Carbon dioxide cylinders forming part of a fixed fire extinguishing system must be maintained as follows:Cylinders must be retested at least every 12 years. MI noted CO2 bottle in Engineers Electrical shop missing 12 year maintenance.

Each vessel must have posters or signs displayed in the vicinity of each survival craft and the survival craft's launching controls that— (a) Illustrate the purpose of controls; (b) Illustrate the procedures for operating the launching device; (c) Give relevant instructions or warnings; (d) Can be easily seen under emergency lighting conditions; and(e) Display symbols in accordance with IMO Resolution A.760(18). MI noted missing IBA launching instructions.

Air damper on the in port generator failed to close upon overspeed shut down. Service actuator to enable damper to function.

One of two Siemens control drive motors is inoperable. Control drive motor needs to be operational. A report attesting to the repairs is required to clear deficiency. 46CFR58.01-35

It shall be the duty of the owner, master, or person in charge to require and have performed at least once in every twelve months the tests and inspections of all hand portable fire extinguishers, semiportable fire extinguishing systems, and fixed fire extinguishing systems on board. 46 CFR 78.17-80(b)

Each davit, winch, fall, and other launching appliance must be thoroughly inspected and repaired, as needed, once each year. The #2 rescue boat launching device did not have updated yearly inspection documentation, please provide. 46 CFR 199.190(f)(2)

All doors, except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a position at the door. Local release of the FSD-19 missing a part, making manual release of the FSD difficult. Repair as necessary. 46 CFR 72.05-25(b)(9)(ii)

The lifejackets stowage positions must be marked with the words “LIFEJACKETS” or “CHILD LIFEJACKETS” as appropriate. There were a few missing lifejacket placards throughout the vessel on the bench stowage locations. Please correct as necessary. 46 CFR 199.170(b)(2)(iii)

The in-port generator was OOC during the inspection. Prove the proper operation of the in-port via all applicable PSTPs to include CO2 system shutdown and power management systems prior to placing the generator back in service.

Repair the ground on the T420 electrical panel. Repair the No. 2 excitation fan for the No.1 drive motor.

Repair the motor operator on the E-Bus tie breaker. Manually charge after each use and verify charge status prior to putting the vessel in service each morning.

Prove that the No.3 prop alternator can be connected using the No.1 tie breaker.

Repair the No. 2 excitation fan for the No.1 drive motor.

Prove that the F/O instrumentation for the No. 3 & 4 prop alternator accurately displays the F/O pressure between the analog gauges and the AMS.

Vessel experienced a crank case explosion on the #3 main engine and experienced a subsequent fire on the #2 end and had to be towed to Eagle Harbor. A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel.

The electrical characteristics of each overcurrent protective device must be considered in the design of the entire protective system. Fire/bilge pump breaker has a 250 amp breaker installed, drawing lists a 200 amp breaker. Resolve descripancy. Replace with a correctly rated breaker

The General alarm J-boxes on the #2 end sundeck under the piolot house and on the passender deck, #1 end are wated and not watertight. Repair to original condition. Additionally, one more J-box on the passenger deck, for 03 total. 46CFR111.01-1

Vessel has fixed fire suppression system installed in the galley range hood which was required for cooking appliances no longer installed in the galley. System was last serviced in January of 2018 and requires servicing every 12 months. Additionally, cylinder gauge is beginning to read in the red unsatisfactory zone. Fire systems installed but not required must be maintained in operational condition. System must either be serviced or removed.

If transformers are used to supply the ship's service distribution system there must be at least two installed, independent power transformers. The HT2 transformer for the ships service bus is not operational

46 CFR 71.55-1 / 50c - Wide areas of pitting corrosion were discovered at various areas on the car deck and upper passenger deck. A full gauging survey which shows the extent and depth of pitting on the car and passenger decks, including both the centerline and wing tunnels, must be completed and submitted to this office within 60 days. The pitted areas must be inspected for cracking or structural deformation once a week and the results of this inspection must be sent to this office. A repair plan must be submitted to address any plate which is pitted by more than a quarter of its original thickness. Any welding proposed as part of the repair plan must be conducted by qualified welders using an approved procedure.

Sprinkler pipe in zone 4 at bulkhead of vehicle deck has hole requiring replacement of pipe section.

Panelboard directories in pilothouse missing descriptions of some loads and circuit designations. All were missing rating of overcurrent devices on directories. Update all pilothouse panelboards to include complete directory information.

Vehicles were parked blocking fire hose stations and passenger egress aisle spaces. Vehicles to be parked in accordance with loading policy and and painted lines. Discussed with 2nd Mate and Master, who will inspect closer in the future.

Vehicles at ends of ferry not properly chocked as required. 2nd Mate counseled and corrected on spot.

Galley reefer deck plate welds and rivets broken, creating trip hazard for galley staff. Fasten deck to remove hazard.

Galley dry stores wall outlet GP204-13-B duct-taped over. Repair or renew to ensure no electrical hazard present.

Self-closing door to passenger lounge on sun-deck weatherdeck not closing on its own. Adjust or renew to make self-closing in accordance with fire and life-saving plans.

Steering system reported by ship's crew as not working properly. Make necessary repairs to steering system and prove proper operation prior to carrying passengers. (46 CFR 58.25)

03 fire hoses burst under pressure. Replaced 03 hoses on scene; all sat.

While on board, found in the Watch Handover Notes the following entry: "#1 Rescue Boat Davit "Limit Switch" valve not working." Crew did not follow SMS procedure for documenting a non-comformity nor was USCG notified of this issue. No work order/requisition was made for this non-conformity. No resolution was identified on board. Master ([name withheld]) indicated that the vessel would be issued a non-conformity. Vessel is not in compliance with the SMS. Conduct an audit of the vessel's SMS and submit findings to the USCG.

Fire Station # 19, 22 and 40 had leaking hoses. Cut in front of inspector and replace with new. 46 CFR 76.10-10

Fire Station # 13 and 20 nozzles failed inspection. Replaced with new. 46 CFR 76.10-10(j)

Fire Station # 33 had a leaking stem valve. Pipe shop repaired. 46 CFR 76.10-10

Masthead and Port light alarm on No 1 End Panel did not work. Electrician corrected on the spot.

CHECK ALL BATTERIES IN LIFE-RING LIGHTS, REPLACE AS NECESSARY.

PROPERLY TERMINATE OR REMOVE DEAD END WIRE IN GALLEY

REPAIR OR REPLACE FIRE EXTINGUISHER #54A PLEXY GLASS.

Engine order telegraph on the # 2 end does not have an audible alarm when the order and answer arrows do not match.

Make perminant proper repairs to SIMADYN D system on #1 end and test prior to opperations with passengers.

Safety trip controls at ECC and both pilot house locations are not independant and must be seperate from systems that they protect. Make controls independant to the satisfaction of attending Marine Inspector.

Vsl has inset in #2 End starboard bow rub rail. Deck plate, overhanging plate, and longitudinal stiffener in steering gear room are buckled 3-4 inches. Repair no later than next drydock in July 2018.

Launch #2 rescue boat at the #1 end. Vessel unable to launch at COI due to obstruction of pier.

02 Emergency lights did not operate and must be replaced.
