RESOLVE IBIS

Tank vessels over 100 gross tons using navigable waters of the United States must have a valid certificate of financial responsibility (COFR) issued by the USCG. Owner/operator shall provide proof of a valid COFR for vessel.

Records to verify that cargo pump relief valves were tested at least once each year to determine that they function satisfactorily at the pressure at which they are set to open, were not available during inspection.

Load lines did not appear to be permanently marked on each side of the vessel; only painted on the hull. Load lines must be permanently marked on the sides of vessels to the satisfaction of the assigning authority. Due at next drydock.

Vessels are required to have a valid Vessel Secuirty Plan or Alternate Secutiry Plan approved by USCG. Provide security plan that is current and valid, or provide CG-approved alternative/exemption through the Marine Safety Center. 33 CFR 104.105(a)(8)

Cargo piping must be tested 1.5x MAWP annually; vessel must test its piping and mark accordingly.

Cargo hatches must be marked "DANGER - KEEP OUT;" some cargo hatches' signage missing and must be replaced.

Electrical installations must be installed IAW 46 CFR Subchapter J; alarm panel must be fitted with blanks to protect against shock hazards.

Exposed and dangerous machinery places must be properly protected; exposed exhaust pipin on #3 and #4 gnerators must be properly insulated to prtect personnel from hazard.

Vessel's dry dock will expire on 31 May 2021. After 31 May 2021, the vessel shall remain out of operation until a dry dock inspection has satisfactorily been completed by the USCG. After 31 May 2021 the vessel shall proceed directly form Dutch Harbor, AK to Jag Shipyard in Seward, AK; voyage shall not to exceed 07 June 2021. The above shall be completed to the satisfaction of the attending USCG Marine Inspector.

Annual surveys for endorsements. Vessels subject to initial and periodic surveys shall have annual surveys, within 3 months either way of the certificate's anniversary date. The annual surveys shall be made by and prove satisfactory to the assigning and issuing authority prior to executing the required annual endorsements on load line certificates. Barge's International Load Line Certificate was issued on 30 May 2019 and has not had an annual endorsement. The Barge is not to travel past the U.S. Boundary Line. ABS needs to endorse the annual survey. Demonstrate that an annual endorsement has been conducted on the International Load Line Certificate, to the satisfaction of the attending USCG Marine Inspector in order to clear this.

The deck line is a horizontal line 12 inches and 1 inch in breath. It shall be marked amidships on each side of the vessel, and its upper edge shall normally pass through the point where the continuation outwards of the upper surface of the freeboard deck intersects the outer surface shell. Barge is missing the deck line. In accordance with the International Load Line Certificate, apply a permanent deck line to both Port and Stbd sides of the barge, to the satisfaction of the attending USCG Marine Inspector.

Cargo pump pressure gage shall be tested at least once a year for accuracy. Barge's pressure gages did not have recent test date. Provide current gage test documentation to the attending USCG Marine Inspector.

Vessels portable fire extinguisher located in the pump room (below deck) had not been serviced since June 2017.

Vessel's transfer hoses were not marked with intended use or hydrostatic testing, as required by 33 CFR 155.800.

Required to have periodic inspection within 90 days of anniversary date. O/O failed to have inspection conducted to maintain valididy of COI. Barge is restricted from operating until a satisfactory inspection is completed.

#3 cargo pump emergency stop inoperable.

Since the vessel's last annual load-line survey, by ABS on 10JUN16, five 20-foot portable storage containers were added on the top of the vessel's deck. Company shall verify acceptability of such containers on deck for stability and load-line purposes.

Vessel's side navigation lights were not working properly. All navigation lights shall be made to work/illuminate properly.

Vessel's starboard-side deck hatch to the pump room did not close to be water-tight. Hatch shall be made to close to be water-tight.

Vessel's portable fire extinguishers were last serviced in June, 2015. All portable fire extinguishers shall be serviced, tested and inspected as required.

Vessel's transfer hoses did not have documentation for annual testing and all the hoses and vessel transfer piping were not labeled to relfect such (test date, etc.). All transfer hoses shall be tested and all hoses and piping labeled per requirements.

Vessel did not have documentation that the cargo pump relief valves or pressure gages had been tested annually. All relief valves and pressure gages shall be tested.

Vessel's pipeline end for skimmer operations was not blanked-off, when not in use. Pipeline end shall be blanked-off when not in use.

Make barge available for inspection including internal structural exam-prior to placing barge in service.

Provide marine chemist to certify barge is safe for entry to satisfaction of attending marine inspector.

Monitor barge for water ingress until damage survey complete.

REMOVE WIRE NUTS AND PROVIDE PROPER CONNECTORS FOR HOUR METER LOCATED ON THE STARBOARD SIDE OF THE GENERATOR COMPARTMENT.

REMOVE DEAD ENDED WIRE HANGING FROM THE OVERHEAD ON THE STARBOARD SIDE OF THE CARGO PUMP ROOM.

REMOVE IMPROPERLY INSTALLED CABLES AND PROVIDE PROPER INSTALLATION OF OUTPUT LEADS FROM BATTERY CHARGER IN GENERATOR ROOM.

MAKE HAILING PORT ON STERN READABLE.

REPAIR OR REPLACE WATER TEMPERATURE GAUGE AND OIL PRESSURE GAUGE ON #1 CARGO PUMP ENGINE.

REPAIR SIGHT GLASS AND PROVIDE GUARD ON HYDRAULIC OIL TANK.

PROVIDE PV VALVES FOR TANKS #1 AND 2 PORT, AND #1,2,3,AND 4 STARBOARD.

PROVE PROPER OPERATION OF STERN LIGHT AND STBD RUNNING LIGHT.

PROVIDE GUARD RAILS FOR BARGE AS PER 46 CFR 32.01-10(C).

INSTALL PROTECTIVE COVER ON SIGHT GLASS ON DIESEL FUEL TANK.

PROPERLY SECURE, GROUND AND MAKE PROPER ELECTRICAL CONNECTIONS TO ALL GALLEY EQUIPMENT.

COVER OPEN SPACES IN ELECTRICAL PANEL IN OFFICE.

PROVE PROPER OPERATION OF OIL PRESSURE GAUGE ON #1 DIESEL ENGINE.

REPAIR OR REPLACE HATCH COVER ON STARBOARD BOW.

REPAIR AND PROVE OPERATION OF NAV. LIGHTS.

TEST PRESSURE RELIEF VALVES ON CARGO PUMPS.

INSTALL ELECTRIC MOTOR ON CARGO PUMP ROOM VENTILATION AND PROVE OPERATION OF SAME.

MAKE REPAIRS TO OVERSPEED TRIP ON S/S GENERATOR; MAKE POSITIVE CONNECTION TO SWITCH.

REPLACE THE MOUNTING FOUNDATIONS FOR BOTH THE STBD AND PORT RUNNING LIGHTS.

INSTALL A SEATING SURFACE TO HOLD FLAME SCREEN IN PLACE ON THE MANHOLE COVER S-3 TANK.

REPLACE THE MUFFLER ON THE S/S GENERATOR.

CORRECT VESSEL DOCUMENT TO REFLECT NEW OWNERS' NAME AND ADDRESS.

REPAIR OR REPLACE THE STARBOARD RUNNING LIGHT.

PROVIDE [name withheld]/LB WITH PLANS FOR REVIEW FOR NEW ELECTRICAL INSTALLATION TO AFTER DECK HOUSE.

MAKE PERMANENT REPAIRS TO HOLD VEN, 3 PORT AT DRY DOCK.
