On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces:(1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette;(2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and(3) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. The lazarette's and engine room's high water visual and audible alarms at the operator's station is inoperable. Make repairs and prove proper operation to the satisfaction of the attending Marine Inspector prior to the carriage of passengers.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible;(2) U.S. Coast Pilot or similar publication;(3) Coast Guard Light List;(4) Tide tables; and(5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority.(b) Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Provide up to date U.S. Coast Pilot [name withheld] List, and Broadcast Notice to Mariners.

Gauges to indicate engine RPM, jacket water discharge temperature, and lube oil pressure must be provided for all propulsion engines installed in the vessel. The port engine lube oil pressure game and stbd fuel tank gauge is inoperable. 46 CFR 182.410(b)

As appropriate for the intended voyage, a vessel must carry adequate and up to date Charts, US Coast Pilot, CG light list and tide tables. The CG light list was not up to date. 46 CFR 184.420(a)(3)

Port side fuel tank emergency shut off is broken and unable to close.

EXCEPT AS OTHERWISE PROVIDED IN THIS SECTION EVERY VESSEL MUST BE FITTED WITH A SUITABLE MAGNETIC COMPASS DESIGNED FOR MARINE USE, TO BE MOUNTED AT THE PRIMARY OPERATING STATION. REPLACE UNREADABLE COMPASS WITH AN APPROVED MARINE TYPE.

Found expired eye wash solution in first aid kit.

Found life ring buoy light missing a corrosion resistant clip.

Update current light list.

No current employee list from the consortium onboard.

Ring buoy water light did not have an approved screw type battery.

Provide proof of FCC station authorization license as required by 47 CFR 80.13.

Provide proof of Marine Radio Operators license as required by 47 CFR 80.155.

Replace the battery in the life ring buoy and life floats floating water light with expiration date of DEC 2018 IAW 46 CFR 185.726(d).

Port remote fuel oil cut off valve is not functional, make repairs to the remote cut off as required by 46 CFR 182.455(b)(4).

Clean or replace port and starboard navigational lights, current lights are obstructed by a glaze on the lenses, 46 CFR 183.420.

To maintain a valid Certificate of Inspection, you must complete your annual inspection within the periods specified in 46 CFR 176.500(b)(1) (within 3 months before or after each anniversary date) and your Certificate of Inspection must be endorsed. Anniversary date for 2019 annual inspection is 11 February 2019 allowing for an end of period date not to exceed 11 May 2019. 2019 annual inspection was not completed within the allowed period, deeming the COI invalid. Vessel is prohibited from carrying passengers for hire until a successful inspection is completed and the COI is endorsed.

Provide emergency drill records.

Replace main deck forward hatch seal.

Remove dead-end and improperly spliced conductors. Provide labeled over-current protection for all conductors.

Repair machinery space duct covers.

Provide copy of latest CG light list.

Provide copy of latest Coast Pilot

Crop and Renew de-laminated or wasted portions of the collision bulkhead.

Repair hull locations impacted by penetrating blisters.

Crop and renew wasted portion of swim step support on starboard side.

Grind cracked FRP around forward void hatch cover to allow further inspection. Repair FRP as needed.

Grind cracked corner where main deck and swim step meet on starboard side of vessel to allow for further inspection. Repair as needed.

Indicators for port engine not responding. At time of USCG visit at 1145, to respond to report of indicators, port engine unable to turn.

Demonstrate proper operation of the stbd MDE prior to carrying passengers.

Provide proof that company is part of a drug testing consortum. No sail issued pending completion of annual inspection.

Provide proof that all employees in safety sensitive postions are drug tested(pre-employment and randomly 25%). Prior to carrying passengers.

Life ring wasted, replace with new. Prior to carrying passengers.

Potable water vent missing screen, replace with new.

Potable water line in aft steering is connected with a hose in hose connection. Repair as required.

Single hose clamps found on the sewage overbaord lines. Provide double clamps.

Wood rot found in aft steering compartment. Repair as required.

Port and starboard exhaust pipes in steering gear leaking water. Temprary repairs complete and SAT till 25JUN16

Port engine tachometer inoperable. Repair as required.

Sewage "Y" valve not closing. Repair as required.

VHF radio not properly programmed with MSSI data. Repair as required.

On passenger decks of a vessel on an excursion trip the following must be installed on vessels with 39.5 inch handrails: 1. Bulwarks 2. Chain link fencing or wire mesh that has openings of not more than 4 inches in diameter or 3. Bars, slats, rail courses, or an equivalent spaced at intervals of not more 100 millimeters (4 inches) IAW 46 CFR 177.900 (g)(1) (i)-(iii)

Replace 05 unserviceable type I lifejackets. Vessel limited to 46 persons until lifejacket are replaced on board.

Rotted plywood and delaminated FRP found in aft portion of weatherdeck, both port and starboard sides. Correct prior to credit drydock.

Repair and make watertight rotted portions of collision bulkhead forward.

Ensure both lifefloats (2) are in a float free arrangement when vsl is on voyages >3 miles from shore. (i.e. not tied down).

Provide (1) additional adult type 1 PFD for a total of 48 onboard. Vsl restricted to NMT 47 total persons until corrected.

Provide servicing records (3rd party or in-house) for all portable fire extinguishers.

Submit proposal for recent Fireboy (pre-engineered fire system) replacement.

Repair/replace corroded breakers in DC power panel.

Provide portable hand bilge pump.

Repair Laz WT hatch/scuttle. (dog mechanism)

Clean/arrest stray current corrosion on bonding (green) wire connections on stbd fuel tank, fwd and aft connections.

Replace MSD thru hull ball valve.

Engine room bilge alarm tested & not working.

Tested light with un-sat results.

Provide valid FCC Safety Certificate

Provide sea painter for both lifefloats.

Provide fire buckets of at least 2 1/2 gallons.

Repair starboard lifefloat.

Obtain waterlight for other life float.
