It was discovered that the vessel was hauled out at North River Shipyard, to have a shaft replaced. The local Officer in Charge, Marine Inspection (OCMI) was never notified of the haul-out or of the damaged shaft. The master, owner, operator, or agent of the vessel shall notify the OCMI whenever the vessel is to be dry-docked regardless of the reason for dry-docking, the OCMI, upon notification, will determine whether to assign a marine inspector to examine the underwater hull of the vessel.No repairs affecting the safety of the vessel shall be made without the knowledge of the Officer in Charge, Marine Inspection (OCMI), when necessary, the repairs or alterations must be inspected by a marine inspector.
**Prior to going back in the water**
(a) A USCG Marine Inspector is to attend the vessel to examine the installation of the repairs or replacement made to the vessel shaft. **MUST CALL TO SCHEDULED**
(b) Provide to the OCMI the following:
1) What was the cause of the need to replace the tail shaft
2) The date, time and location when damage to the tail shaft was discovered.
3) The name of the Master of the vessel when the damage occurred.
