THE COMPANY SHOULD ISSUE PROCEDURES TO ENSURE THAT THE SHIP IS MAINTAINED IN CONFORMITY WITH THE PROVISIONS OF THE RELEVANT RULES AND REGULATIONS AND WITH ANY ADDITIONAL REQUIREMENTS WHICH MAY BE ESTABLISHED BY THE COMPANY. THE VESSEL DID NOT HAVE SUFFICIENT FUEL ONBOARD TO MAKE THE NECESSARY VOYAGE INTENDED. AN INTERNAL ISM AUDIT AND RECORD OF CORRECTIVE ACTIONS, INCLUDING ACTIONS TO PREVENT RECURRENCE IAW ISM 9, IS REQUIRED. 74 SOLAS (20) IX/3.1 ISM CODE 10.1

TIMES FOR SUBMITTING NOTICE OF ARRIVALS ARE AS FOLLOWS: IF YOUR VOYAGE TIME IS LESS THAN 96 HOURS, BEFORE DEPARTURE BUT AT LEAST 24 HOURS BEFORE ARRIVING AT THE PORT OR PLACE OF DESTINATION. THE VESSEL DID NOT SUBMIT AN NOA IN THE REQUIRED TIME FRAME. 33 CFR 160.212 (a)(4)(ii)

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. PSCO found combustible materials being stored in A/C station 8735 (ire door 08.7.07) in a room without fire suppression system. 74SOLAS97II-2/26/2.2. 16ac.

The marking must enable passengers to identify all the routes of escape and readily identify escape exits. All escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. PSCO Observed that the Main Office Front Desk Reception in deck 4, leading to Back Office through a stairways to deck e below lacks of adequate emergency escape signage. Also, there are combustible materials stored under the stairways. 74SOLAS97II-2/28.1.10 16ac.

The construction of all doors and door frames in ``A'' class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. Such doors and door frames shall be constructed of steel or other equivalent material. PSCO observed fire door FSD 06.7.07 not closing, its hose port not closing due to damage spring. 74SOLAS97II-2/30.2 16ac.

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. Fire hose cabinet and portable fire extinguisher, and emergency exit door were being blocked by boxes with supplies in deck 4, adjacent to FSD 4.7.20. 74SOLAS20 II-2/14.1.1 10C.

All life-saving appliances and arrangements shall comply with the applicable requirements of the Code. The propeller shafting shall be so arranged that the propeller can be disengaged from the engine. Provision shall be made for ahead and astern propulsion of the lifeboat. PSCO observed lifeboat number 11 without astern propulsion.

All "A" class doors located in stairways, public spaces and main vertical zone bulkheads in escape routes shall be equipped with self-closing hose port of material, construction and fire resistance which is equivalent to the door into which it is fitted, and shall be inset into the lower edge of the door, opposite the door hinges or, in the case of sliding doors, nearest the opening. PSCO observed FSD 15.3.01 not fully closing.

The capacity of these generating sets shall be such that in the event of any one generating set being stopped it will still be possible to supply those services necessary to provide normal operational conditions of propulsion and safety. On May 19th 2025, the GRAND PRINCESS experienced a loss of propulsion while operating on two diesel generators. When DG #1 experienced a cooling water pump failure and shutdown. DG #6 was unable to provide operational propulsion, and the vessel was without propulsion for approximately 9 minutes. [name withheld] stated the vessel routinely operated with only 2 generators. SOLAS 74 (2004) II-1/41.1.2

Every ship of 100 gross tonnage and above, and every ship which is certified to carry 15 or more persons, and fixed or floating platforms shall carry a garbage management plan which the crew shall follow. PSCO observed a 6 ft high pile of unsorted garbage including hard plastic, food waste, paper, and other items in the recycling center. The pile of garbage extended into the recycling center blocking access to the space's fixed foam system activation point. Ship's crew stated that this system was not in accordance with the ship's procedures. In addition, a plastic container of unsorted garbage was found on deck 6, crew stair, forward by the bosun's store- crew stated garbage was collected here during the day. MARPOL 2017 Annex V / 10.2

PSCO observed a 6 ft high pile of unsorted garbage including hard plastic, food waste, paper, and other items in the recycling center. The pile of garbage extended into the recycling center blocking access to the space's fixed foam system activation point. Ship's crew stated that this system was not in accordance with the ship's procedures. 74 SOLAS 20 II-2/14.1.1

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. PSCO observed eight full oxygen cylinders without protective caps on full cylinders. 46 CFR 147.60 b.

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. PSCO observed fire suppression system in forward purifier room did not operate upon remote activation. The crew discovered the solenoid connector was loose at the valve. SOLAS 20 II-2/14.1.1

Fire doors in main vertical zone bulkheads and stairway enclosures shall be self-closing and shall have an approximately uniform rate of closure of no more than 40 s and no less than 10 s. PSCO observed fire screen doors 01.05.05 and 5.03.03 did not close when tested. SOLAS 97 II-2/30.4.1 17 c

All life-saving appliances and arrangements shall comply 74 with the applicable requirements of the Code. Except as otherwise stated, the normal equipment of every rescue boat shall consist of a searchlight with a horizontal and vertical sector of at least 6° and a measured luminous intensity of 2,500 cd which can work continuously for not less than 3 h. PSCO observed port side rescue boat searchlight inoperative. SOLAS 20 III/34 LSA Code 5.1.2.2.11 17 ac

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Starboard midship life raft launching crew were not familiar with procedures for launching life raft as witnessed by PSCO during launching of the training life raft during abandon ship drill. 74 SOLAS 20 III/19.2.1

All waste-receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. PSCO observed plastic waste-receptacles used with combustible materials in multiple galley spaces, and the ventilation officer office. 74 SOLAS 97 II-2/18.5

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. Following onboard procedures were not maintained in accordance with the vessels safety management system (SMS): 1. Garbage management plan, section 4.2.3 (Garbage assortment) - Garbage was left unassorted in recycling center and crew stairway. 2. SOLAS training manual, Section 4.11 (Instructions for Life Raft Preparation Party - Life raft deployment team displayed lack of familiarity during life raft deployment evolution. Crew needed assistance from shoreside safety management team to identify safety hazards and proper deployment procedures. 3. ), MED-4004 Medical Guide. Section 2.2 (Storage of Medical Gases) - Crew in charge of verifying storage of more that a dozen 100lbs pressurized storage tanks were unaware that they were stored incorrectly. Full oxygen tanks were stored without valve protection caps. Per staff, shoreside training did not provide accurate instructions on how to properly store the pressurized tanks. Due to multiple areas of the SMS system being affected, an external audit is required. 74 SOLAS 20 IX/3.1 ISM Code A.10.1

General arrangement plans shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "B" class divisions together with particulars of the fire detection and fire alarm systems the sprinkler installation, the fire-extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating system, including particulars of the fan control positions, the position of dampers and identification numbers of the ventilating fans serving each section. PSCO observed a storage closet containing combustible materials in the crew office on deck 4 MVZ 5. The space was not outlined on the fire control plan with categorization nor did the space contain fire suppression or detection. 74 SOLAS 20 II-2/15.2.4.1

Within the perimeter of such stairway enclosures, only public toilets, lockers of non-combustible material providing storage for non-hazardous safety equipment and open information counters are permitted. On deck 6,forward, by the bosun's store, PSCO observed plastic bins storing plastic bags of unsorted garbage. 74 SOLAS 97 II-2/29.2.

Cabin doors in ``B'' class divisions shall be of a self-closing type. Hold-backs are not permitted. PSCO observed cabin 3342 door unable to lock in the closed position. 74 SOLAS 97 II-2/31.1.2

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. Fire extinguishers 1.IV.01 and 1.IV.07 were observed to be blocked in the engine control room. Additionally, fire station in the gym on deck 15 would not open due to handle falling off. Fire station on deck 14 zone 4 port side, near the pool and women's restroom was unable to open, the surrounding frame blocked fire station door from opening. 74 SOLAS 20 II-2/14.1.1 10c

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. Low location lighting was being blocked by insulation in the vicinity of FSD 08.1.04. In addition, low location lighting in the vicinity of semi-portable fire extinguisher 5.VI.11 on deck 5, was extinguished. 74 SOLAS 01 II-2/41-2.4.7 16ac

Fire doors in main vertical zone bulkheads and stairway enclosures, remote-controlled sliding or power-operated doors shall be equipped with an alarm that sounds at least 5 s but no more than 10 s before the door begins to move and continues sounding until the door is completely closed. Numerous sliding doors including FSD 15/III/09, FSD 10/VII/05 & FSD 10.7.04 did not alarm for at least 5 s but no more than 10 s before the doors began to move. 74 SOLAS 97 II-2/30.4.2 16ac

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed soft patch on jacket water plate cooler salt water discharge lin. 74 SOLAS 04 II-1/26.1 16ac

It shall be possible for each door to be opened and closed from each side of the bulkhead by one person only. FSD 15/III/09, FSD 10/VII/05 & FSD 10.7.04 would not close when manual release was activated. Additionally, FSD 15/III/09 & FSD 10/7/04 would not open once closed while the manual release was activated. 74 SOLAS 97 II-2/30.3 40ac

Additionally, escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. Emergency escape signage IVO lifeboat 19 was extinguished. 74 SOLAS 01 II-2/41-2.4.7 10c

All "A" class doors located in stairways, public spaces and main vertical zone bulkheads in escape routes shall be equipped with a self-closing hose port of material, construction and fire resistance which is equivalent to the door into which it is fitted. Hose port in FSD 06.2.06 was not self-closing and remained open when the door was closed. 74 SOLAS 97 II-2/30.7 10c

The arrangements for the storage, distribution and utilization of oil used shall be such as to ensure the safety of the ship and persons onboard. PSCO observed oiil accumulation below main engine #1. 74 SOLAS 97 II-2/15.2 10c

The means of escape, including stairways and exits shall be marked by lighting or photoluminescent strop indicators...at all points of the escape route including angles and intersections. Low location lighting on deck 7, aft IVO the atrium and mid passenger stair tower was extinguished. 74 SOLAS 97 II-2/28.1.10

Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any fire-fighting systems and appliances that they may be called upon to use. Two crewmembers in the dishwashing area deck 14, aft were unfamiliar with shipboard procedures in the event of a fire. In addition , during the fire drill, two crewmember in the cabins directly outside of the crew laundry (deck 4, fwd) were unfamiliar with shipboard procedures in the event of a fire. Furthermore, in the main laundry room, one crewmember was unfamiliar with the operation of firefighting appliances. 74 SOLAS 20 II-2/15.2.2.1

All escape route signs...shall be of photoluminescent material or marked by lighting. Backstage Princess Theater exit signage was not visible. In addition, emergency exit sign in the backstage area of the Vista Lounge was extinguished. 74 SOLAS 97 II-2/28.1.10

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power. Two embarkation lights on the port side were extinguished. 74 SOLAS 97 III/11.4

Lifeboats should demonstrate that the gearbox and gearbox train are engaging satisfactorily. Lifeboat #12 would not engage in reverse while testing propulsion with vessel in the cradle. SOLAS 97 III/19

The addition to the emergency lighting required...The marking must enable passengers to identify all the routes of escape and readily identify the escape exits. The men's and women's changing areas in the spa had no escape signage. SOLAS 97 II-2/28

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Aftmost fire station on the 17th deck, starboard side was inaccessible due to a seized latch on the access door. SOLAS 20 II-2/14.2.1.2

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. The life boat #8 commander was not familiar with the starting procedures for life boat engine. SOLAS 20 III/19.2.1

Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space. The pantry adjacent to the Oasis bar on the 17th deck was blocked at both exit locations. SOLAS 97 II-2/28

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Light in the male sauna lacking protective cover. 74SOLAS(04)II-1/45

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Dead end wires found on the lighting mount in fan room next to CO2 space. 74SOLAS(04)II-1/45

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Portable extinguishers which have been discharged shall be immediately recharged or replaced with an equivalent unit. CO2 door unopenable due to corroded locking mechanism. 74SOLAS(20) II-2/14.2.1.2

Openings in "A" class divisions. Fire door 14.501 could not close due to the kick plate getting caught. 74SOLAS(97)II-2/30

For any vessel that is equipped with ballast water tanks and bound for ports or places in the United States and not addressed in paragraph (b)(1) and (2) of this section submit a ballast water report no later than 6 hours after arrival at the port or place of destination, whichever is earlier. NBIC report not submitted for separate port calls within Honolulu COTP zone.

Materials used in the ships' structure shall ensure that the structural integrity is not degraded due to fire. Structural fire protection insulation found damaged in space with sprinkler section valve 20.

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. A section of low location lights in deck 5, zone 1, port side, near the crew cabins were inoperable.

Relief valves shall be provided in conjuction with all fire pumps if the pumps are capable of developing a pressure exceeding the design pressure. Pumps 02 and 03 are not equipped with relief valves. SOLAS(1997) II-2 / 4.3.5

Means shall be provided to control leaks of flammable liquids. The after starboard engine space bilges had substantial amounts of oil in the bilge. SOLAS(2014) II-2 / 4.1.1

The construction of floors and doorframes in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke. Fire screen door FS006.1.08 failed to latch. SOLAS(2014) II-2/4.1.1.2

The minimum fire integrity of all bulkheads and decks shall be prescribed in tables 23.1 to 23.4. Combustible material has been impermissably stored in category 10 and category 3 spaces, changing their categorization. SOLAS(1997) II-2 / 26.2

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent step indicators. Multiple escape routes are missing low locating lighting. SOLAS(1997)II-2/28.1.10

All waste receptacles shall be constructed of non-combustable materials with no openings in the sides or bottom. A paper waste receptacle is present in the international cafe pantry.

Each notice of arrival must contain all of the information items specified in table 160.206, uncluding the name and 24-hour contact information for the Company Security Officer. The vesse's NOA listed the Vessel Security Officer in lieu of the CSO.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Found an inoperable davit for lifeboat number 14 on the port side. The crew is unable to launch or recover lifeboat number 14. The Administration may permit the substitution of lifeboats by liferafts of equivalent total capacity provided that there shall never be less than sufficient lifeboats on each side of the ship to accommodate 37.5% of the total number of persons on board in accordance with SOLAS 1997 cons. Ch. III Reg 20. Total number of persons on board equals 3893 persons.

Crew members were unable to latch the incinerator space fire door, 4-6-45, closed after multiple attempts and maintenance.

Each separate circuit shall be protected against short circuit or overload. Multi-outlet adapters (power strips) found connected to other adapters (“daisy-chained”), or otherwise used in a manner that could overload the capacity of a receptacle on Deck 6 in the computer officer room, 6227.

Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. The mounting bracket for the port side rescue boat shore power electrical outlet was found wasted/corroded. Further corrosion damage to the mounting bracket may result in damage to the shore power electrical outlet.

07103- Division - bulkheads - Bulkheads forming fire boundaries between category (13) storerooms and category (8) public spaces shall be of "A-15" rating. The boundary between the disco and the pantry storeroom on deck 15, port, frames 38-43 is indicated on the SFP plan as being of "A-0" rating. - 74 SOLAS (97) II-2/26.1 - 50ac

0610: Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Lifeboat 4 was not capable of starting during the lifeboat drill. 74 SOLAS 2014 Ch. III 20.2

0710 - Sliding fire doors in main vertical zone bulkheads and stairway enclosures designed to re-open upon contacting an object in its path shall re-open sufficiently to allow a clear passage of at least 0.75 m but no more than 1 m. FSD 07\II\10 & 11 are sliding doors which opens more than 1m when contacting an object in its path. - 74 SOLAS (97) Ch. II-2 Reg. 30.4.2 - 50abc

0710 - Fire doors in main vertical zone bulkheads and stairway enclosures shall be self-closing and double-leaf doors equipped with a latch necessary to their fire integrity shall have a latch that is automatically activated by the operation of the doors when released by the system. FSD 11.4.06, 14.7.07, 04.3.05 & 06.7.05 did not self close. - 10c - 74 SOLAS (97) Ch. II-2 Reg. 30.4.1,5

Found the tender platform located starboard side, between frames 156 and 164, hanging from hinges. Found upon internal examination located aft laundry store space in way of the centre hinge a hole approximately 10cm that was temporarily plugged to stop water ingress

0900 - Safety in General - On-board training and instructions in the use of life saving appliances...and fire extinguishing appliances shall be given as soon as possible. 2nd boat commanders unable to demonstrate competence in operating life boat systems, including emergency steering, bilge pumps and turning on engines. Cooks were unable to demonstrate usage of installed firefighting systems.

0710 - Fire Prevention - The condition of the ship shall be maintained to conform with the provisions of the regulations to ensure that the ship in all respects will remain fit to proceed to sea...main galley, deck 5, Chef's Office window covered, reducing communicable openings to below 30%. SOLAS 74 (1997 Cons) Ch I Reg 11. 10c.

2050:Machinery, bilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction for the service for which they are intended. Vessel's bow thruster #2 has an internal hydraulic oil leak and is inoperable. 74 SOLAS (14) 11-1/26.1
