A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel representative reported damage of a hole in vessel's hull approximately four feet above the waterline. Vessel is prohibited from operating in commercial service until damage survey is conducted and repairs are made to the satisfaction of the attending Coast Guard marine inspector. 46 CFR 91.30-1(a)

Repair/replace broken hand rails on starboard side.

Permanently mark load line on port side in accordance with load line certificate.

AT NEXT SCHEDULED DRY DOCK, CROP AND RENEW THE FOLLOWING AS ORIGINAL; INSETS IN THE VICINITY OF: #2 PT - FRM 9/FRM7, #3 PT - FRM 15, #4 PT - FRM 19, #8 PT - FRM 38/FRM 39, AND #8 STBD - FRM 38.

OUTLET BOX HANGING UNSURPORTED PORT SIDE MIDSHIP. REPAIR PRIOR TO VSL BEINGPLACED IN SERVICE.

CONTAINMENT FOR FUEL OIL VENTS COMPLETELY WASTED THRU. REPAIR PRIOR TO CONDUCTING FUEL OIL TRANSFER.

DURING DECK WALK 2 DECK LIGHTS (STOOL LIGHTS) WERE FOUND BROKEN AND BENT OVER. LIGHTS TO BE REPAIRED PRIOR TO VESSEL BEING PLACED IN SERVICE.

MAKE PERMANENT REPAIRS TO DAMAGED AREA IN BOW RAKE TO THE SATISFACTION OF COGIZANT OCMI PRIOR TO 14 FEB 91

MAINTAIN TEMPORARY REPAIRS TO FRACTURES IN WAY OF BOW RAKE AT CENTERLINE

MAKE PERMANENT REPAIRS INWAY OF SIDESHELL/MAIN DECK DAMAGE, FRAME 27, PORTSIDE.
