-10
