HAGGAN TASI

This vessel sustained catastrophic structural damage as a result of Typhoon Sinlaku (April 2026), indcluding forcible separation of all four outboard engines from the transom, structural failure to both hulls, and partial submersion on the starboard side with the port side aground.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. The child life jackets were moved to a different compartment. Identify the location using the appropriate markings for life jackets. 46 CFR 185.604(f)

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity: (i) The vital systems listed in § 182.710 of this chapter; (ii) Interior lighting except for decorative lights; (iii) Communication systems including a public address system required under § 184.610 of this chapter; and (iv) Navigation equipment and lights. A vessel with batteries of adequate capacity to supply the loads specified in paragraph (a)(1) of this section for three hours, and a generator or alternator driven by a propulsion engine, complies with the requirement in paragraph (a)(1) of this section. The following vital systems: VHF radio, navigation lights, and high water alarms are powered by a single 12 V battery with no second source of power. The electric bilge pump (vital system) for each hull is powered by a single battery power source and inverter with no second source of power. 46 CFR 183.310

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: (1) The pump is listed by an independent laboratory as meeting the requirements in UL 1113 (incorporated by reference; see 46 CFR 175.600); The vessel is greater than 65 feet and carries more than 49 passengers. Two electric submersible bilge pumps were found in the forward void in each hull. The original bilge piping approved diagram shows that a fixed bilge piping system was originally installed in these spaces. 46 CFR 182.520(e)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. The starboard running light housing was found fractured. 46 CFR 183.420

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The bilge pump installed on the starboard side was found with a corroded mounting bracket. 46 CFR 176.830

Conductors in power and lighting circuits must be No. 14 American Wire Gauge (AWG) or larger. Conductors in control and indicator circuits must be No. 22 AWG or larger. 16 AWG conductors were found intstalled throughout the vessel in power and lighting circuits. 46 CFR 183.340(c)

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Each circuit breaker must meet UL 489 (incorporated by reference; see 46 CFR 175.600) or other standard specified by the Commandant. There is no DC primary overcurrent protection on the battery located under the center console used to power the VHF, stereo, navigation lights, and electric bilge pumps located in the fwd void. There is also missing DC overcurrent branch protection for the remaining circuit under the console. There is incorrect primary overcurrent protection (ANL fuse not UL listed) between the batteries and inverters. There is no AC overcurrent protection for the branch circuit after the inverters which power the bilge pumps and AC outlets. 46 CFR 183.380

An emergency switch must be provided in the normally ungrounded main supply conductor from a battery. The switch must be accessible and located as close to the battery as practicable. Disconnect means must be provided on the supply side of and adjacent to all fuses for the purpose of de-energizing the fuses for inspection and maintenance purposes. The battery under the center console does not have a battery switch installed on the supply side of the fuse. 46 CFR 183.380(j)

When the cognizant OCMI or the Commanding Officer, Marine Safety Center determines, in accordance with § 178.210, that a vessel must have stability details indicated on its Certificate of Inspection or a stability letter, the owner or operator must submit the information listed in paragraph (b) of this section. A vessel's lightship characteristics form the foundation for calculations that demonstrate its compliance with applicable stability requirements. Changes to a vessel, through the addition, removal, or relocation of items which are included as a part of the vessel's lightship, could jeopardize the accuracy of the lightship characteristics. The MSC will determine whether calculations alone are sufficient, in lieu of a stability test or deadweight survey, based on the accuracy of the calculations and/or the application of appropriate conservative assumptions in vertical center of gravity (VCG). In making this determination, the MSC will consider the summation of all weight changes that have been made since the last stability test. The owner removed two generators previously installed in the vessel after the 2011 stability test. Submit all weight changes to the MSC in accordance with MTN 04-95 Ch-2. Extended deficiency to December 1, 2024 while waiting on calculations from the Naval Architect. (10/7/2024 [name withheld]) 46 CFR 178.230

Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by Table 181.500(b) and other provisions of this section. The vessel has an approved OCMI special consideration from Sector Guam in December 26, 1997 to permit a gasoline powered trash pump to be onboard as a primary fire pump. The approval letter requires an addtional two 40 B:C portable fire extinguishers to be onboard. 46 CFR 181.500(a)

A vessel of more than 19.8 meters (65 feet) in length must carry at least one rescue boat unless the cognizant OCMI determines that: (1) The vessel is sufficiently maneuverable, arranged, and equipped to allow the crew to recover a helpless person from the water; (2) Recovery of a helpless person can be observed from the operating station; and (3) The vessel does not regularly engage in operations that restrict its maneuverability. The vessel acts as its own rescue boat and the crew use a rescue ladder to recover persons overboard. The rescue ladder is not equipped with a platform at the bottom. The crew must hold the ladder with one hand and retrieve the person overboard with the other while balancing on a ladder rung. Provide a platform at the bottom of the rescue ladder. 46 CFR 180.210

Fire pump must be self priming and power driven on a vessel more than 65' in length. Minimum capacity of 50 gallons per minute with not less than 60 PSI at the pump outlet. Pump outlet must be fitted with pressure gauge. Fire pump must be capable of both remote operations from operating station and local operations at the pump. Provide plans for re-fabricating the existing system to meet the fire pump requirements and demonstrate system operations to Coast Guard Marine Inspector.

VSL more than 65' length must have 2 fixed power pumps. Minimum Capacity of 50 gallons per minute. pump must be self priming. Provide plans for re-fabricating the existing system to meet the Bilge pumps requirements and demonstrate system operations to Coast Guard Marine Inspector.

Remove damaged extra ring life buoy from the vessel.

Each dedicated pump is driven by an independent source of power. Show that each pump have independent source of power.

80% of the ceiling in passenger accommodation space was displaced during storm. Submit repair plan fro CG approval.

Fuel filter must be in serviceable condition as required by 46 CFR 182.455. Replace rusted fuel filter for port and starboard fuel supply lines.

Shutoff valves must be serviceable condition as required by 46 CFR 182.455. Replace rusted fuel shutoff valves on the fuel supply lines.

Life jacket donning placard must be posted conspicuously on board vessel as required by 46 CFR 185.516. Post placard for life jacket donning.

Vessel must have navigation lights as required by 46 CFR 183.420. Repair inoperable port running light.

Battery must be installed to provide poser to start generator as required by 46 CFR 183.200. replace weak starting battery for #2 generator.

Ring Life buoy must be in serviceable condition as required by 46 CFR 180.70. Replace damaged aft ring life buoy.

PA must be fixed installation as required by 46 CFR 184.610. Repair inoperable PA system.

Vessel operating at night time must have illuminated compass as required by 46 CFR 184.402. Repair inoperable compass light.

Life jackets must have 31 sq. inches retro- reflective tape front and back as required by 46 CFR 185.604. Replace all identified old/deteriorated retro- reflective tape.

Lifebuoy must be in satisfactory condition and one must be attached to 60 foot non-kinking black lifeline as required by 46 CFR 180.70. Replace (03) damaged lifebuoy and proved lifeline to one.

Replace deteriorated non-metallic hose on fuel tank fill as required by 46 CFR 182.720.

Replace wasted and deteriorated valves on fuel tank supply line as required by 46 CFR 185.608.

Repair inoperable light fixture in heads as required by 46 CFR 183.410

Replace expired eye wash sollution in first aid kit as required by 46 CFR 184.710

Provide proof of conducted drills/training by crew as required by 46 CFR 185.420

Repair all identified damaged passenger seats as required by 46 CFR 177.820

Vessel must have 2 fixed power bilge pumps as required by 46 CFR 182.520. Demonstrate the proper operation of the port bilge pump.

Install missing junction box covers as required by 46 CFR 183.340

Electrical equipment exposed to weather must be watertight a required by 46 CFR 183.210. Install weather protect for aft switchboard.

Found installed battery not listed as approved battery. Floating water light must be approved IAW 46 CFR 161.010. Install manufacture approved battery for life buoy light.

Locations for Port and STBD generators allows water to enter and accumulate in the space. Generator space must be protected from wet and corrosive environment as required by 46 CFR 183.210. Submit repair proposal for water/weather proofing of the generator spaces.

Found electrical wires, cables and equipments under control station exposed to sea water. All cable and wire must be protected from the weather as required by 46CFR186.340. Make the area under control station watertight.

Found main switch board exposed the weather. Each distribution panel & Main switch board must be in an dry location, protected from splashing water as required by 46 CFR 183.330. Install main switch board weather protection.

Generator compartments are not properly vented. At least two rigid permanent construction natural exhaust and supply ducts for a space containing diesel machinery as required by 46 CFR 182.465. Install rigid permanent ventilation ducts for generator spaces.

Port generator is not in operational state. Electrical equipment must be installed and maintain as required by 46 CFR 183.200. Show proper operations of the port generator.

Found a non marine grade receptacle outlet located 6 inched from main deck. electrical components must be protected from wet and corrosive environment as required by 46 CFR 183.210. Replace all receptacle outlets by passenger seats with marine grade/water proof receptacle outlets.

Found batteries stored in area that is exposed to weather. Batteries exposed to weather must be watertight as required by 46 CFR 183.210. Make battery storage watertight.

Found deteriorated flexible nonmetallic hoses on generator fuel system. Hoses must meet SAE standards J-1942, " Marine Applications" as required by 46 CFR 182.720. Replace all identified flexible fuel hoses on port & STBD generator prime movers.

Found missing metal supports for wires in generator spaces. All cables and wires must be supported with metal brackets as required by 46 CFR 183.340. Install all indentified missing metal supports.

Found toilet space used as cleaning equipments storage. Toilet space must be serviceable and sanitary condition as required by 46 CFR 176.818. Remove gasoline container, 2- wet vac, and cleaning materials from toilet space.

Found forward CO2 mounting bracket & storage missing break away glass cover. Install missing cover per manufacture requirement.

Found deteriorated flexible hoses on generator prime mover jacket water cooling system. All machinery installation must meet the standard acceptable to OCMI as required by 46 CFR 176.800. Replace deteriorated hoses as original.

Found disconnected bilge pipe on the cross connect between hulls. Bilge system on catamaran must have permanent cross connect between hulls as required by 46 CFR 182.520. Make permanent repair to bilge pipe.

Each watertight bulkhead and collision bulkheads must remain watertight with the head of the water to the top of the bulkheads as required by 46 CFR 179.320. Install all open man hole covers.

Found electric bilge pump in space forward of collision bulkhead not permanently mounted and no shutoff valve at the hull penetration. Pump must be permanently mounted and with a positive shutoff valve installed at the hull penetration as required by 46 CFR 182.520. Mount the pump and install valve at the hull penetration.

Found deck lights not operational. Light fixtures must meet standards specified in UL 1570 as required by 46 CFR 183.410. Show proper operations of the deck fixture lights.

Found corroded old bus for battery charger. Remove or repair as original.

Vessel has one fire hydrant with one fire hose of more that 50 feet in length. Vessel over 65 feet in length and have power driven fire pump must have sufficient number of fire hydrant to reach any part of vessel using single length of fire hose. Each hose be 50 feet in length, conforms to UL 19 as required by 46 CFR 181.310. Submit proposal plan for second fire hydrant.

Provide adequate ventilation with vent blower and vent closures for gas tank space in accordance with 46 CFR 182.460.

Paint draft loading marks on port and starboard hulls, fore and aft, 1"x 8"

Weld fractured joints in center and starboard channel of bow ramp.

Provide flame impingment bowls for the generator fuel filters.

Double clamp all bilge, exhaust, and fuel hose connections.

Place on board a ship's bell.

Replace hard plastic piping for generator cooling with suitable metal piping.

Place in the vessel's first aid kit; a wire splint and tourniquet.

Obtain and carry on board one portable manual hand bilge pump of minimum 20gallon per minute pumping capacity. This pump is required as a condition for the OCMI approved variance for the required bilge pumps.

Obtain an additional eleven (11) adult and two (2) child sized lifejackets to bring total on board capacity to 165 adult and 17 child. Until such time that the lifejackes are on board, vessel carrying capacity shall be limited to no more than 154 persons.

Replace generator fuel hoses with hoses that meet SAE J-1942 standards.

Install vent closures on the supply and exhaust vents forthe gasoline fuel tank space. Vent closures to be activated when the fixed fire extinguishingsystem is activated.

Install a bilge high level alarm in each bow thruster space, and in the generator space with a visual and audible alarm at the operating station.

Install flammable vapor detection system in gasoline tank space. System tobe tested on a regular schedule, and such testing logged in the vessel's log.

Post vessel stability letter and Certificate of Inspection under glass, or other transparent material, on board the vessel.

Post operating instructions for the use of the fixed fire extinguishing system at the manual pull station. Instructions to include the closing of space ventilation when the system is activated.

Obtain and post a valid FCC Communications Act Safety Radiotelephony Certificate for vessel's VHF-FM radio.

Install an additional vent on the outboard side of the gasoline tank space to allow for cross ventilation of the space. Reposition the after vent on the inboard side to provide better ventilation supply flow.
