All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Port and starboard Hydraulic pump hose lines forward of both engines worn and wasted. Both hoses require renewal. 46 CFR 182.100

The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant OCMI, but must be not less than the minimum number required by table 46 CFR 181.500(a)and other provisions of this section. Vessel carries (04) 10 B:C portable fire extinguishers. Vessels COI requires 1-10 B:C and 2-40 B:C. Provide (02) 40 B:C's as required by COI. 46 CFR 181.500(a)

At inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each hydrostatic release unit (HRU) to determine that it is in compliance with the servicing and usage requirements of 185.740 of this chapter. Hydrostatic release for EPIRB expired. Install new HRU. 46 CFR 176.808(a)(5)

At each inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the inspection and operation of all watertight closures in decks. Starboard engine hatch support rod, outboard side plate detached from frame and port forward hatch support rod broken. Both require repair or replacement. 46 CFR 176.802(a)(2)

A duct must be of rigid permanent construction, made of the same material as the hull or of noncombustible material. Provision must be made for closing all exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Engine compartment vent openings require closing mechanism. Blocking vent opening with a rag is not accepted. 46 CFR 182.460(h)(l)

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited the operator must secure each Type I or Type II device in a manner which prevents discharge. Acceptable methods include: Padlocking the seacock in the closed position. MSD overboard discharge valve located in head unsecured. Requires new padlock. 33 CFR 159.7(b)(2)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First aid kit on board found with expired contents and unsuitable condition. Requires replacement. 46 CFR 184.710

A vessel on an oceans, coastwise, or limited coastwise route must carry (1)Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q and(2)Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter. Vessels distress signals found expired. 46 CFR 180.68(a)(1)(2)

An adult life jacket must be provided for each person carried on board a vessel. Vessel COI calls for 38 adult like jackets. 37 counted on board. Requires 01 additional. 46 CFR 180.71(a)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel. Vessel does not have any means to test for presence of alcohol. 46 CFR 4.06-15 (a)

The employer shall provide an Employee Assistance Program for all crewmembers that includes both education and training on the effects and consequences of drug and alcohol use. Crewmember must receive training. Vessel's O/O has established an Employee Assistance Program as part of the company's chemical testing program, but has not completed the program. Provide satisfactory evidence of an Employee Assistance Program training for crew. 46 CFR 16.401

Each vessel must be equipment with at least two operable portable battery lights and one for each engine space. Vessel does not have light for each engine space. Provide portable lights. 46 CFR 183.430

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The vessel's stbd fuel shutoff valve did not fully close. Provide fully operational fuel shutoff. 46 CFR 182.455(b)(4)

Machinery and equipment installed must be suitable for the vessel and its operation. Hydraulic hose fittings both port and starboard showing signs of corrosion. Remove fitting and provide new approved J-1942 hydraulic hose and fitting. 46 CFR 182.100 46 CFR 182.720

The Life Float Light must be attached by 3/8 inch lanyard, resistant to UV, and at least 18 feet in length. Life Float light line deteriorated and not of proper length. Provide new line that meets length. 46 CFR 180.175

Ring buoy must be fitted with a lifeline 60 ft in length, be at least 5/16 inch, be dark color if synthetic or a type certified to be resistant to deterioration from UV light. Line from ring buoy deteriorated. Provide new line. 46 CFR 180.70

Life float painter must be at least 100 ft in length, be of a dark color if synthetic, or of a type certified to be resistant to deterioration from UV light, and be stowed in a way that it runs out freely when the life float floats away. Painter deteriorated. Provide new painter.

Each survival craft must be secured by a painter with a float-free link permanently attached to the vessel. Weak link(float free device)did not have tag showing its ability to float-free. Provide weak link with appropriate tag. 46 CFR 180.130(a)

Vessel must be provided with one fixed power bilge pump and one portable hand pump. Portable hand pump was leaking. Provide new portable hand pump. 46 CFR 182.520

Vessel does not currently meet the requirements of Work Instruction 31. Mandated crew posted on COI may not enter the water or act as lifeguard until policy and procedure provided. Provide written procedure. 46 CFR 176.100(b)

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Crewmembers from the HOKUA reported sucking the spring line into the portside jet drive which caused an approximate one inch wide by seven inch crack in the port side grate and is splintering approximately one half inch into the hull. Vessel must be hauled out, damage assessment conducted and permanent repairs made to engine and/or hull.

Vessel must be equipped with portable lights at the operating station and access to the machinery space. No portable lights on vessel. Provide dedicated portable lights for operating station and engine space. 46 CFR 183.430

Installation of portable fire extinguisher must be in accordance with manufacture. Extinguisher not placed in bracket. Provide bracket for lower forward fire extinguisher. 46 CFR 181.520

A visual and audible alarm must be provided at the operating station to indicate a high water level in each machinery space. Visual indication of high water not operational at operating station for starboard engine room. Provide operational visual indicator at operating station for stbd engine room. Conduct and rounds every 30 minutes until operational. Rectify within 14 days. 46 CFR 182.530

All electrical equipment must be suitable for the voltage and current. Dead end wires found behind helm. Remove dead end wiring.

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and Supervisory personnel must receive at least 60 minutes of training. Provide proof of completed EAP training for all personnel. 46 CFR 16.401(b)

Starboard engine room bilge high water visual indicator at helm not operational. Provide operational high water visual indicator at helm.

Radio Safety Certificate was not provided. Provide in accordance with 46 CFR 184.502.

VHF DSC radio function was not programmed. Prove function in accordance with 46 CFR 184.502.

Port installed extinguishing system audible and visual alarm did not function at operating station when tested. Prove correct operation in accordance with 46 CFR 181.420.

Stbd Fireboy system did not secure engine after activation. Prove proper function of Fireboy system in accordance with 46 CFR 181.420.

Port Fireboy system did not allow engine to start after activation. Prove proper function of Fireboy system in accordance with 46 CFR 181.420.

Cabin and operating station portable extinguishers were not mounted. Mount in accordance with 46 CFR 181.520.
