Exhaust gas must not leak from the piping or any connections. The piping must be properly supported. Exhaust piping for the No.4 MDE had cracks in the flex and was visibly leaking exhaust gas. Exhaust piping for the MDE No.1 had salt water leakage. Raw water piping on the port side was wasted away needs replaced.

Each vessel that operates on the high seas must have on board a FCC Type Accepted Category I, 406 MHz EPIRB installed to automatically float free and activate. EPIRB was accidentally activated during inspection, needs replaced.

Repairs or alterations to the hull or machinery that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. A hairline fracture was discovered where the #2 and the #1 SSDG exhausts are welded to the hull. Make permanent repairs to the satisfaction of an attending CG marine inspector.

Except as otherwise provided all engines must be water cooled. The engine head, block and exhaust manifold must be water jacketed and cooled by water from a pump that operates whenever the engine is operating. The #3 MDE jacket water pump mechanical seal is failed. Make repairs to the satisfaction of attending CG marine inspector.

The master of a vessel making an oceans or coastwise voyage must prepare a voyage plan. No voyage plans are being prepared. Provide objective evidence that voyage plans are being completed and communicated to a person ashore.

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. No documentation of crew training could be provided during annual inspection. Provide objective evidence that crew training is being conducted and logged/documented.

For flammable or combustible liquid being transported on board a vessel in a portable tank, at least one B-V semi-portable foam, dry chemical or equivalent fire extinguisher, or a fire hose fitted with an approved portable mechanical foam nozzle with pick-up tube and two 5 gallon cans of foam must be carried. The foam inductor provided onboard is not operational. Repair or replace and prove proper operation prior to carrying portable tanks containing hazardous materials but no later than 30 days which ever is sooner.

Flexible nonmetallic hose must meet SAE J-1942. Bow thruster generator fuel line is cotton jacketed. Vessel must provide fuel line that meets SAE J-1942.

The owner or master shall instruct each crew member at, upon first being employed, and prior to getting underway for the first time on a particular vessel, and at least once every three months, as to the duties that the crew member is expected to perform in an emergency, included, but not limited to, the emergency instructions listed on the emergency instructions placard. During the inspection, it was discovered that both deckhands were unfamiliar with how to energize the vessels fire pump and activate the vessels fixed CO2 system. Vessel shall ensure adequate training is provided to all crewmembers on board and incorporate this aspect of training as part of vessels training procedures. 46 CFR 185.420(a)

Battery chargers must have an ammeter connected in the charging circuit. Vessel's batter charger ammeter for the #1 generator was inoperable.

EPA VGP permit missing. Vessel not required to submit NOI to EPA. Vessel required to obtain EPA VGP permit and perform monthly/annual inspections.

Vessel First Aid/CPR for 50% of crew missing. Vessel shall provide proof of training.

Engine Room and Generator Room Bilges full of oil. Vessel shall remove by VacTruck and provide proof of completion with receipts.

Dirty Oil Tank, STBD side Lazarette, sight glass missing. Vessel shall provide proof of completion with new sight glass installed.

Slop Tank, Stbd side lazarette, vent missing Flame screen. Vessel shall install flame screen and provide proof of completion.

Lifering, stbd side, main deck, missing USCG approval sticker. Vessel shall replace and provide proof of completion.

The starboard navigation light was extinguished.
