Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Currently installed valves are inside the engine space and not shielded from flames.

Vessel suffered smoke and heat damage to port hull and surrounding equipment after adjacent vessel at marina caught fire. Submit damage assessment, repair proposals prior to carrying passengers.

Vessel must conduct full power sea trials with CGMI after engine installation and rebuild is complete.

All battery cables and switches were replaced with new and vessel operates on a 24 volt system that can be parallel; must provide documentation to prove new installation can handle the volts/amps and is an in kind replacement.

Found hole in port quarter indicative of puncture. Mat and resin breached. Hole was covered over with unknowne compound. Hole was approximately 3"x3".

In forward void, two transverse frames (port and starboard) found cracked longitudinally. Glass was cracked but unknown whether frame was cracked beneath. Starboard had been previously repaired and had re-cracked. Indicative of structural failure of frames.

Port rudder sheared off, vessel proceeding to shipyard to make repairs. Reported starboard rudder also had significant crack in weld requiring replacement.

Company management shall provide USCG w/ written procedures for vessel (start-up/securing).

Post start-up/shut-down procedures at operating station.

NY-02 Re-lable valves for bilge manifold.

NY-03 Post instructions for fixed CO2 operations.

NY-01 Replace 1 adult lifejacket.

4 Through-hull bolts in #3 void, stbd side, are leaking water. Make permanent watertight repairs.

The top welds on all 4 brackets for the fuel tank are cracked. V/W all top welds.

There is a wire splice in the lazarette lying in the bilge. Properly mount wiring and place in an enclosure.

Provide CPR training for 50% of vsl crew.

Provide servicing certificates for fixed fire fighting system.

Properly repair navigation lights.

Properly repair "weeping" areas in forepeak & void #2.

Properly repair collision bulkhead to prevent water leaking into void #2.

Place 3' to 6' of line on waterlight.

Properly repair leaks in exhaust (water leaks) for MDE.

Place bonding wire on fuel fill lines.

Reconnect grounding wire in lazaret.

Properly repair under deck cross member in forepeak, stbd side.

Replace fire ext bracket in pilot area

Prove proper operation of bilge high level alarm audible indicators. Vessel personnel must check each space every 30 minutes and log results in ship's log while carrying passengers.

V/W numerous cracked welds to forward stanchions IWO fuel tank. LEGACY COMPONENT DESC: Tank

Replace discharge hose to electric submersible pump with hose which is salt water, petroleum oil, heat and vibration resistant.

INSTALL PORT RUDDER POST ASSEMBLY IN ACCORDANCE WITH 46CFR176.610 LEGACY COMPONENT DESC: Rudder

REPLACE VESSELS 25 POUND DANFORTH ANCHOR WITH ANCHOR SUITABLE FOR VESSELS LENGTH/WEIGHT IN ACCORDANCE WITH 46CFR184.300. LEGACY COMPONENT DESC: Anchor

REPAIR FORWARD, STARBOARD SIDE FUEL TANK SUPPORT BRACKET IN WAY OF OVERHEAD TEE PLATE. LEGACY COMPONENT DESC: Tank

Install and demonstrate the proper operation of externally mounted activation device for fixed fire extinguishing system.

Provide proof of valid renewal sticker for Certificate of Documentation on board the vessel. Confirmed with USCG VDOC that reneal has been mailed to the vessel, but as of the inspection date, the vessel had not received the new sticker.

PROVIDE PROPER OPERATION OF DEPLOYING LIFE FLOATS AND PROPER USE OF THE WEAK LINK.

INSTALL ILLUMINATED MAGNETIC COMPASS. (MISSING LIGHT)

INSTALL FFF PIPING THAT IS OF APPROVED TYPE. (A-53) GRADE A, OR B OR (A- 106) GRADE A, B, OR C.

BILGE ALARMS NEED TO BE IDENTIFIED ON BRIDGE AS TO LOCATION OF OPERATION.

PROVIDE PROOF OF PROPER OPERATION OF FFF AUTOMATIC SHUTDOWNS, AND REPAIR CO2 CYCLINDER HEADS AND CONDUCT OPERATIONAL TEST. (NO SAIL) INTILL THIS IS COMPLETE.

PROVIDE WATERPROOF AND BRIGHTLY STENCILED DISTRESS SIGNAL CONTAINER FOR FLARES.

LIFE FLOATS NEED VSL NAME AND MAX CAPACITY STENCILED ON BOTH SIDES.

PROVIDE AND PROPERLY POST EMERGENCY CHECKOFF LIST.

LABEL VALVES AT BILGE MANIFOLD TO INDICATE COMPARTMENT SERVED.

PROVE PROPER OPERATION OF EMERGENCY REMOTE SHUT OFF FOR STARBOARD MAIN ENGINE.

PROVIDE GUARDS FOR FIRE/BILGE PUMP DRIVE BELTS.

PROVIDE DOCUMENTATION THAT THE MARINE SANITATION DEVICE IS U.S. COAST GUARDAPPROVED._x001C_
