176.25-5(a),(2) At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections. (2) Examine and test by operation all watertight closures in the hull, decks and bulkheads. MI observed water tight door in below deck main transverse water tight bulkhead with torn and missing gasket at bottom section. The gasket was dryrotted and possibly a "weather tight" gasket. The WTD also need to be adjusted. Inspect all WTD gaskets and ensure all have silicone watertight gaskets. Conduct chalk test on all WTD's.

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment. All vessels of 26 feet in length and over shall be provided with individual bilge lines and bilge suctions for each watertight compartment. The individual bilge suction lines shall be led to a central control point or manifold. Each line shall be provided with a stop valve at the control point or manifold and in addition shall be proved with a check valve at some accessible point in the bilge line. Vessel shall be provided with bilge pumps in accordance with table 182.25-10(a), 50 GPM. Marine Inspector observed that check valves were not installed and crew was unable to demonstrate working operation of the installed bilge system. Install check valves and conduct satisfactory operational test of the bilge system in the presence of Marine Inspector. 46 CFR 182.25-1(a); 46 CFR 182.25-5(a),(c); 46 CFR 182.25-10(a)

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's stability letter. The vessel may not depart until it is in compliance with these requirements. Marine Inspector observed water intrusion in the generator space in the vicinity of overhead penetrations and approximately 40 gallons of oily-water in the bilge. Remove water from bilge, identify location of water intrusion, and provide repair proposal plan to local MSU St. [name withheld] in Charge, Marine Inspection for review and approval prior to conducting repairs. 46 CFR 185.315

Each vessel certificated to operate on a limited coastwise route in warm water must be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board. Marine Inspector observed vessel is missing inflatable buoyant apparatus’s for 20 persons. Vessel’s COI allows for 239 persons onboard and vessel’s current routes and conditions requires 50 percent life-saving for warm waters beyond. Vessel had four 25-man inflatable buoyant apparatus’s mounted on the aft upper deck. 46 CFR 180.205(c)

Marine employers must establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels who:(1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection. Marine Inspector observed that company could not provide proof that vessel Captain is enrolled in a program for the chemical testing for dangerous drugs on a random basis. 46 CFR 16.230(a)(1)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The white stern light must be placed as nearly as practicable at the stern, on the centerline, showing an unbroken light over an arc of the horizon of (on each side). It must be visible for 2 nautical miles and usually sits lower than the masthead light. Marine Inspector observed that the white stern light was mounted on the fwd mast. 46 CFR 183.420 Nav Rule 21

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The licensed mate bay be substituted by a deckhand, provided this meets the qualifications and is designated in writing by the master, or company as the senior deckhand in accordance with the U.S. Coast Guard Navigation and Vessel Inspection Circular No. 1-91. A copy of the designated letter shall be maintained onboard the vessel. The senior deckhand shall be capable of directing emergency response actions of the vessel’s crew. Marine Inspector observed that the senior deckhand was not designated in writing by the vessel Master or company. 46 CFR 185.420 NVIC 1-91

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics. Marine Inspector observed that vessel could not show proof of crew conducting quarterly fire, man overboard, and/or abandon ship drills. It was evident the crew had not been properly trained. 46 CFR 185.420

An adult life jacket must be provided for each person carried on board a vessel. Marine Inspector observed that 06 adult life jackets were found unserviceable. 46 CFR 180.71(a)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alterations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to, the: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of livesaving, fire detecting, or fire extinguishing equipment. Make necessary repairs to starboard engine starter motor and conduct satisfactory operational test to the satisfaction of the attending marine inspector prior to carrying passengers.

Vessel is prohibited from operations with passengers until a Coast Guard marine inspector is able to conduct a damage survey and operational test of the vessels propulsion. This is a result of the Allision with the Redhook American Yacht Harbor "P" dock piling approximately 1634 on 17FEB2023.

Vessel suffered a casualty to the center shaft on 11DEC16. Vessel is prohibited form carrying passengers or cargo until damage survey has been conducted and repairs have been made to the satisfaction of the attending U.S. Coast Guard Marine Inspector.

Crop and renew pitted and holed bottom plating in way of center main diesel engine raw water intake.

Secure hose clamp on starboard side exhaust piping in steering compartment and double clamp all flex exhaust piping.

Replace wasted gasket on side shell valve in starboard side engine room servicing starboard engine cooling.

Crop and insert 18 inch x 18 inch hull insert in way damage found in port hull within lower passenger salon compartment, as shown. In addition, crop and renew damaged horizontal plate stiffener as needed. Marked as #16.

Renew port shaft strut cutlass bearing in its entirety.

Install vessel name and hailing port on stern of vessel as per COD. Lettering shall be a minimum of four (4) inches in height.

Prior to carrying passengers, provide USCG with written proposal and complete all work to make port, forward, engine room bulkhead watertight.

Starboard shaft found fractured at key-way. Renew starboard shaft and provide non-destructive testing for both remaining shafts prior to re-installation. All shafts found to be fractured shall be renewed.

Two of the mounting bolts for the port side generator were discovered loose. Ensure that all mounting bolts for both port and starboard generators are sufficiently tight.

The overhead hanging bracket/mount for the port side generator exhasut piping was discovered disconnected. Ensure that the bracket is re-attached to the piping by means of existing mount and that nut/bolt is sufficiently tight.

Hatch/access area to aft steering/lazarette found not watertight. Ensure watertight integrity for entire assembly.

Potable water pump and pressure tank assembly found not securely mounted. Ensure assembly is securely mounted.

Interlock required to prevent generators from being simultaneously connected to the switchboard was found inadequate. Insure the interlocking device functions properly.

Racor fuel filter in engine compartment found missing the required flame shield. Renew flame shield.

Seal all numerous open holes found in WTB between steering gear room and engine room.

Repair weld fractured weld for longitudinal frame in way of hull insert, portside, amidship, as show. This item is located at item marked as # 16.

All exhaust system piping for each of the MDEs were missing either hanging brackets of support blocks as required by 46CFR119.430. As such there was observed possilbe leakage in way of the flexible connection aft of the raw water injection for each of the exhausts. All exhast piping shall have either installed hanging brackets or supporleak source shall either be proven ironeous or arrested.

Center light diffuser in aft engineroom noted missing. Each lighting fixture globe, lens or diffuser must be fitted with a guard or be made of high strength material, as required by 46CFR120.410. High strength diffuser must be renewed or light must be removed. If light is removed, all associated wiring shall be removed as well.

Ensure that wire connections made for exhaust fan in way of aft starboard engineroom are secured to prevent loosening and are made within the associated enclosure (junction).

Make sufficient repair to the support bracket for starboard main diesel engine coolant water discharge piping where aft elbow is mounted to bulkhead.

Make necessary adjustments to aft inboard flourescent lighting fixture above center exhast piping in engineroom to ensure that cover may be re-attached.

Ensure all bolts are provided and tightened for each flange where strainer is installed in raw water coolant piping.

Renew course rail where noted missing from starboard side handrail. Until this item has been corrected passengers may not be allowed access to bow seating.

Renew light guard/globe for fixture in lazarette.

The air control system for the transmission failed causing the captain to lose control of the vessel.

The voltage regulator on the starboard compressor malfunctioned, causing the air compressor to trip-off line. This caused the M/V LADY VENTURE to lose control air for maneuvering the vessel properly. Portside compressor used until voltage regulator is repaired.

Repair light fixture in ladies head.

Properly bolt all loose bench seats to the deck as directed prior to carrying passengers.

Re-install captive chain on aft deck hatch.

Provide a properly marked watertight container for flares.

Provide a logbook and properly log emergency drills and inspection of lifesaving equipment.

Stencil all 5 new 22 person lifefloats with vessel's name & capacity.

Install proper size weak links on both inboard lifefloat painters.

Renew cracked window on starboard side, main deck.

Stencil PFD locker on upper deck with number and type of PFD's enclosed.

Properly mark entry to upper deck with passenger limit (maximum of 50 persons allowed).

Properly mark emergency fuel shutoffs.

PERMANENTLY REPAIR INSETS ON STARBOARD SIDE AND LAZERETTE AT NEXT DRY DOCK TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

SECURE COVERS ON ALL ELECTRICAL PANELS.

MAKE PERMANANT REPAIRS TO THE STBD RUNNING LIGHT.

MAKE PERMANANT REPAIRS TO THE STBD ELECT BILGE PUMP.

PROPERLY SEAL ALL THROUGH BULKHEAD PENETRATIONS.

MAKE ACCESSABLE THE FWD STBD SIDE COMPARTMENT IN THE FWD/LOWER PASSENGER SPACE FOR CG INSPECTION.

STENCIL LIFEJACKET NUMBER ON CONTAINER LOCATED ON THE UPPER DECK.

PROVIDE PORT SIDE ENGINE ROOM VENT COVER.

PROVE OPERATION OF STARBOARD GENERATOR.

PROPERLY LABEL ALL ELECTRICAL BREAKER PANELS.

REMOVE ALL LIFEFLOAT COVERS FOR COAST GUARD INSPECTION - ONLY -.

COMPLETE IFR WORKLIST ISSUED 05DEC96 PRIOR TO 01FEB97

PROVIDE RETRO-REFLECTIVE TAPE FOR RING BUOY.

REPAIR PORT ENGINE TACHOMETER ON THE BRIDGE.

Vessel may not operate under its Morgan City issued COI within another OCMI/COTP zone without completing an inspection to the satisfaction of the cognizant OCMI.

The watertight door between the engine room & generator room shall be welded shut prior to carrying passengers. (Allowed to use during delivery voyage for crew's safety.)

INSTALL START AND STOPS FOR MAIN ENGINES ON BRIDGE OR PROVIDE MEANS OF COMMUNICATIONS FROM BRIDGE TO ENGINE ROOM TO THE SATISFACTION OF AN ATTENDING COAST GUARD INSPECTOR.

REMOVE UNSECURED WASHER AND DRYER FROM LOWER PASSENGER AREA AND PROPERLY SECURE ELSEWHERE.

PROPERLY SECURE OR REMOVE SCUTTLE BUTT IN UPPER PASSENGER AREA.

REPLACE CRACKED CENTER PILOT HOUSE WINDOW.

RENEW CLUTCH CABLE ON STARBOARD MAIN ENGINE.

MAKE FAST THE FIRE PUMP, BILGE PUMP, AND PORTABLE WATER PUMP.

PUMP OUT AND CLEAN LAZARETTE BILGES OF COMBUSTIBLE PRODUCT.

REPLACE NAVLIGHT BULBS WITH PROPER STYLE.

RENEW SECURING LINE ON EPIRB.

REPLACE MISSING PLUGS (VENT COVERS) ON TANK COVERS.

INSTALL COVER OVER CENTER VENT STARBOARD SIDE (ENGINE ROOM).

SECURE WIRE RUNS AS NEEDED IN ENGINE ROOM.

REPAIR WIRE LEADS TO STARBOARD BATTERY BOX ENGINE ROOM.

REPAIR OR REPLACE INOPERABLE COMODE IN CREW HEAD.

INSTALL COVERS THAT ARE MISSING OVER TWO FLORESCENT FIXTURES.

REPAIR HOLE IN ENGINE ROOM SUPPLY VENT.

RENEW EXHAUST WRAPPING PORT ENGINE.

SECURE ALL JUNCTION BOX COVERS.

REMOVE ALL "DEAD END" WIRES FROM DISUSED ALARM SYSTEM ON MAIN ENGINES.

RENEW STARBOARD GENERATOR BATTERY BOX.

PROPERLY SECURE HATCHES OVER PORT AND STARBOARD MAIN ENGINES.

REMOVE IMPROPER WIRES BETWEEN CONSTAVOLT CHARGER AND SWITCH BOX IMMEDIATELY ADJACENT.

MAKE PERMANENT REPAIRS TO FRACTURED SWASH BULKHEAD NEXT TO STARBOARD RUDDER POST IN LAZARETTE.

CONTACT THE INSPECTION DEPARTMENT AT THE ABOVE ADDRESS AND REQUEST THAT THE VESSEL'S STABILITY LETTER BE REISSUED REFLECTING THE VESSELS CURRENT NAME AND OFFICIAL NUMBER. WHEN REISSUED ENSURE THAT THE ORIGINAL COPY OF THE STABILITY LETTER IS POSTED IN THE PILOTHOUSE OF THE VESSEL.

REWELD AROUND THE CENTER DOG ON THE WATERTIGHT DOOR TO THE MAIN DECK CABIN.

PROPERLY INSTALL RETRO=-REFLECTIVE TAPE ON ALL LIFEFLOATS.
