Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Dead end wires found in the following areas: above pilothouse (2 sets), under pilothouse console (2 sets), forward voids (port and stbd), lazarettes (port and stbd). 46 CFR 183.200(b)

A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. 02 penetrations in the aft bulkhead of the port forward void are not watertight. 46 CFR 179.320(c)

Nonmetallic hose may be used for wet exhaust systems provided it is...double clamped where practicable. Flexible hose connections to the port main engine exhaust were made with one clamp. 46 CFR 182.430(e)

Each battery must be…secured to protect against shifting with the roll and pitch of the vessel. Battery boxes not secured, both port and stbd. 46 CFR 183.350(b)

Nonconducting mats or grating must be provided on the deck in front of each switchboard. Mat missing in front of switchboard. 46 CFR 183.330(f)

All electrical equipment and circuits must be clearly marked and identified. Breaker panel near switchboard has multiple labels for same breaker. 46 CFR 183.220(d)

Exhaust gas must not leak from the piping or any connections. The starboard main engine exhaust was leaking above the aft end of the engine. 46 CFR 182.430(b)

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. The IBA painter was found not secured to the vessel. 46 CFR 180.130(a)(1)

A qualified servicing facility must conduct inspections on portable extinguishers and fixed-gas extinguishing systems. Annual fire protection inspection has lapsed. Owner must provide satisfactory evidence of required servicing. 46CFR176.810(b)(1)(2)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure and personal injury. Port motor has excessive vibration. 46CFR182.100

An inflatable buoyant apparatus must be serviced at a facility approved by the Commandant no later than the month and year on its servicing sticker. IBA servicing has lapsed. Must provide proof of servicing. 46CFR185.730(a)(1)

Penetrations and openings in watertight bulkheads must have means to make them watertight. Non watertight penetrations found in the following spaces: port and starboard battery room to port and starboard fuel tank void, port and starboard fuel tank void to port and starboard engine room. 46CFR171.114(a)(2)

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arrestors. Fuel tank vent pipe missing flame screen or flame arrestor. 46CFR182.450(e)

Penetrations and openings in watertight bulkheads must have means to make them watertight. Conduit running from bow to engine room does not have any cap or plug and is fractured in battery room rendering all spaces non-watertight. 46CFR171.114(a)(2)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Found the following three items expired in the first-aid kit: burn dressing, ammonia inhalants, and iodine wipes. 46CFR184.710

Penetrations of required watertight decks and bulkheads by any rigid plastic pipe are prohibited unless the following requirements are met: each penetration must be accomplished using an acceptable metallic through deck or through bulkhead fitting that is welded or otherwise attached to the bulkhead or deck by an accepted method. Vessels submersible electric bilge pumps utilize PVC piping to lead into the common bilge overboard pipe. A metallic shutoff valve must be attached at the overboard pipe in lieu of PVC. 46 CFR 182.720 (d)(1)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Received notification from operator that vessel was taken out of service due to a leaking pipe associated with port main engine sea chest. Provide repair proposal and affect permanent repair to the satisfaction of attending marine inspector. 46 CFR 176.840

The requirements for machinery for electrically propelled vessels are contained in applicable regulations in subchapter J (Electrical Engineering) of this chapter. Each electric propulsion installation must meet section 4-8-5/5.11 of ABS Steel Vessel Rules (incorporated by reference; see 46 CFR 110.10-1). Where two or more control stations are provided outside the engine room, or where the propulsion machinery space is intended for centralized control or unattended operation, the provisions of Part 4, Chapter 9 are to be complied with. Electric power for control, monitoring and safety systems is to be fed from two feeders, one from the main switchboard or other suitable distribution board and the other from the emergency switchboard or an emergency distribution board. The supply status of these feeders is to be displayed and the main power supply failure is to be alarmed. The electric power supply to each of the control, monitoring and safety systems is to be individually monitored. The two feeders are to be connected to a transfer switch in the remote control station. Vessel currently only has one power source for control system. 46 CFR 182.220(b), 46 CFR 111.35-1, ABS Steel Vessel Rules 4-8-5/5.11.2, 4-9-2/11.7.1, 4-9-2/11.7.2

At each inspection for certification all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Vessel bilge/fire piping possessed multiple leaks. Leak witnessed in bilge pipe within the port engine room at bend located above the manifold. Second leak witnessed in crossover pipe connecting the port and starboard engine room bilge systems. Vessel must submit repair proposal to include permanent repair plan to the affected system.

Prove proper operation of override/means or reset at the operating station. 46CFR181.420

Provide repair plan for approval and make permanent watertight repairs for the nickel sized hole in the deck leading into the starboard side lazarette. 46CFR177.100

Repair damage to handrails on port bow. No passengers are allowed on bow until repairs are completed.

Provide a repair proposal for identified through bulkhead cable penetrations for engineering spaces bulkheads in laz spaces, eng rms, and fuel tank voids, as per NVIC 9-97.

Vessel had a loss of propulsion when the variable frequency device tripped offline. O/O must prove proper operation of the propulsion system to the satisfaction of the attending marine inspector.

Provide a repair proposal for the damaged stbd bow as a result of the allision with a pier.
