Exhaust gas must not leak from the piping or any connections. The piping must be properly supported. Exhaust piping for the No.4 MDE had cracks in the flex and was visibly leaking exhaust gas. Exhaust piping for the MDE No.1 had salt water leakage. Raw water piping on the port side was wasted away needs replaced.

The foam inductor provided onboard is not operational. Repair or replace and prove proper operation prior to carrying portable tanks containing hazardous materials but no later than 30 days which ever is sooner.

The owner or master shall instruct each crew member at, upon first being employed, and prior to getting underway for the first time on a particular vessel, and at least once every three months, as to the duties that the crew member is expected to perform in an emergency, included, but not limited to, the emergency instructions listed on the emergency instructions placard. During the inspection, it was discovered that both deckhands were unfamiliar with how to energize the vessels fire pump and activate the vessels fixed CO2 system. Vessel shall ensure adequate training is provided to all crewmembers on board and incorporate this aspect of training as part of vessels training procedures.
