A pre-engineered fixed gas fire extinguishing system must: (1) Be approved by the Commandant; (2) Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector; (3) Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space; and (4) Be installed in accordance with the manufacturer's instructions. The port side engine room's fireboy cylinder is below the required pressure as designed.

Pre-employement testing - Marine employer was not able to provide proof of a drug testing background check in accordance with 49 CFR 40.25. Marine employer to maintain a copy of the form on file and complete for each new hire.

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. The stbd main engine RPM gauge is inoperable.

FCC safety radio certificate was expired.

Ring buoy water light battery is expired. Vessel restricted to daylight operations.

Stbd side Fireboy system will not shut down the engine when restarted after being activated.

Port aft plywood frame for wheel house window is soft and needs to be renewed.

Port & Stbd engine room ventilations is missing covers.

FCC station license and safety certificate has incorrect vessel official number.

Install 30 X 30 flame screen on port and starboard fuel oil tank vents IAW 46 CFR 182.450(e).

Repair/replace starboard engine wiring harness prior to the carriage of passengers.

Repair/replace port and starboard engine tachometers to reflect current engine RPM.

Install state registration numbers on the vessel prior to operations.

Any vessel of at least five net tons which engages in coastwise trade, unless exempt under 46 CFR 67.9(c), must have a Certificate of Documentation (COD) bearing a valid endorsement appropriate for the activity in which engaged. A simplified measurement was conducted using form CG-5397 to validate the vessel's net and gross tonnage while the vessel was out of the water, current tonnages are; 4 Net and 4 Gross. Measurements taken during inspection were: Overall Length(L)= 33', Overall Breath(B)= 12'-4", Overall Depth(D)= 4'-8" and Individual Hull Breadth(b1)= 3'-10" resulting in 6 net tons and 8 Gross tons. New tonnage calculation require that the vessel apply for and be issued a COD with a coastwise endorsement. Attain a valid COD with coastwise endorsement or provide proof that the vessel is under 5 net tons.

Port fuel filter device (Racor) bowl is required to be fitted with a cap or plug to prevent fuel leakage IAW 46 CFR 182.455(a)(9).

Replace 06 adult type I life jackets to align with COI carriage requirements IAW 46 CFR 180.71(a).

Make repairs to the port propulsion engines forward inboard mounts foundation bolt IAW 46 CFR 182.200(a).

Replace the starboard propulsion engines forward outboard engine mount due to metal wastage at next dry docking IAW 46 CFR 182.200(a).

Install turbo charger intake air filters on both propulsion engines to ensure compliance with 46 CFR 182.200(a).

Properly label port fuel oil tank fuel oil shut off valve remote handle IAW 46 CFR 185.608.

Repair the handrail stanchion bases at the port and forward rails due to cracked aluminum at the next dry docking to ensure compliance with the required 200lb point load as required by 46 CFR 177.900(b).

Renew forward lifelines at the bow to withstand a point load of 200lbs as required by 46 CFR 177.900(b).

Renew vital system raw water supply lines to both propulsion engines to ensure the safety and survivability of the vessel as required by 46 CFR 182.710(a)(6).

Vessel operates on a Limited Coastwise route requiring 06 Red hand flares and 06 orange smoke flares, vessel was short 02 Red hand flares and 03 orange smoke flares IAW 46 CFR 180.64(a).

Vessel is required to have at a minimum 60' of lifeline IAW 46 CFR 180.70(c).

Port fuel filter device (Racor) bowl is required to be fitted with a cap or plug to prevent fuel leakage IAW 46 CFR 182.455(a)(9).

Port pre-engineered fire suppression system (Fire boy) has a toggle switch for energizing and de-energizing the ignition circuit for the port main engine and operating station instrumentation causing the automatic shutdown feature of the fire boy system to not function if the switch is in the OFF position. Remove switch from ignition circuit to allow for proper operation of fire protection system IAW 46 CFR 181.420(a)(3).

Renew port and starboard lifelines at the amidships embarkation/debarkation stations to withstand a point load of 200lbs as required by 46 CFR 177.900(b).

Repair/replace visual indicating light at the operating station for the automatic actuation of the port engine compartment electric submersible pump as required by 46 CFR 182.530(c).

Audible discharge alarm at operators station was inoperable.

Provide onboard a copy of the vsl's current stability letter.

Provide proof of current drug testing program (i.e. list of crew enrolled, proof of enrollment).

Replace expired hydrostatic release on EPIRB. Vsl restricted to LBS route until corrected.

Start conducting and logging man overboard, fire, and abandon vsl drills.

Install correct mast light.

Provide a B-II portable fire extinguisher with correct marine bracket. (missing/never installed per COI)

Update emergency broadcast placard to reflect new (as of 2007) call sign.

Provide up to date charts, coast pilot, and light list for areas of operation.

Provide (3) fire buckets at least 2.5 gallon capacity each, w/lanyard, labled "fire bucket".

Provide portable hand bilge pump.

Provide correct pins for Fireboy cable housing(s).

Mark all electrical switches at helm/operating station.

Provide means to close all machinery space(s) natural vents. (vsl fitted w/pre-eng fire system(s))

Provide means to cap unused thru hull fitting in head (when not in use).

Conduct satisfactory u/w MOB, fire, and abandon vsl drills to the satisfaction of the attending CG MI.

Provide current enrollment list for the crew.

Repair aft deck light.

Repair bilge alarm starboard enigne compartment.

Repair lose engine speed throttle mount.

Add metal hangors to all wire runs in engine room.

Light float not working.

Install air silencers on both MDE'S.

Electricly bond both diesel fuel tanks.

Add double hose clamps to salt water circuit both MDE's.

Remove plastic 90 degree elbow from sea suction port MDE and replace with brass type elbow.

Date on new Guam reg is date 1/8/08 and doesn't coinside with vessels sticker, reg mistake from issuing party.

Install ovbd valve with securing device for MSD system.

Demonstrate proper fire and MOB drill.

Post placard for "no sewage discharge with 3 miles of land."

Prove operation of engine automatic shutdown controls for fixed fire extinguishing systems. Until control units to be proven functional, vessel operator to manually shutdown engine(s) at such time that the fixed fire extinguishing system activates due to fire. Prove operation of starboard engine shutdown.

Replace three of the existing smoke pyrotecnic devices that have exceeded their expiration due date.

Prove sufficient ventilation or install additional ventilation for vessel'sengine spaces.

Relocate existing fuel shutoffs or install additional remote fuel shutoff valves at vessel's fuel tanks.

Unknown Component - Unknown Cause/Condition

Replace parted steering cable and repair damage to the port engine outdriveto the satisfaction of the attending marine inspector prior to cariage of passengers.

Plug off existing unused thru hull penetrations located port and starboard hulls, just forward of the engine space to main void bulkhead.

Obtain the necessary navigational charts for vessel's area of operation, Apra Harbor and Island of Guam.

Install closure for opening between starboard engine space and main void toprevent the loss of fire fighting agent in the event of system discharge.

Attach a 3' to 6' lanyard to the ring buoy waterlight. Lanyard to have a clip on the end allowing waterlight to be disattached during daylight hours.

Conduct and log in the vessel log the quarterly man overboard and fire fighting drills required by 46 CFR 185.520 and 524.

Drug and Alcohol Program to be administered by personnel not involved in the possibility of being selected for the random testing.

Bond the fuel fill connections to the fuel tanks.

Update the vessel's 1st Aid Kit to comply with 46 CFR 184.710, or vessel tocomply with the listing of equipment listed on the list provided to owner by the CG inspector.

Prove the proper operation of both life float water lights.

The hydrostatic tests on all on board portable fire extinguishing systems have expired. Conduct a hydro test on each of them or replace as necessary. The vessel is required 2 B-II, 1 A-II, and 1 B-I type extinguishers.

Obtain 6 hand orange smoke distress signals approved by the Coast Guard. These distress signals must be located on the vessel.

Provide an electrical power source to the magnetic compass light to facilitate proper illumination of the compass during night operations

Prove the proper operation of the mooring light to the satisfaction of the local OCMI.

Install second hose clamps on all flexible sea water hoses on the port engine as required.

Obtain and lash 2 paddles to each life float. Paddles must be able to float, and have the vessels name marked/painted in clearly legible letters.

Obtain and post a life jacket donning placard on board the vessel in a location accessible and visible to the crew and passengers.

Update the current FCC Radio Station license to reflect the current owner of the vessel.

Install a flame screen on the starboard fuel tank vent.

Install battery covers constructed of material resistant to electrolyte corrosion of sufficient strength to protect against accidental contact of metallic objects across both battery terminals.

Obtain a valid FCC Safety Radiotelephone Communications certificate. This document shall be located on the vessel at all times.

Otain valid FCC Communications Safety Radiotelephony Safety Certificate. Owner will need to contact local private concern to conduct inspection of unit prior to issuance of certificate.

Obtain and install waterlight for each life float.

Obtain FCC Safety Radiotelephone Certificate, or show proof of application for safety inspection.

Mark location of portable fire extinguishers.

INSTALL DRIP PANS UNDER RACOR FUEL FILTERS IN BOTH ENGINE ROOMS.

INSTALL A TOILET AS REQUIRED BY 46 CFR 177.30-5
