The vessel is permitted temporary manning allowance and deviate from the manning requirements set forth in the Certificate of Inspection (COI) reducing the manning to: * 01 Master * 01 Deckhand Conditions of Deviation: * Vessel passenger count is reduced to 90 * The upper deck must remain secured at all times. * No passengers are permitted on the second deck under any circumstances during operations, unless an emergency situation dictates. This manning reduction is authorized under the condition that the vessel operates within the limitations specified above. During vessel operation, vessels horn appeared to sound weak and distorted. Horn shall be audible for one-half nautical mile.

Remote fuel oil shutoff valve was inoperable. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located.

Battery cables on the port and stbd main engine switches were sound to be very loose. Other battery connections and one other switch was found to have very loose battery cable connections. Connectors must be secured to prevent loosening due to vibration.

Found lifejacket locker door handle that is able to be locked. Each storage container for life jackets must not be capable of being locked.

Port main propulsion engine found to be missing alternator safety guard. An exposed hazard, such as gears or rotating machinery, must be protected by a cover or rail.

A vessel of 12 meters or more in length shall be provided with a whistle. The vessels whistle was inoperable due to weathering.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot or similar publication. Vessel did not have a U.S. Coast Pilot on board

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel did not have a first aid kit that met the requirements of 46 CFR 199.75(b)(10) or a kit with equivalent contents and instructions.

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Vessels marker light lanyard was dry rotted and not serviceable

(d) Each nozzle must be of corrosion-resistant material and be capable of being changed between a solid stream and a spray pattern. A nozzle on a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, must: (1) Be of a type approved in accordance with approval series 162.027; or (2) Be of a type recognized by the Commandant as being equivalent in performance. Vessels fire hose nozzle was not servicable when tested owner needs to replace nozzle folling the above regulation.

(b)(4)All cable and wire must: Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. Vessel had a wire secured using zip tie in overhead of engine room, change zip ties to approved supports.

An adult life jacket must be provided for each person carried on board a vessel. The vessel had 17 non-serviceable adult lifejackets on board

Frequency meter on the main bus panel was not working during the operation of the generator on vessel power. For each alternating current generator, a means for measuring frequency must be provided.

Electrical power available indicator light on the main bus panel was not working in either shore power or vessel power. Electrical equipment on a vessel must be installed and maintained to: protect crew from electrical hazards.

Found handles on engine room hatch froze. Watertight hatch handles should move freely enabling hatch to properly close and make watertight.

Found electrical wires in deck penetration into engine room behind bar area near engine room hatch had no fire rated caulk.

First aid kit was found with missing or expired items.

Vessel did not have required navigation publications on board.

Portable fire extinguishers were found not having monthly inspections as required by NFPA 10.

Remote fuel shutoff valve cable was found to be difficult to operate. Cable worked to close the fuel valve after multiple tries. Cable was difficult to reset. Replace/Repair cable to ensure frictionless operation of cable in either direction.

Found loose battery connections of auxiliary wires. Reconnect wires to ensure all fittings are tight to the battery terminal.

Stbd propulsion engine failed to restart after securing running engine. Vsl rep suspected an inoperable starter.

Oil pressure gauge at the operating station found not working.

Conduct fire and man-overboard drills with summer crews.

Provide proof of DAPI testing results for CY-15.
