CAROLINA

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces, galleys, main pantries and storerooms, other than small service lockers, shall be of “A” Class construction. MI's noted missing bolts in man covers on both accommodation levels adjacent to trunk space as well as compromised MCT on bulkhead separating office from machinery space next to E-Gen. Repair to keep boundary classification intact. 46 CFR 92.07-10(c)

Means shall be provided for stopping all fans in ventilation systems serving machinery and cargo spaces and for closing all doorways, ventilators and annular spaces around funnels and other openings to such spaces, from outside these spaces, in case of fire. Transformer room has natural ventilation openings with no means of closure in event of a fire. 46 CFR 92.15-10(b)

Each refrigerated space that is accessible to the vessel's personnel and that can be locked from the outside so that it cannot be opened from the inside, must have an audible alarm system that can be operated from within the refrigerated space. Alarm must meet requirements of 113.45-5 (a)-(d). 46 CFR 113.45-5

The generator set must be capable of carrying its full rated load within 45 seconds after cranking is started. Emergency generator failed to start within 45 seconds.

Bulkheads of galleys, paint and lamp lockers, and emergency generator rooms shall be of “A” Class construction. Galley fire dampers did not fully close.

All vessels shall have efficient guard rails or bulwarks on decks and bridges. The height of rails or bulwarks shall be at least 39 1/2 inches from the deck. At exposed peripheries of the freeboard and superstructure decks, the rails shall be in at least three courses, including the top. Openings in bulwark lacked rails.

The surveyor shall examine on all vessels the items listed in this paragraph to determine if in satisfactory condition. Transverse watertight subdivision bulkheads, including any openings therein and closures for such openings. Identified multiple openings in watertight bulkheads: pump room (2) and stbd superstructure.

Barges with sleeping accommodations for more than six persons and all self-propelled vessels shall have permanently exhibited the following plans: General arrangement plans showing for each deck the various sections enclosed by fire-resisting bulkheads. Vessel's Fire Plan lacks PAM escape hatch and includes gym PAM that was removed.

The bunker delivery note shall be kept on board the ship in such a place as to be readily available for inspection. It shall be retained for a period of three years after the fuel oil has been delivered on board. Vessel lacked bunker delivery notes.

Each ship subject to Regulation 6.1 shall maintain a list of ozone-depleting substances. Vessel lacked updated IAPP certificate including list of ozone-depleting substances. Vessel also lacked record book as required by Regulation 12.6. MARPOL Annex VI, Reg. 12.5

There must be a sufficient number of loudspeakers throughout the vessel. With the vessel underway in normal conditions, the minimum sound pressure levels for broadcasting emergency announcements must be in interior spaces, 75 dB(A). Public Address loudspeakers were difficult to hear with no background noise in the galley and pump room.

Where fitted, it shall be the duty of the master to see that the emergency lighting and power systems are operated and inspected at least once in each week that the vessel is navigated to be assured that the system is in proper operating condition. One e-light in berthing area corridor failed to automatically energize following loss of power.

Electric installations on vessels must ensure: Protection of passengers, crew, other persons, and the vessel from electrical hazards. Freshwater pump and 3 electrical boxes not secured, including the heater box for the port generator and 2 boxes behind generator control room.

Suitable hand covers, guards, or rails shall be installed in way of all exposed and dangerous places such as gears, machinery, etc. Vessel lacked multiple guards on rotating machinery.

Piping subject to internal head pressure from oil in the tank must be fitted with positive shutoff valves located at the tank. Remote fuel shutoff for emergency generator not installed at the tank.

Safety or relief valve discharges, when permitted to terminate in the machinery space, must be led below the floorplates or to a remote position. Potable water piping in pump room supported by rope.

In a space described in § 113.25-9(a), where the general emergency alarm signal cannot be heard over the background noise, there must be a red-flashing light or rotating beacon. The installation must be in accordance with the requirements of part 111, subpart 111.105, of this chapter. Light in upper fwd machinery space blocked by ventilation duct and light in space fwd of pump room not mounted properly.

Plastic pipe installations must be in accordance with IMO Resolution A.753(18) and IMO Resolution MSC.313(88) and the supplemental requirements in 46 CFR 56.60-25. Plastic pipe in PAM has not been approved for use.

Within the accommodation and service areas the following conditions shall apply: Corridor bulkheads in accommodation spaces shall be of the “A” or “B” Class intact from deck to deck. Stateroom doors in such bulkheads may have a louver in the lower half. Vessel's Fire Plan does not list “A” or “B” Class boundaries as required. Additionally, the space between the galley and corridor is open to adjacent spaces.

Each fire hydrant shall be identified in red letters and figures at least two inches high “FIRE STATION NO. 1,” “2,” “3,” etc. Multiple fire stations not marked as required.

There shall be at least two means of escape from all general areas where the crew may be quartered or normally employed. At least one of these two means of escape shall be independent of watertight doors. The two means of escape shall be as remote as practicable so as to minimize the possibility of one incident blocking both escapes. The outboard means of escape on PAM CG1537257 leads directly overboard resulting in the forward stateroom only having one means of escape.

Pressure vessels described in paragraph (c)(3) of this section must have detailed plans that include the information required by § 54.01-18. The plans required by paragraph (d)(1) of this section must be certified by a registered professional engineer. The certification must appear on all drawings and must be made available to the Coast Guard prior to the inspection required by § 54.10-3(c). Pressure vessel added in 2022 lacks U-1A.

Pressure vessels for low temperature operation shall be as required by section VIII of the ASME boiler and pressure vessel code. The cutter gear oil head tank was reported by the vessel to have a leak which resulted in approximately 10 gallons of spilled biodegradable gear oil. Vessel shall provide a plan to repair or replace the system within 7 days to the satisfaction of the duty marine inspector

All plans required by 46 CFR 91.55-1/46 CFR 91.55-5/46 CFR 91.55-10 may be submitted directly to OCMI in the district in which the vessel is to be built or directly to Commandant CG-ENG for approval. Vessel has a PAM CG1658092 installed on the port aft deck to be used as a gym. There are no records of approval and the PAM does not comply with CG ENG policy letter 01-16/ ABS standards for PAMs. Submit plans for approval prior to any further occupancy of the crew. 705 Prior to use 46 CFR 91.55-5

Each life raft or group of life rafts must be arranged for float free launching. The arrangement must ensure that the life raft or life rafts, when released and inflated are not dragged under by the sinking vessel. The aft life raft on the STBD side of the vessel on the deck and under a deck railing that may catch the painter and drag the life raft down with the vessel. Re-mount the life raft above the railing as the other three are mounted to the satisfaction of the attending MI. 46 CFR 199.130 (c) (7)

Servicing of inflatable lifesaving appliances, Each inflatable lifesaving appliance and marine evac system must be serviced with in 12 months of each subsequent servicing. Vessel has 04 life rafts and only servicing documents for 03. Provide proof of servicing report for the fourth life raft. 46 CFR 199.190 (g) (1)

Each vessel must have a line throwing appliance approved under 46 CFR 160.040. 160.040-4 Four rocket projectiles. Vessel currently has one of four required line throwing appliances. Provide three more to the satisfaction of the attending MI prior to sailing on Oceans route. 46 CR 199.170 (a)

Emergency instructions, illustrations and instructions in English must be posted at the muster station and include the method of donning life jackets. The vessel has two different type of life jackets and the posted illustrations on the container match one of the provided life jackets but the other is not the correct illustration for proper donning. Replace the illustration with the type matching the life jackets on board to the satisfaction of the attending MI. 46 CFR 199.80 (c) (5)

The master or person in charge of a ship under paragraph (a)(1) of this section shall ensure that the ship is not operated unless a waste management plan meeting paragraph (c) of this section is on the ship and that each person handling garbage follows the plan. The vessels plan contains conflicting forms for disposal of trash, amend the plan to reflect the actual disposal of trash when operating near coastal, to the satisfaction of the attending MI. 33 CFR 151.57 (b)

Radio life saving appliance installations and arrangements must meet the requirements of 47 CFR part 80. At least one radar transponder must be carried on each side of every cargo ship of 500 GT and upwards (47 CFR 80.1095 (b)). The vessel is currently not equipped with any Search and Rescue Transponders. Provide two. 46 CFR 199.60

The inspection for certification shall include an inspection of the structure and equipment to ensure it is in satisfactory condition and fit for the service which it is intended. Currently the vessels freezer latch is out of adjustment, not latching or operating properly. Repair latch or install a trapped in freezer alarm IAW 46 CFR 113.45-5 to the satisfaction of the attending MI. 46 CFR 91.25-10

It shall be the duty of the Master to see that the vessel and in particular the quarters are in a clean and sanitary condition. Damper, electrical components and installed fire fighting fusible link over the griddle and deep fat fryer has excessive oil and grease built up. Clean to the satisfaction of the attending MI. 46 CFR 97.15-10

Cable applications, damp or wet locations. Electrical cable for installation in damp or wet locations shall have an outer covering of lead and armor, moisture resistant jacket and armor or mineral insulated metal sheathed. The cable insulation may be either rubber, thermoplastic, varnished cloth, asbestos varnished cloth or silicone. The cable installation powering the griddle and the main oven has exposed wires from the cable sheathing and the breaker cabinet is wasted at the connection. Renew to the satisfaction of attending MI. 46 CFR 111.60 (d) Old-old J

Where natural ventilation is provide for spaces protected by a carbon dioxide extinguishing system, provisions shall be made for easily and effectively closing off the ventilation. Both the aft STBD and port ventilation closure for the flame storage will not close effectively allowing for CO2 to escape in the event of a fire. Repair to the satisfaction of the attending MI. 46 CFR 95.15-35

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practical. Vessels laundry space has excessive build up of lint in overhead at opening for valve access and the non tight door requires adjustment to fully close with door closure device. Clean and adjust to the satisfaction of the attending MI. 46 CFR 92.05-1

In general compliance with the standard established by the American Bureau of Shipping will be considered as satisfactory evidence of the structural efficiency of the vessel. On the 2nd deck (02 deck) just aft of the house port side the deck is wasted with holes and thin metal on the walk way at the railing. Submit a repair proposal to renew the wasted steel at this area. At owners request this will be due at drydock. 46 CFR 92.01-10

On vessels of 500 GT on international voyage, must be provided with at least one international shore connection complying with ASTM F 1121. The vessel does not have an international shore connection for either side of the vessel. Replace to the satisfaction of the attending MI. 46 CFR 95.10-10 (c)

The general alarm system shall consist of electric vibrating bells and in certain cases flashing red lights located through out crew quarters, machinery spaces and work spaces and so located as to warn all occupants in an emergency. The GE alarm bell on the main deck accommodations has ceiling tiles laying on it and causes the bell to not ring properly. Repair ceiling tiles to the satisfaction of the attending MI. 46 CFR 113.25-5 (a) Old old J

Each battery operated relay controlled lantern used must have rechargeable batteries; have an automatic battery charger that maintains the battery in a fully charged conditions; and not be readily portable. The emergency lanterns in the 8 man PAM rooms 3 and 4 are not operable. Repair or replace to the satisfaction of the attending MI. 46 CFR 112.39-1

Fire detecting, manual alarm and supervised patrol systems are not required except in special cases; but if installed, the system shall meet the requirements of sub chapter H. The hard wired smoke detector in the 8 man PAM room 3 is hanging from the overhead and is required to be installed properly. Unit was tested and did alarm. 46 CFR 95.05-1 ABS Guide PAM

Each fire hydrant must have at least one length of fire hose, a spanner wrench and a hose rack or other device for stowing the hose. Ensure each fire station is equipped with spanner wrench for the hose to nozzle and hose to hydrant connection to the satisfaction of the attending MI. 46 CFR 95.10-10 (g)

If the annual inspection reveals deficiencies in your vessel's maintenance you must make any or all repairs or improvements within the time period specified. The 8 man PAM is required to have a PA system IAW ABS Guide for Portable Accommodation Modules, it does not work and would not awaken any persons sleeping in the PAM. Repair to the satisfaction of the attending MI. 46 CFR 91.27-1 (a) (3)

If the annual inspection reveals deficiencies in your vessel's maintenance you must make any or all repairs or improvements within the time period specified. IAW ABS guide for Portable Accommodation Modules all exterior boundaries must have a minimum fire rating of A-O. The 8 man PAM has a cable penetration above the door that is not A-0 and the door is not fully self closing. Repair to the satisfaction of the attending MI. 46 CFR 91.27-1 (a) (3)

Except as provided in paragraphs (b) through (d) of this section, this subpart applies to vessels with watertight doors in bulkheads that have been made watertight to comply with the flooding or damage stability regulations in this subchapter. WTD between Fwd hold and Main Pump hold did not close properly. Repair or replace to the satisfaction of the attending MI 46 CFR 170.248(a)

If a pipe, scupper, or electric cable passes through a main transverse watertight bulkhead, the opening through which it passes must be watertight. Pipe Penetrations between Generator hold and Pump Hold had multiple cables going thru one penetration. Repair or replace to the satisfaction of the attending MI. 46 CFR 171.111 (e)

If a pipe, scupper, or electric cable passes through a main transverse watertight bulkhead, the opening through which it passes must be watertight. Multiple penetrations thru out machinery space were not properly capped or made water tight. Repair or replace to the satisfaction of the attending MI. 46 CFR 171.111 (e)

Pipe that is to be used for potable water must bear the appropriate certification mark of a nationally-recognized, ANSI-accredited third-party certification laboratory. Plastic pipe fitting and bonding techniques must follow the manufacturer's installation guidelines. Bonders must hold certifications required by the manufacturer's guidelines and provide documentation of current certification to the Marine Inspector when requested. Potable water line is not properly supported. Repair or replace to the satisfaction of the attending MI. 46 CFR 56.60-25 (a)(4)

If a pipe, scupper, or electric cable passes through a main transverse watertight bulkhead, the opening through which it passes must be watertight. Ship shore through hole was not properly capped. Repair or replace to the satisfaction of the attending MI. 46 CFR 171.111 (e)

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment required by the regulations in this subchapter is provided, maintained, and replaced as indicated. Fire Pump was leaking during operational. Repair or replace to the satisfaction of the attending MI. 46 CFR 97.15-60(a)

Fire pumps must be fitted with a pressure gage on the discharge side of the pumps. Improper gage was installed. Repair or replace to the satisfaction of the attending MI. 46 CFR 95.10-5(e)

Each machinery space must be designed to minimize the exposure of personnel to noise in accordance with IMO A.468(XII) (incorporated by reference, see 46 CFR 58.03–1). No person may encounter a 24-hour effective noise level greater than 82 dB(A) when noise is measured using a sound-level meter and an A-weighting filter. (b) Except as allowed by paragraph (c) of this section, no machinery space may exceed the following noise levels: (1) Machinery control room—75 dB(A) (c) If adding a source of noise would cause a machinery space to exceed the noise level permitted by paragraph (b) of this section, the new source must be suitably insulated or isolated so that the space does not exceed that noise level. If the space is manned, a refuge from noise must be provided within the space. Space is not properly insulated for proper operation monitoring. Repair or replace to the satisfaction of the attending MI. 705 Drydock 58.01-50 (a)(b)(c)

The inspections and tests shall insure that the equipment and associated structure are in satisfactory operating conditions and fit for the service for which they are intended. Overspeed visual alarm is not working. Repair or replace to the satisfaction of the attending MI. 46 CFR 61.01-1

Each Class 1 door on a vessel in ocean service must be designed to withstand a head of water equivalent to the depth from the sill of the door to the margin line but in no case less than 10 feet (3.05 meters). Port and Starboard main deck water tight doors are not properly secured correctly. Repair or replace to the satisfaction of the attending MI. 46 CFR 170.270 (b)

The installations of all systems of marine engineering nature, to together with the details of design, construction and installation, shall be in accordance with the requirements of subchapter F (Marine Engineering). The aft fuel transfer pump is not operational. Repair or replace to the satisfaction of the attending MI. 46 CFR 96.03-1

It shall be the duty of the master and chief engineer to see that the vessel, and, in particular, the quarters are in a clean and sanitary condition. The chief engineer shall be responsible only for the sanitary condition of the engineering department. Marine Inspector observed the vent hood, vent hood lighting and vent hood grease traps in the galley were covered in a thick layer of grease. Also observed was excessive lint on lint traps and walls within the laundry room. Both issues observed create a fire hazard, which needs to be eliminated.

Each refrigerated space that is accessible to the vessel's personnel and that can be locked from the outside so that it cannot be opened from the inside, must have an audible alarm system that can be operated from within the refrigerated space. Marine Inspector observed the inside release mechanism of the freezer storage room was inoperable, which needs to be repaired or have a audible alarm installed.

Within the accommodation and service areas the following conditions shall apply: The integrity of any deck in way of a stairway opening, other than a stairtower, shall be maintained by means of “A” or “B” class bulkheads and doors at one level. The integrity of a stairtower shall be maintained by “A” Class doors at every level. The doors shall be of self-closing type. Holdback hooks, or other means of permanently holding the door open will permanently holding the door open will not be permitted. However, magnetic holdbacks operated from the bridge or from other suitable remote control positions are acceptable. Marine Inspector observed "A-60" class door between the galley and the pantry tied at the handle holding the door open.

The fire detection and alarm system must be capable of immediate operation at all times that the vessel is in service. Fire detection panel showed fault and was not in proper working order.

Each vessel must have a general emergency alarm system that meets the requirements of this subpart. Vessel was unable to sound the general emergency alarm from any location on the vessel. Vessel must affect repairs to allow the sounding of the general emergency alarm from all contact marker locations.

At each inspection for certification the inspector shall examine the bilges in in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. The port and starboard exhaust stack in the engine room thickly coated in oil creating fire hazard. Vessel shall clean excess oil out of the exhaust.

Each hand portable fire extinguisher shall be marked with a number and location where the stowed shall be marked with the corresponding number at least a half inch high. The majority of the vessel's portable extinguishers were not marked, and the majority of stowage locations were marked. Vessel shall mark extinguishers and locations.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Mitigating factors have been presented not limited to the Damage Stability Assessment, Longitudinal strength analysis, and Statement of Intent dated 28 July 20. Vessel shall make permanent repairs in the booster pump hold void at a non-credit dry dock to the satisfaction of the attending marine inspector prior to COI renewal.

Each vessel must have a general emergency alarm system that meets the requirements of this subpart. 2 GE alarm bells not operational in engine room deck.

Each general emergency alarm signal must be marked “GENERAL ALARM—WHEN EMERGENCY ALARM SIGNAL RINGS GO TO YOUR STATION” in red letters at least 1/2 inch high.

Wire must be in an enclosure.(b) Wire must be component insulated. Dead end wire present in Generator room.

Servicing of inflatable lifesaving appliances, inflated rescue boats, and marine evacuation systems. (1) Each inflatable lifesaving appliance and marine evacuation system must be serviced Provided report of initial servicing for 2 new life rafts. Service sticker has an expiration of 2022.

Emergency instructions. Illustrations and instructions in English, and any other appropriate language as determined by the OCMI, must be posted in each passenger cabin and in spaces occupied by persons other than crew, and must be conspicuously displayed at each muster station. The method of donning lifejackets Missing lifejacket donning placard at muster station.

A log for records of inspections and maintenance. Life raft data cards were not filled out.

All persons employed or engaged onboard vessels to which STCW applies will be issued a medical certificate valid for 2 years unless the mariner is under the age of 18, in which case the maximum period of validity will be 1 year. life boatmen had missing medical card or STCW expired

Periodic servicing of hydrostatic release units. Each hydrostatic release unit, other than a disposable hydrostatic release unit, must be serviced in accordance with repair and testing procedures meeting the requirements of part 160. Hydrostatic release was improperly installed.

For vessels constructed on or after September 30, 1997 or for existing vessels which have their plans redrawn, the symbols used to identify the aforementioned details shall be in accordance with IMO Assembly resolution A.654(16). These identical symbols can also be found in ASTM Adjunct F 1626 (incorporated by reference, see §97.01-2). The aforementioned information shall be kept up-to-date, any alteration being recorded in the applicable medium as soon as practicable. Vessel did not match plans on board and required equipment on the COI

The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. On inspection, vessel found with wastage presenting multiple holes allowing open communication with sea, in the double bottom void shell plate in way of the booster pump hold. Affix proper permanent repairs OR descale booster pump hold to completely assess the condition of the space to the satisfaction of attending Marine Inspector and attending class surveyor.

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. The self-igniting light may not be attached to the lifebuoys required by this section to be fitted with lifelines. Main deck fwd and aft stbd lifebouy lights had improper battery installed.

Each vessel must carry lifebuoys approved under approval series 160.150. Port fwd life buoy was a bridge buoy approved under approval series 160.157.

Run fire, man overboard and abandon ship drills once CAROLINA and GL 10 aare coupled together and prior to commencing dredging operations.

Issued 835 extending liferaft servicing date for 5 months or until next scheduled vessel exam whichever comes first as per 46 CFR 199.190(g).

Update vessel’s fire/safety plan to reflect vessel’s equipment and location.

Remove flexible hose and properly install hard piping for gear box oil tank 2. As discussed.

Provide heat resistant sight glass on #1 gear box oil tank and provide protection from breakage.
