No extensive alterations involving the safety of a tank vessel in regard to the hull shall be made without the approval of the Commandant. Before such alterations are carried out, copies of plans and specifications for the work involved shall be forwarded for submission to Headquarters (USCG Marine Safety Center) for approval. During Internal Structure Exam, Marine Inspectors discovered that all intermediate floor frames in the wing voids had been cropped back from the knuckle by approximately 3 feet and angled. The floor frames had previously extended all the way to the knuckle before the modification. The modifications had not previously been approved by Marine Safety Center, as the letter that was thought to have approved the modifications (MSC Letter #C1-1001654) was an approval for a different class of barge and CBC 114 was not included in the approval. Submit modification plans to Marine Safety Center for approval. Vessel may not operate until approval from Marine Safety Center is received and presented to attending USCG Marine Inspectors.

During loading, a vapor leak was found coming from a pin hole in a weld on the tank top of the #3 cargo tank. After temporary repair was made and passed leak test barge was allowed to transit for offload and then proceed to get permanent repairs prior to next loading.
