Spool piece reducer on the bow thruster fire pump hads a pin hole leak. Renew spool piece.

Identified the port side engine room bracket that supports the handel for the damper was found wasted. Crop and renew damper bracket.

Identified the junction box for the port aft deck capstan sealed with electrical tape and not weather tight. Make junction box weather tight.

Manal sprinkler system shall be so designed and arranged that the overhead is effectively sprayed and all portions of the deck are covered. The sprinkler system on the car deck has many ineffective sprinker heads.

Doors in A class bulkheads shall have a latch with a minmum 3/4 inch throw which can be operated from either side of the door. The A class door in the deli does not have a latch.

Fill E/R BLKD penetrations (wire run) iwo of aft blkd behind switchboard.

During black out test the emergency generator failed to start; following investigation it was found that the battery post was broken. The battery was replaced and black out test conducted satisfactorily. Emergency generator tested under load. Issued 10c CG-835 for emergency generator.

Fwd E/R bilge was not operational.

Vessel recently completed a M/E re-power and engine control system upgrade. During the plan review process it was identified that the Port and Starboard bridge wing control stations are daisy-chained via a single connector to the EC300 Controllers and not in compliance in 46 CFR 62.30-5(a) and 46 CFR 62.30-5(b)(1). A failure of the cable from the EC300 controller will result in the loss of both bridge wing control stations for the corresponding engine. The twin disc control system was originally designed and installed with two wing stations connected. In order to gain USCG approval for the control system the plan was revised to only show one wing station. Vessel owner requested SLIS OCMI to operate with both wing stations as originally designed temporarily in order to develop a solution that will fully comply with the regulations in 46 CFR 62.30-5(a). A local OCMI risk assessment was carried out to evaluate the risk of having a single point failure of both wing stations vs. having only one wing station. Risk assessment acknowledged a single point failure does not meet the regulations; however it does not present any additional risk if the other station was not connected from the outset. Vessel is allowed to temporarily operate with both wing stations pending a resolution. This temporary authorization is good for 30 days. If a resolution is not identified within 30 days the vessel owner must request an extension detailing the progress made and why corrective action is still outstanding.

Identified 03 cracks in the deck rail where the superstructure ties in to the rail. Issued CG-835V to make permanent repairs to deck rails.

Vsl is broadcasting invalid IMO # and no static draft information

Rudder stock post snapped inside the rudder bearing and the rudder dropped from the vessel

Inadequate audible alarm for fixed CO2 system located in the engine room.

Labels for valve release and cylinder release for CO2 system in engine room are switched.

Aft engine room bilge suction valve found inadequate.

Fixed fire system system in deli space are overdue for annual service.

Demonstrate operational readness for low lube oil alarm or bow thruster motor.

Provide approved plans for boiler and demonstrate tests of unit's safties. Install lagging on boiler exhaust. To be completed prior to operation of unit.

Remove unncessary circuits from emergency electical power system (ie outlets, vending, etc). Provide updated one-line diagram and laod analysis for MSC review/approval.

Properly bundle electrical cables in engineering spaces, main deck heads, and under wheelhouse console.

Label fire station #2.

Label new watertight doors.

Label CO2 protected spaces with warning statement.

Re-install SFP insulation in engine room and A/C equipment space overheads.

Provide monthly safety equipment check sheet.

Provide placard for fuel tank cross connect valve. "to be closed while underway".
