The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. No. 3 Anchor Windlass and Mooring Line Winch is in-operable due to sheared bolts.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The crew reported that a hydraulic line parted from hydraulic cylinder while using the hydraulic cargo door on the weather deck. The hydraulic line fitting was found corroded and fractured which resulted in a loss of hydraulic pressure to the cargo door. Provide a class report attesting to the replacement of the hydraulic line/fitting and proper operation of the cargo door on the weather deck. SOLAS (2004 Cons.) Ch. II-1 Reg 26

In accordance with Table 91.40-3(a) in Title 46 CFR, states a 2.5 year examination interval- a vessel must undergo two examinations within any five year period, no more than three years may elapse between any two examinations. The vessel’s last two examinations of the outside of the ship’s bottom were completed on 30MAR2017 (UWILD) and 28MAR2020 (haul out/dry dock). The vessel was required to conduct its next examination of the ship’s bottom in March 2022. Vessel has no later than 30JUN2022 to conduct its next examination of the ship’s bottom (UWILD) to the satisfaction of the attending USCG MI and Class Surveyor.

Repair/renew ruptured fuel line on main engine to the satisfaction of Coast Guard Marine Inspector prior to operating. Vessel is required to be escorted by two tugs while maneuvering into Sector Hampton Roads Zone. The two tugs shall consist of one tug greater than or equal to 3,000 HP and one tug greater than or equal to 2,000 HP.

Prove vessel meets safe manning certificate prior to sailing.

Provide current publications prior to sailing.

Place all lifesaving equipment on station prior to sailing.

Demonstrate fire and abandon ship drills in presence of a Marine Inspector prior to sailing.

Replace aft paint locker fan and prove proper operation.

Prove proper operation of #1 and #2 auxiliary boilers and all safeties.

Replace wiring kick pipes around main deck as needed.

Prove proper servicing of all lifesaving equipment.

Replace weathertight doors around helo deck as needed.

Submit updated automation procedures for approval to local OCMI.

Repair/replace CO2 siren located in E/R Control Room & submit service report upon completion of repair.

Complete test of high pressure CO2 system.

Perform fire/boat/security drills in the presence of a CG marine inspector upon ship activation prior to getting underway.

Provide proof of proper manning with all licenses/MMD's/STCW upon ship activation prior to getting underway.

Repair #2 Central fresh water cooling pump.

Replace and properly mount all expiring lifejacket and immersion suit lights.

Replace or hydro medical oxygen bottles located in the operating room.

Provide approved ASME relief valve for the potable water hydro-pneumatic tank.

Make temporary repairs to the centerline stiffeners and breast plate IWO the bulbous bow between the 22'03" level and the 28' 8 5/16" level. Temporary repairs shall be completed to the satisfaction of the attending ABS Surveyor prior to the vessels departure from [name withheld], B.I.O.T.

Make permanent repairs to the centerline stiffeners and breast plate IWO the bulbous bow between the 22'03" level and the 28' 8 5/16" level. Repairs shall be completed to the satisfaction of the attending marine inspector.

Install removable, or hinged covers on the rudder to permit examination of the rudder retaining nut during UWILD examination. Current covers are welded in place and do not permit examination of the nuts. This items shall be completed at the vessels next drydocking.
