The company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting reliability of such equipment or systems. These measures should include regular testing of stand-by arrangements and equipment or technical systems that are not in continuous use. During ACP annual oversight inspection, Marine Inspector found that oxygen sensors that are installed at EDG and paint locker were not operational and/or been rendered inoperable. The oxygen sensor installed at the CO2 storage room displayed 19.8% oxygen concentration. Additionally, there were no records of calibration for this equipment. After looking into the SMS, it was determined that the SMS does not provide instructions to personnel on how to safely enter (or not enter) a space monitored by oxygen sensing equipment, what to do if the oxygen sensors are inoperable, how to maintain and test the oxygen sensors, and how and when to calibrate the sensors. Develop procedures for maintaining/calibrating and usage of these oxygen sensors and ensure that measures are integrated into ship’s operational maintenance routine as required by ISM 10.4. 74 SOLAS (20) IX/3.1 ISM 10.3

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. Vessel has an outdated checklist for bringing vessel from ROS to FOS status. Vessel also decommissions lifesaving equipment (such as lifeboats not in davits) and firefighting equipment (such as fixed foam system in cargo spaces) while in ROS. The pre-deployment preparation of these items are not captured in any current SMS product. Develop updated checklist, plan, procedure, or instruction that includes all necessary items for preparing vessel for fully operational status and ensure tasks are assigned to qualified personnel. 74 SOLAS (20) IX/3.1 ISM 7

During inspection, Marine Inspector found the following deficiencies that need to be resolved to the satisfaction of ABS: -Battery Powered E-Light inoperable at the EDG Room -SSDG exhaust fan excessively oil covered creating a fire hazard -Bulkhead valves (engine room to aft steering) providing watertight boundary were not on test schedule or emergency procedures for flooding -The following propulsion boiler system gauges found inoperable: Port main steam inlet pressure, stbd HP Bleed pressure, and chemical feed pump discharge pressure. -Holes found on the operating flat decking forward of the stbd boiler. -Inoperable light with damaged support structure identified for the port main turbine L/O sightglass -Leaking Y Gate valves noted on Fire Stations 66 &68. 74 SOLAS (20) I/11(a)

Fire-fighting systems and applicances shall be kept in good working order and readily available for immediate use. Jubilee patch was found on the fire main adjacent to the C-deck riser cut out valve and a leak was identified at the cutout valve for stations 169, 162, 153, & 146. 74 SOLAS (20) II-2/2.1.2

Where ‘A’ Class divisions are pierced for the passage of electric cables, arrangements shall be made to ensure that the fire resistance is not impaired. Horizontal deck cable transits under the bridge console (03), in the bridge electrical station room (01), and in the gyro room (01) were found modified with sealants not part of the transit design or type approval. 60 SOLAS II/38(a)

Bulkheads at the exposed ends of superstructures shall be of efficient construction and shall be to the satisfaction of the administration. The enclosed forward mooring deck found with two deteriorated chock drains that are now open to the exterior of the vessel. ICLL (88) II/11

The watertight door at bulkhead 198 (cargo hold C) is missing a section of lower channel gasket rendering it non-watertight. Eft engine room bulkhead on the 3 deck found with large bolted openings; only 2 bolts per plate were found installed leaving the remaining holes open. This bulkhead should be a A-60 machinery space boundary. 60SOLAS II/9(b)

The lights prescribed in these Rules shall have an intensity so as to be visible at the following minimum ranges: in a vessel of 50 meters or more in length, a masthead light - 6 miles. Masthead light onboard is inoperable. Repair light to meet minimum visibility requirements above. 33 CFR 83.22

A marine casualty occurred on 27 May 2018 in which a mooring line parted and severely injured multiple crew. Upon inspection, the vessel's mooring lines were found heavily worn and deteriorated and the vessel had no accurate records of installation, maintenance, or inspections. Vessel shall develop and implement planned maintenance as outlined in the vessel's SMS. 33 CFR 96.240(b) ISM Code Part A 10.2

Vessels in oceans service shall be fitted with hawsers in general agreement with the Standards established by the American Bureau of Shipping. Based on material condition of the hawsers, the ALGOL shall provide objective evidence that they are in keeping with the standards published by ABS. 46 CFR 96.07-5(a)
