The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. Cracked cylinder head in no. 4 cylinder, #4 M/E. Approved to operate scheduled route at half power until maintenance team from Eagle Harbor can arrive and effect repairs. Provide root cause analysis, and repair plan. USCG Marine Inspector attendance at conclusion of repairs required.

Each electrical conductor penetration into a battery room must be made watertight. Main car deck battery storage/galley service lift space deck cable penetration had missing blocks and large hole around loose wires.

Fire hazards are to be minimized within the general construction and normal operations of the vessel as is reasonable and practicable. Combustible materials (cardboard boxes) were discovered stored inside the main car deck battery storage/galley service lift space.

Electric installations on vessels must ensure: Maintenance of services necessary for safety under normal and emergency conditions. Fuse holder for #1 PLC is loose and needs to be repaired/replaced. 46CFR111.01-1(a)(1)

Vessels required to have a temporary emergency power source; lighting for passageways, stairways and escape trunk in public spaces, machinery spaces, and work spaces, must be sufficient to allow passengers and crew to find their way to open decks and to survival craft, muster stations, and embarkation stations. Replacement of emergency lighting is not sufficient to temporary power source installed on vessel.

Minimum instrumentation and alarms required for specific types of automated vital systems; must clearly distinguish normal, alarm, and acknowledged alarm conditions. Occasional faults requiring computer restart on OS1 identified, restore to proper working order.

Each boiler must be made available for examination at intervals specified. No. 2 Boiler failed hydrostatic testing due to leak in between two of the sectional headers. Boiler is to remain out of service and tagged out IAW comapny procedures until such time that permanent repairs are made and a satisfactory hydrostatic test is witnessed. 46 CFR 61.05-10(a)

VESSEL REPORTED APPROX. 3/8 INCH HOLE IN MAIN BILGE LINE IN THE #2 ENGINE ROOM. ALL VESSELS SHALL BE PROVIDED WITH A SATISFACTORY BILGE PUMPING PLANT CAPABLE OF PUMPING FROM AND DRAINING ANY WATERTIGHT COMPARTMENT. VESSEL SHALL CONDUCT PERMANENT REPAIRS OF BILGE PIPE AT NEXT OUT OF SERVICE DATE AND PROVIDE SECTOR PUGET SOUND WITH EVIDENCE OF SATISFACTORY REPAIR. 46CFR56.50-50

Galley Vent hood found with build up of grease and dust. Clean vent hood. 46 CFR 71.30-10

Crop and renew to original the buckled frames in way of the No.2 end starboard side at frames 78 and 79. Renew the stiffener brackets in between the affected transverse frames. Prior to DDX credit. 46 CFR 72.01-15 ABS Rules 2021 3.1.2.15.3(i)

Bulkheads or decks of the "A" Class shall be composed of steel or equivalent metal construction, suitably stiffened. Constructed so that if subject to the standard fire test, smoke or flame would not pass for 1 hour. The cable penetration leading to the ER from the #1 side car deck was missing the fire retardant fire block. 46CFR72.05-10(c)1

In all ventilation systems, manually operated dampers of other suitable means shall be provided in accessible locations, outside the space served by the system. The fire damper # 04-(5-1)-2 for the UPD MEN'S and WOMEN'S room would not close. 46CFR72.05-55(i)

Use of non-approved fire detection system may be acceptable if it is installed, tested and maintained in accordance with manufacturer's guidance. The smoke detectors in the crew cabins and associated passageways were found to be 10 years pasts the manufactures recommended expiration. 46CFR76.27-15(b)

All piping, valves and fittings shall meet the applicable requirements of sub chapter F. Fire station 21 packing was found to be leaking. 46CFR76.10-15(a)

Each firehose on each hydrant must have a combination solid stream and water spray firehose nozzle that meets the requirements in 46CFR162.027. Fire stations # 5, 10 and 19 nozzles were found to be leaking. 46CFR76.10-10(j)

Complete but simple instructions for the operation of the system must be located in a conspicuous place at or near all pull boxes. Vessel had an old pull box, that no one knows if its still in use or if it is still active. Located outside the crew lounge, #1 end car deck. 46CFR76.15-10(h)

Each vessel must have a current magnetic compass deviation table or graph in the wheelhouse. The current deviation table was issued 7 years ago. 33CFR164.35(c)

Each general emergency alarm signal must be marked "GENERAL ALARM-WHEN EMERGENCY ALARM SIGNAL RINGS GO TO YOUR STATION" in red letters at least 1/2 inch high. The general emergency alarm on the #2 end pickle fork does not have a sign. 46CFR113.25-20(d)

ach self propelled vessel must have an electric or mechanical engine order telegraph system from the navigation bridge to the engine room. #2 engine order telegraph resistor burned out. Determine cause and ensure that there are two (02) means of communication between the navigation bridge and engine control room while the telegraph is being repaired.

The vessel master had an expired medical certificate with his MMC (pilot date). The mariner's medical certificate was up-to-date in MISLE, but the hard copy certificate onboard was expired. A new copy of the certificate must be obtained within 30-days. 46CFR15.401(c)

Repair emergency switchboard ABT.

Test forward engine room #1 supply damper. Failed to close during testing of CO2 system.

Repair #2 sewage aeration blower trip with CO2 activation.

Test EOS supply room damper with CO2.

Fix accommodation automatic ventilation shutdown from pilot house and engine room.

Each muster station and embarkation station must be adequately illuminated by lighting with power supplied from the vessel's emergency source of electrical power. #7 and #8 IBA embarkation/flood lights are extinguished.

Prove operation of #1 end fire pump soft start.

The #2 end aft mast light and anchor light were inoperable.

Each lifejacket must be marked with type 1 retro-reflective material approved under approval series 164.018. 30 lifejackets require new retro-reflective material. Remove lifejackets from vessel until approved retro reflective material is replaced.

Each fire hose on each hydrant must have a combination solid stream and water spray fire nozzle that meets the the requirements of in subpart 162.027. #7 bail requires servicing and #13 is leaking excessively due to a missing gasket.

A fire control plan (FCP) must be carried on board. The FCP in the pilot hose is outdated and must be updated with most recent revision. Remove an replaced fire control plan in pilot house.

Repair/replace guards for 7 lights located in the vehicle decks that are cracked and/or broken. Locations are as follows: (1)near #2 rescue boat (2)#1 end by port side stairwell,(3)#1 embarkation near amber light,(4)#1 end stbd side stairway tunnel,(5)MES #4 embarkation (e-light) and (6)one inboard of MES #4,(7) e-light near #2 end stbd side tunnel,(8) #1 end tunnel port side forward of #6 fire station.

Each panelboard must have: (iii) The rating or setting of the overcurrent protective device for each circuit. All three panelboards in both bridges are not properly labeled with the rating or setting of each overcurrent protective device. Identify rating or setting and add to label on the panel.

Pinhole leak discovered in #1 end firemain system. Repair to original.

13199 - Sea water pipe leading to the heat exchanger on the #1 side, #2 engine suffered a fractured weld near intake manifold. You must provide repair updates to the Coast Guard and make the vessel available for inspection of repairs. - 46 CFR 71.55-5 - 16C (Rectify deficiencies w/in 14 days)

#2 engine room fire relay allows breakers to be reset while still activated. Prover proper operation. 46 CFR 76.15-35

#1 end propulsion motor supply ventilation damper did not close w/ co2 activation.

#1 boiler low fuel oil trip did not function.

#4 air reciever needs new gauge.

The #2 fire pump was found to have a pin hole leak that was temp repaired using epoxy patch.

#2 Propulsion generator cooling fan shroud for the windings is cracked causing risk of fan failure. Repair to the satisfaction of Coast Guard Inspector. 46 CFR 50.20-35(a).

Prove proper operation of MDE shutdowns on #2 end.

Prove proper operation of bilge system.

Ensure FSD #7 closes completely.

Prove proper operation of follow up failure alarm #2 end.

Prove proper operation of public address/loudspeaker system from both pilot houses so as can be reasonably heard over ambient noise in passenger areas to include car deck.

Properly block off bulkhead cable run (corrected on the spot).

Complete repair to the AMS system steering gear alarm.

Repair MES strobe light.

Repair or replace two valves: F-637-31 & #3 valve from #2 End Gen Overboard & prove proper operation to the satisfaction of the attending marine inspector.

Replace 7 wafer type check valves with lugged check valves and prove proper operation to the satisfaction of the attending marine inspector.

3) Conduct the following steel repair: - Void #3F @ FR#37 thru FR#41, cropout & renew 12” sections of the 7 tripped transverse under deck longitudinals, at the outboard ends IWO guard plate structure. - Vee & reweld 3 weld stress fractures outboard near guards, Void #6F @ FR#57,Void#5A FR#57 and Void #2A @ FR#20. - Void #8A FR#74-76 cropout & renew 3 tripping brackets (approx 6”x 6” brackets)

Repair all equipment affected by casualty to include exciter board for the #2 drive motor and #4 generator to the satisfaction of the attending Marine Inspector.

#3 generator tripped offline causing other 03 generators to trip offline resulting in loss of propulsion.
