Electrical systems on vessel must ensure maintenance of services necessary for safety under normal and emergency conditions. Emergency generator is in the final stages of comissioning and requires the coolant fluid prior to running the final load test.

Port and STBD bridge wing lifebuoy with attached smoke was not in compliance with manufacturers instructions. The line attaching the smoke float to the body of the lifebuoy was not at proper length requirement. 74SOLAS(20) III/20.2

Electrical installations shall be such that electrical services essential for safety will be ensured under various emergency conditions. The embarkation lighting near the port side lifeboats was found operable but had the wrong cable plug connection and was very loose. 74SOLAS(04) II-I/40.1.2

MI observed multiple holes approx. 1/4" diameter drilled through B-class bulkhead of the 02 deck unlicensed crew lounge. 74SOLAS(01)II-2/9.1

The purpose of this regulation is to control the spread of smoke in order to minimize the hazards from smoke. For this purpose, means for controlling smoke in atrium's, control stations, machinery spaces and concealed spaces shall be provided. MI observed several marine cable transits located on the 02 deck partially opened due to threading of new Internet cables. 74SOLAS(01)II-2/Reg 8.1

Lifesaving appliances and arrangements shall be approved by the Administration. For vessels enrolled in ACP (such as the USNS Red Cloud), lifejackets are required to hold type approval from the CG under approval series 160.155. At the time of attendance, only 6 of the 121 life jackets onboard are 160.155 lifejackets. The remaining are primarily 160.055/190 lifejackets marked as "Type V PFD - For Official Department of Defense Use Only". The MSC-USCG Memorandum of Agreement (MOA), Annex III, Section 4.a discusses lifesaving equipment and states that "The USCG may accept alternative lifesaving equipment or arrangements...when any USCG-certified MSC vessel replaces lifesaving appliances or arrangements...each new lifesaving appliance or arrangement must meet current regulations. The 160.055/190 lifejackets are date stamped as inspected and passed in 2018. Provide 111 lifejackets with the 160.155 CG type approval (# required per SAFEQ Form E), or objective evidence that the CG has accepted the use of the 160.055/190 lifejackets on MSC vessels such as the USNS Red Cloud that hold a valid COI under ACP. Update 9/29/23: it was agreed that ~40 SOLAS lifejackets will be provided to cover the number of persons onboard for the transit from Baltimore to Bayonne. Disposition regarding the remaining 70+ lifejackets will be due by the end of the dry dock.

The thimble of the Jacob's ladder for one of the port side lifeboats is wasted and requires replacement prior to departure from port. The deck padeye for the same Jacob's ladder is corroded and requires replacement during the upcoming dry dock.

main propulsion system must be provided in duplicate unless the system served is provided in independent duplicate, or otherwise provides continued or restored propulsion capability in the event of a failure or malfunction of any single auxiliary component. Vessel had propulsion failure due to new software components. Item to be cleared by Class Society.

Audible alarm for door 5-62-1 did not work during testing.

Door handle to galley, over/fryer area, did not work properly.

Sliding water tight door failed to close from remote bridge control when testing.

Lube oil found on deck, starboard side E/R from sight glass valve.

.25" x .0625" hole on salt water cooling pipe, discharge side of S/W cooling pump. Conduct permanent repairs on the pipe no later than next schedule dry dock.

Lifeboat No. 1 (STBD) was found with the aft davit locking pin bent & cracked. This pin secures the davit in the stowed position. Conduct davit repair IAW OEM instructions.

Lifeboat No. 4 (Port) was found with a puncture in the hull at the aft end near the waterline on the outboard side. Conduct hull repairs IAW OEM instructions.
