Electrical installations on vessels must ensure maintenance of service necessary for safety under normal and emergency conditions. Vessel experienced black-out loss of power due to failed transfer from ships service power to power take off/shaft generator. Provide root cause analysis and demonstrate satisfactory functionality of transfer.

The emergency power source must have the capacity to supply all loads, at a unity (1.0) service factor, that are simultaneously connected to it. Emergency generator failed to supply all loads during emergency loss of power. Provide root cause analysis and demonstrate satisfactory operation of the emergency generator to automatically supply all loads upon emergency loss of power.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. Vessel reported port main shutdown due to loss of sea water cooling pump. 46 CFR 58.05-1(a)

Accommodation ladders and gangways required under regulation II-1/3-9 of the 1974 SOLAS Convention. Vessel renewed wire on both port and STBD gangway's. Load test in accordance with IMO MSC.1/cIRC.1331. testing procedure.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its equipment. The bow thruster is not operating as approved and designed, make repairs as necessary to assist the vessel in mooring and maneuvering situations,no later than the next drydocking of the vessel.

Onboard communications and alarm systems. Each vessel must meet the requirements for onboard communications between emergency control stations, muster and embarkation stations, and strategic positions on board. Found the Portside MES talk-back station inoperable. Repair MES talk-back station.

Master [name withheld] - The owner of a vessel subject to inspection under section 3301 shall ensure such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key of which access shall only be available to an established list of crew, by position, allowed to access and use the master key. This list must be maintained onboard the vessel and included within vessel’s safety management system. Additionally, a log book must be maintained with records of access and use of the mater key and shall include the following information at a minimum; date and time of access, room or location accessed, and name and rank of crewmember using the master key. This log book is required to be included in the safety management system. It may be in electronic format and shall be located in a centralized location that is readily accessible to law enforcement personnel. Due to the number of master keys required to conduct daily operations, the vessel needs to re-key staterooms in order to satisfactorily implement the master key control program and limit access to private spaces. Demonstrate satisfactory implementation of master key program prior to completion of next scheduled drydock. 46 U.S.C. § 3106

On each side of the vessel, lifeboats with an aggregate capacity sufficient to accommodate at least 30 percent of the total number of persons on board is required. The STBD side lifeboat is inoperative due to engine failure. Vessel may continue operating on the restricted route of Lakes Bays and Sound and Non-International, until the Lifeboat is repaired.

All lifesaving appliances shall be in working order and ready for immediate use before the ship leaves port and at all times during the voyage. Make repairs to the STBD side lifeboat using OEM parts and technician. When repairs are made demonstrate operation to attending CG Marine Inspector.

In addition to other items required to be entered in the 46 CFR 78.37-lO(a) official logbook on every vessel where an official logbook is required, all items relative to the crew and passengers, including the count of passengers carried, and to casualties shall also be entered. Master of the vessel stated that the passenger accountability system has not been fully addressed even with recent updates to account for individuals disembarking the vessel during port calls. Non Conformity Report (NCR) 16-002 regarding passenger accountability, which was identified as critical has been open since 2016, and no final corrective action(s) have been implemented. Within 30 days, provide a plan to address passenge accountability to the satisfaction of the attending marine inspector. 46 CFR 78.37-10

Operating gernerators must provide a continuous and uninterupted source of power for the ship's service load under normal operational conditions. The vessel reported a brown out after the Port PTO failed to parallel with the SSDG while udnerway. The vessel shall not parallel power with the Port PTO while underway unless the vessel is in US waters, open water, not restricted in ability to maneuver, and the master deems it is safe to do so. Provide a tech report within 14 days to the attending marine inspector. 46 CFR 111.10-4(d)

While underway and transitioning power from the ship's service diesel generator to the port PTO, ship experienced a loss of power. Since the above mentioned loss of power has been reported twice, ship is only permitted to shift from the SSDG to the PTO while moored. Prior to shifting from the SSDG to the PTO while underway, USCG and technician attendance is required.

Signage - In each crew berthing areas and washing areas information on the procedures and resources to report crimes, including sexual assault and sexual harassment, must be adequately posted. These procedures and resources must include at a minimum the following information: 1. Company policies regarding sexual assault, sexual harassment, and drug and alcohol usage. 2. Telephone numbers, websites and email addresses for reporting allegations of sexual assault and sexual harassment to the Coast Guard. 3. Company procedures to report violations of company policy and how to access resources. 4. Resources provided by outside organizations such as sexual assault hotlines and counseling. 46 U.S.C. § 11101

Safety Management System - With respect to sexual harassment and sexual assault, procedures for and annual training requirements for all responsible persons and vessels to which this chapter applies on: Prevention; bystander intervention; reporting; response; and investigation. Provide a plan to incorporate the sexual assault and sexual harassment requirements into the Safety Management System to the satisfaction of the attending Marine Inspector. 46 USC 3203 & 46 USC 3205

made every time a defect is discoverd which affects the safety of the vessel. While underway and transitioning power from the #2 ship service generator to the port shaft generator, the service generator turned off before the power was fully transferred. Ensure the proper operation of the port shaft generator to the satisfaction of the marine inspector. 705: Rectify within 7 days.

Results of Testing the Periodic Safety Test Procedures showed several need to be re—written as they were not in accordance with the approved procedure. Resubmit to the Marine Safety Center for approval, then demonstrate to attending Coast Guard Marine Inspector.

It is mandatory that the design, fabrication and erection of piping constructed under the regulations in this subchapter demonstrate leak tightness. The vessel reported a pinhole leak in the fire main ovbd piping. A temp repair is currently in place. Permanent repairs must be made within 30 days. 46 CFR 56.97(b)

Update vessel lifesaving plan as there is missing equipment on the plans. The vessel had four lifeboats and two were removed, immersion suits have been reduced and the lifesaving plan should indicate these changes. 705 prior to completion of drydock.

Temporary clad weld repair the SW cooling pipe for the PORT Main Engine. Make permanent repairs during the next shipyard/overhaul period. Demonstrate proper operations to the satisfaction of attending CG Marine Inspector. 705 prior to completion of drydock.

All lifesaving appliance must be in working order and ready for immediate use. Repair or replace the lifeboat faulty hydrostatic interlock diaphragm and re-test. Provide technicians report. Test should be demonstrated in the presence of a CG Marine Inspector.

Leak on the of #1 R/G saltwater cooler leading to discharge of oil into the ocean. Repair leak to satisfaction of attending CGMI prior to using R/G.

To assist in casualty investigations, ships, when engaged on international voyages, subject to the provisions of reg 1.4, shall be fitted with a Voyage Data Recorder (VDR). The VDR installed is not operating properly. Make permanent repairs and provide a technician report to MSD Ketchikan. (SOLAS, Chapter V, Regulation 20).

Each accommodation ladder or gangway should be clearly marked at each end with a plate showing the restrictions on the safe operation and loading, including the maximum and minimum permitted design angles of inclination, design load, maximum load on bottom end plate, etc. Where the maximum operational load is less than the design load, it should also be shown on the marking plate. Install plates on both accommodations ladders as required by SOLAS. To be completed by next dry-dock.

A survey shall be made every time a defect occurs which affects the safety of the vessel. The survey shall be such as to ensure that the repairs have been effectively made. Main Diesel Engine is out of service until repairs to two cylinder liners are completed. Prove proper operation of Main Diesel Engines to attending Marine Inspector.

Ships main diesel engines shall be of safe and sound construction. Vessels starboard main diesel engine developed a lube oil leak on an external lube oil pipe that feeds the cam shafts.

There is a pinhole leak in the overboard discharge pipe of the fire main. Vessel is placing a temporary patch on the pipe until the vessel arrvies in Ketchikan on Monday. Required to permanently repair the pipe. 46 CFR 71.40-1 46 CFR 76.10-15

Emergency Generator auto starts uncontrollably. Prove proper operation of the emergency generator.

Vessel is required to establish a standby tug of sufficient size and horsepower to be available for assistance during the one time transit from Petersburg to Juneau.

Vital systems that are automatically controlled or remotely controlled must be provided with instrumentation to monitor system parameters necessary for the safe and effective operation of the system; and an alarm system if instrumentation is not continuously monitored or is in appropriate for detection of a failure or unsafe condition. Currently the vessel's Machinery Alarm and monitoring system (Siemens S7) is not functioning as designed and is intermittently showing faults with alarms and equipment that is monitored including vital equipment. Vessel is required to increase the engineering watch by One (1) oiler. Each watch must consist of One(1) licensed Engineer and Two (2) Oilers. The Chief Engineer shall not be included as a watch stander. Work/rest requirements shall be adhered to.

Prove proper operation of the Port engine to a Coast Guard Marine Inspector. Until permanent repairs are made and attested to by a Coast Guard Marine Inspector, the Port Engine may not be used.

The #2 Ship Service Generator tripped off-line while in-port. This has been an ongoing problem of approximately 6 weeks Provide technician report. Prove satisfactory operation to Coast Guard Marine Inspector.

Provide technician report and prove proper operation of the SSDG #2 to Coast Guard Marine Inspector.

Prove proper operation of the #2 SSDG to an attending USCG Marine Inspector.

Prior to carrying passengers complete fire and abandon ship drill's.

Machinery spaces of category A and above are required a fixed water-based fire extinguishing system (LAFF System). The motherboard for Loop 2 is causing the fault light to stay on. Repair or replace within 90 days.

At next dry-dock make permanent repairs to the STBD side overboard discharge 6" piping/spool piece for the Main Engine discharge cooling system for the boiler, in way of void 1-60-3. Gauge, crop and renew as needed. Repairs are to be made in accordance with 46 CFR 72.01-15 and to the satisfaction of attending CG Marine Inspector.

At next dry-dock make permanent repairs to the STBD side overboard discharge 2" piping/spool piece for the waste heat/boiler blow down, in way of void 1-60-3. Gauge, crop and renew as needed. Repairs are to be made in accordance with 46 CFR 72.01-15/46 CFR 71.55 and to the satisfaction of attending CG Marine Inspector.

The Low Start Air Pressure PSTP requires the high pressure alarm to sound when the pressure reaches 190 psi. The normal operating parameters for this system are between 70 and 135 psi. Update PSTP with the correct high pressure limit. The High Operating Pressure PSTP for the oil fired boiler requires the closure of three separate in-line valves in order to increase the pressure. The current installation is not fitted with the three in-line valves as reference in the PSTP. Update the PSTP to accurately reflect the current installation. The relief valve PSTS for the oil fired boiler does not require the high operating pressure set point to be raised. Without raising the set point, the boiler will shut down before the relief valves lift at the prescribed 80 psi. Additionally, the current PSTP requires the closure of three isolation valves that are not installed on the current installation. Completely re-write the oil fired boiler relief valve PSTS to accurrately reflect the current installation.

Make permanent repairs to the port rescue boat. The boat's sponsons were found to be leaking. The port lifeboat will serve as the port rescue boat. To be corrected by 30AUG18.

Investigate cause and repair of blown capacitor for internal phone system.

Demonstrate proper operation of all Public Address (PA) speakers in Zone 3. Found that the speakers for the P.A. and General Alarm, although working did not meet the minimum sound producing requirements of 46 CFR 113.50-15(c)(1).

Remove and/or reconnect loose wires for internal phone system. Correct by 04SEP18

Monitor the STBD PTO inner coupling backlash every two (2) weeks and record. The expectancy of the coupling is in question. If the tolerance becomes out of Manufacturers (Lufkin)acceptable range, replace. The PTO should only be used as necessary. Replace coupling's at next overhaul period in accordance with Manufactures and ABS approved couplings. 46 CFR 110.30-7 & 46 CFR 111.10-4

Prove proper operation of upgraded 24 volt ECC console interface with installed machinery IAW 46CFR62.25-15.

Pilothouse EOT loss of power audible alarm is not functioning, Repair/replace annunciator IAW 46CFR 113.35-5(e)(1).

Emergency generator flexible fuel oil hose is in excess of 30", reduce sections of flexible hose to allowable lengths IAW 46 CFR 56.60-25(b)(4).

Prove operation of indicating lights/switches for the starboard vehicle deck loading doors IAW 46 CFR 78.17-3.

Repair/replace Fire Tight Door 3008 to dumpster area due to wastage on door bottom IAW 46 CFR 72.05-25(b)(6).

Vessel is allowed to continue on scheduled route until the Port of Ketchikan where vessel must make permanent repairs to the port Controllable Pitch Propeller which is losing hydraulic oil before vessel is allowed to depart Ketchikan.

Repair the steering failure alarm in accordance with 46 CFR 58.25-25 (c) and the approved PSTP.

Respond to the MSC letter dated April 7, 2017. Single point failure of generators (E2-1701246)

Verify diode D2 is installed correctly as per MSC Letter item (4). Demonstrate to the satisfaction of attending CG Marine Inspector.

Identified conductors with worn insulation in the #1 SSDG 24vDC panel. Affect permanent repairs by renewing worn conductors.

Complete permanent repairs on the hole found in the bilge piping to the emergency bilge pump located in the port void under shaft alley between frames 96 to 108.

Re-submit Periodic Safety Test Procedures to the CG Marine Safety Center (MSC). 46 CFR 111.97-5

Repair or replace all four accumulators for the watertight tight doors. 46 CFR 111.97-5

Submit plans and test procedure for the new automatic auxiliary boiler burner. 46 CFR 63.10

Prove proper operation of all fire dampers that failed to function as designed and as noted on F/D checklist.
