Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. Special consideration shall be given to the malfunctioning of: the hydraulic, pneumatic or electrical means for control in main propulsion machinery including controllable pitch propellers. However, the administration, having regard to overall safety considerations, may accept a partial reduction in propulsion capability from normal operation. Vessel reported a ground fault in both windings of port Propulsion Electric Motor (PEM)

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Outboard forward life raft davit was not immediately ready for use due to an electrical failure. Life boat number 5's engine could not be stopped utilizing the engine stop button. - 74 SOLAS 20/III 20.2

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. The loft and video game room on deck 9, MVZ 1, is classified as a category 8 space. PSCO observed unauthorized stowage of approx. 30 cardboard boxes and other items. - 74 SOLAS 97 II-2/26.2.2

Fire doors in main vertical zone bulkheads and stairway enclosures, designed to re-open upon contacting an object in its path shall re-open sufficiently to allow a clear passage of at least 0.75 m but no more than 1 m. Sliding fire door 3.4.028 to atrium was not re-opening after contact was made.- 74 SOLAS 97 II-2/30.4.2

Where "A" class divisions are penetrated for the passage of electric cables, pipes, trunks, ducts, etc., or for girders, beams or other structural members, arrangements shall be made to ensure that the fire resistance is not impaired. Piping going through bulkhead in "Cascade Room" on deck 09 has gaps in SFP. 74 SOLAS 97 II-2/18.1.1 (16ac)

Where "A" class divisions are penetrated for the passage of electric cables, pipes, trunks, ducts, etc., or for the girders, beams or other structural members, arrangements shall be made to ensure that the fire resistance is not impaired. Pipe going through A-60 is made of non-metallic material and is free moving with in the penetration in way of fire door A.1.098. 74 SOLAS 97 II-2/18.1.1 (16ac)

Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space or group of spaces at least one of which shall give access to a stairway forming a vertical escape. There has been a curtain added to the "Kings Room" on deck 5 turning it into a space within a space. No secondary escape route or audible alarm was in place. 74 Solas 97 II-2/28.1.2 (16ac)

The purpose of this requlation is to maintain and monitor the effectiveness of the fire safety measures the ship is provided with. Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. Fire nozzle was found defective during drill. 74 SOLAS 97 II-2/14.1

For determining the appropriate fire integrity standards tobe applied spaces are classified according to the fire risk. Gangway corridors on Decks 2 and 3, prot and starboard sides, are being imporperly used for storage; not meeting the fire integrity requirements of the designated space. 74 SOLAS 97 II-2/14.1

Furniture shall not be permitted in the passenger and crew corridors forming escape routes in cabin areas. PSCO's observed a bench on Deck 6, Zone 1, stairwell not secured. 74 SOLAS 97 II-2/41-2.4.10

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Section valve indication on the SMCS does not match the correct indication on the fire detection system. 74 SOLAS 01 II-2/14.2.1.2

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures other than the power operated watertight doors and those which are normally locked shall satisfy the following requirements. A door designed to re-open not more than 1 m from the point of contact. All sliding fire doors do not immediately stop and re-open upon actuation of the crash bar. 74 SOLAS 97 II-2/9.4.1.1.5.11

ELECTRICAL CONDITIONS SHALL BE SUCH THAT: THE SAFETY OF PASSENGERS, CREW, AND SHIP FROM ELECTRICAL HAZARDS WIL BE ENSURED. PSCO OBSERVED EXPOSED WIRING ON ELECTRICAL BOX ABOVE DOOR A-053.

THE CONSTRUCTION OF ALL DOORS AND DOOR FRAMES IN "A" CLASS DIVISIONS, WITH THE MEANS OF SECURING THEM WHEN CLOSED, SHALL PROVIDE RESISTANCE TO FIRE AS WELL AS TO THE PASSAGE OF SMOKE AND FLAME, AS FAR AS PRACTICABLE, EQUIVALENT TO THAT OF THE BULKHEADS IN WHICH THE DOORS ARE SITUATED. SUCH DOORS AND DOOR FRAMES SHALL BE CONSTRUCTED OF STEEL OR OTHER EQUIVALENT MATERIAL. PSCO OBSERVED DOOR A041-A053 IN FIRE ZONE 4 WAS NOT SECURING AS INTENDED.

The construction of all doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. Such doors and door frames shall be constructed of steel or other equivalent material. Fire doors 6.3.018, 7.3.020 & 7.4.016 gaskets were deteriorated rendering the fire doors unable to seal appropriately. 74 SOLAS (97 CONS) II-2/30.2

Hydraulic power-operated steering gear shall be provided with a low-level alarm for each hydraulic fluid reservoir to give the earliest practicable indication of hydraulic fluid leakage. Audible and visual alarms shall be given on the navigation bridge and in the machinery space where they can be readily observed. After testing low hydrualic level at both port and starboard steering rooms, audible and visual low level alarms inoperable at both engine control room and navigation bridge. Steering alarm panel at engine control room and navigation bridge is not receiving alarm signal. 74SOLAS(04)II-1/29.12.2 Deficiency code was initially a code 17ac, but then updated to a 10c, as it was corrected on the spot prior to the team departing the vessel.

Any main and auxiliary steering gear control system operable from the navigation bridge shall in the event of a failure of electrical power supply to the control system, an audible and visual alarm shall be given on the navigation bridge. After testing loss of power alarm at both port and starboard steering rooms, audible and visual loss of power alarms inoperable at both engine control room and navigation bridge. Steering alarm panel at engine control room and navigation bridge is not receiving alarm signal. 74SOLAS(04)II-1/29.8.4 Deficiency code was initially a code 17ac, but then updated to a 10c, as it was corrected on the spot prior to the team departing the vessel.

Muster and embarkation stations shall be adequately illuminated by lighting supplied from emergency source of electrical power. 02 embarkation lights on the starboard side were found to be inoperable. 01 between life boat #1 and lifeboat # 3, and 01 aft of the starboard side launching station. 74SOLAS(97)iii/11.4.

Fire-extingushing appliances shall be kept in good order and be available for immediate use at all times. On bulkhead deck, starboard side near sideshell door opening, section valves were blocked by excessive storage of palletized garbage/ship's stores. Improperly stored items were removed from area. Additionally at sprinkler station #22, 02 section valve gauges were missing fluid and out of callibration specifications, needing to be replaced. Gauges each read approx. 7 bar, which was the appropriate pressure for the system. 74SOLAS(97)II-2/21

The public address system shall be clearly audible above the ambient noise in all spaces, and shall be provided with an override function controlled from one location on the navigational bridge. The entertainment sound system in the Ocean Bar on Deck 05 does not have an override function. 74 SOLAS 97 III/6.5.3

To assist in casualty investigations, ships, when engaged on international voyages, subject to the provisions of regulation 1.4, shall be fitted with a voyage data recorder (VDR). if the ship's emergency source of electrical power supply fails, the VDR should continue to record Bridge Audio from a dedicated reserve source of power for a period of 2 h (resolution A.861(20)). Vessel's VDR alarm panel states "no battery pack connected". 74 SOLAS 20 V/20.1

All life-saving appliances and arrangements shall comply with the applicable requirements of the Code. All lifeboats shall be properly constructed...Lifeboat No 10 has an approximate 12 inch crack around the forward part of the starboard fiberglass strut joint and an approximate 5 inch crack on the aft part of the strut joint. Lifeboat No 9 has an approximate 3 inch crack on the forward port side fiberglass strut joint. 74 SOLAS 20 III/34/ LSA Code 4.4.1 See Pictures in Documents to support Deficiency findings.

The safety management system should include procedures ensuring that non-conformities, accidents and hazardout situations are reported to the company, investigated and analyzed. The company should establish procedures for the implementation of corrective action, including measures intended to prevent recurrence. Vessel does not have written procedures to properly montior structural cracking. Crew provided an inaccurate "crack log." Area of cracks and poor welding repairs at FSD A.5.011 deck A are not captured accurately. Picture and written description in the log did not represent the observed area. - ISM Code 9.1 & 9.2

Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space. The digital workshop located on deck 5 is a room-in-room configuration. Additionally items #4&5 on letter 16710/PO4436/plc from the USCG Marine Safety Center have not been addressed. - 74SOLAS(2014)II-2/28.1.2

In all ships general arrangement plans shall be permanently exhibited; Plans shall be kept up to date. Posted fire control plans & the escape & evacuation plans show conflicting arrangements for the digital workshop located on deck 5; to include but not limited to escape routes and fire doors. - 74SOLAS(1997)II-2/20.1

Sprinklers shall be placed in an overhead position and spaced in a suitable pattern. Numerous sprinkler heads located throughout the ship have been damaged and/or misaligned, thereby altering their intended spray pattern. - 74SOLAS(1997)II-2/12.3

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Several stairway guides failed to display familiarity with their assigned emergency duties - 74SOLAS(2014)III/19.2.1
