PRESQUE ISLE

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to ensure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. The vessel must make permanent repairs to the 38 identified weeper and corrosion points that are leaking ballast water into the cargo holds and tunnel and conduct internal ballast tank and cargo hold inspection to the satisfaction of both class and Coast Guard inspectors prior to departing repair facility as outlined in the issued permit to proceed and ABS survey report. 46 CFR 91.30-1

For vessels assigned a load line, see subchapter E (Load Lines) of this chapter. During and after any survey made by the assigning and issuing authority, those items, fittings, etc., which are found to be in an unsatisfactory condition by the surveyor shall be repaired or renewed in order to place the vessel in a satisfactory condition. 2' crack in side shell plating below the waterline in the #2 starboard ballast wing tank must be repaired in accordance with ABS rules to the satisfaction of attending USCG Marine Inspector and Class Surveyor. Vessel shall provide written plan to MSU Duluth for review prior to repair completion. 17ac 46 CFR 92.01-5 46 CFR 42.09-45(a)

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter F. Vessels Bow thruster is inoperable. Vessel must have tug assist when operating in Duluth harbor in a loaded condition. Notify all applicable VTS along planned route. 46 CFR 96.03-1

Each Vessel Must - Carry not less than 12 rocket parachute flares approved under approval series 160.136; and Stow the flares on or near the vessel's navigating bridge. All of the vessels flares were expired. 46 CFR 199.60(c) 46 CFR 199.640(a) Table 46 CFR 199.630(b)

Firehose must be connected to the outlets at all times. Hoses throughout the house, and accommodation spaces were found not connected to the outlets. 46 CFR 95.10-10(h)

Each liferaft or group of liferafts must be arranged for float-free launching. The arrangement must ensure that the liferaft or liferafts, when released and inflated, are not dragged under by the sinking vessel. Vessel's primary lifesaving was installed such that the liferaft(s) would be dragged under by the vessel if sinking. Arrange correct float-free arrangement as per manufacturer specifications and to the satisfaction of attending Marine Inspector.

There are 2 areas of hull damage in the #1P ballast tank resulting from an allision with an underwater object. These 2 areas of damage are located approximately 29' below the water-line in the vessel's current loaded condition. There is one 4" crack at the turn of the bilge between FR's 30 and 31, with an oblong inset of approximately 3". Another crack also approximately 4" long was discovered at the turn of the bilge between FR's 24 and 25, with a rounded inset of approximately 4". In addition FR's 30 to 33 exhibit minor deformation at the turn of the bilge. Vessel is authorized to affect temporary concrete-box repairs in both of these locations as approved by the attending USCG Marine Inspector. These repairs must be completed prior to departure from the Port of Duluth, MN. All associated damage to be permanently repaired during the vessel's next drydock period.

Rescue boat release gear must be operationally tested under a load of 1.1 times the total mass of the lifeboat when loaded with its full complement of persons and equipment whenever overhauled or at least once every 5 years. Vessel's rescue boat weight test is past due. Vessel is required to complete applicable testing in presence of attending USCG Marine Inspector prior to 25 April 2020.

Crack at the upper toe of the support bracket at Gate 82 in the starboard outboard cargo tunnel shall be cropped and renewed to the satisfaction of the attending marine inspector. Crack was temporarily repaired using a doubler.

The barge stbd-side aft inflatable liferaft (ILR) is not configured with an approved weak-link as part of its float-free arrangement. Properly install approved weak-link, or hydrostatic release, in accordance with 46 CFR 199.130(c)(7).

Effect permanent repairs to the 3/4" crack in the #l port ballast tank to the satisfaction of ABS and the cognizant U. S. Coast Guard Officer In Charge, Marine Inspection at next dry dock, no later than December 15, 2018.

Contact MSU Duluth for repair proposal and conduct repairs to port side #4 ballast tank to the satisfaction of US Coast Guard Marine inspector.

There are (02) slope plate fractures ("weepers") in way of the #3 cargo hold (stbd side) located on the upper slope strake directly below hatches #13 and #15 respectively. Each fracture is approx 20"-24" long and located opposite a common longitudinal within the ballast tank. Make repairs to both weepers, as original, prior to departure from the port of Two Harbors, MN.

2400 Volt supply wiring located outboard in portside forward tunnel at frame 114 damaged by small fire. Submit repair plan to MSU Duluth & effect permanent repairs ensuring proper operation of associated equipment the supply line serves & bow thruster to the satisfaction of attending CG marine inspector prior to departure from next port.

Sideshell found fractured above rubrail IWO #5 starboard ballast tank/FR#113. Vessel required to complete temp repair prior to departure from port

Sideshell found fractured above rubrail IWO #5 starboard ballast tank/FR#113 with temp repairs completed. Provide plan for permanent repairs which shall be completed prior to completion of 2015-2016 winter layup.

Anchor box damage noted port approx 8" fracture, stbd approx 12" fracture required to gouge and weld.

Fire main port side hatch 8 has a leak approx 1" with temp patch, permanent repair required.

Master of ITB PRESQUE ISLE, reported a 6' crack in hull plating on starboard quarter of barge. This breach resulted in water ingress into the #5 ballast tank. Installed pumps are keeping up with flooding. Owner/Operator decided to send vessel back to Duluth to effect repairs.

Windings failed. Prove proper operation of #3 SSG prior to departure.

Vessel's anchor became unattached while at anchor northeast of Manitowoc WI. All chain is onboard and it appeared that the shackle failed. Replace/renew anchor and attachment; Provide ABS class surveyor report that vessel is properly fitted with anchor(s) in accordance with ABS rules. 46 CFR 96.07-5

At or around 1230 the M/V PRESQUE ISLE (553416) allided with a loading crane at the US Steel dock on Zug Island in Detroit, damaging the barges's (553417) loading boom.

WBTs #2 port and starboard have cracks and/or wastage on fore and aft bulkheads, approximately 3 feet from bottom. Make proper repairs at earliest opportunity after ballast tanks #1 and #3 are emptied, to the satisfaction of attending USCG Marine Inspector.

Vessel's depth sounder INOP.

Make permanent repairs to crack sustained in #5 port side ballast tank.

#4 Center Ballast tank, cut, crop and renew 2 brackets stbd side forward of swash bulkhead as noted in and pending from MISLE Activity 2810828 narrative.

#5 Stbd Ballast; Crop & renew Frames 8, 9, 10, 11 at side tank bhd to slope connection. Crop & renew section of bilge frames apx 4' in from bilge. Apx size of crop 24inchs x 10inches at frames 6, 12, 13, 14, 16 & 17th from forward. Crop & renew shell longitudinal deep web at catwalk level at 15th frame outboard. Crop and renew Frame 8 at inbd side tank bhd at tank top level.

Both Air Receiver, located in the Aft Machinery room are over due for inspection. Open and/or have each pressure vessel inspected to the satisfaction of an attending USCG Marine Inspector IAW 46CFR 61.10-5.

Rescue boat in deplorable condition, including holes in hull near bow, rotted flooring, transom cracked/exposed, no battery boxes, and unapproved wiring. Company to submit proposal to replace existing rescue boat with a USCG approved rescue boat.

Crack in main deck, Stbd side cargo hatch #12. Repairs to crack in main deck to be inserted and crack in hatch combing to consist of gouge and weld.

Prior to operating in the 2006 season, following winter layup, make proper repairs to cracks noted in ballast tanks 3 & 5 starboard.

Make #3 Port ballast tank available for internal structure exam by a CG Marine Inspector, after vessel goes into 2005-06 winter lay-up status.

EFFECT PERMANENT REPAIRS TO INOPERATIVE #3 SHIP'S SERVICE DIESEL GENERATOR. REPAIRS TO BE COMPLETED NOT LATER THAN 31 MARCH 2006. TEMPORARY PROVISIONS FOR DECK-MOUNTED GENERATOR APPROVED BY DULMS AND ABS.

Port MDE #2 B High pressure fuel pump leaking excessively.

Three of the four ring buoys attached line needs to be replaced. Line no longer servicable due to deterioration. Vessel to submit copy of CR to [name withheld].

Repair and/or replace port outer shaft seal where leaking.

Damage sustained to hull port bow hull plating and frame numbers 8 - 9 in paint locker and below in forepeak. Permanent repairs are to be immediately addressed the company. All repairs are to be made to the satisfaction of the attending CG Marine Inspector.

Make permanent repairs to port shaft generator to the satisfaction of attending class surveyor. Fax copy of completion report to [name withheld].

Replace emergency generator starting batteries.

PRIOR TO 26APR99, INSPECT AND/OR REPLACE AS NECESSARY THE OUTBOARD SEALS ONTHE PORT AND STBD BOW THRUSTERS. THE PRESENCE OF A MARINE INSPECTOR IS REQUIRED TO WITNESS POST INSPECTION/REPLACEMENT TESTING TO ENSURE SEALS AREPROPERLY FUNCTIONING/NOT LOSING OIL.

Maintain temporary repairs to damage in way of stbd anchor pocket, near frame 6, in forepeak. Monitor water level in tank every 30 minutes. Make permanent repairs to affected area NLT 06Dec96 to the satisfaction of an attending marine inspector.

CONDUCT HYDROSTATIC TEST ON VESSEL'S PORTABLE FUEL HOSES.

NEXT CREDIT DRYDOCKING, MAKE PERMANENT REPAIRS TO SHELL PLATE AND ASSOCIATED INTERNALS IN WAY OF CONCRETE BOX IN FOREPEAK PORTSIDE 3RD FRAME FORWARD OF BOW THRUSTER TUNNEL.

Make permanent repairs to the floor frames where cropped for renewal in #3 center ballast tank, from frames 68 through 77 port and starboard prior to the compliance date.

THE BARGE PRESQUE ISLE WAS OBSERVED DISCHARING CARGO RESIDUES INTO LAKE HURON FROM A OVERBOARD DISCHARGE ON THE PORT SIDE.

PRESS UP P/S BALLAST TANKS IN WAY OF #2,3,4 CARGO HOLDS AND PROVE SOUNDNESSOF OF REPAIRS TO UPPER CARGO HOLD SLOPES ADLACENT TO SIDE TANKS.

NEW BALLAST STRIPPING SYSTEM IS TO BE INSPECTED BY A CG MARINE INSPECTOR INACCORDANCE WITH APPROVED PLANS PRIOR TO PLACING SYSTEM IN SERVICE.
