Clean or relace life jackets on 01 deck to the satisfaction of attending CG Marine Inspector. passenger count reduced to 204 adult and 20 children until cleared.

The vessel's main steering gear and rudder stock must be of adequate strength and capable of steering the vessel at maximum ahead service speed. The SILVER LAKE is unable to adequately maneuverer. Ensure rudder is capable of moving from 35 degrees on one side to 30 degrees on the opposite side (at max service speed) within 28 seconds.

Indication of the rudder angle must be provided both at the main steering station in the pilothouse and in the steering compartment. Rudder angle indicator is off by 7 degrees in the pilothouse.

Vital systems that are automatically or remotely controlled must me provided with - (1) An effective primary control system... Port and Stbd mde throttle controls are extremely mismatched; port engine at wot is 100 rmp below stbd.

Vessel must comply to requirements issued by the FCC. Prove the DSC function of the radio is operational with vessels MMSI.

An EAP training program must be conducted for employers supervisors. Captain has not conducted training.

The damper shall be made accessible for periodic inspection by means of a hinged or bolted plate in the duct. Two dampers had bolts missing and/or loose.

The sidelights of vessels of 20 meters or more in length shall be fitted with mat black inboard screens and meet the requirements of 33 CFR 84.15. Port and Starboard running lights were not properly installed with mat black screens.

The installation of all systems of a marine engineering nature together with the details off design, construction and installation shall be in accordance with the requirements of subchapter F. Pressure fuel piping not properly secured on the portside generator.

Bow thruster is inoperable

Port and starboard aft outboard fire dampeners are not closing as designed.

The bow thruster accuator has missing dust cover.

The crew berthing and passenger lounge has loose and damaged tiles, creating slips and trips hazards.

The port and starboard propellers have reported damage and caused excessive vibration to vessel during operation.

Renew wasted EDG fuel piping & attached hoses at the next DD.

Correct vibration issue IWO propulsion shafting which occurs during light deck loads & shallow water. Provide dive report & report of tailshaft alignment.

#4 Life raft (IBA) hydro is not marked on expiration.

Rescue boat ladder was found damaged during COI.

Bow thrusters emergency stop button was inoperative when tested during COI.

Several tiles in the (02) passenger lounge are damaged, creating slips, tripping, and falling hazards.

The vessels aft fire pump and sprinkler system could not provide pressure as designed during COI when tested.

Volton frive coupling in stbd clutch failure.

Engine control room speaker missing horn and electrical wires exposed.

STBD fwd outboard sprinker clogged.

Ammonia packets expired. The box had one date of 02/2017 and a sticker was palced over it with a dat of 2022.

#1 air receiver had corrosion and was need of clean out for further inspection this date. Company to provide documentation of U1A for max allowable corrosion limits.

The emergency lighting at #3 life raft embarkation station was tripping the breaker.

Cut section of plating approximately 51" wide port side 12 feet from centerline crossing frames 104 and 105. Cut section of plating approximately 48" X 51" below the chine 8 feet off of center line also crossing frames 104 and 105. Replace with 3/8" steel due to excessive pitting.
