No repairs or alterations affecting the safety of the vessel with regard to the hull, shall be made without the knowledge of the [name withheld] of alterations shall be approved before work is started, unless deemed unnecessary by the OCMI. Identified starboard drain along cardeck was removed and pipe blanked off without approval. Submit alteration proposal to ABS or repair in kind with original drawings.

The diesel oil transfer pump #1 failed to secure the pump when the switch outside the space was activated. repair the controller and re-test to the satisfaction of attending CG Marine Inspector.

Watchmen shall be under the direct charge of the master or officer in command of the vessel, and shall report to such officer at the pilothouse at fixed intervals not exceeding one hour. Watchman use a wand system which uploads completed rounds to a computer on the bridge. Computer records indicated rounds were incomplete and the crew could not provide an explanation. Identified objective evidence that the crew was not verifying completion of rounds and the SMS lacked procedures for the verification of completed rounds within the computer system. The ISM Code states: The company should establish procedures... for key shipboard operations concerning the safety the personnel, ship, and protection of the environment. Develop procedures for the watchman system which ensure completion of watchman rounds.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. Vessel reported a pinhole leak (approx. one drop every two minutes) on the fire piping manifold for the deck sprinkler system, in vicinity of the crossover valve to the secondary pump source. Conduct gauging approximately 12 inches around the affected area of piping to assess extent of deterioration. Complete repairs , to satisfaction of the attending Marine Inspector while vessel is in Ketchikan on/or around 21MAY2025. 46CFR71.40-1

Gyrocompass has intermittent uncorrelated rate of turn alarm and Gyro Heading Error alarm. Prove proper operation of gyro compass without alarms to attending USCG marine inspector.

In addition to the radar system under 164.35(a), the vessel is required a second marine radar system that operates independently of the first. Independent operation means two completely separate systems, so that failure of any component of one system will not render the other system inoperative.
