The owner, managing operator, or a qualified servicing facility as applicable shall conduct the following inspections and tests: For portable fire extinguishers, the inspections, maintenance procedures and hydrostatic pressure tests required by NFPA 10. Vessels servicing record indicated portables failed due to expired hydro test dates however service tags indicated that extinguishers were inspected and service ready. Provide documentation from the servicing company that all portable extinguishers have passed full inspection.

The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. In addition, the OCMI may require the vessel to get underway. Due to safety concerns in regards to deficiency items 1-3, underway drills and DVTP's were not observed by the attending [name withheld] and underway drills in the presence of MI prior to carrying passengers.

The main steering gear and rudder stock must be capable of moving the rudder from 35 degrees on either side to 35 degrees on the other with the vessel at its deepest loadline draft and running at maximum ahead service speed. While underway with passengers to Salem, vessel reported a loss of maneuverability as steering gear froze in the hard over port position, preventing the vessel from safely maneuvering. Once repairs are affected, prepare and have vessel ready to perform sea trials to the satisfaction of attending Marine Inspector. 46 CFR 58.25(b)2

Propulsion machinery must be capable of operating at constant marine load. Vessel experienced a loss of propulsion of the starboard engine. Affect necessary repairs and conduct satisfactory sea trial in presence of Marine Inspector.

A storage cylinder must be securely fastened, support, and protected against damage. CO2 cylinder bracket in starboard lazarrette is wasted.

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. Battery covers were not properly secured.

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the structure, machinery, equipment, and on a sailing vessel, rigging and sails. During vessel's annual inspection, Marine inspector noticed pitting in the forward transverse bulkhead in Port Lazarette. Vessel shall address pitting at next scheduled drydock examination.

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge, embarkation stations, the location of fire protection/emergency equipment, compartment titles and hazard classification of accommodation and service spaces, and structural fire protection boundaries. IAW 46 CFR 116.202 the owner shall submit for approval to the Marine Safety Center, three copies of the following plan, to Marine Safety Center: Structural fire protection details. Post approved fire control plan prior to carrying passengers.

On a vessel without overnight accommodations, or with overnight accommodations for not more than 49 passengers, the fire pump must be capable of delivering a single hose stream from the highest hydrant, through the hose and nozzle required by § 118.320 of this part, at a pitot tube pressure of 345 kPa (50 psi). Unable to demonstrate requirement, due to frozen pipes. Demonstrate 50 psi from the highest hydrant, prior to carrying passengers.

Each vessel must be provided with bilge pumps in accordance with 46 CFR 56.50-55, which states: each hull of a vessel with more than one hull must have at least two means for pumping the bilges in each hull. The port side bilge pump was unable to pump a starboard void space. Vessel cannot carry any passengers until proper operation of the bilge pump and cross connect are verified.

Except as allowed by paragraphs (b),(c) and(d) of this section, each vessel certificated to operate on a limited coastwise route shall be provided with the survival craft required by §§ 117.204 (a) through (d) of this part, as applicable. All of the vessel's IBAs were removed for annual servicing. Vessel is limited to a route within one mile from shore until the IBAs are re-installed onboard to the satisfaction of the OCMI.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First- Aid kit medications were expired.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space.

Cable/Wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. Wire & Cables were installed with zip ties instead of metal brackets.

Electrical equipment on a vessel must be installed and maintained to:Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Expose bonding at the Port side ENG, and Control panel on STDB engine room was not blanked.

Inspection of all accessible parts of the exterior and interior of the hull, watertight bulkheads and weather deck shall be made. Cracks were found on welds in the starboard #2 void, located midway up on the most aft frame where the longitudinal is welded,on both the outboard and inboard sides.

The number and identification of the items stowed inside and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. The lettering on the upper deck life jacket stowage containers are faded and not clearly legible.

Each ring life buoy with a waterlight must have a lanyard of at least 3 feet, but more more than 6 feet securing the waterlight around the body of the ring life bouy. The lanyard exceeded 6 feet and needs to be replaced due to deterioration.

No flammable or combustible liquids may be stowed in any accommodation, control, or service space (other than than a paint locker). 10 gallons of hydraulic fluid ware stored at the Bridge.

Penetrations in A Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. The CO2 switch cable going through the starboard side engine room overhead fire boundary (rox blocks) is compromised and will not prevent the passage of flame and smoke.

Piping in vital systems shall comply with the provisions in Subchapter F. The vessels bilge pick up lines in both hulls have approximately 2 feet of rubber hose vice metal pipe.

Connections to batteries must be made with permanent type connectors. The connections to both port and starboard 24V batteries are connected by wing nuts.

A vessel must be provided with a piping system that meets §56.50-50 in subchapter F of this chapter. Port hull bilge line to the #2 void space leaks water into the engine room while the bilge pump in in operation. Determine source of leak and make repairs as needed to restore port hull bilge system to full operation.

A fire drill must be conducted at least once each month. Vessels most recent fire drill was conducted in July 2018.

Crew must be trained on procedures listed in the station bill at least once every three months. Vessels most recent man overboard drill was conducted in July 2018.

The COI must be posted under glass, such that all pages are visible, in a conspicuous place where observation by passengers is likely. COI is currently posted in the wheelhouse.

A fire control plan must be posted on the vessel visible to passengers. Vessel does not have a plan.

Suitable means for shutting off the passage of air through the ventilation in the event of fire. Port aft manual roll down damper does not fully block air flow.

Vsl is broadcasting invalid IMO #

STBD side steering ram was ripped off its mount on a transverse web frame. Crop and renew web frame and prove proper operation of steering system.

Make the stbd steering compartment available for internal structural examination. Clean all hull plating and framing to bare aluminum.

Sewage tank has severe pitting. Repair/replace tank during next drydock.

One of the vessel's 50-person IBAs has been removed for annual servicing. The total number of persons allowed on board during cold water operations more than three nautical miles from shore is limited to 150 until the IBA has been re-installed to the satisfaction of an attending marine inspector.
