Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. The remote reach rods for “emer. fire pump cut out valve” located on the starboard side of the aft deck are seized, and one valve handle is broken. Repair and prove proper operation of all reach rods for emergency fire pump / fire main to the attending ABS surveyor. Update 12/27/23: at request of the vessel PIC, the due date has been extended for 30 days; now due 1/28/24. Update 1/26/24: at request of the vessel PIC, the due date has been extended for an additional 30 days; now due 2/28/24.

Emergency lighting shall be provided at every boat station on deck and oversides and in all alleywells, stairways, and exits. All emergency lights shall be marked with a letter “E” at least ½ inch high. Various e-lighting for emergency escape scuttles/trunks from the starboard tunnel space and some aft cargo holds were found not illuminated. Additionally, various e-lighting around the lifeboats and in the aforementioned escape scuttles were not properly marked. Ensure all e-lights throughout the vessel are illuminated and properly marked to the satisfaction of the attending ABS surveyor.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. The “vent fans fwd” button on the "vent-fan emerg stop station" on the bridge did not stop either the forward accommodation or forward cargo hold vent fans when tested. Determine which vent fan(s) this remote stop controls and demonstrate its proper functionality to the satisfaction of the attending ABS surveyor. Additionally, provide a hammer or other arrangement to expediently access the ‘break-glass’ enclosure in case of an emergency.

A ship of >300 gross tons must have a fixed container or enclosed deck area under or around each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe that has a capacity of one barrel. Fuel oil containment for vent for a double bottom tank on starboard side, approximately halfway on the aft deck (specific label not legible) is wasted and holed. Nearby containments also appear suspect; all to be close-up inspected and repaired as necessary to the satisfaction of the attending ABS surveyor. Update 12/27/23: at request of the vessel PIC, the due date has been extended for 30 days; now due 1/28/24. Update 1/26/24: at request of the vessel PIC, the due date has been extended for an additional 30 days; now due 2/28/24.

Maintenance, testing, and inspection shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. A sampling of detectors and manual call points shall be tested monthly so that all devices have been tested within 5 years. At time of attendance there was not objective evidence available onboard that a percentage of manual call points were being tested monthly, and locations tracked to ensure all are tested within a 5 year period.

Lifesaving appliances and arrangements required by this part shall be approved by the Administration. The approval series for thermal protective aids is 160.174. NVIC 02-19 discusses approval under a MRA. All items covered under the MRA must be marked with the appropriate USCG approval number. The thermal protective aids found in the starboard lifeboat are Datrex model TPS, part number DX1529M and do not have any markings indicating CG type approval. Provide proof that these thermal protective aids have CG type approval, or replace any non-CG type approved thermal protective aids with the proper type.

The navigational equipment and systems referred to in this regulation shall be so installed, tested, and maintained as to minimize malfunction. The rudder angle indicator located on the port side of the wheelhouse near the GMDSS suite reads approximately 5 degrees to port when the rudder is amidship. Also, the rudder angle indicator on the port bridge wing reads approximately 2 degrees off.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. The following fire doors were found with issues as noted below: -Cargo hold E deck centerline stairwell door does not close and latch. -Door 232 from officer’s mess into galley; door catches on the frame. -Aft house 01 deck port side stairwell door does not close and latch. -Aft house 02 deck port side stairwell door doorknob pulled off when tested.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: fire detection and fire alarm systems. FOS fire detection panel unit in the aft house showed a trouble alarm at time of attendance.

Maintenance, testing, and inspection shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. Portable containers or portable tanks containing foam concentrate, excluding protein based concentrates, less than 10 years old, that remain factory sealed can normally be accepted without the periodical foam control tests required in MSC.1/Circ.1312 being carried out. Portable foam containers in the emergency gear locker in the aft house have a date label of 11/04/04, and there is no record of periodical foam testing being carried out since.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. The following fire station valves found leaking by when fire main was pressurized: -Station 40. -Station 26.

Every ship to which chapter I applies shall carry an appropriate portable atmosphere testing instrument or instruments. Suitable means shall be provided for the calibration of all such equipment. Vessel currently has MSA multigas meters, for which the manufacturer’s instructions call for bump testing prior to use. The calibration gas currently provided onboard expired as of 7/13/23, and the SR 3212310 raised for new calibration gas has not been filled/new calibration gas supplied onboard as of date of CG inspection (8/16/23).

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. The following electrical penetrations in fire boundaries found not properly sealed: -Starboard tunnel rock-block style penetration near cargo hold 6-7. -Cargo hold E deck port side near cargo hold 1.

The Company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The safety management system should provide for sufficient measures aimed at promoting the reliability of such equipment or systems. The inventory for various critical spare parts are below the minimum ROB inventory established by the ISM Company. This issue was noted during the external ISM-DOC audit on 11/28/22 (finding no. 435) and also during the internal ISM-SMC audit on 6/8/23 (finding no. 3). The provided corrective action plan for internal audit finding no. 3 indicates that some spares were ordered, but the budget for ordering critical spares in FY23 has been exhausted prior to orders being placed for all critical spare parts needed to obtain the minimum ROB quantity, with SR3212271 for replenishing critical spares for FY24. Provide a corrective action plan to the satisfaction of both CG and ABS for how the critical spare parts requirements will be achieved in a timely fashion. Update 9/29/23: The due date for this deficiency has been extended to 11/1/23 based on the operator's report that MARAD intends to allocate funding for continued procurement of spare parts, and work to revise the critical spare parts list is in progress.

After any survey of the ship under regulations 7,8, 9, or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment, or other items covered by the survey, without the sanction of the Administration. -Drain pipe leading out to port side forecastle deck in vicinity of frame 300 is holed. -Electrical kickup penetration on port side forecastle deck is wasted and holed.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: [.1] structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. -Fire screen doors for entry to cargo hold 2 from main deck of forward house, port and starboard side, are not properly sealing to prevent the passage of flame and smoke as required for class A divisions. -Electrical penetration above the door for entry to cargo hold 2 from the forward house, main deck, port side, is not properly sealed. -Electrical penetration in aft house, main deck, amidships, on aft bulkhead, is of mismatched type (e.g. mixing rock-block type and fire resistant putty). -Ventilation dampers for the heaters in the port and starboard fan rooms (main deck, forward house) are severely wasted and seized. -Manual ventilation cover for supply for CO2 room (located on aft bulkhead of aft house) has a wasted lip.

The master of every new ship shall be supplied with sufficient information, in an approved form, to enable him to arrange for the loading and ballasting of his ship. Current ILLC, certificate no. 72707401-4259263-043 states “The vessel shall be operated in accordance with the Trim and Stability Booklet, Drawing Number 4843856, Rev D, bearing the ABS Americas approval stamp dated 22 November 2006.” The most current version of the Trim and Stability Booklet onboard is Rev F, with associated ABS Houston Letter Ref T1300718 dated 24-Dec-14. The version of CargoMax currently in use is build 2.1.0243 dated 9/22/2014. Also, Ref F of the Trim and Stability Booklet makes multiple references to Military Sealift Command (ex: “These differences should be reported immediately to the MSC Headquarters’ Naval Architecture Branch (MSC Code N721)”). Resolve to the satisfaction of ABS.

Manholes and flush scuttles in position 1 or 2 shall be closed by substantial covers capable of being made watertight. The following scuttles were found with wasted or holed lids/covers: -cargo hold 7 aft port side. -cargo hold 7 aft starboard side. -cargo hold 8 forward starboard side.

Hot surfaces likely to come into contact with the crew during operations are to be suitably guarded or insulated. Lagging is missing on flanges for steam piping inboard of the port condenser.

Ships shall be provided with a main steering gear to the satisfaction of the Administration. When operating on emergency steering from the steering flat, there is an intermittent “steering failure” alarm that activates at the bridge console. This seems to routinely activate when going to from amidships to starboard, when passing around 10 degrees to starboard.

All electrical apparatus shall be so constructed and so installed that danger of injury in ordinary handling shall not exist. Dead end wiring noted near purifiers and in starboard side shaft alley.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. Multiple debris screens for cargo hold ventilation trunks are wasted or holed. Repair or replace to the satisfaction of ABS.

While ships are operating within an Emission Control Area (ECA), the sulphur content of fuel oil used on board ships shall not exceed the following limits: 0.10% m/m [14.4]. The sulphur content of fuel oil used or carried for use on board a ship shall not exceed 0.50% m/m [14.1]. Supplement to IAPP (SOVC), no. 7207401-4259263-391, indicates that when the ship operates inside an ECA, the ship uses fuel oil with a sulphur content that does not exceed 0.10% m/m. Vessel used HFO with a sulphur content >0.5 % during recent turbo-activation in August-September 2021 (voyage entirely within North American ECA). Additionally, approximately 4000 mt of HFO with a sulphur content of >0.5 % remains board at this time. Develop and implement corrective action plan to the satisfaction of CG and ABS. Note: any exemption from the requirements of MARPOL VI/14 would require approval from CG HQ.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. There are six items relating to the CO2 system called out on Harris Fire Protection report no. 1226353 of 9/29/21, including ER hose reel hose needing replacement due to dry rotting, and multiple flex hoses. In addition to the listed items in report no. 1226353, during ACP annual it was noted that CO2 flex hoses for two bottles in forward battery room are located directly against each other and thus liable to chafing.

Every fuel oil suction pipe from a storage, settling, or daily service tank situated above the double bottom shall be fitted with a cock or valve capable of being closed from outside the space concerned in the vent of a fire arising in the space in which such tanks are situated. Remote pull/actuation cable for EDG fuel shutdown at governor parted when tested during inspection.

Maintenance, testing, and inspections shall be carried out based on the guidelines developed by the Organization [IMO MSC Circ 1432] and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. IMO MSC Circ 1432/5.9 requires that portable fire extinguishers are checked monthly to verify that they are in place, properly arranged, and in proper condition. There is no objective evidence of checks of the portable fire extinguishers being carried out at the required monthly intervals.

All arrangements for pilot transfer shall efficiently fulfill their purpose of enabling pilots to embark and disembark safely. The appliances shall be kept clean, properly maintained and stowed and shall be regularly inspected to ensure that they are safe to use [23.2.1]. Arrangements shall be provided to enable the pilot to embark and disembark safely on either side of the ship [23.3.1]. One of pilot ladders found with three damaged steps.

Ships shall carry a line-throwing appliance of an approved type. Line throwing appliances have expiry date of 10/2021.

The means for securing and maintaining weathertightness shall be to the satisfaction of the Administration. Multiple locations of pooled water found on “A” deck. It is reported that water is leaking into the cargo hold from the hatch coaming of the cargo accordion-style hatch above.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. “A” deck to “B” deck cargo ramp has transverse support member that is completely wasted through in several locations. Additionally, one dog has hydraulic leak that is dripping onto the flexible water drain hose and the ramp below. *No cargo authorized for carriage on B deck and below until transverse support member is repaired.

The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations [10.1]. In meeting these requirements, the Company should ensure that appropriate corrective action is taken for any non-conformity reported [10.2.3]. Subject vessel has PMP class notation. Most recent Condition Monitoring Service Report, no. 2021051, dated 11-2021, includes six red (“major discrepancy, repair immediately”) findings from thermal imaging and eight red (“major discrepancy, repair immediately”) findings from vibration analysis. Three of the red items from the vibration analysis (standby fire pump, stbd A/C fan fwd, and supply Fan TAS-3) have been noted as red in the last 3 annual reports.

Port and starboard bunkering manifold containment are missing drain plug fittings, thereby not providing an oil-tight enclosure. *Must be corrected prior to bunkering operations.

Satisfactory means shall be provided for the protection of the crew in getting to and from their quarters, the machinery space and all other parts used in the necessary work on the ship. Broken handrails noted in aft house port side stair tower on main deck and O2 deck and near main cargo hold ramp between B and C deck.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and readily available for immediate use. Life ring buoy at starboard girder box needs new retro-reflective tape and plug.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Electrical panel in SOPEP locker is not readily accessible by the crew due to storage of items around the panel.

Each ship shall keep on board a ship specific Energy Efficient Management Plan (SEEMP). Supplement to IEEC (SOVC), no. 7207401-2406546-003, indicates that the ship has a SEEMP. No SEEMP has been provided for the vessel / available onboard at time of inspection. Develop and implement corrective action plan to the satisfaction of CG and ABS. Note: any exemption from the requirements of MARPOL VI/22 would require approval from CG HQ.

Demonstrate emergency drills, prove proper operation of the port lifeboat, and verify status of watertight doors in accordance with submitted voyage plan to the satisfaction of the attending marine inspector prior to departing [name withheld] Anchorage.

REMOVE DEAD GROUND " A & C" PHASE 120 VOLTS DISTR, BUS AFT OF THE MAIN SWITCHBOARD.

REMOVE PARTIAL GROUND 120 VOLTS AT EMERGENCY BUS FWD OF THE MAIN SWITCHBOARD.

REPLACE BROKEN VIEW PORT GLASS OF STBD BOILER (AFT CORNER)

FREE UP REMOTE QUICK ACTING FUEL OIL VALVES (2 EA) OF THE STBD BOILER.

FREE UP REMOTE WATER SIGHT GLASS VALVE (4EA) OF STBD BOILER.

INSTALL FIRE EXTINGUISHERS AT STATION #136 ENGINEROOM NEXT TO CONSOLE. INCLUDE STATION 137 & 132.

REPAIR TELEPHONE SYSTEM IN ENGINEROOM.

MAKE PERMANENT REPAIR TO PIPING OF THE STBD EVAPORATOR WHERE " RED HAND" ARE INSTALLED. ( INBOARD OVERHEAD AREA)

COVER ALL EXPOSED VALVE ON STEAM LINE PORT AND STBD BOILER.

INSTALL RE-SET BUTTON AUX MAIN FEED PUMP CONTROLLER AFT PORT AND STBD SIDE.

MAKE PORT MAIN PROPULSION BOILER AVAILABLE FOR COMPLETE FIRESIDE AND WATERSIDE INSPECTION BY USCG PRIOR TO SAILING FROM NEXT US PORT.

DEMONSTRATE PROPER OPERATION OF THE SELF-CONTAINED GENERATOR MOUNTED ON THEFANTAIL. THIS REQUIREMENT INCLUDES ELECTRICAL CONTROL/S, AND THE PERSONNEL SAFETY AND POLLUTION PREVENTION MEASURES.

DEMONSTRATE PROPER OPERATION OF THE PORT BOILER PRIOR TO DEPARTING THE NEXTU.S. PORT.

DEMONSTRATE PROPER OPERATION OF STBD SSTG REVERSE POWER TRIP. (RELAY)

SET AND SEAL THE PORT & STBD PROPULSION BOILER SAFETY VALVES & SUPERHEATER SAFETY VALVES.

CONDUCT A BOAT DRILL.

PERFORM A WEIGHT TEST OF PORT/STBD LIFE BOATS & DAVITS.

PROVE PROPER OPERATION OF THE VESSELS EVAPORATOR.
