Identified starboard drain along cardeck was removed and pipe blanked off without approval. Submit alteration proposal to ABS or repair in kind with original drawings.

Where automatic fire dampers are required, they shall be made accessible for periodic inspection by means of a hinged or bolted plate in the duct. Inspect all installed fire dampers within the next 30 days and demonstrate to the satisfaction of a CG Marine Inspector in accordance with 46 CFR 72.05-50 (e).

Each forced draft fan, fuel oil transfer pump, fuel oil unit, fuel oil service pump, and any other fuel oil pumps must have a stop control that is outside of the space containing the pump or fan. The diesel oil transfer pump #1 failed to secure the pump when the switch outside the space was activated. repair the controller and re-test to the satisfaction of attending CG Marine Inspector.

Watchmen shall be under the direct charge of the master or officer in command of the vessel, and shall report to such officer at the pilothouse at fixed intervals not exceeding one hour. Watchman use a wand system which uploads completed rounds to a computer on the bridge. Computer records indicated rounds were incomplete and the crew could not provide an explanation. Identified objective evidence that the crew was not verifying completion of rounds and the SMS lacked procedures for the verification of completed rounds within the computer system. The ISM Code states: The company should establish procedures... for key shipboard operations concerning the safety the personnel, ship, and protection of the environment. Develop procedures for the watchman system which ensure completion of watchman rounds.

Each inflatable lifesaving appliance and marine evacuation system must be serviced within 12 months of each subsequent servicing, except when servicing is delayed until the next scheduled inspection of the vessel, provided the delay does not exceed 5 months. Survival craft servicing due to expire 1NOV25. Request for an extension for the annual servicing is granted until 31JAN26.

Vessel reported a pinhole leak (approx. one drop every two minutes) on the fire piping manifold for the deck sprinkler system, in vicinity of the crossover valve to the secondary pump source. Conduct gauging approximately 12 inches around the affected area of piping to assess extent of deterioration. Complete repairs , to satisfaction of the attending Marine Inspector while vessel is in Ketchikan on/or around 21MAY2025. 46CFR71.40-1

The bow thruster is not operating as approved and designed, make repairs as necessary to assist the vessel in mooring and maneuvering situations. Provide technician report and notification when the skin valve has been purchased and replaced.

All mechanically propelled vessels 1,600 gross tons and over in ocean or coastwise service must be fitted with a gyrocompass in addition to the magnetic compass. Gyrocompass has intermittent uncorrelated rate of turn alarm and Gyro Heading Error alarm. Prove proper operation of gyro compass without alarms to attending USCG marine inspector.

In addition to the radar system under 164.35(a), the vessel is required a second marine radar system that operates independently of the first. Independent operation means two completely separate systems, so that failure of any component of one system will not render the other system inoperative. The vessel reported that two of the three radar systems on board were not operating properly. Due to the restricted waters the vessel operates in, the vessel is limited to daylight operations with 3nm visibility until repairs are made and the vessel has two operating radar systems. 33 CFR 164.37(a)

Piping systems shall be tested by means of hydrostatic leak test in the presence of CG marine inspector at 1.5 times maximum allowable working pressure, to be determined by the largest pump. The vessel completed permanent repairs to the fire main that were tested via dye pen and operational pressure test. A hydrostatic pressure test at 1.5 times MAWP shall be conducted at next scheduled dry-dock of the entire system. Make all repairs necessary and test to the satisfaction of attending CG Marine Inspector. 46 CFR 56.97-40

All marine engineering details, such as boilers, pressure vessels, main and auxiliary machinery, piping, valves, and fittings, shall be designed, constructed, and installed in accordance with the provisions of this subchapter. Vessel reported inoperative bow thruster. Provide dive report and repair proposal to attending USCG Marine Inspectors prior to departure from Ketchikan, AK.

MSD Ketchikan was made aware of a temp patch to the SW pipe on the discharge side of the #2 gen engine driven pump. Permanent repairs shall be made prior to departing next port. 46 CFR 71.55-1(a)

It is mandatory that the design, fabrication and erection of piping constructed under the regulations in this subchapter demonstrate leak tightness. The vessel reported a two pinhole leaks in the fire main. A metal sleeve was welded around the suspect areas of the fire main as a temp repair. 56.97-1(b)

IN accordance with ABS and vessels engine manufacturer, MDE's are required to have installed oil mist detectors. Port MDE oil mist detector is faulty and shuts the MDE down without the presence of oil. The shutdown function is currently being bypassed. The oil mist detector is still functional and can be heard in the engine control room. Restore the port MDE oil mist detector to proper operation and to the satisfaction of the attending Coast Guard Marine Inspector. Additionally provide technician report attesting to the repair and causal analysis of the failure.

Rescue boat release gear must be operationally tested under a load of 1.1 times the total mass when loaded with its full complement of persons and equipment, whenever overhauled or falls renewed. Demonstrate testing of the falls to the satisfaction of attending CG marine Inspector.

Each passenger vessel of 500 tons gross tonnage and over must carry on each side of the vessel at least one rescue boat. Repair fast recue and install in it's appropriate cradle. Demonstrate proper operation by lowering running and recovery of both rescue boats.

Provide a repair proposal for the holes near the grease traps in way of the upper car deck and fiddly space (class A fire boundary), Prior to carrying passengers. 46 CFR 72.01-15

The emergency generator must shut down in an emergency situation by all installed safety devices. Currently the generator will not shut-down when in manual mode with the activation of the CO2 system or by use of the local emergency switch and the fire dampers are not functioning automatically. Submit plans to the MSC for approval to correct issue. 46 CFR 112.50

Identified wasted steel on the galley deck below the steamer in way of frame 104 & 107. Crop and renew to good steel. 46 CFR 72.01-15

Provide pit/gauge report of the sprinkler tank internal bottom due to notable pits. If repairable do in accordance with Navigation and Vessel Inspection Circular NO. 11-92. A registered professional engineer must prepare and certify Form R-l meeting the requirements of the NBIC, ASME Code, part 59.

Replace the two valves that were temporarily repaired in the shaft Alley for the emergency fire pump/ballast system. 46 CFR 56.20

Make permanent repairs to starboard rescue boat to satisfaction of attending CG marine inspector or replace boat.

Water leaking from the boiler blowdown piping from flange between temporarily-repaired piping and skin valve. Effect repairs to restore watertight integrity to the satisfaction of the attending marine inspector. Until such time as repairs are made and verified by the marine inspector, monitor leak and alert the cognizant OCMI of any increase in the rate of leakage. - 46 CFR 56.01-1(b)

Exhaust manifolds and piping are to be insulated with fireproof material or are to be water-cooled such that the temperature of exposed parts does not exceed 204°C (400°F). The number 2 ship generator exhaust system has insufficient insulation on the upper level in way of the walk area exposing personnel to the hot surface. Piping or exhaust systems over 150 degrees F is required to be lagged to protect personnel. 46 CFR 58.10-5(d), Incorporates ABYC P-1 and NFPA 302. ABS rules, 46 CFR 56.50-1(k)

At next dry-dock make permanant repairs to the port side overboard discharge piping/spool piece for the boiler blow down. Repairs are to be made in accordance with 46 CFR 72.01-15 and 46 CFR 71.55.

In accordance with the engine manufacturer & (ABS) the Main Propulsion Engines are required to have installed oil mist detectors (OMD). Wartsila requires specified Coalescers requiring modifications to the OMD, and providing no known delivery date. Due to this, this 835 will be allowed for one (01) year to be completed no later than the next dry-dock. Both MPE's to be modified and tested by CG Marine inspector. 46 CFR 62.35-50

Exhaust manifold connections are to be constructed of pipe or pipe fittings with screwed or welded connections or bolted connections with fireproof gaskets." In addition, piping over 150 degrees F is required to be lagged to protect personnel. Bow thruster was missing lagging, make it up properly, to the satisfaction of CG Marine Inspector.46 CFR 58.10-5(d) Incorporates ABYC P-1 and NFPA 302,ABS rules,46 CFR 56.50-1(k)

Lower, release, operationally test and recover the port rescue boat.

Prove proper operation of bow thruster starting system.

A complete survey of all equipment in the bow thruster compartment must be completed to assess if any components were damaged during the exhaust manifold overpressure and CO2 discharge incident as required by 46 CFR 76.40-1. Any affected equipment must be renewed as original to the satisfaction of the attending marine inspector prior to carrying passengers.

Make permanent repairs to the starboard overboard discharge piping for waste heat boiler blow down line. Additionally, the sea valve for the line is to be relocated, on the discharge line, as close as possible to the side shell. The current location of the sea valve is too far from the side shell.

Pinhole leak on waste heat boiler blowdown line. Temp repair made with synthoglass pathc. Vessel authorized to transit normal itinerary to Ketchikan to conduct non-destructive examination of pipe and effect repairs to the satisfaction of the attending CG MI from MSD Ketchikan.

Each passenger vessel of 500 tons gross tonnage and over must carry on each side of the vessel at least one rescue boat. The propulsion system on the port fast rescue boat is currently partially inoperable. The vessel is limited to a maximum of 300 passengers until the deficiency is corrected. The port rescue boat must be repaired to the satisfaction of the attending marine inspector by 27APR18.

Gauge thickness of Firemain pipe in way of wasted area to the horizontal connecting pipe and all the way to the fire station #67 (vertical section). Provide repair proposal to MSD Ketchikan. Make approved repairs in accordance with attending CG Marine Inspector.

The rudder angle indicator connecting rod broke. Repair prior to sailing.

Oil mist detector on starboard main engine activates without presence of oil, resulting in engine shutdown. Restore starboard main engine oil mist detector to proper operation to the satisfaction of the attending Coast Guard marine inspector and provide tech's report.

Provide approval letter from MSC for Revision I of the Design Verification Testing Procedures. 46 CFR 61.40

Make complete repairs to starboard side CPP system. Provide technical report identifying the root cause of CPP failure, explanation of effected components, and documentation of all repairs made. Report must be reviewed by the Coast Guard and your classification society prior to any vessel movements. (46 CFR 58.05-1)

Conduct emergency drills with vessel's entire crew in the presence of a Marine Inspector prior to carrying passengers. At time of inspection, 08 crew members were absent.

Properly repair inoperable fast rescue boat and conduct a speed trial proving proper operation in the presence of a Marine Inspector prior to carrying passengers.

Make permanent repairs to the #1 generator's main display to show proper voltage readings in idle mode.

Provide approved DVTPs and prove proper operation of the steering stand prior to the carriage of passengers. Until this item is complete, a helmsman shall remain on watch at the main steering console AT ALL TIMES while u/w. Additionally, until this item is complete, when maneuvering or while navigating the river systems, the engine control room shall be manned with one licensed engineer.

Properly label #84, #85, #86, #87 fire screen doors.

Properly label and restore the 06 inoperable emergency lights.

Repair damage to starboard side propeller. 46 CFR 71.40-1

Vessel shall install new oil mist detector on the starboard main engine. Vessel must continually monitor engine temps, bearing temps, and crankcase pressure until detector is replaced.

Identify the source and location of oil leak, (internal or external). Provide dive report. Monitor the amount lost per day and notify CG if it exceeds 2 gallons.

Provide repair proposal once source and location are identified.

Vessel is restricted from operations until the malfunction to the starboard main engine is determined, corrected, and inspected by the attending marine inspector.

Remove the small hydraulic oil polishing pumps located booth steering system. Currently fed by the steering controllers, not part of the original system or approved to be wired to the controllers. If this system is considered vital then submit plans to the MSC for approval.

Make permanent repairs to the temporary repaired pin hole leak for the sprinkling system piping located STBD side shaft alley bilge area. Inspect length of the 4-6" piping for additional wasted. Inform via email local OCMI of the results. Pressure test piping in the presence of a CG Marine Inspector once permanently repaired.
