The safety management system should provide for specific measures aimed at promoting reliability of such equipment or systems. These measures should include regular testing of stand-by arrangements and equipment or technical systems that are not in continuous use. During ACP annual oversight inspection, Marine Inspector found that oxygen sensors that are installed at EDG and paint locker were not operational and/or been rendered inoperable. The oxygen sensor installed at the CO2 storage room displayed 19.8% oxygen concentration. Additionally, there were no records of calibration for this equipment. After looking into the SMS, it was determined that the SMS does not provide instructions to personnel on how to safely enter (or not enter) a space monitored by oxygen sensing equipment, what to do if the oxygen sensors are inoperable, how to maintain and test the oxygen sensors, and how and when to calibrate the sensors. Develop procedures for maintaining/calibrating and usage of these oxygen sensors and ensure that measures are integrated into ship’s operational maintenance routine as required by ISM 10.4.

Upon inspection, the vessel's mooring lines were found heavily worn and deteriorated and the vessel had no accurate records of installation, maintenance, or inspections. Vessel shall develop and implement planned maintenance as outlined in the vessel's SMS. 33 CFR 96.240(b) ISM Code Part A 10.2
