While validating vessel's Fire Control Plan it was discovered that the physical locations on the barge and the tug that were identified to be the location to access copies of the Fire Control Plan, where not physically labeled nor had the plan present. Ensure copies of the Fire Control Plan are made available as indicated in the Fire Control Plan and are labeled properly.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to ensure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. The vessel is directed to proceed directly to an approved repair facility under the conditions specified in the permit to proceed and approved transit plan. The vessel shall utilize tug assistance when transiting restricted waters and complete all necessary repairs to the starboard rudder and controllable pitch propeller (CPP) system. All repairs must be executed to the satisfaction of both the cognizant Officer in Charge of Marine Inspection (OCMI) and the attending ABS Class surveyor before the vessel may return to service. 46 CFR 91.30-1

Each Vessel Must - Carry not less than 12 rocket parachute flares approved under approval series 160.136; and Stow the flares on or near the vessel's navigating bridge. All of the vessels flares were expired. 46 CFR 199.60(c) 46 CFR 199.640(a) Table 46 CFR 199.630(b)

Firehose must be connected to the outlets at all times. Hoses throughout the house, and accommodation spaces were found not connected to the outlets. 46 CFR 95.10-10(h)

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter F. Vessels Bow thruster is inoperable. Vessel must have tug assist when operating in Duluth harbor in a loaded condition. Notify all applicable VTS along planned route. 46 CFR 96.03-1

Lower level engine room CO2 nozzle missing horn.

Design Verification and Periodic Safety test procedures described in subpart 61.40 of this chapter must be submitted to the Marine Safety Center for approval. Vessel owner must provide a copy of the submitted plans, correspondence and acceptance from the MSC of the phase 3 automation upgrade to the local Marine Inspector. Vessel shall maintain a manned engine control room, until MSC accepted plans are received at the local OCMI.

Starboard main engine inoperable due to a failed oil pump on one cylinder. Vessel is authorized to proceed to Gary, IN under reduced propulsion utilizing the Port engine. Vessel shall conduct repairs and provide inspection/testing to the satisfaction of the local OCMI prior to departure from Indiana Harbor. Vessel shall keep local OCMI informed of any changes in propulsion capabilities during transit.

Each liferaft or group of liferafts must be arranged for float-free launching. The arrangement must ensure that the liferaft or liferafts, when released and inflated, are not dragged under by the sinking vessel. Vessel's primary lifesaving was installed such that the liferaft(s) would be dragged under by the vessel if sinking. Arrange correct float-free arrangement as per manufacturer specifications and to the satisfaction of attending Marine Inspector.

All of the inflatable liferafts (with the exception of the forward liferaft on the barge) on the ITB PRESQUE ISLE (tug and barge) are improperly attached to their cradles due to missing liferaft lashing-straps and slip-hook attachments. Properly configure installed hydrostatic release units with liferaft lashing-straps and slip-hooks in accordance with the manufacturer's instructions. Note: As currently installed, the liferafts will still float-free/deploy normally; however, they could potentially be inadvertantly knocked free from their cradles by a heavy sea-state.

During the year 2020 fit-out; inspection(s) of the stbd-stack waste-heat boiler (#181902-02-2) were missed by the USCG due to attendance restrictions stemming from the COVID-19 pandemic. This system was however self-inspected by the vessel's Engineers IAW the applicable SIP ICR. Complete fireside, waterside, and hydro inspections of this auxiliary unit prior to completion of the 2021 return-to-service fit-out inspection (must be witnessed by a USCG Marine Inspector).

GLF Port Engineer reported mechanical failure of the #1 (stbd -outboard) ship's service generator prime mover due to an overspeed issue that did not result in shutdown. Repair/replace generator prime mover as original and submit service technician's report indicating satisfactory operation and testing IAW the vessel's approved PSTPs.

The barge stbd-side aft inflatable liferaft (ILR) is not configured with an approved weak-link as part of its float-free arrangement. Properly install approved weak-link, or hydrostatic release, in accordance with 46 CFR 199.130(c)(7).

#2 SSDG prime mover has been lost due to #4 cylinder damage. Repair or replace #2 SSDG prime mover and demonstrate satisfactory functional tests in the presence of a USCG Marine Inspector.

ONE INCH CRACK IDENTIFIED IN BULKHEAD BETWEEN #5 & #6 STARBOARD BALLST TANKS, IWO BOTTOM PLATE, SECOND WELD PASS OF PREVIOUS INSERT. TEMPORARY CEMENT BOX PATCH INSTALLED. DETERMINE EXTENT OF CRACK, CROP AND RENEW AFFECTED AREAS TO ORIGINAL SCANTLINGS MINIMUM.

Prove proper operation of auxiliary steering pump (#2) independently controlling both the port and starboard side rudders. To be completed to the satisfaction of class or the attending C. G. Marine Inspector.

Replace the failed (#1) steering pump & proved proper operation to the satisfaction of class or the attending C. G. Marine Inspector.

Make repairs to starboard waste heat boiler where casing failed. Repairs must be completed and tested in accordance with requirements in 46 CFR 61.05.

Have both hybrid waste heat boilers (port and starboard MDE exhaust stacks) tested in accordance with the requirements for hybrid boilers in 46 CFR 61.05-10.

Test safety/relief valves on both waste heat boilers in accordance with 46 CFR 61.05-10. This should be completed in annually in conjuction with the annual/COI exam but has not been completed.

Update fire/safety/general arrangment plans to reflect change in life raft locations.

Starboard Controllable Pitch Propeller (CPP) system is malfunctioning. The starboard CPP is locked in the 9' ahead propulsion position.

Provide evidence of current class endorsement of Great Lakes Load Line Certificate.

Both starboard and port side liferaft embarkation locations lack emergency lighting. Provide lighting hooked up to emergency bus by 22APR14.

Deck cradles for Viking Liferafts are mounted incorrectly. The higher side of the cradle should face inboard to facilitate manual launch. This is illustrated in Viking manuals that accompany the rafts. Cradles need to be removed from the deck, turned around and re-welded.

The remote pull box for the CO2 system protection the paint locker at the bow is located behind a permanently installed ladder. This makes is difficult to break the glass and pull the release. Relocate this box to facilitiate clear unobstructed access to the pull.

The paint locker at the bow is penetrated by a rubber drain hose. The drain hose services a deck line running through the paint locker overhead. This rubber hose compromises the fire boundary of the space. Remove and plug the pentration with threaded steel plug or replace rubber hose with proper steel piping.

Throughout vessel suvey and identify penetrations in fire rated bulkheads (galley, engine casing, paint lockers, main vertical zones) and seal properly with approved fire stop material.

AUX boiler requires fireside and waterside exams prior to 04JUN14 to remain within interval limits. Arrange for a marine inspector to attend.

Rescue boat installation does not utilize a Coast Guard approved launching appliance (davit). Provide documentation of weight test on existing hoist, cable, winch and brake that meets Coast Guard requirements prior to departure from port.

Provide written proposal to correct to install a Coast Guard approved davit, obtain an equivalency determination from COMDT or provide documentation of plan review and approval of the current arrangement within 30 days.

Vessel is missing the 6 person life raft required at the bow. This requirement exists because the vessel is inspected as a single mode (push only) ITB (single cargo vessel instead of tug and barge). Install raft in accordance with requirements.

Launching arrangements and placement of four 25 person rafts do not meet requirements for 100% on each side nor do they meet the requirements for manually launching over the side. See details in narrative. Corrections required within 60 days.

6" fracture located #5 center void of tug port side, at frame 60 of aft bulkhead. Temp repair completed 20MAR14. Crop and renew required at next dry dock. Surrounding steel UT'd and is within tolerance. Do not recommend extension be granted.

Ice cover in the harbor prevented launch, recovery of MOB and retrieval of rescue boat. This must be done at earliest opportunity but no later than 18PR14. Record in ship's log and send photo of drill to [email withheld].

Crew unable to start outboard engine of rescue boat due to lack of winter maintenance and cold temperatures. Require demonstration of operational status prior to departing from port.

#2 SSDG engine failed due to a bad valve and camshaft damage. Engine will be removed and rebuilt - to be completed about 12/10/13.

Although Buffalo cleared requirement for DVTP/PSTP for modifications to propulsion system, CE cannot produce a stamped approved copy of procedures on board. The operator has until the start of the 2014 shipping season to correct this.

Propulsion control system modified without USCG plan review. Installation does not match current plan configuration. Submit plans IAW 46 CFR 62.20 to MSC for review and approval. Installation verification to approved plans is required to clear this requirement.

New remote propulsion control system installed without Coast Guard plan review. Submit plans IAW 46CFR62.20 to Marine Safety Center by 25 March 2011.

Effect permanent repairs to port shaft stern seal prior to engagement or operation of port shaft oil lube system.

Make permanent repairs to the damage area located on the port stern corner of the vessel to the satisfaction of a qualified USCG Marine Inspector.

Have vessels rudder asymbly removed and lower bushing and alignment inspected during the drydocking of the barge by 30 Nov 06 or when the barge is placed in drydock.

Make permanent repairs to the ship's #1 Service Generator.

Make permanent repairs to port shaft stern tube seal to the satisfaction of the attending Coast Guard inspector and/or the attending class surveyor prior to 01APR06. Vessel permitted to depart Two Harbors, MN on 05JAN06 and proceed to Conneaut, OH to offload cargo, thence to Erie, PA for winter lay-up/repairs. In restricted waters, #2MDE is on line/available for emergency use.

Make permanent repairs to leaking starboard shaft stern tube seal to the satisfaction of the attending Coast Guard marine inspector and/or the attending classification society surveyor.

Main steering stand, control board to port rudder motor in-op

Port servo amp board burned out. One of 3 steering gear motors.

Repair or replace the port shaft seal to the satisfaction of the class surveyor.

Repair bulk heads in port and starboard #2 wing tanks. In the port #2 wing tank repair 2-4 " inset that runs from the deck approx 8' up from the deck. In the stb #2 wing tank repair the 1-2" inset that runs approx. 4' from the deck. Repairs to be inserts to the damaged areas.

Repair turbocharger on the starboard main engine to the satisfaction of theattending marine inspector prior to the vessels departure from Gary, IN.

Due to STBD engine casualty, while vsl is transitting the St Mars River fm Pt Aux Fresnes to Round Is. Lt #26, vsl must be accompanined by tug escort of adequate number & h.p. to give assist for maneuvering. Upon arrival Duluth, conduct operational test to satisfaction of attending Marine Insp.

TEST OVERSPEED AND LOW LUBE OIL TRIPS ON STBD MAIN DIESEL ENGINE.

PROVE PROPER OPERATION OF BARGE GENERAL ALARMS.

Make permanent repairs to port main diesel engine to the satisfaction of the attending USCG Marine Inspector. In the interim, until 31DEC95, the provisions of MILMS COTP Order 03-95 dated 25OCT95 shall be followed.
