No major repairs or alternations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the the knowledge and approval of the Officer in Charge, Marine Inspection. The following items were identified as requiring repair or needing further inspection: The area in vicinity of port strut backing plate, transverse frame, supporting structures including bottom hull plate were experiencing excessive vibration. The transverse frame and the hull plate were no longer attached structurally.

The operating station deck is rotted about the engine room making a fixed firefighting protected space not vapor tight.
