Electrical systems on vessel must ensure maintenance of services necessary for safety under normal and emergency conditions. Emergency generator is in the final stages of comissioning and requires the coolant fluid prior to running the final load test.

Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. Port and STBD bridge wing lifebuoy with attached smoke was not in compliance with manufacturers instructions. The line attaching the smoke float to the body of the lifebuoy was not at proper length requirement. 74SOLAS(20) III/20.2

Electrical installations shall be such that electrical services essential for safety will be ensured under various emergency conditions. The embarkation lighting near the port side lifeboats was found operable but had the wrong cable plug connection and was very loose. 74SOLAS(04) II-I/40.1.2

The purpose of this regulation is to contain a fire in the space of origin. For this purpose, the following functional requirements shall be met: The fire integrity of the divisions shall be maintained at openings and penetrations. MI observed multiple holes approx. 1/4" diameter drilled through B-class bulkhead of the 02 deck unlicensed crew lounge. 74SOLAS(01)II-2/9.1

The purpose of this regulation is to control the spread of smoke in order to minimize the hazards from smoke. For this purpose, means for controlling smoke in atrium's, control stations, machinery spaces and concealed spaces shall be provided. MI observed several marine cable transits located on the 02 deck partially opened due to threading of new Internet cables. 74SOLAS(01)II-2/Reg 8.1

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. The fire dampers 4-109-3 and 4-108-1 would not fully close. 74SOLAS(20) II-2/14.1.1

Lifesaving appliances and arrangements shall be approved by the Administration. For vessels enrolled in ACP (such as the USNS Red Cloud), lifejackets are required to hold type approval from the CG under approval series 160.155. At the time of attendance, only 6 of the 121 life jackets onboard are 160.155 lifejackets. The remaining are primarily 160.055/190 lifejackets marked as "Type V PFD - For Official Department of Defense Use Only". The MSC-USCG Memorandum of Agreement (MOA), Annex III, Section 4.a discusses lifesaving equipment and states that "The USCG may accept alternative lifesaving equipment or arrangements...when any USCG-certified MSC vessel replaces lifesaving appliances or arrangements...each new lifesaving appliance or arrangement must meet current regulations. The 160.055/190 lifejackets are date stamped as inspected and passed in 2018. Provide 111 lifejackets with the 160.155 CG type approval (# required per SAFEQ Form E), or objective evidence that the CG has accepted the use of the 160.055/190 lifejackets on MSC vessels such as the USNS Red Cloud that hold a valid COI under ACP. Update 9/29/23: it was agreed that ~40 SOLAS lifejackets will be provided to cover the number of persons onboard for the transit from Baltimore to Bayonne. Disposition regarding the remaining 70+ lifejackets will be due by the end of the dry dock.

The thimble of the Jacob's ladder for one of the port side lifeboats is wasted and requires replacement prior to departure from port. The deck padeye for the same Jacob's ladder is corroded and requires replacement during the upcoming dry dock.

main propulsion system must be provided in duplicate unless the system served is provided in independent duplicate, or otherwise provides continued or restored propulsion capability in the event of a failure or malfunction of any single auxiliary component. Vessel had propulsion failure due to new software components. Item to be cleared by Class Society.

Audible alarm for door 5-62-1 did not work during testing.

Door handle to galley, over/fryer area, did not work properly.

Sliding water tight door failed to close from remote bridge control when testing.

Lube oil found on deck, starboard side E/R from sight glass valve.

.25" x .0625" hole on salt water cooling pipe, discharge side of S/W cooling pump. Conduct permanent repairs on the pipe no later than next schedule dry dock.

Lifeboat No. 1 (STBD) was found with the aft davit locking pin bent & cracked. This pin secures the davit in the stowed position. Conduct davit repair IAW OEM instructions.

Lifeboat No. 4 (Port) was found with a puncture in the hull at the aft end near the waterline on the outboard side. Conduct hull repairs IAW OEM instructions.

Vessel shall update periodic safety test procedures to accurately reflect testing methods and submit for review/approval to USCG Marine Safety Center. Verification of updated test procedures to be witnessed by USCG Marine Inspector.

Vessel does not have a SOLAS approved rescue boat onboard and will use #1 lifeboat as a rescue boat. SOLAS approved rescue boat shall be acquired and proper operation demonstrated prior to departing Boston, MA.

Complete credit drydock exam prior to departing Boston, MA.

Provide all International and class certificates prior to departing Boston, MA.

Prove proper operation of sprinkler for forward foam system prior to loading cargo.

Conduct 1.1 % overload weight test on of rescue boat davit prior to using the davit and prior to leaving Boston, MA.

Conduct hydro, fireside and waterside exam for # 1auxiliary boiler (forward) prior to using the boiler and prior to leaving Boston.

Complete required annual survey of SOLAS Safety Equipment Certificate upon activation.

Replace expired life saving equipment for all lifeboats as needed upon activation.

Service all inflatable liferafts as required upon activation.

Replace expired lifebuoys smokes as required upon activation.

Replace 60s time delay for Bow thruster (time delay released at 51seconds).

Provide approved structural fire protection insulation (fire-stop) to cable run, located portside E-1 level.

Conduct satisfactory fire and abandon ship drills.

Provide proof of manning IAW vessels current COI.

At next Dry-dock / UWILD make available the fixed ballast tank coupons for inspection.

Repair leaking fire hydrant valves for fire stations FM 108 7-91-2, and FM 106 7-73-2. LEGACY COMPONENT DESC: Condition

Install replacement part (ordered) for ruptured hand pump housing for sliding watertight door between cargo bays 3 & 4 on D-Deck. Demonstrate proper operation of door. Door must remain closed while underway.

Install ordered bells for e/r sliding watertight doors so that they provide distinct audible alarms to indicate door movements.

Adjust or otherwise reduce interfaces between sliding watertight doors & frames in e/r so that sealing surfaces are sufficiently tight. Interfaces were found to be loose enough to accept .075 - .125 feeler gauge between machined sealing surfaces.

Prove proper operation of port gas turbine propulsion engine.

Proceed to a port to make permanent repairs to the port shaft and/or strut bearing upon offloading cargo in the U.S. and notify local OCMI of vessel's condition and your plan for repairs. LEGACY COMPONENT DESC: Bearing

Make permanent repairs and demonstrate operation to include hose test of sealing surfaces to sliding watertight door 6-86-2.
