A pre-engineered fixed gas fire extinguishing system must: (1) Be approved by the Commandant; (2) Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector; (3) Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space; and (4) Be installed in accordance with the manufacturer's instructions. The port side engine room's fireboy cylinder is below the required pressure as designed.

Pre-employement testing - Marine employer was not able to provide proof of a drug testing background check in accordance with 49 CFR 40.25. Marine employer to maintain a copy of the form on file and complete for each new hire.

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. The stbd main engine RPM gauge is inoperable.

FCC safety radio certificate was expired.

Ring buoy water light battery is expired. Vessel restricted to daylight operations.

Stbd side Fireboy system will not shut down the engine when restarted after being activated.

Port aft plywood frame for wheel house window is soft and needs to be renewed.

Port & Stbd engine room ventilations is missing covers.

FCC station license and safety certificate has incorrect vessel official number.

Install 30 X 30 flame screen on port and starboard fuel oil tank vents IAW 46 CFR 182.450(e).

Repair/replace starboard engine wiring harness prior to the carriage of passengers.

Repair/replace port and starboard engine tachometers to reflect current engine RPM.

Install state registration numbers on the vessel prior to operations.

Any vessel of at least five net tons which engages in coastwise trade, unless exempt under 46 CFR 67.9(c), must have a Certificate of Documentation (COD) bearing a valid endorsement appropriate for the activity in which engaged. A simplified measurement was conducted using form CG-5397 to validate the vessel's net and gross tonnage while the vessel was out of the water, current tonnages are; 4 Net and 4 Gross. Measurements taken during inspection were: Overall Length(L)= 33', Overall Breath(B)= 12'-4", Overall Depth(D)= 4'-8" and Individual Hull Breadth(b1)= 3'-10" resulting in 6 net tons and 8 Gross tons. New tonnage calculation require that the vessel apply for and be issued a COD with a coastwise endorsement. Attain a valid COD with coastwise endorsement or provide proof that the vessel is under 5 net tons.

Port fuel filter device (Racor) bowl is required to be fitted with a cap or plug to prevent fuel leakage IAW 46 CFR 182.455(a)(9).

Replace 06 adult type I life jackets to align with COI carriage requirements IAW 46 CFR 180.71(a).

Make repairs to the port propulsion engines forward inboard mounts foundation bolt IAW 46 CFR 182.200(a).

Replace the starboard propulsion engines forward outboard engine mount due to metal wastage at next dry docking IAW 46 CFR 182.200(a).

Install turbo charger intake air filters on both propulsion engines to ensure compliance with 46 CFR 182.200(a).

Properly label port fuel oil tank fuel oil shut off valve remote handle IAW 46 CFR 185.608.

Repair the handrail stanchion bases at the port and forward rails due to cracked aluminum at the next dry docking to ensure compliance with the required 200lb point load as required by 46 CFR 177.900(b).

Renew forward lifelines at the bow to withstand a point load of 200lbs as required by 46 CFR 177.900(b).

Renew vital system raw water supply lines to both propulsion engines to ensure the safety and survivability of the vessel as required by 46 CFR 182.710(a)(6).

Vessel operates on a Limited Coastwise route requiring 06 Red hand flares and 06 orange smoke flares, vessel was short 02 Red hand flares and 03 orange smoke flares IAW 46 CFR 180.64(a).

Vessel is required to have at a minimum 60' of lifeline IAW 46 CFR 180.70(c).

Port fuel filter device (Racor) bowl is required to be fitted with a cap or plug to prevent fuel leakage IAW 46 CFR 182.455(a)(9).

Port pre-engineered fire suppression system (Fire boy) has a toggle switch for energizing and de-energizing the ignition circuit for the port main engine and operating station instrumentation causing the automatic shutdown feature of the fire boy system to not function if the switch is in the OFF position. Remove switch from ignition circuit to allow for proper operation of fire protection system IAW 46 CFR 181.420(a)(3).

Renew port and starboard lifelines at the amidships embarkation/debarkation stations to withstand a point load of 200lbs as required by 46 CFR 177.900(b).

Repair/replace visual indicating light at the operating station for the automatic actuation of the port engine compartment electric submersible pump as required by 46 CFR 182.530(c).

Audible discharge alarm at operators station was inoperable.
