THE OWNER OR MANAGING OPERATOR SHALL SUBMIT DRAWINGS, SKETCHES, OR WRITTEN SPECIFICATIONS DESCRIBING THE DETAILS OF ANY PROPOSED ALTERATIONS TO THE COGNIZANT [name withheld] MUST BE APPROVED BY THE [name withheld] IS STARTED. THE GANGWAY INSTALLATION IS NOT APPROVED. MSC REQUIREMENTS MUST BE MET. A PROOF TEST ON EACH VESSEL WITH AN INSTALLED GANGWAY MUST BE WITNESSED BY THE COAST GUARD PRIOR TO ALLOWING PASSENGERS USE OF EACH GANGWAY.

Repairs or alterations to the hull may not be made without the approval of the cognizant [name withheld] vessel is to proceed to Morgan City, while affixed to the deck barge, with no passengers or cargo on board. Cognizant OCMI (MSU Morgan City) shall be notified when the vessel arrives in port and advised of plans for temporary and/or permanent repairs to the hull prior to lowering the vessel back in the water or making those such repairs.
