The position of the rudder, if power-operated, must be shown at the principal steering station. If there is non-follow-up steering control at the alternative steering station, there must be a separate rudder angle indicator system for that station that is electrically independent from each other rudder angle indicator system. Vessel reported loss of rudder position indication at all stations on the bridge. Troubleshoot and repair the steering system to properly operate from all stations as designed to the satisfaction of the attending Marine Inspector. Vessel must notify USCG prior to any repositioning. 46 CFR 113.40-5

Inspection after new installations shall comply with the applicable regulations and in accordance with approved plans. Vessel has approved plans from ABS for the new steering system but not from CG. Vessel may operate under ABS approved plans but must comply with any additional CG plan review requirements. All DVTP must be witnessed by both CG and ABS, prior to sailing.

Inspection after new installations shall comply with the applicable regulations and in accordance with approved plans. Vessel has approved plans to install an ABT Switch for the fire pump in the Engine room, and a new cable run from the main switchboard. The new cable requirement is in accordance with the new IEEE-Standard but not available. The vessel may use temporary cable in accordance with the old IEEE standard, until the new cable arrives.

The prime mover of a generator set must have:(1) All accessories necessary for operation and protection of the prime mover. L/O lower pressure shut down occurs at 2-3psi, as indicated on the analog pressure gauge. Per approved PSTPs shut down should occur at 10psi or higher. 46CFR70.25-1 46CFR112.50-1

Operating generators must provide a continouous and uninterrupted source of power for rhe ships service load under normal operational conditions. Vessel reported inoperable circuit breaker on generator #2.

A survey, either general or partial, according to the circumstances, shall be made every time a defect is discovered which affects the safety of the vessel. While completing car deck repairs required by ABS belt gauging Survey, other areas of the car deck and stern car ramp showed excessive wastage. AMHS requested deferment until December 2025. This request is approved by the [name withheld] all repairs in accordance with submitted plans to the satisfaction of the attending CG Marine Inspector.

Piping subject to internal head pressure from oil in the tank must be fitted with positive shutoff valve located at the tank with a means to remotely close the valve from outside the space. Close and Lock-out the low suction valve on the STBD side supplying fuel oil to your engines. Means of securing the valve outside the space is ineffective, repair and or replace valve at next overhaul.

The OWS must be equipped with an approved and calibrated 15 PPM monitor alarm control system. Retain oily waste onboard and or dispose properly to shoreside reception facility. Prior to operating the OWS install a calibrated control alarm system.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. Vessel reported a jacket water leak on the #10 cylinder, STBD main engine. Complete repairs to the satisfaction of the Coast Guard. 46CFR58.05-1

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. ABS conducted a Special Periodical Survey which identified excessive wastage.AMHS requested to defer repairs on the Main Car Deck and Exposed Deck until October. Sector SEAK approved deferment until October for repairs to the Main Car Deck and Exposed Deck. All other areas identified as excessive wastage within the repair plan must be addressed to the satisfaction of the attending Marine Inspector before the vessel goes back into service.

All magnetically controlled fire doors shall be operated locally and by the remote control and shall be checked to determine that they are in an operable condition. Fire screen doors 12, 12a did not fully close when released. Prove proper operation of fire screen doors when released in local and remote control.

The inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that it is in a satisfactory condition. Emergency generator fuel shutoff valve padlock is corroded. Replace with new padlock.

The inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that it is in a satisfactory condition. Emergency generator fuel shutoff break glass key lock is corroded. Replace with new lock.

The inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that it is in a satisfactory condition. Emergency generator room, starboard side access door electronic keypad is corroded. Replace with new lock keypad.

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Identified dead end wires hanging from overhead in the Fan Room aft of bridge crew accommodations. Properly terminate or remove dead end wire.

Vent outlets from all tanks which may emit flammable or combustible vapors, such as bilge slop tanks and contaminated drain tanks, must be fitted with a single screen of corrosion-resistant wire of at least 30 by 30 mesh, or two screens of at least 20 by 20 mesh. Goose neck vent for fuel/oil on port side of car deck requires screen of adequate size. Currently installed screen does not meet requirements. Repalce with proper size screen.

Each panelboard must have a circuit directory that has a description of the load of each circuit. In the Fan Room on Vent Panel P420, circuit breakers 15 and 16 are not labeled. Label circuit breakers 15 and 16 to identify what equipment it is protecting.

Each panelboard switching unit must be numbered. In the Fan Room on circuit breaker panel P-420-12, all of the circuit breakers are not numbered. Number the circuit breakers to match their description on the directory listed in the panel.

The emergency generator space has an approved fixed CO2 system designed with a 30 second time delay, prior to discharging to allow those in the space to exit. this delay tested at 25 seconds repair or replace the time delay with an approved 30 second time delay by a certified technician and re-test to the satisfaction of attending CG Marine Inspector.

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment required by the regulations in this subchapter is provided, maintained, and replaced as indicated. Vessel reported smoke detector in the car deck stairway, IVO where sprinkler system failed, is inoperable.

Doors, other than watertight; All doors, except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a position at the door. The release mechanism shall be so designed that the door will automatically close in the event of disruption to the control system; however, approved power operated watertight doors will be considered acceptable for this purpose. Holdback hooks, or other means of permanently holding the door open, not subject to control station release, will not be permitted. When double swing doors are permitted, they shall have a latch arrangement which is automatically engaged by the operation of the door release system. The A60 Food Stores Door on the starboard side, main deck aft is binding on the door frame making it hard to close and has a rope hold back installed. Properly repair the A60 Food Stores Door and remove the holdback device. 46 CFR 72.05-25

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2/HALON cylinder storage room. On systems in which the CO2/HALON cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Each control valve to branch lines must be marked to indicate the related space served. The HALON fixed fire system in shaft alley, control valves are missing labels. Verify that the correct numbers are associated with each control valve. 46 CFR 76.15-10.

Bulkheads or decks of the “A” Class shall be composed of steel or equivalent metal construction, suitably stiffened and made intact with the main structure of the vessel, such as shell, structural bulkheads, and decks. They shall be so constructed that, if subjected to the standard fire test, they would be capable of preventing the passage of smoke and flame for 1 hour. In addition, they shall be so insulated with approved structural insulation, bulkhead panels, or deck covering that the average temperatures on the unexposed side would not rise more than 250 °F. above the original temperature, nor would the temperature at any one point, including any joint, rise more than 325 °F. above the original temperature, within the time listed below: Properly repair the damaged A60 insulation on the car deck overhead, aprox 50' forward of the aft car ramp door. 46 CFR 72.05-10

If port lights are fitted in spaces used alternatively for the carriage of cargo or passengers as permitted by § 171.116(d) of this chapter, dead covers must be fitted on the port lights when cargo is carried. Properly repair cracked damaged glass in the aft port light located inboard of the aft car deck ramp. 46 CFR 78.16-1

There shall be at least two means of escape from all general areas accessible to the passengers or where the crew may be quartered or normally employed. At least one of these two means of escape shall be independent of watertight doors. For stairway continuity and general requirements for stairways see § 72.05-20. Remove stored items away from the vertical emergency means of escape in the vessels shaft alley space. 46 CFR 72.10-5

Fire hazards to be minimized. (a) The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. The oily rag waste receptacle and trash receptacles in the engine room are missing metal lids. 46 CFR § 72.03-5

The inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that it is in a satisfactory condition. Properly repair the lock and key lock box for the Emergency Diesel Generator remote emergency fuel shut off valve. 46 CFR § 71.30-5

The system shall be so designed and arranged that the overhead is effectively sprayed and all portions of the deck are covered. The capacity shall be such that at least 12 gallons of water per minute are applied to each 100 square feet of deck area. Properly store all items below the fixed sprinkler system nozzles in galley pantry's and ship storage locations throughout the vessel, noted items stored above sprinklers in the forward upper storage locker above the ships paint locker/shop area and in the ships food stores locker by the galley. 46 CFR § 76.23-10

Each self-propelled vessel must have duplicate light sources for the side, masthead, and stern lights. Port side navigation light on bridge wing is inoperable. Prove proper operation of port navigation light to attending USCG marine inspector prior to departure.

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port required to be marked on the stern of the vessel. The vessel name and port on multiple ring buoys had worn off and was illegible. Ensure all ring buoys are clearly marked in block capital letters with the vessel name and port. 46 CFR 199.70

The Type II retro-reflective material approved under approval series 164.018 must be placed on the boat to meet the arrangement requirements in IMO Resolution A.658(16). Vessels lifeboat retro-flective material is worn off and faded. Replace all worn retro-flective material and add missing retro-flective material to the bottom of each lifeboat iaw IMO Resolution A.658(16). 46 CFR 199.176

Oily water separating equipment and bilge alarm approval standards.(a) On U.S. inspected ships, oily water separating equipment and bilge alarms must be approved under 46 CFR 162.050. Vessels OWS is currently out of commission due to faulty circulation pump motor. Repair faulty motor to satisfaction of the attending CG Marine Inspector. 33 CFR 155.380

Vessel reported fire on starboard MDE. Oil had accumulated on top of engine gallery. Remove all pooled oil from both MDEs and troubleshoot STBD engine for any oil or exhaust leak causing pooling/splashing of oil.

Fire on STBD MDE damaged exhaust lagging. Cleanup lagging and replace all damaged lagging.

Oil pooled on top of both MDE galleries. Submit a nonconformity and corrective action to amend the SMS/PMM procedures for MDE housekeeping following spring valve weekly inspections.

The Day Tank fuel vents on the port side car deck were missing the flame screens. Re-install the flame screens as required.

Wood scaffolding material was left in the exhaust stack space from dockside maintenance. Remove combustible material.

A temporary card board tape repair was covering a hole left by a removed light fixture. Permanently repair light fixture or construct a more durable temporary repair while awaiting parts.

The high pressure limit switch for the aux boiler is not incorporated into the Periodic Safety Test Procedures (PTSPs). Amend PTSPs to include the limit switch test.

Cans of flammable cleaner were stored in the gallery deck shelving . Remove flammable material or store in proper storage area.

The car deck leak detector was not attached to the mounting bracket. Remount leak detector.

46 CFR 61.40-10 Periodic Safety Test Procedure (PSTP) document must be in a step-by-step or checkoff list format and must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety controls and alarm set points, the procedure to be followed, and the expected test results. The PSTP for testing of bilge pump needs to be corrected/updated. As currently written the diapham pump is unable to prime the system and the ships fire pump/auxillary SW pump needs to be used to accomplish priming.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. The interlock switch for the stbd lifeboat hand crank was not operational. Make repairs and provide proof to the attending marine inspector.

Each lifesaving appliance must be inspected monthly using the checklists required under paragraph (b)(1)(i) of this section to make sure the appliance and the equipment are complete and in good working order. Several lifejacket lights were not operational and one crew lifejacket was missing a light. Monthly material condition inspections were conducted by crew. Inspections were not all-encompassing of the requirements set forth in the vessel's checklist. Conduct a full monthly inspection of all lifejackets onboard to make sure all equipment is in good working order. Provide proof to the Coast Guard within one week.

All lifesaving appliances and arrangements on passenger vessels must be in accordance with Subchapter W (lifesaving appliances and arrangements) of this subchapter. The #1 Lifeboat’s alternator for the engine has failed and no longer is charging the starting batteries. Repair #1 Lifeboat to original condition and notify the USCG Sector Juneau Inspections office when permanent repairs are complete. The vessel may continue to operate the #1 Lifeboat under the following conditions: - Outfit vessel with an additional portable all around white light to be used for supplemental navigation light. - Monitor battery status with daily pre-departure battery voltage checks. - Notify USCG Sector Juneau Inspections office with any status change to the #1 Lifeboat. 46 CFR 70.28-1

Vessel reported that the #1 fire pump was not functional due to a faulty transformer. Vessel conducted temporary repairs and the fire pump is now working properly. Vessel is required to conduct permanent repairs and notify the CG of any status change to the vital system operation. CG attendance will be required to clear the deficiency. 46 CFR 71.40-1(a)

Each fixture, globe, lens, or diffuser must have a high strength or be made of high strength material. The fixture on the bridge deck aft in the solarium did not have a glass globe and there were two other emergency lights on the port side that were extinguished. Re-install globe into fixture and renew two extinguished e-lights.

Lighting for survival craft launching, including the area of the water wherein is to be launched. Both life boats and both MES lights for the water were not pointing at the water below the survivial craft. Re-aim lights to illuminate water prior to carrying passengers.

Protection of passengers, crew, other persons and the vessel from electrical hazards. On the car deck, the port freezers refrigeration unit’s condenser is frozen and melting on/near a light switch. Remove ice and/or cover light switch.

The escape scuttle from the sewage space to the car deck was not properly marked. Mark scuttle “Emergency escape, Do not Block”

The launching appliance must be capable of launching and recovering the lifeboat with its crew. The vessels crew noted damaged and missing grease fittings on the forward lifeboat hooks and the hooks are extremely stiff and take more effort than usual to open. Replace forward lifeboat hooks at next drydock.

Each lifeboat must be provided with a launching appliance. The launching appliance must be capable of launching and recovering the lifeboat with its crew. The vessels crew noted damaged and missing grease fittings on the forward lifeboat hooks and the hooks are extremely stiff and take more effort than usual to open. Provide plan for repair or replacement of forward lifeboat hooks to USCG Marine Inspector.

Each vessel must have a means of communication. Multiple stations must be able to communicate at the same time. The Talkback System is not functioning properly. Prove proper operation of Talkback system to attending USCG Marine Inspector.

An automatic damper shall be installed in exhaust ducts over frying vats. The Gaylord fire suppression system for cooking appliances in the galley is not functioning properly. The deep fat fryer, grill and other cooking appliances effected by the inoperative exhaust hood must be secured until proper operation is proved by attending USCG Marine Inspector or tech service report is received.

46 CFR 199.180 Prior to carrying passengers complete fire and abandon ship drill's, to include lowering, operation, and recovery of the starboard lifeboat.

46 CFR 61.40-10 Periodic Safety Test Procedure (PSTP) document must be in a step-by-step or checkoff list format and must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety controls and alarm set points, the procedure to be followed, and the expected test results. The following PSTPs from test from draft PSTP Rev E, Elliot Bay job # 09109-001-436-4, dated 1/15/21 must be updated: 1) The loss of lube oil alarm and shutdown on the Ships Service Diesel Generators are written as one test with the same test point used to achieve the alarm and shutdown. Because there are two separate test points (one for the alarm and one for the shutdown), this test procedure needs to be rewritten as two separate tests. 2) The PSTP outlines procedures for testing the leak off alarm on the ships diesel propulsion engines, however, this device has never been installed. Move this testing procedure to the Ships Service Diesel Generator where it is installed. 3) In accordance with the 2004 CAT Tech Note, Add the -/+ 25 RPM over-speed allowance to the Ships Service Diesel Generator PSTP. 4) Update the number, location, and type of fire damper installed on the vessel. Fire damper 40-42 located in the crew laundry were not listed in the PSTP.

46 CFR 72.15-15 All enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. The supply air fans to the steering gear and shaft alley remote shutdown from the bridge was not operational. Make permanent repairs to the bridge remote shutdown for both spaces and prove proper operation of local and remote means for securing the ventilation to these spaces.

46 CFR 72.05-50 Due to material degradation during visual inspection, replace fire damper AFD #10.

Center steering station helm control occasionally wanders causing the rudder to deflect 5 deg when the system is started up or autopilot is disgengaged. Switching to NFU, cycling pumps, or switching to bridge wing controls corrects the problem. Technicians are looking to diagnose cause and make repairs.

#2 SSG waterpump is failing causing a leak throught the seals. Prove proper operation of SSG

Conduct all Tag/lock out requirements as per the Vessels SMS. Vessel’s SMS requires tagging/locking out of all machines/equipments protect against accidental or inadvertent operations which could cause injury to personnel. This also applies when working on or near de-energized electrical circuits/ parts. Additionally a check and audits of all lockout/tag out are to performed once every two weeks. Last entries in tag out/ lockout log was 9-16-19 and there has not been any log entries of any audits or checks conducted. Lockout/ Tag out request forms are not being correctly fill out.

Conduct external audit on the SMS for the Lock-out/Tag-out Procedures

Vessel experienced a shutdown on it's #1 Ship Service Generator. Vessel must have 2 sources of electrical generation for redundancy.

Periodic Safety Testing Procedures demonstrate proper operation of the primary and alternate controls and alarms addressed in 46 CFR 62. The following test points need to be rewritten and submitted to the Marine Safety Center, 46 CFR 61.40: a) The Auxiliary Boiler. 46 CFR 61.35 b) The Steering Gear controls. 46 CFR 58.25-25 c) Remove CO detection. 46 CFR 78.83-1

46 CFR 62.30-10: Periodic Safety tests must demonstrate the proper operation of primary and alternate controls, power sources, over-ride arrangement, interlocks, and safety controls. The PSTP (Rev D) for the Boiler was not appropriate for the boiler and the newly installed controls. Submit PSTP to MSC for Review within 30 days.

Stbd steering system has an anomaly that on rare occasions causes it to not return to the degree indicated by the helm wheel.

The Boiler (#12) PSTP does not contain any of the time-frame or psi parameters needed to effectively determine whether the safety controls operate as designed within the proper times/pressures. Update PSTP with proper parameters. The Steering Gear (#10H) Motor Overload PSTP needs to be amended to require the ships crew to energize the system prior to conducting tests. Remove the CO detection (#8E) PSTP from the procedures. The company has removed the fixed CO detection system and only utilizes a portable detection system.

During the 2018 COI renewal, it was determined that the water column on the ships service boiler had been replaced and improperly installed, preventing the high water level alarm to actuate as per the parameters in the PSTP. No later than January 1, 2020, properly install water column and demonstrate proper operation of the high water alarms safety shut down to a Coast Guard Marine Inspector.

ach Class 3 watertight door must be designed and constructed in accordance with ASTM F1197. When being operated/closed electronically from a remote operation, there must be a 5 second delay and an audible alarm must sound prior to the door moving. The door did not have a 5 second delay or the audible alarm. Correct deficiency with 60 days.

Properly repair stbd MDE keel cooler iwo shaft alley 3' aft of the fwd shaft alley space bulk head, under the shaft. Temp repair is currently in place, 6" x 10", 5/16" steel doubler plate w/concrete patch overtop of the doubler. Conduct u/t testing of metal iwo keel coolers to ensure integrity of system both port and stbd. Permanent repairs to be made at the vsl's next dry dock scheduled Jan 2017.
