An interconnected detection and alarm system must: Be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d). Vessel did not have visual/audible in accommodations(crew) and no visual in Gen Room. Due to Maritime Safety Solutions inventory and techs, vsl is on list to have system complete inline w/ rest of fleet. Provide proof when completed. 46 CFR 181.450(a)(2)

Vessel required to be able to recover a helpless person from the water to OCMI satisfaction. Rescue platform does not meet OCMI standards. Platform was 24 in X 28 in. not the required 36 in x 36 in.

Vessels operating more than 100 miles required to have NAVTEX. Vessel not equipped with NAVTEX, Restricted to no more than 100 miles until installed. 47 CFR 80.905(a)(3)(v)

Installed fire systems shall be labeled and have instructions posted. Remote stations for the fixed CO2 system for generator and Main machinery space labeling or instructions posted. Crew was not sure of the purpose or operation of the remote stations.

Each battery must be secured from shifting. Battery for port generator doesn't fit in securing bracket and was not secured.

Exhaust gas must not leak from piping or any connections. Port Generator had a pinhole exhaust leak in the exhaust piping into the ER. 46 CFR 182.430(b)

A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Bulkhead at the FWD engine room compartment & bulkhead between ER and generator space had multiple empty bulkhead penetration fittings as light was seen penetrating. 46 CFR 179.320(c)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Starboard generator crankshaft drive belt is exposed. 46 CFR 177.960

Each vessel must be marked as required by part 67. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Hailing port on the stern of the vessel (Morgan City) did not match the COI and COD (New Orleans). 46 CFR 185.602 46 CFR 67.123

Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. All MDE primary fuel filters had non-metallic bowls and did not have a flame shields. 46 CFR 182.455(6)

Exhaust system protection must be provided at such locations where persons or equipment might come in contact with an exhaust pipe. MDE #4 had missing exhaust lagging. 46 CFR 182.430(a)

A vessel must comply with the requirements for any radio installations, including the requirements for a station license and installation certificates issued by the FCC, in 47 CFR part 80. 705- Limited to <20NM from land. Vessel did not have a MF, SSB, and NAVTEX. 46 CFR 184.502

A vessel must comply with the requirements for any radio installations, including the requirements for a station license and installation certificates issued by the FCC, in 47 CFR part 80. Vessels Radio Station Certificate call sign did not match the Radio Safety Telephony & Bridge-to-Bridge certificate, and EPIRB registration. 46 CFR 184.502

Each distribution panel and switchboard must protected from falling debris and dripping or splashing water. Distribution panel front access door IVO starboard generator will not close. 46 CFR 183.330

All machinery and equipment must be installed to provide protection from causing a fire, explosion, machinery failure, and personnel injury. Inboard port MDE has excessive corrosion on the wet exhaust connection. Complete prior to dry dock credit. 46 CFR 182.100

Spanner wrenches must be provided where a 1.5" diameter firehose is required. Spanner missing from hose station next to fire pump. 46 CFR 181.310

All machinery and equipment must be installed to provide protection from causing a fire, explosion, machinery failure, and personnel injury. Starboard generator injection pump delivery pipes were leaking diesel. Both generators had a pooling mixture of oil/diesel and oil soaked rags. 46 CFR 182.100

At each inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: all smoke and fire detection systems. The fire detection panel was exhibiting a fault in Zone 1, engine room. 46 CFR 176.810

All machinery and equipment must be installed to provide protection from causing a fire, explosion, machinery failure, and personnel injury. MDE #3 shaft seal was leaking an excessive amount of water. 46 CFR 181.200

A vessel must comply with the requirements for any radio installations, including the requirements for a station license and installation certificates issued by the FCC, in 47 CFR part 80. Master had a Restricted Radio Operator Permit and the mate did not have Marine Radio Operator Permit on-board. 46 CFR 184.502

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. A temp repair was found on STBD stern hull plate, Repair to approved proposal. Repair approximate 1/4" hole in engine room exhaust duct. 46 CFR 177.100.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck. Add lighting to aft engine room to properly illuminate aft door. 46 CFR 183.432.

Batteries must be secured to protect against shifting with the roll and pitch of the vessel and must be mounted in trays lined with, or constructed of, a material that is resistant to damage by the electrolyte. Properly secure generator batteries. 46 CFR 183.350.

Each distribution panel or switchboard must be totally enclosed and of the dead front type. Repair power distribution panel door in generator room. 46 CFR 183.330 (b).

All safety and relief valves for use on pressure vessels or piping systems shall be designed to meet the protection and service requirements for which they are intended and shall be set to relieve at a pressure which does not exceed the “maximum allowable working pressure” of the pressure vessel. Unable to verify relief valve setting due to corrosion on tags. 46 CFR 182.330, 46 CFR 54.15-10 (a).

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. During inspection of the hull exterior, the MI observed the following cracks to hull welds: 19” crack on stbd to stern hull weld, 16”x.5” crack at stern to main deck hull weld and a 9”x1/4” crack at main deck to port quarter side shell hull weld. Crack to stern main deck weld showed light penetrating into steering compartment. Crew stated they were unaware of hull damage. Identification of hazardous situation, in combination with the additional deficiencies identified during the inspection shows objective evidence that a serious substandard condition of the vessel is not being proactively managed onboard.

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's…stability letter...The vessel may not depart until it is in compliance with these requirements. The vessel master was unaware of how to complete loading calculations as required by vessels stability letter. Additionally, vessel did not have onboard the ability to determine weight of under deck cargo/fuel/ballast to assist in making this determination. Company shall provide master with necessary training and tools to efficiently calculate safe loading conditions prior to departure.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties. Each fire drill must include: Instruction in the use and location of fire alarms, extinguishers, and any other fire fighting equipment on board. When questioning the deckhand on previous training and knowledge for operating the vessels fixed CO2 system, the deckhand had not received instruction nor had knowledge of the operation of the system. Vessel shall ensure all crewmembers are familiar with operating the fixed CO2 system prior to departure.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Main engines exhausts had numerous excessive leaks inside engine room. Excessive amounts of soot was observed by the MI throughout the entire engine room. Vessel corrected all leaks prior to arrival however, vessel shall proactively manage leaks to ensure hazardous gases do not accumulate inside enclosed spaces.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems…Approximately 6” of oily water mixture was identified in the engine room bilges. Vessel shall remove oil from bilge prior to departure.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Vessel has several 12V wires in engine room overhead with excessive deterioration to outer sheathing. Company electrician made temporary repairs using electrical tape however, permanent repairs shall be carried out at vessels next drydock.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Charts onboard vessel had not been corrected or updated with the most recent local notice to mariners. Additionally electronic chart system onboard had not been updated since March, 2021. Vessel could not provide a copy of the latest notice to mariners. Vessel shall ensure charts used onboard are updated for intended voyage prior to departure.

Each EPIRB…must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; The EPIRB test…shall be logged or otherwise documented, as applicable. No records of the monthly EPIRB tests could be presented to the MI. Vessel shall ensure monthly tests are performed and logged prior to departure.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets...Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18.) Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. Personal flotation devices were stored under seating without any markings notifying passengers of storage location in the event of emergency use. Vessel shall apply appropriate markings within 30 days. Where pressure-indicating gages are used, they shall be chosen to be compatible with the pressure to be indicated. The size of the visual display, the fineness of graduations, and the orientation of the display will be considered. In no case shall the upper range of the gage be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function. Pressure gauge on starboard outboard air receiver was unreadable. Pressure gauge seal was missing and fluid inside gauge filled with soot from exhaust leaks, making the gauge unreadable. Vessel shall replace gauge within 30 days to ensure proper indication of pressure can be verified.

Any vessel of at least Five net tonswhich engages in Coastwise trade must have a Certificate of Documentation with a valid endorsement. Vessel underwent a name change from CANDY MAKER to GOL MAKER and is awaiting an updated Certificate of Documentation.

On a vessel of more than 65 feet in length, each hose must conform to UL19. 02 Fire hoses ruptured while under pressure. Hoses were destroyed and two new hoses were purchased.
