A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or defect is discovered which affects the safety of the vessel. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel ran aground and is currently leaking oil from stbd Z-drive. Conduct damage assessment and make proper repairs as necessary to the satisfaction of Marine Inspector prior to commercial operatins.

Bulkheads of galleys shall be of "A" Class construction. "A" class boundaries shall be constructed that if subjected to the standard fire test, they would be capable of preventing the passage of flame and smoke for one hour. Class "A" boundary in galley not maintained due to warped gasket and frame. 46 CFR 92.07-5(b) 46 CFR 92.07-10(b)

Seafarers employed or engaged on a seagoing ship shall receive approved security-related familiarization training. 02 crewmembers without adequate security familiarization training needed for STCW. STCW A-VI/6

The following automatic shutdowns are required for the emergency generator set: Overspeed. The Periodic Safety Test Procedures dated April 18, 2016 did not include a test procedure for an overspeed shutdown. Provide satisfactory evidence of testing the overspeed shutdown to the satisfaction of the Marine Inspector.

Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must include electric power generation and distribution. The Periodic Safety Test Procedures dated April 18, 2016 did not include a test procedure for an overspeed shutdown. Submit revised procedures to the Marine Safety Center for approval.

On vessels required by 112.05-5(a) to have a temporary emergency power source, the following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source: (c) lighting for escape trunks in machinery spaces and work spaces sufficient to allow crew to find their way to open decks with all watertight doors and fire doors closed. Emergency escape trunk in upper level of starboard mezzanine not sufficiently illuminated.

For ships constructed on or after 1 July 2014, a minimum of two two-way portable radiotelephone apparatus for each fire party for fire-fighter’s communication shall be carried on board. Those two-way portable radiotelephone apparatus shall be of an explosion -proof type or intrinsically safe. Ships constructed before 1 July 2014 shall comply with the requirements of this paragraph not later than the first survey after 1 July 2018. Vessel was not outfitted with portable radiotelephone apparatus for fire fighters. SOLAS 74(20) II-2/10.4

The means of control of any fixed gas fire extinguishing system shall be readily accessible and simple to operate and shall be grouped together in as few locations as possible at positions not likely to be cut off by a fire in a protected space. At each location there shall be clear instructions relating to the operation of the system having regard to the safety of personnel. CO2 remote activation on bridge does not contain system operation instructions. SOLAS 74(97) II-2/5.1.7

The revised NOx Technical Code 2008 shall be applied in the certification, testing and measurement procedures for the standards set forth in this regulation. In accordance with Regulation 13.2.1.1 of MARPOL Annex VI all engines over 130kw placed on board the vessel after 1 January 2000 must meet emission standards set forth by the convention. Chapter 2.1 of the NOx Technical Code 2008 states that this is certified by issuance of an EIAPP Certificate(Engine International Air Pollution Prevention Certificate). The vessel currently does not have EIAPP certificates for the Port and Starboard Jet Pumps. Prior to sailing foreign provide EIAPP and Technical Files for each engine. MARPOL 73/78 (2011) Annex VI Reg. 13.8

For each ship subject to regulations 5 and 6 of this Annex, details of fuel oil for combustion purposes delivered to and used on board shall be recorded by means of a bunker delivery note that shall contain at least the information specified in appendix V to this Annex. The bunker delivery note shall be kept on board the ship in such a place as to be readily available for inspection at all reasonable times. There was no bunker delivery note for the fuel on board the vessel at the time of inspection. MARPOL 73/78 (2011) Annex VI Reg. 18.5 and Reg. 18.6

The generator set must shut down automatically upon loss of lubricating oil pressure, overspeed, or operation of a fixed fire extinguishing system in the emergency generator room. The emergency generator set did not shut down automatically upon testing of the fixed CO2 in the E-Generator room space. During the test of the CO2 system, all test CO2 was taken up by the engine and blown out of the exhaust stack, without the engine ever shutting off. 46 CFR 112.50-1

Each panelboard must have: A circuit directory cardholder; and A circuit directory that has: The circuit designation of each circuit; a description of the load of each circuit; and the rating of settin of the overcurrent protective device for each circuit. The forward panel board located in the port hydraulic room contained inaccurate information and was unclear as to which breakers were spares and which were being utilized. Insure all panels on board are updated to reflect the current approved electrical drawings. 46 CFR 111.40-11(b)

Navigating bridge, ECC, maneuvering platform, and manual alternate control locations must include- Shaft speed and thrust direction indicators. The RPM gauges for the main engine controls located on the main deck in the engine rooms were not operable. 46 CFR 62.35-5(b)(3)

Each vessel must be provided with a sound-powered telephone system, complying with subpart 113.30 of this chapter, to communicate between the pilothouse and the steering-gear compartment. The sound-powered phone located in the port engine room had a broken call button. 46 CFR 58.25-15

A ship must have a fixed container or enclosed deck area under or around each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe, that: for a ship of 1600 or more gross tons has a capacity of one barrel. There were multiple fixed containments throughout the vessel missing the plug for the containment, not allowing for the full capacity as required. 33 CFR 155.320(a)(2)

To assist in casualty investigations, cargo ships shall be fitted with a VDR which may be a simplified voyage data recorder. S-VDR battery failed test by technicians and parts are still being sourced. Provide technician report that the battery has been replaced and the system has been tested and certified fully functional. 74 SOLAS (2014) V/20.2

The first aid kit in a rescue boat must be approved under approval series 160.041. First aid kits on board the vessel all had expired medication.

All ships, irrespective of size shall have, when the ship's bridge is totally enclosed, a sound reception system, or other means, to enable the officer in charge of the navigational watch to hear sound signals and determine their direction. No sound reception system was fitted onboard the vessel as required. 74 SOLAS (2014) V/19.2.1.8

The grab line shall be 3/8 inch diameter polyethylene, polypropylene, or other suitable material having a minimum breaking strength of 1,350 pounds. The grab lines on two of the vessel lifebuoys were found wasted beyond repair. Vessel is to replace the grab lines to the satisfaction of a USCG Marine Inspector.

Electric installations on vessel must ensure: protection of passengers, crew, other persons, and the vessel from electrical hazards. Vessel had an installed outlet directly above cooking equipment that was accumulating hot oil and grease. Vessel is to remove and replace the outlet to avoid fire and water hazards to the satisfaction of a USCG Marine Inspector.

Machinery driving fuel-oil transfer pumps must be fitted with remote controls from a readily accessible position outside the space concerned so that the pumps may be stopped in case of fire in the compartment in which they are located. There is no remote control to stop the fuel-oil transfer pump for filling the EDG fuel tank. Vessel shall install a means of remote control stop of the fuel-oil transfer pump by next scheduled dry dock.

Bridge buoys with attached smoke/lights were attached to hand rail on the inboard side so as not to be able to be easily cast loose. Additionally, there was a deck located underneath the drop path of the buoy located on the port side. Issued 835 SOLAS III/7.1.3 (30 days).

The engine powering the starboard dredge pump was leaking oil and discharging directly into the waterway. The lube-oil shutoff switch was cracked, causing the leak.

Replace damaged fire sleeves and properly install on all required hoses on all generators and main diesel engines mas per SAE J-1942 46CFR56.60-25(b)

Repair hydraulic oil leak at electrical position sender on port Z drive.

Repair exhaust leak on inboard main generator turbo outlet.

Replace float light on ring bouy, outside of Bos'n locker. Ship's crew ground away part of case so it would clear the mounting bracket attaching bolt. Case cracked at ground out area.

Prove proper operation of OWS. OWS did not cycle the water properly.

Remove mechanical holdbacks from all fire boundary doors. 46CFR92.07-10(d)(4) Many of the fire boundary doors, including stairwell doors had hold back clips installed allowing required fire doors to be held open with no way to release them from the bridge.

Adjust all fire boundary doors so they will close properly.

Properly label all CO2 systems to identify space protected. Provide directions for use that match the system. Provide schematic of system and any alternate method of discharge. 46CFR95-15-10(h)

Provide proposal for safe means of local discharge of EDG space CO2 system. 46CFR95.15-10(c) To activate EDG CO2 system locally, crew member would need to straddle an open ladder way to access the CO2 bottle.

Provide low lube oil shutdowns on both MDE'S and prove proper operation IAW 46 CFR 58.05-10.
