176.25-5(a),(2) At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections. (2) Examine and test by operation all watertight closures in the hull, decks and bulkheads. MI observed water tight door in below deck main transverse water tight bulkhead with torn and missing gasket at bottom section. The gasket was dryrotted and possibly a "weather tight" gasket. The WTD also need to be adjusted. Inspect all WTD gaskets and ensure all have silicone watertight gaskets. Conduct chalk test on all WTD's.

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment. All vessels of 26 feet in length and over shall be provided with individual bilge lines and bilge suctions for each watertight compartment. The individual bilge suction lines shall be led to a central control point or manifold. Each line shall be provided with a stop valve at the control point or manifold and in addition shall be proved with a check valve at some accessible point in the bilge line. Vessel shall be provided with bilge pumps in accordance with table 182.25-10(a), 50 GPM. Marine Inspector observed that check valves were not installed and crew was unable to demonstrate working operation of the installed bilge system. Install check valves and conduct satisfactory operational test of the bilge system in the presence of Marine Inspector. 46 CFR 182.25-1(a); 46 CFR 182.25-5(a),(c); 46 CFR 182.25-10(a)

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's stability letter. The vessel may not depart until it is in compliance with these requirements. Marine Inspector observed water intrusion in the generator space in the vicinity of overhead penetrations and approximately 40 gallons of oily-water in the bilge. Remove water from bilge, identify location of water intrusion, and provide repair proposal plan to local MSU St. [name withheld] in Charge, Marine Inspection for review and approval prior to conducting repairs. 46 CFR 185.315

Each vessel certificated to operate on a limited coastwise route in warm water must be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board. Marine Inspector observed vessel is missing inflatable buoyant apparatus’s for 20 persons. Vessel’s COI allows for 239 persons onboard and vessel’s current routes and conditions requires 50 percent life-saving for warm waters beyond. Vessel had four 25-man inflatable buoyant apparatus’s mounted on the aft upper deck. 46 CFR 180.205(c)

Marine employers must establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels who:(1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection. Marine Inspector observed that company could not provide proof that vessel Captain is enrolled in a program for the chemical testing for dangerous drugs on a random basis. 46 CFR 16.230(a)(1)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The white stern light must be placed as nearly as practicable at the stern, on the centerline, showing an unbroken light over an arc of the horizon of (on each side). It must be visible for 2 nautical miles and usually sits lower than the masthead light. Marine Inspector observed that the white stern light was mounted on the fwd mast. 46 CFR 183.420 Nav Rule 21

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The licensed mate bay be substituted by a deckhand, provided this meets the qualifications and is designated in writing by the master, or company as the senior deckhand in accordance with the U.S. Coast Guard Navigation and Vessel Inspection Circular No. 1-91. A copy of the designated letter shall be maintained onboard the vessel. The senior deckhand shall be capable of directing emergency response actions of the vessel’s crew. Marine Inspector observed that the senior deckhand was not designated in writing by the vessel Master or company. 46 CFR 185.420 NVIC 1-91

he owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part and, when applicable, the duties listed in the station bill required by § 185.514 of this part. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics. Marine Inspector observed that vessel could not show proof of crew conducting quarterly fire, man overboard, and/or abandon ship drills. It was evident the crew had not been properly trained. 46 CFR 185.420

An adult life jacket must be provided for each person carried on board a vessel. Marine Inspector observed that 06 adult life jackets were found unserviceable. 46 CFR 180.71(a)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alterations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to, the: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of livesaving, fire detecting, or fire extinguishing equipment. Make necessary repairs to starboard engine starter motor and conduct satisfactory operational test to the satisfaction of the attending marine inspector prior to carrying passengers.

Vessel is prohibited from operations with passengers until a Coast Guard marine inspector is able to conduct a damage survey and operational test of the vessels propulsion. This is a result of the Allision with the Redhook American Yacht Harbor "P" dock piling approximately 1634 on 17FEB2023.

Vessel suffered a casualty to the center shaft on 11DEC16. Vessel is prohibited form carrying passengers or cargo until damage survey has been conducted and repairs have been made to the satisfaction of the attending U.S. Coast Guard Marine Inspector.

Crop and renew pitted and holed bottom plating in way of center main diesel engine raw water intake.

Secure hose clamp on starboard side exhaust piping in steering compartment and double clamp all flex exhaust piping.

Replace wasted gasket on side shell valve in starboard side engine room servicing starboard engine cooling.

Remove flex hose outboard of port side shell valve servicing port engine cooling and replace with hard pipe.

Repair or replace wasted and holed cooling water piping servicing starboard side generator.

Provide closure device for goose neck ventilation on starboard side of operating station.
