The inspections and tests must ensure that the equipment and associated structure are in satisfactory operating conditions and fit for the service for which they are intended. - Number 2 service generator was inoperable.

Except as provided in paragraph (a)(3) of this section, no person may operate an oceangoing ship of 400 gross tons and above but less than 10,000 gross tons, excluding a ship that carries ballast water in its fuel oil tanks, unless it is fitted with approved 15 parts per million (ppm) oily-water separating equipment for the processing of oily mixtures from bilges or fuel oil tank ballast. - Oily water seperator was inoperable.

The inspections and tests must ensure that the equipment and associated structure are in satisfactory operating conditions and fit for the service for which they are intended. - MI observed fuel leaks on emergency diesel generator and several oil leaks on number two (02) generator.

The construction of each luminaire (lighting fixture) for a non-hazardous location must meet ANSI/UL 1598A, or IEC 60092-306:2009 (both incorporated by reference; see § 110.10-1 of this subchapter). - E-lighting in the converter room had a cracked casing.

Fire detection, manual alarm, and supervised patrol systems are not required except in special cases; but if installed, the systems must meet the applicable requirements of 46 CFR, part 76 of subchapter H (Passenger Vessels) of this chapter. - Smoke detector in DC motor control room not installed and is without battery.

The purpose of fire detection systems is to give warning of the presence of fire in the protected spaces. To meet this end, the basic requirements of these systems are reliability, sturdiness, simplicity of design, ease of servicing, and the ability to withstand shipboard shock and vibration and the adverse effects of sea humidity. All fire detection systems must be designed, constructed, tested, marked, and installed according to the applicable standards as incorporated by reference in § 161.002-1 and 46 CFR chapter I, subchapter J (Electrical Engineering) of this chapter. - Smoke detectors on board need to be UL 217 or UL 218 if part of a integrated smoke detection system.

No means shall be provided for locking doors giving access to either of the two required means of escape, except that crash doors or locking devices, capable of being easily forced in an emergency, may be employed provided a permanent and conspicuous notice to this effect is attached to both sides of the door. This paragraph shall not apply to outside doors to deckhouses where such doors are locked by key only and such key is under the control of one of the vessel's officers. - Interior emergency escape freezer handle was inoperable.

The unprotected perimeter of each helicopter facility must have a safety net at least 1.5 meters (4.92 ft.) wide. The outer edge of the net must not extend more than 15 centimeters (6 in.) above the surface of the deck. - Various parts of the safety net perimeter needed tie downs to secure to its framing system.

In the inspection of hulls, boilers, and machinery of vessels, the standards established by the American Bureau of Shipping, see part 90, subpart 90.35 of this chapter, respecting material and inspection of hulls, boilers, and machinery, and the certificate of classification referring thereto, except where otherwise provided for by the rules and regulations in this subchapter, subchapter E (Load Lines), subchapter F (Marine Engineering), subchapter J (Electrical Engineering), and subchapter W (Lifesaving Appliances and Arrangements) of this chapter, shall be accepted as standard by the inspectors. - Stbd crane engine had inoperable oil and fuel pressure gauge.

Gears, couplings, flywheels, and all rotating machinery capable of injuring personnel must be provided with adequate covers or guards. - Port crane engine missing shroud.

Operational readiness. Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. - Safety radios were inoperable (batteries).

Each vessel must have posters or signs displayed in the vicinity of each survival craft and the survival craft's launching controls that— (a) Illustrate the purpose of controls; (b) Illustrate the procedures for operating the launching device; (c) Give relevant instructions or warnings; (d) Can be easily seen under emergency lighting conditions; and (e) Display symbols in accordance with IMO Resolution A.760(18). - Liferaft launching instructions deteriorated, unreadable.

(a) The Commandant may authorize a recognized classification society to issue certain international convention certificates. Authorization will be based on review of: (1) Applicable class rules; and (2) Applicable classification society procedures. (b) The Coast Guard may delegate issuance of the following international convention certificates to a recognized classification society: (1) International Load Line Certificate; (2) International Tonnage Certificate (1969); (3) SOLAS Cargo Ship Safety Construction Certificate; (4) SOLAS Cargo Ship Safety Equipment Certificate; (5) International Certificate of Fitness for the Carriage of Dangerous Chemicals in Bulk; (6) International Certificate of Fitness for the Carriage of Liquefied Gases in Bulk; (7) International Maritime Organization (IMO) Mobile Offshore Drilling Unit Safety Certificate; (8) MARPOL 73/78 International Oil Pollution Prevention Certificate; (9) MARPOL 73/78 International Pollution Prevention Certificate for the Carriage of Noxious Liquid Substances in Bulk; (10) SOLAS Passenger Ship Safety Certificate; (11) High-Speed Craft Safety Certificate; (12) MARPOL 73/78 International Air Pollution Prevention Certificate; (13) International Anti-fouling System Certificate; (14) MARPOL 73/78 International Energy Efficiency Certificate; (15) Polar Ship Certificate. - International certificates for vessel issued by Class were expired and not renewed by Class at the time of inspection.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. - Vessel had temporary / unauthorized repair to water line in the electrician's workshop.

Barges with sleeping accommodations for more than six persons and all self-propelled vessels shall have permanently exhibited for the guidance of the officer in charge of the vessel the following plans: (a) General arrangement plans showing for each deck the fire control stations, the various sections enclosed by fire-resisting bulkheads, together with particulars of the fire alarms, detecting systems, the sprinkler installation (if any), the fire extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating systems including particulars of the master fan controls, the positions of dampers, the location of the remote means of stopping fans, and identification numbers of the ventilating fans serving each section. The aforementioned information shall be kept up-to-date, any alteration being recorded in the applicable medium as soon as practicable. - Fire Control plan needs to be updated.

Each fire drill must include—(i) Reporting to stations and preparing for the duties described in the muster list for the particular fire emergency being simulated;(ii) Starting of fire pumps and the use of two jets of water to determine that the system is in proper working order;(iii) Checking the firemen's outfits and other personal rescue equipment;(iv) Checking the relevant communications equipment;(v) Checking the operation of watertight doors, fire doors, and fire dampers and main inlets and outlets of ventilation systems in the drill area; and(vi) Checking the necessary arrangements for subsequent abandonment of the vessel. - Fire drill conducted during COI renewal and failed to meet the standard. Another fire drill will be required prior to COI issuance.

Each ship shall keep on board a ship specific Ship Energy Efficiency Management Plan (SEEMP). SEEMP was not maintained by the current management company. The plan on board is from CAL DIVE in 2013 and specific to that companies’ management system. That company that is no longer associated with the vessel.

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. CO2 instructions for paint locker not clear. The instructions mentioned which valves to open to operate the system, however the valves were not labeled for use.

A space for the stowage or mixing of paint must not have any electric equipment, except: Explosion-proof electric equipment approved for a Class I, Division 1, Group D (Zone 0 or Zone 1) location. Cover to prevent damage on the explosion-proof light in the paint locker was missing.

Where natural ventilation is provided for spaces protected by a carbon dioxide extinguishing system, provisions shall be made for easily and effectively closing off the ventilation. Fire damper for paint locker ventilation had wasted gasket.

Except as noted in paragraph (c) of this section, all enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. Large ventilation damper for machinery space located on main deck by paint locker did not fully close at the top of the damper and the bottom portion was wasted and holed.

Machinery space openings in position 1 or 2 shall be properly framed and efficiently enclosed by steel casings of ample strength. The corner plating on the outside of the exhaust trunk below the paint locker has a soft patch.

Where air pipes to ballast and other tanks extend above the freeboard or superstructure decks, the exposed parts of the pipes shall be of substantial construction. The fuel tank vent near the life rafts by the paint locker is wasted and has multiple soft patches.

All vessels shall have efficient guard rails or bulwarks on decks and bridges. The height of rails or bulwarks shall be at least 39 1/2 inches from the deck. The ladder leading to the life-rafts was wasted and longer serviceable. The ladder was also missing handrails to allow safe access to launch liferafts.

Each helicopter deck must be fitted around the perimeter with yellow and blue lights in alternate order, not more than 3 meters (10 ft.) apart. The lights on the helo deck are blue, orange, and red which does not follow any lighting standard.

Each cable installation must meet—Sections 6, of IEEE 45.8-2016 or Cables manufactured to IEC 60092-353:2016 must be installed in accordance with IEC 60092-352:2005. Multiple cable penetrations on the main deck, which penetrated a watertight boundary were not watertight.

Except as provided in paragraph (c) of this section ventilator openings shall be provided with efficient weathertight closing appliances. Large ventilation damper for machinery space located on main deck by the emergency generator was sealed shut using red hand which is a temporary solution.

Ventilators in position 1 or 2 to spaces below the freeboard decks or decks of enclosed superstructures shall have coamings of steel or other equivalent material, substantially constructed and efficiently connected to the deck. The ventilation trunk for the galley and the cable penetration were wasted and holed.

Where air pipes to ballast and other tanks extend above the freeboard or superstructure decks, the exposed parts of the pipes shall be of substantial construction. A void goose neck vent in the aft winch area was holed at the bottom of the pipe near the main deck.

Ventilators in position 1 or 2 to spaces below the freeboard decks or decks of enclosed superstructures shall have coamings of steel or other equivalent material, substantially constructed and efficiently connected to the deck. Ventilation trunk aft of the galley ventilation truck on the main deck was wasted and holed in multiple places.

General emergency alarm signal must: Be located in passenger and crew quarters areas where they can alart person in spaces where those persons may be transiting. The old dive control room is not used as an accommodation space or recreational space. There is no fire extinguisher, PA, or general alarm in that space.

Oil piping drains, strainers, and other equipment subject to normal oil leakage must be fitted with drip pans or other means to prevent oil draining into the bilge. On the first deck of the crane space, there was a large hydraulic leak on deck that was not contained.

Each cable installation must meet—Sections 6, of IEEE 45.8-2016 or Cables manufactured to IEC 60092-353:2016 must be installed in accordance with IEC 60092-352:2005. Multiple dead-end wires and unsecured electrical junction boxes were found in crane house on all 3 levels.

Each switchboard must meet the location and installation requirements in section 5.3 of IEEE 45.7-2012 or IEC 60092-302:1997. The switchboard in the crane house had open covers into the switchboard.

An electric apparatus must be designed, constructed, and installed to prevent any person from accidentally contacting energized parts. The #1 generator in the crane house had open covers on the generator with exposed wires and hanging breakers.

A suitable twin strainer must be fitted in the fuel supply line in the engine compartment. A drip pan must be fitted under the strainer. The #1 prime mover in the crane house had fuel pooled in the racor strainer.

Nonmetallic flexible hose may be used in lube oil, fuel oil and fluid power systems only where flexibility is required and in lengths not exceeding 30 inches. Fuel hoses shall be no longer than 30 inches and must meet J-1942 standard. Hoses to and from the racor filters were longer than 30 inches.

All installations must be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. The #2 prime mover was leaking lube oil excessively. Repair in accordance with manufacture recommendations and provide report.

Electric equipment must be arranged, as far as practicable, to prevent mechanical damage to the equipment from the accumulation of dust, oil vapors, steam, or dripping liquids. The #2 generator aft bearing cover was missing.

Each outlet and junction box must be suitable for the environment in which it is installed and be constructed to the appropriate NEMA or IEC standard. The #2 generator junction box had unsecured wire penetrations.

Each cable installation must meet—Sections 6, of IEEE 45.8-2016 or Cables manufactured to IEC 60092-353:2016 must be installed in accordance with IEC 60092-352:2005. The cables leading from the #1 generator to the junction box were unsupported and laying on top of the generator.

Suitable hand covers, guards, or rails shall be installed in way of all exposed and dangerous places such as gears, machinery, etc. The walkway to the inboard side of the #1 prime mover was missing a handrail.

There shall be at least two means of escape from all general areas accessible to the passengers, if carried, or where the crew may be quartered or normally employed. The means of escape from the crane operator station was obstructed. There must be 28 inches of unobstructed walkway.

Under or around each loading manifold and each transfer connection point, a fixed container or enclosed deck area that, in all conditions of ship list or trim encountered during the loading operation, has a capacity of at least: One half barrel if it serves one or more hoses with an inside diameter of 2 inches or less. There must be containment under each transfer station. The fuel transfer station leading to the fuel tank in the crane house does not have containment under it and a sheen was noted on deck.

Each loading arm and each transfer pipe system, including each metallic hose, must not leak under static liquid pressure at least 1 1/2 times the maximum allowable working pressure. Hoses used in the transfer of fuel to the fuel tank in the crane house have not been hydrostatically tested to 1.5 MAWP annually.

Except as noted in paragraph (c) of this section, all enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. All vent closures for the crane machinery spaces were found inoperable.

Each general emergency alarm signal must be marked “GENERAL ALARM—WHEN EMERGENCY ALARM SIGNAL RINGS GO TO YOUR STATION” in red letters at least 1/2 inch high. The general alarm and lights in the crane spaces were not marked appropriately.

Electric equipment must be arranged to prevent mechanical damage to the equipment from the accumulation of dust, oil vapors, steam, or dripping liquids. The welding machine room, located on the main deck was holding water with electrical leads to the welding equipment laying in standing water due to the condition of the bulkhead allowing water into the space.

All piping, valves, and fittings shall meet the applicable requirements of subchapter F (Marine Engineering) of this chapter. The fire hydrant on the main deck located below the paint locker was leaking at the stem during the test of the fire main system.

Nonemergency and inside-type decorative luminaires in environmentally protected, nonhazardous locations must meet the applicable luminaire-type requirements of ANSI/UL 1598 or IEC 60092-306:2009. These luminaires must also meet Clauses 7.4, 8.1, 8.3, 11.2, 13.4, and 17.2 of ANSI/UL 1598A, except in an accommodation space, navigating bridge, gyro room, radio room, galley, or similar space where it is not subject to damage. Light behind the main switchboard was missing a globe.

Each nonmetallic transfer hose must not burst, bulge, leak, or abnormally distort under static liquid pressure at least 1 1/2 times the MAWP. The frequency of the tests and inspections required by this section must be annually or as part of the biennial and mid-period inspections. Main deck fuel transfer hoses require annual test under static liquid pressure of 1.5x MAWP.

Each cable installation must meet—Sections 6, of IEEE 45.8-2016 or Cables manufactured to IEC 60092-353:2016 must be installed in accordance with IEC 60092-352:2005. Multiple cable penetrations near Fire extinguisher 28, which penetrated a watertight boundary were not watertight.

All piping, valves, and fittings shall meet the applicable requirements of subchapter F (Marine Engineering) of this chapter. Noted 2 pipes near crew berthing with severe degradation.

The unprotected perimeter of each helicopter facility must have a safety net at least 1.5 meters (4.92 ft.) wide. The outer edge of the net must not extend more than 15 centimeters (6 in.) above the surface of the deck. Noted hole in helo deck guard rails.

Loudspeakers must be located to eliminate feedback or other interference which would degrade communication. PA system in fwd storage room in-op.

The master shall be responsible for conducting a fire and boat drill at least once in every week. The scheduling of such drills shall be at the discretion of the Master except that at least one fire and boar drill shall be held within 24 hours of leaving a port if more than 25 percent of the crew have been replaced at the port. The vessel OCMI shall witness crew drills prior to commercial operation and vessel departure. 46CFR97.15-35(a)

Each general emergency alarm signal must be an electrically operated bell, klaxon, or other warning device capable of producing a signal or tone distinct from any other audible signal on the vessel. General alarms in the following spaces were found to be inoperable: (02) port bow accommodation hallways; (01) in the lower machinery space in the crane house. 46CFR90.25-1(a) 46CFR113.25-12(a)

Within the accommodation and service areas the following conditions shall apply: Corridor bulkheads in accommodation spaces shall be of the "A" or "B" class. Unauthorized bulkheads penetrations inhibiting the structural integrity of the "B" class bulkheads IAW the vessel's Fire Control Plan were found in the corridors leading to both port/starboard accommodation spaces. 46CFR92.07-10(d)(1)

General arrangement plans showing the various sections enclosed by fire resistant bulkheads shall be displayed. The Class "A" fire door, identified via the vessel's Fire Control Plan, in the entrance to the galley did not close properly. Rectify deficiency prior to the vessel's departure.

The accuracy of the bilge alarm (OWS) must be checked at IOPP Certificate surveys according to the manufacturer's instructions. Upon inspection it was found that a third party technician had reported the oily water separating equipment to be inoperable. 33CFR155.380(d)

The following emergency lighting and power loads must be arranged so that they can be energized...At least one light in each space where a person may be maintaining, repairing, operating equipment, or transiting. Emergency lighting was found missing in the crane operator's passageway from the operating station to the generator space. 46CFR90.25-1(a) 46CFR112.15-1(f)

There shall be at least two means of escape from all general areas accessible to the passengers, if carried, or where the crew may be quartered or normally employed. The double doors from the lower deck of the crane house to the weather deck were found seized. 46CFR92.10-5

Deck lines shall be painted in white or yellow on a dark ground or in black on a light ground. The deck line was found to be incorrectly painted, below the actual raised line. ICLL/8

A fire main system on a unit in international service must have at least one international shore connection that meets ASTM F 1121. The shore connection was missing (01) bolt for its intended service. 46CFR108.427(a)

The owner of a vessel subject to inspection under section 3301 shall-(1) ensure that such vessel is equipped with a vessel master key control and Signage requirements regarding the Coast Guard's Sexual Assault and Sexual Harassment Prevention and Response programs for commercial vessels. Follow guidance in implementation IAW Coast Guard Policy Letters 23-04 and 23-06. 46USC3106 46USC111101

Periodic tests and inspections shall insure that the equipment and associated structure are in satisfactory operating conditions and fit for service for which they are intended. The #2 center generator contained wasted flexible fuel lines. 46CFR90.20-1(a) 46CFR61.01-1(b)

Periodic tests and inspections shall insure that the equipment and associated structure are in satisfactory operating conditions and fit for service for which they are intended. The #3 generator contained and unsupported high pressure fuel line. 46CFR90.20-1(a) 46CFR61.01-1(b)

Each panelboard must have a circuit directory that has the circuit designation for each circuit. The distribution panel located in the main passageway of the accommodations under the helo-deck needs an updated circuit breaker directory. 46CFR90.25-1(a) 46CFR111.40-11(b)(2)(i)

For each ship subject to regulation 5 and 6 of this Annex, details of fuel oil for combustion purposes delivered to and used on board shall be recorded by means of a bunker delivery note that shall contain at least the information specified in appendix V to this Annex. The vessel is missing a bunker delivery note from April 2023. MARPOL VI/18.5

The IOPP certificate shall be issued or endorsed as appropriate either by the Administration or by any person or organization duly authorized by it. The IOPP survey, conducted by ABS on 08FEB24, had an improper entry on the Form A regarding the OWS OCM. The certificate read that the OCM met approval series A.393(X) but upon further investigation the OCM was found to meet MEPC.60(33). MARPOL I/7.2

Vessels that operate in salt water must undergo drydock and internal structural exams with the intervals set forth in Table 91.40-3(a). The following worklist items were discovered during the vessel's drydock and internal structural examinations: -Holes wasted in aft port side shell next to the aft anchor. -Hole in port side shell bottom most keel cooler. Approximately 100 ft from stern of vessel. -Double bottom tank C-3 currently contains approximately 5 inches of water in the forward section of the void, clean out water and descale two bays for more detailed inspection. -Above the C-3 double bottom tank in way of the forward electric storage has wasted deck plating. Descale and gauge deck plating. -Ballast pump room under ballast manifold deck plating found wasted. Descale and gauge sloped deck area under manifold. -Main deck winch flat found wasted. Descale and gauge in aft portion of winch area. -Winch flat aft tool room bulkheads wasted near bottom of watertight bulkheads. Remove tools from bulkhead and gauge bottom 18” of watertight bulkheads. -Multiple watertight penetrations in way of winch flat and tool room on main deck not made watertight on deck and through bulkheads. -Pilot house weathertight penetrations are not properly secured. -The ladders in the following tanks were wasted: F1, F2, B1, C1, D1, and C2. -Wasted deck plating on port side of crane pedestal wasted. Gauge to determine remaining thickness. -Unable to access A4 void due to condition of ladder/platform. Replace angle supports to ensure access is safe for personnel. 46CFR91.40-3(a)(1)

An inspection either general or partial depending upon the circumstances shall be made whenever any important repairs or alterations are undertaken. During the annual inspection multiple repairs and further inspections were required. Due to the number of items and amount of repairs required, an attached word document was created to capture all requirements. See Word Document titled "27FEB2023 Worklist for PACIFIC (O.N.537871)". All repairs shall be made to the satisfaction of the attending marine inspector prior to credit given for annual inspection. 46 CFR 91.45-5

Electric motors must be outside the duct and compartment and have an explosive-proof motor for Class I, Division I Group B location or be at least 10 ft. (3m) from the exhaust end of the duct. Each blower must have a non-sparking fan. Vessel does not have a non-sparking Class 1 Division I fan in the battery compartment.

B” Class bulkheads. Bulkheads of the “B” Class shall be constructed with approved incombustible materials and made intact from deck to deck and to shell or other boundaries. They shall be so constructed that, if subjected to the standard fire test, they would be capable of preventing the passage of flame for one half hour. Vessel has two watertight Class B bulkheads that needs to be replaced.

There must be provided at least 1 toilet, 1 washbasin, and 1 shower or bathtub for each 8 members or portion thereof in the crew who do not occupy rooms to which private or semi-private facilities are attached. The vessel sewage is down for aft berthing due to rupture in piping, that effects 38 potential crew members. Vessel is investigating and correcting the sewage system.

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces, galleys, main pantries and storerooms, other than small service lockers, shall be of “A” Class construction. Numerous penetrations in the winch control room were found not completely sealed to ensure adequate structural fire protection as an "A" Class bulkhead.

The lantern must be relay-controlled so that the loss of normal power causes the lanterns to light. The emergency lighting located in the passageway near the galley had one light bulb not working.

Pilot house Embarkation ladder was too short to reach the water line. The ability to meet the egress and survival craft launching requirements of this part. Crew was unable to reach the water to enter survival craft.

Numerous dead end cables were found throughout the vessel. Electrical installations on vessels must ensure: protection of passengers, crew, other persons, and the vessel from electrical hazards.

Fixed fire extinguishing systems were inoperable during function testing. Fixed fire extinguishing systems shall be repaired as necessary and made operable.

Multiple fire hoses were found to be full of holes and were leaking water during pressure test. Vessel shall be supplied with at least one length of fire hose at each fire hydrant.

Multiple watertight bulkhead penetrations were found not to be watertight. Vessel shall ensure all watertight bulkhead penetrations are made watertight.

Multiple incorrect electrical installations were found, to include an unsecured junction box in the ET repair shop and a dead end armored anode cable in the starboard bow warehouse.

A complete recharge shall be carried for each self-contained breathing apparatus. Vessel's SCBA's not charged properly. 46 CFR 96.35-5(e)

Each fireman’s outfit must consist of one self-contained breathing apparatus, one lifeline with a belt or a suitable harness, one flashlight, one flame safety lamp, one rigid helmet, boots and gloves, protective clothing, and one fire ax. In lieu of the flame safety lamp, vessels may carry an oxygen depletion meter which is listed by a Coast Guard recognized independent laboratory as intrinsically safe. (b) Every vessel shall carry at least two firemen’s outfit. Vessel missing fireman outfit equipment. 46 CFR 96.35-10

Each vessel must – (1) carry not less than 12 rocket parachute flares approved under approval series 160.136; and (2) stow the flares on or near the vessel’s navigating bridge. Vessel had 9 flares onboard. 46 CFR 199.60(c)

Each vessel must have a line-throwing appliance approved under approval series 160.040. Vessel missing line-throwing appliance. 46 CFR 199.170

Each vessel must have posters or signs displayed in the vicinity of each survival craft and the survival craft’s launching controls that – (a) illustrate the purpose of controls; (b) Illustrate the procedures for operating the launching device; (c) give relevant instructions or warnings; (d) can be easily seen under emergency lighting conditions; Vessel missing instructions. 46 CFR 199.90

Each survival craft must be stowed and ready for use so that two crew members can complete preparations for embarkation and launching in less than five minutes. Liferafts are installed improperly; the hydrostatic release units installed in such manner as to prevent manual release safely and one stowage bracket is heavily corroded and requires replacement. 46 CFR 199.130

Approved hand portable fire extinguishers shall be installed in accordance with Table 95.50-10(a). Several fire extinguishers were missing or misplaced throughout the vessel. 46 CFR 95.50-10(a)

All hand portable fire extinguishers shall be checked as noted in Table 91.25-20(a)(1). Several fire extinguishers were found to be unsatisfactorily serviced throughout the vessel as compared to the third-party servicing certificate. 46 CFR 91.25-20(a)(1)

Fixed fire extinguishing systems shall be checked as noted in Table 91.25-20(a)(2). Remove obstructions from fixed CO2 system nozzles and require function test of the CO2 system. 46 CFR 91.25-20(a)(2)

Installations and equipment accepted by the CG as meeting the applicable requirements in this subchapter which are maintained in good and serviceable condition to the satisfaction of the OCMI may be continued in use until replacement is ordered by the OCMI or as specified in the regulations. Multiple areas where electrical wiring was not marine grade type; multiple extension cords used throughout vessel as permanent installation; found uncovered live distribution panel; multiple openings in switchboard panel to allow free access into the switchboard; found multiple dead end wires that need to be removed; and galley distribution panel requires proper labeling. 46 CFR 110.01-1(c)

Each Class 1 door on a vessel in ocean service must be designed to withstand a head of water in no case less than 10 feet. All gaskets in watertight doors are deteriorated and must be replaced. 46 CFR 170.270(b)

Each generator prime mover must have an overspeed device that is independent of the normal operating governor. Each generator must meet the applicable requirements for construction and testing of the ABS Steel Vessel Rules. Vessel unable to conduct generator overspeed and parallel testing. Exhaust insulation missing on #2 generator and must be replaced. 46 CFR 111.12-1(b) 46 CFR 111.12-5

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment shall be made without the knowledge of the OCMI. CG was not notified of new battery installation; CG was not notified of the removal of the #1 generator; repairs were made within ballast tank without CG oversight; and plans must be submitted for addition of DC controllers. 46 CFR 91.45-1

Several fire extinguishers were improperly stowed out of their supplied mounting brackets. 46 CFR 95.01-2(c)

The galley/mess deck rolldown service door inoperable, A60 fire door to galley not self-closing, and mess deck A60 door would not completely shut. 46 CFR 92.07-10
